Dr. Cobb received his doctorate of podiatric medicine from the California School of Podiatric Medicine with Samuel Merritt University in Oakland in 2005. He went on to complete his 3-year surgical residency training program with Kaiser Permanente's San Francisco, Bay Area Foot, and Ankle Residency program. Dr. Haywan Chiu received his undergraduate degree from the University of California, Santa Cruz, and did his doctorate of Podiatric medicine at Temple University. He completed his 3-year surgical residency program with DVA Palo Alto Podiatry residency program and Stanford University. Dr. Matt Cobb and Dr. Haywan Chiu discuss their experiences in podiatry and the development of the academy, providing valuable resources for students, residents, and faculty. Dr. Cobb specializes in reconstructive surgery with a focus on pediatrics, bringing a unique perspective to the field. Tune in to learn more about their journey and the impact of the academy on the podiatric community. Dr. Cobb and Dr. Chiu (along with 2 of their colleagues) began recording their surgeries to share with their students and residents to facilitate distance learning. They hoped to keep their residents engaged in learning surgical procedures even if they were not able to be physically present in the operating room due to pandemic restrictions/limitations. Listen as they describe their experiences with adaptability, flexibility, and leaning into technology to help advance surgical education. We discuss how vulnerability can help broaden opportunities and break down barriers to quality education. The Foot and Ankle Surgery Academy, also known as FASA, was established during the COVID-19 pandemic in response to the challenges faced by students, residents, and faculty in podiatric medicine. Dr. Matt Cobb and Dr. Haywan Chiu, the founders, recognized the impact of the pandemic on surgical training, with elective surgeries being canceled and limited opportunities for residents to gain hands-on experience in the operating room. This led to the idea of creating a platform where surgical procedures could be filmed and shared with the podiatric community. The primary goal of FASA is to provide educational resources through video compilations of various surgeries, accompanied by voiceovers from the primary surgeons. The videos aim to offer a detailed insight into the decision-making process, techniques, and nuances of podiatric surgeries. The founders emphasize showcasing not only the technical aspects of surgery but also the thought process behind each step. The creation of FASA allowed for a unique learning experience for students, residents, and faculty. The videos provided a visual representation of surgeries, allowing viewers to observe the procedures in detail. The platform also offered a space for discussion and reflection on different approaches to surgery, highlighting the importance of decision-making and adaptability in the operating room. Dr. Cobb and Dr. Chiu also discuss how the ability to make sound decisions during surgery is crucial, as it can determine the overall success of the procedure. They shared examples of cases where high-risk decisions led to positive outcomes, showcasing the significance of thoughtful and strategic decision-making in surgical practice. Dr. Cobb shares a valuable insight from his mentor, Shannon Rush, who emphasized the importance of stepping in right before a mistake is made that cannot be fixed. This highlights the critical role of experienced surgeons in guiding residents and younger surgeons to make the right decisions during procedures. Dr. Cobb's experience with Dr. Rush exemplifies how mentorship and guidance from seasoned professionals can shape a surgeon's decision-making process and ultimately lead to better outcomes for patients. The discussion also touched on the challenge of maintaining focus and presence during surgeries, with Dr. Cobb and Dr. Chiu emphasizing the need for residents to stay engaged and attentive throughout the entire procedure. They highlighted the potential consequences of zoning out or losing focus, underscoring the importance of being mentally present and focused on the task at hand to ensure successful surgical outcomes. They also share a personal experience where he and his colleague would remind residents not to zone out during surgeries, especially during critical moments like organ closures. He stressed the impact of losing focus, mentioning the potential consequences such as infections and compromised outcomes due to laziness or distraction. Dr. Chiu mentioned the challenge of teaching this mental integration component, as it goes beyond technical skills and requires a deep level of concentration and mindfulness. The discussion touched on the concept of neuroplasticity and focus, highlighting the variability in individuals' abilities to maintain focus for extended periods. https://explorepodmed.org/ Dean's Chat Website Dean's Chat Episodes Dean's Chat Blog Why Podiatric Medicine? In this episode of Dean's Chat, Dr. Jeffrey Jensen is joined by Dr. Jonathan Leibovitz, the Dean at Western University's College of Podiatric Medicine. Dr. Labovitz discusses leadership, the academic path to becoming a faculty member, and his role at the College of Podiatric Medicine since its inception in 2009. They discuss Dr. Leibovitz's background and experiences in the field, including his education at Cornell University and Temple University in addition to his AO International Trauma Surgery Fellowship experience, and Podiatric Medicine as a career. Tune in to learn more about Dr. Leibovitz's journey and insights into the world of podiatric medicine! Dr. Leibovitz talks about his start in Podiatric medicine and how he chose to channel his love for sports into his profession as a team physician. He highlights how many incoming students have visions of surgery or sports medicine when coming into school, but once they get into the profession, they understand all of the different other areas they can become an expert in, whether it's wound care, whether it's pediatric foot issues. In this episode, Dr. Leibovitz describes his experience during his AO International Trauma Surgery Fellowship in Austria and how that experience impacted his career. Dr. Jensen and Dr. Leibovitz discuss whether they think all students should go to a fellowship to get extra training in one focused area. Dr. Leibovitz emphasizes the significance of listening to students and valuing their suggestions and feedback. He reflects on his own experience as a student, where his opinions were often disregarded, and stresses the importance of changing this dynamic. Dr. Leibovitz acknowledges that the current generation of students expects to be heard and valued, and it is crucial to meet these expectations. https://aacpm.org/about/ https://www.westernu.edu/podiatricmedicine/ Dean's Chat Website Dean's Chat Episodes Dean's Chat Blog Why Podiatric Medicine? On today's episode, Dennis sits down with Sports Chiropractor and mental and physical chemistry researcher, Dr. Carmen Romano. Dr. Carmen Romano received his Doctor of Chiropractic degree from Bridgeport University in December of 1999. After earning his degree, Dr Romano spent a year practicing in Italy before returning to New Jersey. Since 2003, he has owned Madison Medical & Sports Rehabilitation Center in Madison, NJ. Madison Medical is an integrative medical center that includes Internal Medicine, Functional Medicine, Physical Medicine, Chiropractic, Podiatric and Physical Therapy services to patients. He also owns and operates R3 Recovery Lab which offers cryotherapy, infrared sauna and pneumatic compression services as well as cold-pressed juices, detox cleanses and butter brain coffee. At Madison Medical, Dr Romano has created a center where patients receive immediate attention from a collaborating team of whole body wellness focused practitioners. What began as a small Chiropractic office in 2003 has grown to a 10,000 square foot integrative medical center through Dr Romano's philosophy and vision. Patient care at Madison Medical is a collaborative process with providers co-treating to address systemic issues along with current symptoms. Visits may include hormone and micro-nutrient testing, diagnostic ultrasound and x-ray, gait evaluations and orthotics to identify and address root causes of the symptoms being experienced. Dr Romano's newest venture, R3Recovery Lab was born out of the observed need for complimentary services such as Cryotherapy, Infrared Sauna and Pneumatic Compression to aid athletes in real recovery. Recognizing that pro athletes have access to these types of services every day, Dr Romano created R3 so that the “weekend-warrior” type athlete or average patient recovering from injury or illness could also benefit from these services.Today, J.P. gets deep into sharing his story starting from what inspired him to join the Army, the loss of his friend Justin while on mission and how that impacted his view on life, the day of his incident that led to his tragic injuries, his battle with recovery and his 12 suicide attempts, keeping faith when you're in the darkness, the book he wrote "Walking Again" and the support to handle life together. Today, Dennis and Dr. Romano talk about the wear and tear of the "grind", the effects of diet on the body and the brain, tweaking your diet to fit your lifestyle, importance of relationships and how it corresponds with your mental health, creating goals for yourself and business advice. “Fix Your Feet ®” focusing on the surgical correction of painful and embarrassing forefoot deformities, including bunions and hammertoes, using the Tiara-Toe™ technique is the brainchild of Dr. Yolanda Ragland. Her mantra, "Providing medically necessary surgery with a cosmetic result®”, is synonymous with her surgical practice and its final outcome in keeping with her patients' concerns. While most Podiatric and Orthopedic surgeons often substitute remedial foot surgical procedures for aesthetic obligations, Dr. Ragland integrates these two domains and furnishes her patients with both functionality and relief. The revolutionary Tiara-Toe™ technique is a Pedi-Plastic™ approach that yields the best enhancement results consistent with an aesthetic outcome. In addition to her many services, Dr. Ragland offers comprehensive evaluations aimed at educating her patients on the best surgical procedure relative to their specific condition(s). It is her conviction that patients should be more informed and involved in order to make the necessary decisions with regards to obtaining the best recommendations while in pursuit of corrective foot surgery for bunions, hammertoes, cosmetic foot surgery, and other forefoot disorders. Dr Cam Wareham has worked in the area of Podiatric Medicine and Surgery for over 25 years, and he has worked across three countries, New Zealand, Australia, and the United Kingdom. His career started in MSK and sports podiatry in Canberra, and even though he did enjoy it, he could not kick the bug and excitement of becoming a podiatric surgeon. He honed his craft in the National Health Service in the UK, and even though some podiatrists would say he has been lucky, I would say when you work 60 - 70 hours per week for multiple years luck is not a factor. On this episode we also discuss: Why he moved to the UK and worked in the NHS Chasing the surgery dream By working long hours, you make your own luck. Why you sometimes need to buy a round of drinks We’re not medical practitioners, but we are medically-trained Changing your mindset - You are an educated practitioner. Why you are not just a podiatrist. You would never hear a dentist never say I’m just a dentist Empowering the podiatry profession Medical law - A patients idiocy is only part of your defence "There’s something about keeping your patients close and keeping the ones with complications even closer". Final Tip When you see new advances in Podiatric Medicine never stop learning and embrace everything you can. You may not need it today, but tomorrow it could change your practice. If we stand still, we go backwards, so embrace the idea of improving your skillset. If you have any questions about this podcast, please email me at tf@tysonfranklin.com. Diary - Your diary needs to be structured in a way that maximises patient numbers and increases daily profits. Today’s guest is Dr. Emily Splichal, Podiatrist and human movement specialist. When it comes to sprinting fast, and jumping high, the feet are a critical link. We’ve heard it often on this podcast, sprint coaches referencing the feet as a key component of fast athletes. To that end, I’m delighted to have Dr. Emily on the podcast today. Dr. Emily is a true expert on the performance driven foot. Along with being a doctor of Podiatric medicine, she is the Founder of the Evidence Based Fitness Academy, is the creator of multiple barefoot training courses and inventor of Naboso Barefoot Technology. She has over 16 years in the fitness industry and has dedicated her medical career towards studying postural alignment and human movement as it relates to barefoot science, foot to core integration and training from the ground up. Dr. Emily is actively involved in barefoot training research and barefoot education as it relates to athletic performance, injury prevention and movement longevity. She also serves as a consultant for many prominent fitness and training organizations. For today’s episode, we’ll be discussing the feet on many levels in regards to their impact on performance, as well as how we coach and cue the feet in training and lifting movements. We’ll also go into the critical muscles that support the arch and stiffness of the foot, and how to optimally train them. This is a really in depth episode, and one I learned a lot putting together. Today’s episode is brought to you by SimpliFaster, supplier of high-end athletic development tools, such as the Freelap timing system, kBox, Sprint 1080, and more. Key Points: Emily’s background as an athlete and her transition into fitness and movement Aspects of the feet that lead to fast running and sprinting Troubleshooting the rear-foot loading phase of running and sprinting The barometer of supinated feet to pronated feet and associated issues Toe gripping and “overgripping” Connection of the foot to the core in movement Cueing squatting and lifting movements in context of foot action Concepts of the big toe Emily’s favorite strength drill for the feet Quotes “The rate that you can create stiffness in the foot leads to a shorter contact time, which means you get off the ground faster and that’s how you’re fast… that can be built into certain foot types that are able to lock or re-supinate faster. That would be a more inverted foot, technically it would be a more rigid foot” “The foot type that feeds speed would be a more supinated foot” “Regardless of how your foot is striking the ground, whether you are walking, you are a heel-striker, mid-foot striker, or a sprinter, you are always in an inverted rear foot position” “You have to make sure that there sufficient rearfoot inversion, that is the part of the foot that makes sure it is rigid enough for pushoff” “The shortest window for inversion-eversion-inversion is during sprinting, and that’s why they are able to get off the ground so quick” “Our power is in rotations and spirals, the more that you can harness rotational power through your feet, the faster you will be” “Regardless of foot type, you have to make sure that the foot is talking to the core fast enough” “The posterior tibialis is the most powerful locking muscle of the foot” “The up phase of the squat is where you really need the key digit (toe) contact, or the foot engagement” “When they jump, I want them to follow through and push all the way through the digits. I want every last centimeter of the digits to touch the ground when they push off from that jump or that sprint” “That follow through should be off of the big toe” “Your ability to do an optimal push-off and have that high-gear position is linked to that posterior tibialis” About Dr. Emily Splichal Dr. Emily Splichal, Podiatrist and Human Movement Specialist,
