It is simple enough to know that everyone needs protein. Yet, the protein that people are consuming has been decreasing over the years. It is particularly low in adults over the age of 50. To compensate, people are consuming more carbohydrates. Unfortunately, most of those carbohydrates are refined and processed. A simple change that will create a long-lasting and sustainable change is to increase your protein to the appropriate level and keep it there. The Protein Sparing Modified Fast (PSMF) is the best way to start learning about protein. You can then use it as a guideline to create a healthy diet for yourself. Understanding proteinWe need to understand protein. We need to know how much protein we need and how it can change our lives.A different contextWorking with people on various programs and doing panels and labs has given Dr. Karl a different context. He understands that we are not all the same, and we will not all get the same results from doing the same thing.MetabolismTo change your metabolism, you need to look at the bigger picture. You have to get to the root of the problem that you are having metabolically and address that.The ketogenic dietThe ketogenic diet started around treating pediatric epilepsy in the 1920s. At the time, they were most concerned about finding the right diet, and they were worried that they were not going to give the children enough protein. Growing children need protein, so giving them less protein to stop their epilepsy could stunt their growth. That concern held back the roll-out of the classic ketogenic diet.How much protein do you need?Science has now clarified that we need a minimum of one gram of protein per pound of body weight. That converts to a minimum of 2.2 grams of protein per kilogram of body weight. Too much protein can damage the kidneys. The upper limit of our protein intake has not yet been determined, however.Stimulating your muscle massProtein is very satisfying and satiating. Si tuviéramos que diseñar la dieta más efectiva para perder la máxima grasa posible minimizando la pérdida muscular, se basaría en tres pilares principales: Alta en proteína. Para mitigar la pérdida muscular, aumentar la termogénesis y también la saciedad. Muy baja en carbohidrato. Para elevar la producción de cuerpos cetónicos y potenciar la saciedad. Muy baja en calorías. Para que el cuerpo utilice sus propias reservas de grasa. Esto implica que además de baja en carbohidrato será también baja en grasa. Y estas son en realidad las características del protocolo PSMF o Protein Sparing Modified Fast, que podríamos traducir como ayuno modificado que retiene la proteína (el músculo). No es una idea nueva, y se utilizó ya con éxito en los años 70. Demostró ser efectiva para perder peso rápido en personas con obesidad (estudio, estudio). Estudios más recientes confirman que es un método seguro y efectivo para las personas con mucho peso que perder (estudio, estudio). En la última década ha resurgido esta dieta, no solo como estrategia de choque para las personas con muchos kilos de más, sino también para las que quieren reducir rápido su nivel de grasa corporal, como los culturistas antes de una competición. Evaluemos el detalle de esta dieta, con un día de ejemplo, y algunas variantes quizá más recomendables. Los pilares de PSMF Empezaremos profundizando en los tres pilares básicos de este protocolo. Los tres pilares de la dieta PSMF 1) Alta en Proteína Cuanto mayor sea el déficit energético mayor debe ser la ingesta de proteína. En este caso se propone estar por encima de las recomendaciones generales, idealmente en el rango de 2-3 gramos diarios por kilo de peso. Si pesas 70 kg, deberías comer entre 140 y 210 gramos de proteína al día. Si entrenas fuerza, apunta al límite superior (2.5-3 g/kg). Si no, te puedes quedar en la parte inferior (2-2.5 g/kg). Como siempre, si tienes bastante peso que perder, usa como referencia tu peso objetivo, no el actual. 2) Muy baja en Carbohidrato Olvídate de los almidones (cereales, legumbres o tubérculos) y de la mayoría de frutas. Podrás incluir sin embargo mucha verdura baja en carbohidrato, como espinaca, lechuga, kale, pepino, calabacín... La verdura es muy recomendable por dos motivos principales: Nutrientes. Al reducir tanto las calorías existe riesgo de déficit de ciertos minerales y vitaminas, y las verduras aportan una gran densidad nutricional (o muchos nutrientes por cada caloría). Saciedad. La fibra, como la proteína, es saciante, y la combinación de ambas hace que este protocolo sea algo más tolerable. Si realmente lo necesitas, podrías incluir un puñado de fresas, moras o frambuesas al día, que supone unos pocos gramos de carbohidrato. 3) Muy baja en Calorías Las calorías son el factor principal que determinará cuánto peso perderás, por eso están en la base de la pirámide. Sin embargo, no todas las calorías tienen el mismo efecto metabólico, por eso debemos adaptar los macronutrientes según nuestros objetivos. Empezando por la proteína (lo más importante), alguien que pese 70 kg consumirá entre 140 y 200 gramos de proteína al día, representando 600-800 calorías (cada gramo de proteína tiene cuatro calorías). Recuerda además que la proteína tiene un mayor efecto térmico que otros macronutrientes, y aproximadamente el 25% de sus calorías se gastan en su propia digestión. Añade quizá 25 gramos de carbohidrato (100 calorías más) y 20-30 gramos de grasa (180-300 calorías). Esta grasa vendrá seguramente de la carne, huevos o pescado, o también de media cucharada de aceite de oliva en la ensalada. I started keto in October last year and I am a Type 2 diabetic who let my diabetes and weight become out of control due to uncontrolled stress the two previous years. With the exception of a few vacations where I've cheated, I've been strictly keto otherwise eating less than 20g of carbs a day. Hello Jimmy and Dr. Cole, I recently purchased Keto Clarity and read it with a vengeance. Thank you so much for your insights, experience and expertise included in that book, Jimmy. I now feel like I have a great grasp on nutritional ketosis and how it can benefit the body. I've recently been through a trying period with my health and I ended up losing 50 pounds very rapidly due to not eating and malnutrition followed by gaining back 90 pounds when I was "better" again. I recently visited an endocrinologist to help me lose about half my body weight and I was excited when I left the appointment because she told me that I would be on a ketogenic diet! A friend suggested your book, so I purchased it and read it right away. However, after meeting with the dietitian through her office, I found out that I was put on what’s called a protein sparing modified fast. The plan is very limiting allowing only about 600 calories a day. It is also very low in fat and only includes 11 ounces of protein daily. So, after much thought and my sad attempt at researching this online, I thought I would contact you to ask this important question: Does limiting the amount of food you eat on a plan like this equal what a bariatric surgery patient would consume put the body into a more effective state of ketosis than following a more traditional ketogenic diet as described in your book? Is it possible that eating the higher amounts of protein required by the GAPS diet is causing me to plateau and even gain weight? Hi Jimmy and Will, I recently began working with a nutritional therapy practitioner and have been practicing the GAPS diet for about a week now to begin healing my gut and digestive system as well as to detoxify. Before this diet, I was following a ketogenic approach along with intermittent fasting for 16:8 naturally and then practicing 42-hour fasts three times a week and losing weight fairly easily. Now my weight has gone up some and plateaued. My NTP has asked me to stop fasting since it might deprive my body of the nutrients it needs to heal. At the first stage of the elimination portion of the GAPS diet, you eat animal meat boiled in bone broth with specifically approved vegetables like squash, broccoli, onion or cauliflower that are also boiled in bone broth. I am becoming convinced my plateau has to do with the higher amount of protein in this approach. I am only able to add animal fat to the stews and soups that are GAPS-approved and realize it's not the larger amount of fat I was eating on keto. So it seems I’m now eating a high-protein, moderate fat, low-carb diet. Do you have any insight whether or not it’s the protein that is stalling my weight loss and even gaining weight again? Dear Jimmy and Dr. Cole, Can eating a ketogentic diet mask the symptoms of perimenopause? I have been eating a ketogenic diet for over a year after starting my nutritional health journey with a primal/Paleo diet. The pounds on the scale simply are not moving. My doctor had said according to my health markers that there is nothing wrong with my metabolism. My thyroid and adrenals were good and my A1C is 5.0 showing my blood sugar to be well-controlled. According to my doctor, my sex hormone levels indicate I am still fertile at the age of 46. When I test blood ketones, I am .5 or higher and even seeing as high as 2.2 mmol. Prior to eating clean, my cycles were becoming irregular, but after 2 1/2 years of eating well my cycles are now back on track. I am having cyclical symptoms of fatigue, moodiness, and joint pain that is not in correlation with my cycle. It's seems to be happening every 3-4 months and I am no long losing inches while gaining scale weight. Could these symptoms be perimenopausal even with a regular cycle? Both of her parents have been keto since March and I wonder if my granddaughter could eat this way as well so long as she gets enough protein. I’m a registered nurse and it seems to be safe for her to do to help get her extra weight under control. My real concern is with her carbohydrate cravings and whether they will turn into an eating disorder. We’ve already caught her hiding candy and cookies so she can eat them later and then she doesn’t eat the real food she is served at meal times.
