In this deep-dive episode of the Prolonged Field Care Podcast, Dennis sits down with trauma and critical care surgeon Dr. John McClellan ( University of North Carolina) to cut through the noise on tranexamic acid (TXA) in trauma.They cover the mechanism, who actually needs it, why the dosing shifted from 1g + drip to 2g upfront, pre-hospital decision-making when bleeding is controlled, redosing in ongoing hemorrhage, IM/IO options, seizure and hypotension concerns, the critical 3-hour window, and practical advice for the medic who is truly alone and afraid.Whether you're a combat medic, flight medic, or trauma provider, this conversation delivers actionable clarity on one of the most studied — and sometimes misunderstood — tools in hemorrhagic shock resuscitation.Key Takeaways:TXA is a lysine analog that reversibly (and at higher doses irreversibly) binds plasminogen, preventing its conversion to plasmin and stabilizing clots. It is one of the most evidence-backed hemorrhage adjuncts available.The ideal candidate is any patient you suspect will trigger (or has triggered) a massive transfusion protocol — not just obvious amputations. Err on the side of giving it early in pre-hospital/austere settings to avoid missing occult bleeding.Modern trauma practice favors 2g IV push upfront over the older CRASH-2 regimen of 1g bolus + 8-hour drip because traumatic bleeding is an acute event that needs rapid high plasma levels. The 8-hour drip was designed for elective surgical cases with ongoing bleeding over hours.Overall safety is excellent. The bigger practical risks are seizures with doses significantly above 2g and accidental double-dosing due to poor handoff between pre-hospital and hospital teams.Transient hypotension can occur with rapid push, but causality is murky — it is often impossible to separate from the patient's underlying shock state.Redosing is reasonable (another 1–2g) if significant re-bleeding causes hemodynamic instability. Roughly 25% of active TXA can be lost in major hemorrhage/transfusion models.Give TXA within 3 hours of injury for maximum benefit. After 3 hours efficacy drops sharply and some data suggest potential increased bleeding risk.For the solo medic: Preload if your protocol allows. Make TXA automatic once you have access (alongside calcium and blood products). Prioritize rapid transport. Meanwhile, the team takes a trip back to the Pendulum Room for a final farewell to the old studio.   Gamma Ray Bursts are some of the brightest bursts of high energy light in space, usually originating from the collapse of a massive star or two neutron stars colliding. Both of these processes create a black hole, which ejects very bright light into space. GRBs happen frequently, but far away from us!   Simon has begun his post-doc in Copenhagen where he will continue working on transient astronomy. Transient astronomy involves studying objects that vary in their nature. Simon talks us through the accidental discovery of GRBs, how we discovered that GRBs are from very distant galaxies. He also mentions that GRBs originate from collapsing stars as well as the formation of a black hole.   Simon's PhD work focused on trying to capture the optical afterglow of GRBs using the MeerLICHT telescope in Sutherland. He found that the light curves of the optical afterglows observed did not behave as expected. At that point, I've failed as an engineer - I've reached my limit and could not find a solution.Some time later, a solution appeared in my head from nowhere —what if the fan changed with the fire? Not in a crude on-off way, but by tracking temperature, exploiting density changes, and chasing constant mass flow instead of fixed volume.We unpack the moment this clicked, the fan physics behind it, and why hotter smoke can actually make extraction easier if you use the margin correctly. You'll hear how we oversized the fan, ran it at a lower frequency in ambient, then ramped as temperatures rose to keep kilograms per second steady. That adaptive control boosted cubic meters per second right when the layer needed support, eased plug-hole entrainment, and stabilised makeup air velocities. We walk through the thermodynamics, the electrical and pressure implications, and how these pieces form a practical control strategy for retrofits and new builds.To ground the idea, we share two paths to proof. First, CFD with user-defined control that reads gas temperature each time step and updates fan frequency with smoothed delays to prevent oscillations—capturing the real feedback loop between fire and system. Then, full-scale container burns with live control showed the same trends from 20 to over 500 degrees: falling duct pressures, lower fan power at heat, and the headroom to increase volumetric extraction without breaking limits. Thinking about it now, this idea is a part of many other concepts that I describe together. Just after the assassination of Charlie Kirk, host Mark Call did a show outlining the engineering concept of "transient effects," that happen whenever any system - including an entire society - is hit with a "shock to the system." The Truth is - we HAVE seen real martial law, the real weaponization of "intel agencies," and lawfare, and State-sanctioned terror and invasion. It's a bit late to whine about "tyranny," and "dictatorship," and a POTENTIAL police state. We've seen it. And the precedent has been set. The question is - can we survive the Transient necessary - if it's even possible! - to restore the Rule of Law and an actual Republic.
