Dr. Alan Sokoloff is the founder, owner, and clinic director of the ⁠Yalich Clinic Performance and Rehabilitation⁠ in Glen Burnie, MD, where he has practiced for over 39 years. He served 2 terms as President of the Maryland Chiropractic Association, was selected as Maryland's “Chiropractor of the Year 2001-2” and was the American Chiropractic Board of Sports Physician's Sports Chiropractor of the Year 2006. He holds a post-graduate degree as a Diplomat of the American Chiropractic Board of Sports Physicians and has been a member of the University of Maryland Sports Medicine Team since 1991. Dr. Sokoloff served a residency at the United States Olympic Training Center in the summer of 1997 and was selected to the medical team for the United States at the 1998 Goodwill Games. Dr. Sokoloff was appointed to the USA Olympic Medical Team for the 1999 Pan Am Games in Winnipeg, Canada. He is one of the team chiropractors for the two time Super Bowl Champion ⁠Baltimore Ravens⁠ and has been since 1999. He is currently one of the team chiropractors for the Baltimore Orioles and the Baltimore Orioles AA team in Bowie, MD, The ⁠Bowie Baysox⁠.  For over 10 years, Dr. Sokoloff hosted a weekly interactive health radio program in Baltimore called “Talkin' Back” on AM-680 WCBM. Dr. Sokoloff truly enjoys teaching, both live and on video. He is a recipient of the “Award of Excellence” for his DVD series including “Back Talk” (Preventing Back Injuries in the Work Place) and “Hands On” (Preventing Carpal Tunnel in the Work Place). One of Dr. Sokoloff's passions is in the area of injury and concussion prevention. In my opinion, we are in Phase 3 of the evolution of the Sports Chiropractor. There are many exciting aspects of this new generation of Sports Chiropractors, but also some concerns. In this episode, I interview Dr. Alan Sokoloff, a Sports Chiropractor who has been on the leading edge of the profession for many years and has built a great and sustaining practice in addition to his work with professional sports teams. We have a wide-ranging conversation that includes the evolution of the Sports Chiropractor, how to position yourself as an expert in your community, considerations when working with teams, and how to have longevity in this wonderful career. Dr. Sokoloff was selected as Maryland's “Chiropractor of the Year 2001-2” and the American Chiropractic Board of Sports Physician's Sports Chiropractor of the Year 2006. He holds a post-graduate degree as a Diplomat of the American Chiropractic Board of Sports Physicians. He has been a member of the University of Maryland Sports Medicine Team since 1991. Dr Eva Carneiro is this week's guest (I will give you Eva's introduction in two parts). Eva is a Sports Physician and was one of the first eight pioneering doctors recruited nationally to the UK Sports and Exercise Medicine Specialist Training Programme. She worked in the New South Wales Institute of Sport in Sydney and at the Olympic Medical Institute in London in the build up to the Beijing Olympics. Eva worked at Chelsea Football Club from 2009-2015. During her four seasons as First Team doctor the team was successful in winning Champions league, Premier league, Europa league, FA Cup and League Cup titles. During her six and half years with the team she worked with a total of seven elite international football managers. She is the first woman to sit on a team bench pitch-side in Champions League, Premier League, and Europa League competitions and the only woman to become the First Female Assistant Medical Director in a Football Club in the UK.  That's the introduction to Eva's medical and performance credentials. Here are some details about an incident that erupted in 2015. On the first game of the 2015-16 premier league season, Chelsea played Swansea. During the game, Eden Hazard twice called for medical attention following an impact to his abdomen. The medical team were then then summoned on the pitch by the ref on two separate occasions ….  Eva, along with club physio Jon Fearn came onto the pitch to attend following rules of the game. But the manager Jose Mourinho reacted to the situation, because he didn't feel the injury warranted attention. The reaction was pivotal to Eva's case, first there was a demotion from the first team. Footage emerged of Jose Mourinho used abusive phrases towards Eva, which he denied being sexist in motivation. The subsequent furore received extensive press coverage, both for the dismissal, the claim for abuse and the character and personal attention, scrutiny and sensationalism that Eva was exposed to. Eva's lawyers filed for constructive dismissal, but before she gave evidence the tribunal the case was settled on confidential terms. But importantly as part of the settlement, Chelsea issued a statement "We wish to place on record that in running onto the pitch Dr Carneiro was following both the rules of the game and fulfilling her responsibility to the players as a doctor, putting their safety first". This was important because that statement indicates the position that Eva upheld; upheld beyond the stage of the game or its results, the Hazard situation, beyond the clash with the manager and club, beyond football, beyond sport perhaps too - in not only serving her professional duties and duty of care, but in doing what is right and not capitulating to compromise. Eva's case received widespread media and public attention all sorts of accusations, pressures, intrusions into her personal and private life for, as I say, essentially doing her job, but perhaps the difference is that she also held her ground against all of those pressures.  If you've tuned in to the dirt being dealt around this case – you're in the wrong place, that's not what this podcast is about. However, if you want to hear from someone who has operated at the heart of one of the most successful football clubs in recent history, if you want to hear the piercing lessons from someone who has lived through unwarranted scrutiny and exposure but has held the utmost professionalism throughout and is now leading a campaign for higher standards of professionalism, ethics and governance – and ultimately is standing for doing what is right, you are in the right place. The fact that she has pioneered in a ‘male dominated environment' makes her achievements, perspectives and voice all the more profound.   Show notes The key responsibilities of a doctor in a football club – but feeling the pressures and being effective in an elite football environment Eva's dream as a 16 year old, buying an American College of Sports Medicine book about sports injuries on the beach Taking in the atmosphere. Memories of Chelsea playing Barcelona FC in the Champions league behind Fernando Torres and getting the nod from Lionel Messi Being part of the team – adding to the culture, team motivation and dynamics Being an optimistic realist How to weigh risk and judgement for the players, performance and the club when medical issues arise. Going on the journey with the players Football restarting. Systems not in place to respect medical governance and players. Non-medically staff can have a lot of power. You don't learn about what's required in 3 weeks. Huge credit to the medical staff for the immense work that's go in to ready players as best they can. Without the audit and review – competition has restarted, with enhanced protocols, but sustaining these protocols and within the culture of football could face problems. Footballers being the only people in the country going back to work without social distancing. Eva trained in accident and emergency to develop skills and understanding around trauma cases. It helped to be able to speak different languages to calm the players Keeping your mind at the pitch side Knowing your athlete is key Trauma and loss of life is a part of a medic's job, it's sobering and grounds your reference point. How did Eva cope through the ordeal? It's taken time to overcoming the emotions We need to be clearer about the non-negotiables We do need to understand that medical governance is about justice Football lives in a bubble at times A new generation of players and managers that can change the culture Allowing players to feel safe and to be able to speak – leads to better performance A new style of leadership must arise Safeguarding of athlete's medical treatment is central to Eva's cause. Nicole Kew: An elite athlete's story of rhabdomyolysis (commonly known as Rhabdo).

Episode 42 is dedicated to a particularly important topic that can affect some runners; Rhabdomyolysis, more commonly known as Rhabdo. It is a condition caused by damage to skeletal muscle. In sports, the muscle damage happens with excessively strenuous exercise, but it can also be caused by crushing injuries to the body, some medications, toxins, or infections. When the muscle is damaged, potentially toxic contents within the muscle cells (creatine phosphokinase or CPK, myoglobin, potassium, phosphate, uric acid) are released into the blood stream. In small amounts, the body is typically capable of filtering and clearing these toxins. If the injury is severe enough to cause a massive release of toxins, the body may be unable to adequately clear them. These toxins can cause irregular heartbeats, kidney damage, and, in severe cases, death. Signs and symptoms of rhabdomyolysis include: Classic Triad Muscle pain (myalgias) Muscle weakness Dark-coloured urine Other: Decreased urination Fever Malaise Nausea or Vomiting Confusion and/or agitation (delirium) My friend, Nicole Kew came down with Rahbdo following her participation in the 24 hour world championships in France in 2019. This is her story. What a scary story! It would be good for everyone to stay on top of their hydration and urine out-put. Nicole told me later that the doctors had said to her that if she had waited another 24 hours she would very likely have died. Scary stuff. The doctors haven't been able to pinpoint anything specific that caused her rhabdo. The Sports Physician called it 'a perfect storm' of small things that on their own were insignificant but combined may have all contributed. e.g. Nicole could have possibly picked up an infection/virus before the race, it was slightly warmer weather than expected, she may not have fully hydrated properly prerace, the hard surface and twisty/windy nature of the course could have taken its toll on her quads. Having a hard massage on the quads during the race probably sped up the muscle breakdown and release of the myoglobin into the blood stream (the muscle breakdown was already occurring and then rubbing the quads added to the trauma rather than relieving it), the flight home 2 days after the race would also have accelerated the dehydration process as she wasn't able to keep any fluids down and flights are dehydrating anyway. There wasn't any one thing that can be blamed. Other causes are if you have taken certain medications so they did blood tests in hospital to rule out NSAIDS as this is often a contributor but she hadn't taken any during the race so this wasn't a factor and the blood tests ruled out medications as an issue. It can also often be undertraining or over exertion, but Nicole was fully prepared, had ticked all the boxes in training. This was her 8th 24-hour event so her body and legs are used to the repetitive nature of a looped course. Nicole also had to have extensive testing with a Sports Cardiologist after her hospital admission to rule out any potential cardiac issues either pre or post race. So it's all a bit of a mystery but I guess it goes to show that you can do all the right things in training and during the race, follow the same nutrition and hydration plan as you have multiple times before and things can still go wrong. Dr Gooden is a fellowship trained Orthopaedic Surgeon who specialises in surgery of the hip and knee and Orthopaedic trauma. In this episode we talk about the swollen knee, a common condition that most young athletes will experience at some point.   A swollen knee can happen for a variety of reasons and is often managed fairly simply, however, sometimes it can be a more significant issue that requires further review and treatment.    Here’s what we discuss in this episode: Do swollen knees differ in presentation according to the age of the young athlete? Has there been some sort of event that has caused the knee to swell? Could this swelling be because of some other condition such as arthritis? What types of structures could be injured to cause the knee to swell? How soon after an injury where the knee swells should I seek treatment from a physio or Dr? What sorts of things can help parents or coaches to decide if you should be seen immediately or you can wait a few days? Should I get an x-ray, MRI or ultrasound of the knee? CT scans of the knee should be avoided unless the need for this is explained very clearly Swelling in the knee, is the knee’s cry for help – often this will settle We talk about management of the swollen knee and where parents and coaches are best placed to get help for their young athletes. A Physiotherapist or Sports Physician is often a good first port of call before seeing an orthopaedic surgeon Orthopaedic Surgeons are also able to be used as an expert review as part of a team Rare – but more serious knee issues that present as swollen knees When should you use painkillers or anti-inflammatories? The importance of managing the training loads of your young athlete How challenging is it to teach perspective to young athletes about making decisions on their injury for their long term health Encouraging young athletes to not push through injuries Does the management of the swollen knee change with the truly elite young athlete?
