Today, the practice of modern medicine has changed and is now far more intense, complex and demanding of physicians than ever before. It may not come as a surprise to find, therefore, that physicians are now burning out at an ever increasing rate. The issue is one that often remains hidden or unrecognized but has implications for the resilience and sustainability of our healthcare system. On todays episode we're talking about Physician wellness and burnout, and the inspirational work local physicians are engaging in to tackle this growing issue. Dr. Ingrid Cosio is a family physician who has been practicing in Prince George for over 18 years. In addition to being an award-winning family doctor and providing medical leadership to the Northern Gender Clinic, in Summer 2022, she successfully applied on behalf of the PGMSA for the Physician Peer Support Pilot Project, a Physician Health Program initiative. The Prince George Physician Peer Support Program was created as a way to combat burnout and support the mental well-being of other physicians. She also shares her own strategies for self-care such as early morning running and the drawing from stoic philosophy.

I find that all of us have a single emotion which is our fall back. I realize this sounds odd but hear me out. In any given day one can run through the gamut of emotions. I find I seem to always gravitate to one emotion: NOT that the particular emotion actually fits the scenario it just seems to be the comfort zone of emotion. for many many years that emotion was anger. It wasn't always appropriate. in fact, probably most of the time, 'anger' was not the emotion that fir the scenario. It seemed to be the first response to a lot. i now know it was triggered by pain. Living in pain 24/7 can be exhausting, and frustrating and can make you feel like a pot about to boil over at any moment. “When someone suffers from pain for an extended period, this can affect their mood negatively,” Dr. Cosio explains. “These mood changes can negatively impact their levels of activity. People begin to engage in more unhealthy behaviors and less in healthy ones.” Interestingly, a study Conducted by Dr Cosio's research team. in 2018 looked at the prevalence rates of different psychological domains among veterans with chronic pain, they found that women (88%) reported anger more often than men (72%.). They say anger can be a coping mechanism in response to pain. Chronic pain is actually proven to be worsened by anger. Anger releases cortisol, adrenaline and other chemicals that make things worse all round. When you Look for more... As I began to explore alternate modalities to deal with my pain, like meditation, visualization mindfulness and gratitude, I found myself less likely to become angry and had to learn to feel all the other emotions more freely, and allow them to flow and go. Did you know the lifespan of an emotion is about 90 seconds? ( attributed to Dr. Jill Bolte Taylor, a Harvard-trained neuroscientist who specialized in the anatomy of the brain.) Yet, we humans are so attached to the story.. we can drag the emotions out for ages, based on the stories we tell ourselves... I have seen a huge change over the years, where my first go-to was anger, now I find gratitude is my go-to. Finding the gratitude in all the little things seems to increase those things I can be grateful for, all around me. This is an incredibly important shift for me. Not only has it changed how and if I react, this change to mindfulness and gratitude has allowed me to significantly lower my pain medications. I am so grateful for this new lease on life. Being able to interact without the haze of heavy medication lets so much more joy into my day. I learned that all of it was my choice. Viktor Frankl wrote, “When we can no longer change a situation, we are challenged to change ourselves... Everything can be taken from a human but one thing: the last of the human freedoms—to choose one's attitude in any given set of circumstances, to choose one's own way.” in his book Man's Search for Meaning, Being present in all the moments you can, gives you a better appreciation for the little things. Mindfulness can show you a wealth of gifts both large and small for which to be thankful. And making the choice to improve yourself, is the greatest gift you can give to you. This is why I all ways make an effort to make the most of today, every day! And why I sign off my correspondence the same way!

EP31 Kiddo Cosio and the School of Unschooling I call him Captain Fantastic, a monicker from one of my favorite movies of all time of a father that defies convention and raises his kids in the wilderness giving them an education better that is better than what most of us will get going through traditional school. A self confessed drop out Kiddo's life has been anything but conventional. Growing up in a Christian family living abroad coming home and setting up one of the first specialty coffee shops, El Union, in La Union with his wife Amy in 2012. He is one of the most recognizable faces in Philippine specialty coffee and a community leader in the surfing town of San Juan La Union. His mantra is easily summed up as to live life with curiosity and an open mind and this reflects in the way he has ran El Union over the last 8 years and raised his children in what would be considered the most unconventional education, none of his kids have gone to a traditional school but they are very well educated they can surf, run a business, and quote from the myriad of books they read because like their father they have been thought to go through life with inquisitiveness and a drive to learn and digest the world around them. We talk about this Unschooling and how El Union has adapted to the pandemic which includes providing a standard living wage to everyone in their little company including themselves -a novel idea if a bit socialist but something we should all consider moving forward for a more humane economic system. Codella’s Rory Cosio interviews heard from two distinguished Latina women in STEM fields who were the keynote speakers of the 3rd Annual SHE TECH TALKS, Beatris Mendez Gandica, Program Manager at Microsoft, and Margaret Dominguez a TECNOLOchicas 3.0 and an Optical Engineer at NASA Goddard. Ms. Mendez Gandica and Ms. Dominguez engaged the students through interactive sessions focused on their personal journeys towards success in STEM, how to navigate their careers, securing mentorship and showed the girls what their jobs looked like.
People tend to think that having no pain is normal when in fact, having a little is. While that is the case, there are still some ways that can help us manage pain. Talking about effective strategies for pain management is Dr. David Cosio. He is a pain psychologist in the Pain Clinic and interdisciplinary pain program at the Jesse Brown VA Medical Center in Chicago. He earned his PhD from Ohio University with a specialization in Health Psychology. Starting with his journey, he shares how he got involved in health psychology and then into pain management. He explains the difference between a health psychologist and a clinical psychologist. Having both backgrounds, he gives a holistic look into pain—taking into consideration the environmental aspects as well. He gives us a peek into his book, Pain Relief: Managing Chronic Pain Through Traditional, Holistic, and Eastern practices, by talking about the concept of a fifth vital sign.
