Check out today's questions:    Snaz: Hi Dr. Cabral. I am located in Southern CA in an area impacted by fires. The air has been very ashy. When the farmers market reopened after the initial crisis had passed, I had gone shopping at the market and then I was washing my veggies and started to think about how this toxic ash may end up in our food being grown in the region. What if anything can or should we do to prevent the body from absorbing these additional toxins? I can remember "smoky" vintages of wine from previous fires. This obviously ends up in our food system through direct settling on the produce or absorption in the soil. Is there anything extra we can do to protect our health from this additional source of toxins? Or just all of the usual things you talk about? Thank you for your thoughts.                           Gemma: Has the Dr Cabral practice ever worked with Milky Oats (avena sativa)? I see that it is often used for anxiety (amongst other things such as alcohol or drug recovery) but I would like to understand if you have seen this work in practice and if its worthwhile as its quite an expensive herb. Pub med is hinting that it got good results but theres a lot of science in that report! It behaves differently from other herbs for anxiety as other herbs such as passionflower seem to make you sleepy and switch off your brain. I dont quite understand the science behind this one but you take it every day, no sleepiness, no withdrawal symptoms if you forget….seems like a wonder herb in theory. And apparently can also lower cholesterol as an added bonus! Why isn't half the population on this?                        Michael: Hi Dr. Cabral, Thank you for everything you do. My question is regarding cone beam CT scans at dental offices. What is your recommendation for having this imaging done? I'm 38 and had all 4 of my wisdom teeth pulled about 5 years ago. I've had a history of bad oral health that I've been working on. I've also had some intermittent jaw/facial symptoms on one side of my face for the last year or so. I recently started to see a holistic dentist and the CBCT is an option. My worry is the radiation with CT scans.                                                                                                                                                             Michael: Hi Dr. Cabral, I have a question regarding regular sauna use. I've read a lot about the benefits of multiple sauna sessions per week. I have used a dry sauna at a local gym regularly for a couple of years. I recently purchased a sauna blanket from Higher Dose for the convenience of not having to travel to the gym every day. I do sweat a lot using the sauna blanket especially when I use it after a workout. Is there more of a benefit from a sauna that you're inside of and breathing the warm air, or does the benefit simply come from warming the body to the point of sweating which is still provided by the sauna blanket? For somepeople there is NO permanent relief. However, what many people―even some in the medical profession―don't realize is that there are sometimes multiple factors involved. The purpose of this teaching is to present some IDEAS that may help in reducing pain significantly OR altogether. Our GOAL is YOUR complete DELIVERANCE, HEALING and FUTURE HEALTH. ________________________________________ HEALING FROM SHOULDER, ARM & HAND PAINOPTIONS FOR RELIEF The normal treatment sequence for shoulder pain is: as follows: 1. Physical Therapy; 2. Cortisone injections; and, 3. Surgery (after MRI analysis) if #1 and #2 above do NOT help. Notice that #1 and #2 above may sometimes be scheduled by a medical practitioner in reverse order OR simultaneously. THE SEVERITY OF CHRONIC PAIN If left unattended―or IF treatment is suspended due to NO relief―chronic pain can escalate, causing depression and anxiety, sleep deprivation, social isolation, and even economic and financial burden. ____________________________________________ Data from the National Center for Health Statistics finds that more than 1 in 10 adults 65 and older say they have pain that limits their life most days, or all the time! ____________________________________________ Pain management in the last 10 years has been disheartening. Efforts at treating chronic pain with medication resuted in opioid abuse; overdoses are now among the leading causes of death for adults age 50 to 70. In this teaching I want to share with you some non-drug options that may help you. PAIN CAN BE MORE THAN PHYSICAL Your body and your emotions both feel pain: especially chronic pain. You can experience emotional, social and psychological damage in addition to physical damage. FIND OUT WHAT IS DRIVING YOUR PAIN Sometimes it is easy to identify. For example house painters who have reached overhead for years may end up with severe shoulder pain. Or certain athletes who do repeated arm movements may exacerbate the situation. In addition to the “area of pain” there MAY be some other “culprits” contributing to the problem. Ask yourself these questions: When did my pain start? Do I feel other part(s) of my body that seem to be connected to the KEY area at times? What causes my pain to increase: sitting, standing, computer use, reclining (i.e., at night), lifting, eating, driving (steering wheel operation)? What makes my pain feel better? (Does it help for long?) Does my pain ever go away? You may find a “co-conspirator” or the actual cause … like I did! YOU CAN TRY THESE STEPS STEP ONE Schedule an appointment with an Orhopedic Surgeon and explain your problem. They may suggest Physical Therapy and/or Cortisone shots. NOTE: Cortisone injections should NOT be taken over three (3) times a year in a single joint [and no more than six (6) times a year in total if in different joints] preferably four (4) months apart IF needed, Any more can cause deterioration of the soft muscle tissue and is NOT advised. Getting too many cortisone shots can cause serious problems like damaged cartilage and bone death. It can also make it harder to manage other conditions such as diabetes. HOW CORTISONE INJECTIONS WORK The goal of a cortisone injection is to improve pain and inflammation. Cortisone does not treat the underlying condition; it only treats the symptoms. Cortisone injections are used to treat many joint problems, including: Bursitis Tendonitis (including shoulder and arm) Trigger finger (can aggravate OR cause shulder pain) Carpal tunnel syndrome Tennis elbow Knee arthritis Many types of overuse injuries NOTICE: You may find that the cortsone shot helps successfully after the first few days BUT in some cases it may start to “wear off” after a couple months; hopefully, it will last longer. Cortisone is a type of steroid hormone that works by decreasing inflammation. Inflammation is the body's normal response to an injury, infection, or disease as a means to heal itself. However, when a condition is persistent and the inflammation is chronic, the symptoms of inflammation—including pain and swelling—can become intolerable. Ultrasound can also be used to guide injections. Ulrasound isn't just a diagnostic tool. When some type of injection treatment is needed, such a corticosteroid shot into a joint, unltrasound can be used to visualize the structure to ensure that the needle is placed in the correct location. Image-guided joint injections also are done with X-Rays or CT Scans. But with ultrasound, there's NO radiation and NO need to inject a contrast dye. STEP TWO Your Physician OR Medical Assistant MAY recommend Physical Therapy. Physical Therapy usually is done two or three times a week. Most insurance plans cover it BUT check with you insurance before starting. You will probably find out IF it is relieving your pain or not after several treatments (if it is beneficial in reducing your pain). The same with the cortisone shots. You should findout immediately (within the first few days IF the shot is reducing your pain. STEP THREE See if other parts of your arm or hand at times feel pain that seems to connect up your arm to the shoulder. I found that my use of the computer mouse was causing some pain in my right hand near the wrist but between my thumb and right index finger. I went to an Orthopedic Surgeon and had an Ultra Sound of my right hand. Musculoskeletal ultrasound can help to diagnose a range of injuries and chronic conditions, including tendonitis, bursitis, carpal tunnel syndrome, rotator cuff tears, joint problems, and masses such as tumors or cysts. The ultrasound showed that the cushion between my right thumb and index finger had competely eroded. Years of texting plus hours a day operating keyboard and mouse on computers eroded the cushion between my right thumb and index finger. [I have several social media sites plus large websites and large podcast websites.] I got a shot of cortisone near my right thumb plus purchased a MetaGrip (photo below) produced by HealthLab which the doctor recommended. Immediately I noticed a difference. Then I started to remember―and recall―that in the past operating the mouse would send pain up my arm and to my shouder. I go to the gym two or three times a week plus lift at home and use strength bands plus cardio. I have NOT had a pain in weeks, even after working out at the gym with heavy weights. Only once did I sense some slight discomfort in one side of my neck due to heavy curling exercises with probably too heavy of weights but it went away. On my last visit to the Specialist (Orthopedic Surgeon who was giving me advice for the terrible pain in my shoulder) I asked him, “Is there anything I can take to relieve OR heal inflammation in my shoulder?” He said, “Yes, Tumeric.” I started taking Tumeric Curcumin right away. Immediately I noticed a profound affect: NO PAIN. MY TESTIMONY Check with your medical advisor and try Tumeric Curcumin Herbal Supplement and see IF it provides healthy imflammatory response. I have used it successfully. I experienced such terrible shoulder pain. It would keep me awake at night, at times causing me to cry. Even in the daytime, sometmes liftng a light weight small plastic cup of water would cause me such severe pain that I would cry out. Or when driving, just lifting my arm to turn the steering wheel slightly would cause me to cry out. After trying physical therapy and cortisone treaments I got serious about seeking God for the cause of my chronic pain. The physical therapy did NOT seem to help AND the cortisone shots were only helpting reduce pain for two months at the most. What was the problem? WHEN and HOW did it start? Then I got serious about seeking GOD for healing. Of course I had prayed before the shots and the physical therapy BUT figured I would jusr pray and go to a specialist. But when the pain continued to be chronic and affect my emotions AND my life style I then decided to get serious about asking God for help. Especially when I realized that this was NOT a little thing that was going away with normal treatments. I did NOT want to have it the rest of my life! Through prayer―and with several friends praying―I was healed permanently by God. It doesn't matter HOW God works … just so He works. One of God's Names is YHWH Rapha: the LORD who heals.” PRAY THIS PRAYER “Father in Heaven, I ask you to heal me of all pain. I don't care HOW you work, just so you work. I am really hurting and I need your help. Your name is YHWH Rapha: the LORD who heals. If you want me to use any physical means to assist my healing please direct me. I am listening to you. Lead me by your Holy Spirit. I ask you to forgive my sins. I ask Your Son, Yeshua―Jesus the Messiah―to save me and to take over my life. Please help me to live for you here on earth, and take me to Heaven when I die.” If you prayed the prayer above, God heard you. History of National Poetry Writing Month National Poetry Writing Month (also known as NaPoWriMo) is a creative writing project held annually in April in which participants attempt to write a poem each day for one month. NaPoWriMo coincides with National Poetry Month in the United States of America and Canada. This website is owned and operated by Maureen Thorson, a poet living in Washington, DC. Inspired by NaNoWriMo, National Novel Writing Month), she started writing a poem a day for the month of April back in 2003, posting the poems on her blog. When other people started writing poems for April and posting them on their own blogs, Maureen linked to them. After a few years, so many people were doing NaPoWriMo that Maureen decided to launch an independent website for the project. My History with National Poetry Writing Month I started writing poetry in 1988 after I had been exposed to T.S. Elliot in my honors English class in high school. In 1992 I started reading my poetry publicly at Espresso Europia Coffee Shop in Abilene Tx while I was in the United States Air Force. While living in Rockford Illinois I published my first book of poetry Throwing Yourself at the Ground and Missing in 2007 followed by Postcards From Someone You Don't Know in 2008 Wisdom From the Sack in 2010 and Shaving Crop Circles In My Chest Hair in 2017. You can get copies of all of these books in my merch section. In 2020 I started publishing my podcast version of the challenge and those can be viewed here for 2020 and here for 2021. What I am doing differently for Poetry Month For 2023 I am not going to be just writing poems in April, I will be writing poems all year round. Also, since I will be writing all year round, I am using a different source for my prompts. There is a blog called Think Written and you can find it here. I will also be performing these works at Katora Coffee House here in Fredericksburg Virginia on their Friday Night Snaps Open Mic Poetry reading. Some of the podcast episodes you will hear will be a live recording of me reading the poem to a live audience, other times I will be reading it in the comfort of my home studio. I do plan on posting my work to the NaPoWriMo site and interacting with the poets there and see if they want to read their work on this podcast. Todays Poetry Prompt Life Didn't Go As Planned: Write about a recent or memorable experience when nothing went according to plan.  Todays Poem The Last Time I Cried  14 March 2023    It wasn't too cold that day  My car was starting, sporadically  It was a Friday, and I was forgotten in the waiting room  The nurse had me type in my information  She was unable to hold a thought for more than five minutes  Then back to the waiting room of forgetting  The resident came in  Way younger than me  Way more handsome,   Probably was having a threesome that night  He wanted to get out of there  I wanted to get out of there  Friday at 4 PM, both of our minds were elsewhere  He brought in his teacher, the real doctor  His accent was probably not as thick as I perceived   Between the nurse, the resident and this doc  I am surprised any of them hadn't been sued or assaulted  And that's when they told me  Welcome to the world of MS  And they ushered me out  The receptionist had left an hour ago  So, the head nurse practitioner had to check me out  He was the only one with an ounce of kindness  I made it through the hospital in downtown Richmond  I had been here before  Had the ticking timebomb removed from my brain  And I remembered how shitty I was treated back then  And I wondered why I came back  Oh yeah, I'm a De Sade fan  The car didn't start.   I lit a cigarette  The car didn't start  I am not sure why I thought it would  I walked around the car  Smoking another cigarette  Underneath the sign that says don't  Just as I was about to call home  The car started  Driving up I95 on a Friday before Spring Break  There was a lot to time to contemplate  Making my way home  I said no, No, NO  5 years of frustration  5 years of MRI's MRA's, CT Scans  Spikes pressed 5 times in my back  Blood tests galore for every known disease  Finally understanding why so many things happened  Finally realizing that I was never lazy  Finally realizing that what was wrong had a name  It wasn't me all along  Making it through the front door  The children were asleep  The wife was on the couch  I knew we would be there for awhile  Talked till her yawns couldn't keep her eyes opened  And then we slept  Saturday with the children didn't cause the waterworks  It was when my wife got back from errands  That it all hit me  And my kids wonder why Papa doesn't cry  They've never seen it  And I know it's bad to think, but they may never  Even then I couldn't weep for myself in front of them  Retiring to our room  As she followed close by  I made it in, shut the door  And heard my dead mothers voice calling me selfish  And selfishly I cried  Reaching Out To The Podcast To reach out to me, email timothy@createartpodcast.com I would love to hear about your journey and what you are working on. Providing X-Rays, MRIs and CT Scans to Higher-Weight Patients

If you like what you are reading, please consider subscribing and/or sharing!Finding out that you need an x-ray, MRI, CT or other scan can be scary for people of all sizes, but fat* people can face a lot of additional and, unfortunately, well-founded fears about the experience. Here are some tips and tricks for people who offer these scans to make the experience better for higher-weight patients.I know that many people won't be able to do all of these things, what I'm asking is that you use any power/privilege/leverage you have to do what you can (and know that anything you can do WILL make a difference) and then consider finding allies who you can work with to push for larger changes.1.      Know what you are working withKnow everything you can about the equipment and facility. What is the weight-rating of the table/bed/chair? What is the bore size of the scanner? Are there any other size/weight limitations? If you don't have the most accommodating equipment that exists, proactively create a list of who does so that you can refer patients. Make sure all of that information is easy to access for everyone who might be patient-facing.2.      Know the people you are working withBe aware of all the patients you may work with. Sometimes when it comes to inclusion, identities can become siloed. So even if there are protocols for higher-weight patients and protocols for wheelchair users who need to transfer, there may not be protocols for higher-weight patients who are wheelchair users who need to transfer. Sometimes a lot of focus is put toward accommodating patients physically, but less focus is put on making sure patients, including neurodivergent patients, are psychologically comfortable. Consider bringing in educators and/or consultants to help you think through all the patients you might work with and proactively create protocols so that every patient will have a positive, seamless experience.3.      Communicate what you are working with to the people you are working withGive as much information as you can – including the weight ratings and bore sizes of your equipment, any accommodations you offer (for size, disability, neurodivergence etc.) be honest about who you can't accommodate (and offer referrals to places that can). Don't wait for patients to ask and don't make assumptions about who needs to know. Provide this information to every potential patient early, often, and through as many delivery methods as possible – on your website, when patients call, on their MyChart or other online patient record, in appointment reminder texts, literally in every possible way.Communicating about accessibility and accommodations helps people who need the access and accommodations AND it educates people who may currently have privilege that puts them in a situation where they don't know what they don't know. It can help them stop taking their access for granted, help them to be more educated for the people in their lives who may not typically be accommodated, and help them in the event that their needs change.4.      Create an accessible experience start-to-finishIt's not just about the equipment. Do you have armless chairs in your waiting room and in any treatment rooms? Do you have the largest possible gowns? If you take blood pressure do you have cuffs that work for all arm sizes? Don't take the patient's weight for no reason (ie: “routine weigh-in.”) If you truly need to get the patient's weight (for example, if you're using weight-based contrast medium dosing) instead of just asking their weight (which might be jarring/upsetting to them) explain it to the patient in a non-judgmental way. For example “The contrast is dosed by weight. We don't care about your weight except to get the dose right!” There is a size-inclusive healthcare office audit here.5.      Center the patient's experienceStory time! (Shared with permission, of course.) I was recently acting as an advocate at an orthopedic appointment that included an x-ray. Ortho appointments can be some of the most fraught for fat patients so the patient was nervous and I was on high alert. We had talked through scenarios and worked out a signal they would use if they wanted me to jump in (in a nod to The West Wing, they would tug their ear lobe.)Things had gone very well so far - great chair options in the waiting area, no fat-shaming, a positive response to declining weigh-in, and the tech knew exactly what the weight-rating of the x-ray table was (650lbs). The patient had limited mobility and was moving slowly (because they had a knee injury, which is why they were at the orthopedist!) They explained that they could walk from the exam room to the x-ray room at which point the tech said “great, you can stand for the x-ray.” They explained that, in fact, they couldn't do that so they would need to use the table. The tech sighed audibly. As the patient worked to get onto the table without using their injured dominant leg the tech continued to sigh. The patient said “I can tell I'm frustrating you” and the tech said, “this is just taking a LOT longer than normal.” Earlobe tug. I stepped in and cheerily said “Isn't it interesting how there isn't really a ‘normal' and different patients need different amounts of time!” Then I put my body between the the tech and the patient, turned to the patient and said calmly “take all the time you need.” (The rest of the appointment was outstanding. It may be in part that the patient's issue is genetic, but the ortho said that he didn't think surgery was warranted but never mentioned weight loss or suggested that surgery would not be an option for this super fat patient.)This is an example of a tech centering their own experience. I know that often techs are under time pressure and that sucks, but it's certainly not the patient's fault, and making the patient feel guilty and embarrassed for taking the time they need won't make things go any faster, but it may make the patient less likely to try to access care in the future. So even if you are feeling pressured or stressed try to center the patient's experience. Also try to put yourself and the patient on the same team – the two of you against unfair time constraints, rather than you against them.6.      Do The Right Thing When Things Go WrongSometimes things go wrong – it can happen despite the best intentions, and it can often be outside of your control. Maybe the patient was accidentally scheduled on the standard MRI instead of the wide-bore, or even though you tried to get information about the CT weight limit to them they didn't realize that it would fail to accommodate them. I suggest that you always start by apologizing and making it clear that it's not the patient's fault. Even if it's not your fault, apologizing and taking the blame (or at least shifting the blame from the patient) can help them avoid a shame spiral that will lead to them disengaging from care. Ask the patient what they want – do they want help solving this, do they just want to leave, do they want a list of referrals. Respect the patient's requests and support them to the best of your ability.As always, it's unlikely that you can change everything, but you can change something and what you change will make a real difference in the experiences and lives of your patients, any effort you put into this will be more than worth it. This past weekend I attended the 22nd Annual Conference of the Weston A. Price Foundation. Multiple exhibits stressed the harmful effects of Electromagnetic Radiation on Humans including today's guest, Justin Frandson. You can't hear, see, or smell them; electromagnetic fields. The earth emits low level electromagnetic fields. The earth, sun and ionosphere are all natural sources of EMF. However, the subject of this podcast episode isn't how to block EMFs; it is about learning to live with them. Although, manmade magnetic fields are causing a broad spectrum of dangerous radiation to human health. As mentioned in this podcast episode, full blocking (like sleeping in a Faraday bag) would also cut us off from the good energy emitted by the Earth. The increase in biological exposure to manmade EMFs is in the billions, so how can we learn to live with them with the least damage? There are folks who suggest that EMF exposure is not harmful; however, let's keep in mind that doctors denied cigarettes were harmful to public health in the beginning. Keep in mind it's easier to prevent than treat disease, or as I mention in the podcast episode, err on the side of caution without living in fear. Types of EMFs On average, there are four types of manmade EMFs in a home. Dirty Electricity - Itinerant frequencies that create unnatural electromagnetic environments like fluorescent and LED lighting.Radio Frequency/Microwave Radiation - WiFi and Bluetooth devices along with microwave ovens.Magnetic Fields - These occur when electricity is flowing. Frequently used appliances with large motors are typically the worst offenders. (Avoid having these large appliances share an adjoining wall with your bed as the closer you are to these magnetic fields, the stronger. The further you are, the more they fall off.Electric Fields - Emanate from all the wiring in your home. This includes extension cords and power cords. Considering the four types of manmade EMFs, exposure can come from many sources, such as: mobile phones, laptops, iPads, bluetooth headphonespower linesx-raystanning beds, microwave ovens, and other electric appliances The main topic of this podcast conversation will be concerning electronic devices which use WiFi and Bluetooth, however we touch on other types of EMFs briefly. Where Does EMF Exposure Come From? Families are exposed to EMFs at school and at home. Children are exposed to electromagnetic fields in many forms from baby monitors to iPads. In American schools, children are often able to use computers as early as preschool. France banned WiFi in elementary schools years ago. The governments of other countries such as Spain, Germany, and Russia have recommendations to ban WiFi from schools due to potential health challenges. EMFs influence metabolic processes in the human body and exert various biological effects on cells through a range of mechanisms. 1 EMF exposure intensity decreases as you move away from the object that's sending out waves. High-frequency EMFs are the ionizing type of radiation. Scientific literature agrees that large exposures can cause damage. Xray machines, CT Scans, and tanning beds are examples of this low levels of this type of radiation. (National Cancer Institute Fact Sheet.) Low- to mid-frequency EMFs are considered non-ionizing type of radiation. Examples include hair dryers, power lines, cellphones, and microwave ovens. EMF Exposure Symptoms The problem of health effects of RF EMF has not been definitively resolved, however, they suggest it appears necessary to use precautionary principles and ALARA (As Low as Reasonably Achievable) principles. it is recommended that the interpretation of results from studies on health effects of RF radiation should take sponsorship from the telecom or other industry into account. Use Robots and Ultrasound to Treat and Prevent Stroke

To effectively treat stroke and prevent stroke, you need to know just what is literally happening in a patient's head. CT Scans and MRI scans are tools most of us are familiar with. Generally if you suspect a stroke is possible, you need these two scans done. There's another tool out there, too, that's cheaper, more portable, and involves no radiation. It's called Transcranial Doppler Ultrasound. It's a great complement to the other scans, and it can provide impressive insight to supplement the information from the radiologists. A skilled practitioner is a great complement to the care team. I first talked about this technology in my conversation with Dr. Aaron Stayman a few years back (Is my Brain Pregnant? Ultrasound and Stroke: Transcranial Doppler Ultrasound). It's fascinating stuff. The problem is that it does require a skilled technician or someone specifically trained in the technique. Despite the work of advocates like Dr. Stayman and Dr. Mar Rubin, there just aren't enough of those techs. Dr. Mark Hamilton of Nova Signal has a solution. His robotic machine automates the whole process, makes it faster, more reliable, and cheaper while giving more accurate results in a lot of scenarios. Rubin is a vascular neurologist and associate professor of neurology with the University of Tennessee Health Science Center Department of Neurology. He specializes in vascular neurology and is experienced in stroke and cerebrovascular disease, and an experienced sonographer and expert interpreter of carotid duplex ultrasound and transcranial Doppler ultrasonography. He received his medical degree from University of Illinois College of Medicine and completed his Adult Neurology residency and fellowships (Neurohospitalist and Vascular Neurology) at the Mayo Clinic. Robert Hamilton, Ph.D. is the Chief Scientific Officer and Co-Founder of NovaSignal. He is an accomplished entrepreneur, engineer, and clinical researcher with a passion for innovative technologies that allow for increased access to care. Robert, a biomedical engineer by training, is an expert in image/signal processing and machine learning, with extensive experience in cerebral blood flow, traumatic brain injury, stroke, and other neurological disorders. TCD vs Traditional Ultrasound When most of us think of ultrasound in medicine, we think of the sonograms of developing children, where parents and doctors swear they can see a human being in those black and white lines. Personally, they seem more like those Magic Eye pictures from the 90s. Traditional ultrasound can also capture pictures of the heart, the blood vessels in the neck, and the condition of other organs in the body. It can be another way of getting a picture of the structures at play. And then you can frame those pictures or make them your Facebook profile picture. TCD is different. The goal is not to capture a picture of the structures of the brain. The goal instead is to understand blood flow through the brain. The technology helps practitioners understand the rate of flow, where there may be leaks or disruptions, if stuff is flowing with the blood that shouldn't, the direction of flow, and whether things are shunting (or crossing) between the veins and arteries that shouldn't. That shunting is at the core of the recent study Nova Signal was part of and that we talk about in this conversation. There are several reasons shunting can occur, but one of the most common is a PFO, or a hole in the heart. And it doesn't involve the dedicated rooms or radiation that might come with CT  or MRI Scans. While TCD is a great tool, it does take a while to get the scan just right, and we face a dearth of practitioners who are able to do it. The Nova Signal solution is simpler. It's small and automated. It doesn't take up much space in a medical facility and can easily be brought from one patient to another. Plus, an operator can perform a lot more scans per hour or get back to other tasks more quickly. With hospitals perpetually understaffed these days, that speed makes a lot of financial sense. And since, in stroke, time is brain, it can make a lot of medical sense, too. The Nova Signal device was significantly better at detecting the vein-artery shunts indicative of PFO and other conditions than the gold-standard  evaluation technique using Transthoracic Echocardiogram. This matters in stroke because when we talk about shunting in tests we're usually talking about very small bubbles. Outside of the test context we're usually talking about clots and other stuff sneaking across from veins to arteries, bypassing the body's filters. When they do that they can go to the brain and cause a stroke. Accurate detection of shunting allows physicians to make more appropriate treatment recommendations and decisions. Hack of the Week Cultivating a sense of gratitude, as unlikely as it may seem, is something that helps a lot of folks deal with the challenges pf post stroke life. Simply being alive means thing can get better. And being alive in the 21 century with the medical resources available is a huge asset to the community. Of course those medical resources are not evenly distributed within the US and around the world, but they exist, and that's a start. Develop a sense of purpose. After stroke, it can be hard to see a way forward, especially if you have severe disabilities. People who succeed in their recovery, or in their goals in general, typically have a pretty strong "Why?" driving them. Achieving their goals gets them closer to something important to them. In the dark times, when you find yourself feeling it's just not worth it, your why -- your sense of purpose can give you a reason to push through. We sometimes ask, "Why should I go on?" almost rhetorically to express or pain. For some folks, though, it can be an exercise to identify the things that matter to you. So ask yourself that question, but assume there is an answer. And write down everything that could be an answer. Cultivate the sense of purpose to get through the darkness. Dr. Dena Garner is a professor in the Department of Health and Human Performance, the Director of Undergraduate Research and the Assistant Provost for Research and Policy at The Citadel in Charleston, South Carolina. Dr. Garner is best known in fitness circles for her work with performance mouthpieces, including helping to develop the technology behind the AIRWAAV mouthpiece.   Dena started working at The Citadel in 2004 while completing her post-doctoral fellowship in the Department of Neurology at the Medical University of South Carolina, also located in Charleston. Before her move to Charleston, she worked at Oregon State University in Corvallis, Oregon where she also received her doctoral degree in exercise physiology. She received her Master's degree from the University of South Carolina in exercise physiology and an undergraduate degree from Furman University in Greenville, South Carolina.   Dr. Garner has been working in the area of mouthpiece use and effect on human performance since 2005 with studies that have focused on reaction time, lactate, and cortisol and the effect of mouthpiece use on these parameters. In addition, her research has focused on mouthpiece use during steady state exercise and effects on oxygen and carbon dioxide exchange as well as the proposed mechanisms for positive effects on airway dynamics in a healthy population.   In addition to research in the area of mouthpiece use and effect on human performance, Dr. Garner is involved in a research study which encompasses testing and evaluating physiologic neuro-assessment devices in healthy versus traumatic brain injury populations.
