And yet, the single-minded focus, thousands of hours of practice, and high training loads on their musculoskeletal systems mean that musicians and athletes probably have more in common than they have differences. At least from a musculoskeletal rehabilitation perspective. Today's guest is Marianne Roos - a former clarinetist and forever musician-at-heart, who now practices as a physiotherapist and conducts her PhD research from Laval University, Canada. October is awareness month for both Menopause & Breast Cancer, and one area of overlap between these two issues is that of musculoskeletal health Sometimes, when we think about musculoskeletal health at menopause, we might immediately think of osteoporosis. Although that's a really important issues, and one I'll be addressing in upcoming episodes, for today I want to talk about muscle & joint pain at menopause Musculoskeletal dysfunction affects between 70 and 80% of women going through peri to post menopause, with 25% of women having severe symptoms, including rotator cuff dysfunction or gluteal tendinopathy (which affects 1 in 4 women in their 50's and is often misdiagnosed as hip arthritis) In today's episode, I'll discuss the latest research around why this happens (spoilers: oestrogen, inflammation & sleep disruption) and of course the evidence based strategies that we can use to help women live well before, during & after menopause, and (of course!) where pelvic health at menopause fits into the discussion!       In S 5 E 11 I am delighted welcome Dr Bethan Richards to the podcast.   Dr Richards is a Staff Specialist Rheumatologist at Royal Prince Alfred Hospital, Sydney and Deputy Director of the Institute for Musculoskeletal Health. In 2019, following her return from Stanford, Bethan had the honour of being appointed Chief Medical Wellness Officer in Sydney Local Health District – an Australian first. She is also the District's inaugural MDOK Centre Director.            Throughout her career, Dr Richards has had extensive experience and a passion for designing and implementing mentoring, teaching, leadership and wellbeing programs. Following a successful pilot in junior medical staff, her evidence based BPTOK wellbeing program is now being rolled out to over 14,000 staff in Sydney Local Health District as MyDistrictOK (MDOK). Dr Richards is a national thought leader and researcher in the field of workplace wellbeing and has advised and mentored local, national and international organisational leaders on how to improve healthcare staff burnout and promote professional fulfilment.        This is a conversation I have wanted to have on the podcast for some time but feel we have reached a fitting and opportune juncture to have now.       It is hard to go first. In this episode, we meet Dr. Stacey Love, Director of Virtual Physiotherapy and Rehabilitation at Saskatchewan's Virtual Health Hub, and an Assistant Professor at the University of Saskatchewan's School of Rehabilitation. One had traveled 12 hours, the other seven, both to see physiotherapist Stacey Lovo for back pain.   “It was a big undertaking for them,” said Lovo. She said one woman was forced cut her appointment short, to catch the last bus returning north.   The other woman stayed in Saskatoon to treat spinal pain stemming from what Lovo characterized as a ‘very difficult and scary problem.'   “This was devastating for many reasons,” said Lovo who remembered her patient was distraught, unable to return home to care for her young children.   That glaring disparity in healthcare access propelled Dr. Lovo into action. She returned to the classroom, and earned her PhD in Rehabilitation Science, specializing in low back disorders.   “If you're rural and remote resident or if you're Indigenous, your chances of having back pain are 30 percent higher than an urban person who's non-Indigenous,” said Lovo.   Lovo said long waits for care and spending up to twelve hours in a vehicle to attend a one-hour physiotherapy consultation often aggravate spinal damage — rather than healing it.   “When we're traveling that distance, we're undoing everything that's done in the session,” she said.   Working closely with people in Pelican Narrows and the Peter Ballantyne Cree Nation, Dr. Lovo began testing virtual consultations and in-person treatments, advocating for a hybrid model of healthcare.   "These projects are all community driven,” she said. “They're led and driven by the strengths and knowledges from the communities."   Her team's innovative use of Remote Presence Robotics in partnership with local nurse practitioners  in Pelican Narrows demonstrated a successful model of team-based virtual physiotherapy, connecting patients hundreds of kilometres away from health providers with care.   Local health providers are the key, Lovo said.   “Taking reflexes and testing for sensation and muscle strength are done by the nurse on the other side,” said Lovo.  “They have incredible rapport with the patients. They know their families.”   When the pandemic amplified the need for virtual care, Dr. Lovo, Dr. Brenna Bath  and Dr. Lovo's graduate students met the challenge with innovation.   Participants in her back pain study reported their outcomes were noticeably better. Many asked to continue with more treatments.   Dr. Lovo's work goes beyond healthcare delivery; it is a commitment to fulfilling treaty obligations and ensuring equitable healthcare access for Indigenous Peoples.   “Canada's promise to First Nations peoples was that medical care would be taken care of, and so we are  working with communities to try things out that will allow us to provide it,” she said.

Musculoskeletal health is not just a concern for professional athletes. Still, it is paramount for individuals across all walks of life, impacting daily activities from walking to playing sports. It plays a pivotal role in sports performance, allowing athletes to showcase their skills and supporting their every move, from sprinting down the field to executing precision movements on the court. However, men of color face a daunting challenge because access to specialists and timely care is unattainable, especially for those in rural areas or with limited means. It's a disheartening truth that many face these challenges alone, without the support and guidance they deserve.   Therefore, we must acknowledge that the way forward necessitates taking proactive measures in the face of such disparities. We must strive to make informed decisions about our health. It's imperative that we empower ourselves with knowledge through having a deep understanding of the risks, benefits, and alternatives in shaping our well-being. It is also essential to understand that the lack of trust in the medical community can be a significant barrier. Hence, we must be reminded that our health is our most valuable asset, and it's within our power to shape a future.   In this solo episode, we will learn about the critical importance of proactive healthcare engagement, particularly musculoskeletal health among men of color. Dr. Derrick Burgess will enlighten us on the common challenges and disparities that affect African Americans and how they can navigate these hurdles for better health outcomes. Relaxants come in many varieties. Today we come to two herbs who relax tension patterns in the body, yet are quite different from one another.Pedicularis densiflora, P. canadensis, and P. groenlandica are just a few of the “louseworts”, also sometimes known as wood betony. We ourselves usually mean Stachys officinalis when we say “betony”, and east of the Rockies that's usually how it goes. Both betonies release tension, though we think of Stachys as reaching the body via the mind, and Pedicularis as reaching the mind via the body. This is an herb you don't need to take in high doses to get a good effect; even a touch in smoke is palpable.Solomon's seal is Polygonatum biflorum or P. multiflorum, and in some contexts the species P. odoratum is similar enough. Just watch out for certain medicinal processing in some traditions, this can change the properties of the herb from its basic set of moistening, relaxant, and cooling. Sol'seal root is a good one to chew, or take in tincture; we do love it in water but reserve that for special occasions.Both of these herbs appear in our Musculoskeletal Health course as well as our Neurological & Emotional Health course. In this episode Bryan talks with NAON Member, Roz Puleo, ONP-C, FNP-BC, RCEP about her experience as a Nurse Practitioner working in an outpatient sports medicine setting. The number of Orthopaedic NPs in the area of Sports Medicine and overall musculoskeletal health continue to increase. Sports medicine should be looked at as a more comprehensive arc for a person leading an active lifestyle, not just about athletics. Orthopaedic care for a sports medicine issue goes beyond resuming the sporting activity and touching psychological or emotional wellness, return to work and even physical adjustments through rehabilitation. The role of orthopaedics keeps people moving, building on the foundation to overall wellness. Looking ahead to new opportunities in orthopaedics, we talk with Roz about NP sports medicine care in the outpatient setting. NPs have greater acceptance, more autonomy and overall better ownership of primary care. She shares experiences at her clinic that show less referrals out of the clinic to orthopaedics since implementing the NP role onsite, improved patient satisfaction and outcomes, increased comfort and trust, and improved patient education.

Studies have shown that the rates of joint replacement surgeries has increased steadily over the past decade. In Australia alone, there are about 110,000 joint replacements performed every year. This number is expected to rise with the aging population and increasing rates of obesity. Furthermore, COVID-related cancellations and hospital restrictions will likely impact the provision of joint replacements for years to come. Although joint replacement surgery is an effective treatment for end-stage osteoarthritis, the volume of joint replacement surgeries being places a huge burden on the surgical workforce and the healthcare system. Ilana Ackerman is a Professor (Research) in the School of Public Health and Preventive Medicine at Monash University and a Deputy Director of the Monash-Cabrini Department of Musculoskeletal Health and Clinical Epidemiology. Ilana is a musculoskeletal epidemiologist and an experienced orthopaedic physiotherapist. She completed her PhD at The University of Melbourne in 2006.   Alexa Chandler, M.S., CSCS,*D, is a doctoral student studying Exercise Science at the University of South Carolina. She earned her bachelor's degree with honors in Exercise Science from Ithaca College in 2016 and her master's degree in Kinesiology and Applied Physiology from Rutgers University in 2019. While at Rutgers, her research focused on the use of ultrasound to assess body composition in elite ballet dancers. She is currently using ultrasound to assess changes in muscle and tendon thickness following a resistance-training program with and without blood flow restriction. Her interests include exercise physiology, sports nutrition, and sports science.

We’re discussing the notion of amphoteric herbs today, because it’s the Spring equinox and we’re thinking about balance. The term amphoteric is orginally a chemistry term meaning “having characteristics of both an acid and a base”. Herbalists use the word to mean an herb that has a balancing activity, one that is capable of acting in ways that seem opposite, depending on the context in which it’s taken.But to understand how amphoteric herbs could be balancers in this way, first we need to investigate the concept of balance a little bit. What does it mean to find balance, or stay balanced? What does it mean that we’re all seeking balance? How can the experience of building physical balance skills teach us about finding mental & emotional balance? Sallie has a long-standing interest in clinical trials, medical statistics and, rehabilitation of musculoskeletal and chronic conditions in older people. She works with clinicians to develop pragmatic clinical trial designs to capture the effectiveness and cost-effectiveness of a variety of health interventions. She is the Chief Investigator for a number of trials of rehabilitation interventions.  Disease-modifying biological agents and other drug regimens have substantially improved control of disease activity and joint damage in people with rheumatoid arthritis of the hand. However, commensurate changes in function and quality of life are not always noted. Tailored hand exercises might provide additional improvements, but evidence is lacking. We estimated the effectiveness and cost-effectiveness of tailored hand exercises in addition to usual care during 12 months.  We have shown that a tailored hand exercise program is a worthwhile, low-cost intervention to provide as an adjunct to various drug regimens. Maximisation of the benefits of biological and DMARD regimens in terms of function, disability, and health-related quality of life should be an important treatment aim. Improvements in overall hand function were 36 points (95% CI 15 to 57) in the usual care group and 79 points (60 to 99) in the exercise group (mean difference between groups 43, 95% CI 15 to 71; p = 0.0028). Pain, drug regimens, and health-care resource use were stable for 12 months, with no difference between the groups.

Jill Cook is a Professor in Musculoskeletal Health in the La Trobe Sport and Exercise Medicine Research Centre at La Trobe University in Melbourne Australia. Jill’s research areas include sports medicine and tendon injury. After completing her PhD in 2000, she has investigated tendon pathology, treatment options and risk factors for tendon injury. Jill currently supplements her research by conducting a specialist tendon practice and by lecturing and presenting workshops both in Australia and overseas.   Dr. Steve Kamper comes onto the HET Podcast to share his perspective as a researcher in academia.  He discusses issues that exist in current physiotherapy literature and systemic constraints to publishing high quality research in academia. Steve is editor of the Journal of Orthopaedic and Sports Physial Therapy, associate editor in the Cochrane Back and Neck Group, Vice-chair of the Executive Orgainsing Committee of the International Back and Neck Pain Forum, and member of the Scientific Advisory Committee of the Australia New Zealand Musculoskeletal Clinicial Trials Network. Supporting and connecting early career researchers is an important part of Steve's professional life.  In 2011 he established the International Collaboration of Early Career Researchers (http://the icecream.org), he has provided input regarding ECR programming at International conferences, conducts mentoring and training, and gives lectures on academic skills such as writing and presenting. Research Interests: Dr Kamper's main interest is in musculoskeletal pain (especially back and neck pain) in adolescents and adults. In particular he is interested in how pain interacts with broader aspects of health such as physical activity, obesity, substance use, mental health and chronic diseases. The aim of this work is to move the field forward by combining perspectives from clinical management and population health. Other research interests include patient expectations and placebo effects, mechanisms of treatment effect, research methods, and outcome measurement. His track record includes publication of systematic reviews and meta-analyses, RCTs, cohort studies, clinimetric research, and Delphi surveys. Professor Alan Silman is an epidemiologist and a rheumatologist and is the co-author of 'Epidemiological Studies: A Practical Guide', which is the recommended textbook for the module 'Introduction to Study Design and Research Methods'. Alan Silman is currently Professor of Musculoskeletal Health at the University of Oxford. He was Director of the UK's Arthritis Research Epidemiology Unit in Manchester from 1988-2006, and then Medical Director of Arthritis Research UK, before moving to Oxford to take up his current position. He has published over 500 articles in the broad field of arthritis and musculoskeletal disease epidemiology. This talk was held as part of the Introduction to Study Design and Research Methods module, which is part of the MSc in Evidence-Based Health Care.

The number of people in their 30s walking around with osteoporosis and osteopenia is staggering. Why is this issue so largely overlooked? How can people start improving their musculoskeletal systems and bone density? Why is the insurance system within medicine a waste of time and a hamster wheel? On this episode, I am joined by Dr Jim Meehan, Dr Rob Kominiarek, Dr. John Crisler and Dr. Sean Breen continue with our biological systems series, tackling the musculoskeletal system today.   Three Takeaways Geroprotection is a better term to use instead of anti-aging. You can protect yourself from the effects of aging, but you can’t stop it.   The strength of our musculoskeletal system is dependent on microfractures, micro-tears and repairs and this is why movement and resistance training are so necessary. Medical boards have almost unlimited power. They are the end all be all with no supervision and oversight. That’s a huge part of the problem.   At the start of the show, we talked about the issues with the sick care model and the doctors who are getting taken down by the medical board for trying to step out of the system. We also talked about why you just can’t make a difference in the sick care model and if there’s anything we can do to help fellow physicians. We also talked about how to improve bone health through lifestyle changes.   We also discussed: Why anti-aging is a dangerous term to use How hormones can be used regulate a woman’s menstrual cycle How doctors are contributing to bone health issues   People are constantly trying to look for shortcuts to solve issues that can easily get fixed by simple lifestyle changes. When it comes to bone health, every conversation about the musculoskeletal system has to start with getting off the couch, moving and being active. Following from that, we have to cut out sugar and alcohol. All these things will affect the strengths of our musculoskeletal, and make more of a difference than any drug can.
