For too long, getting adequate mental health care has been difficult. There's the stigma that, unfortunately, comes with saying you need help. And once you are ready to seek out care, where do you go? Who do you turn to? How can you get the mental health care you need in a timely fashion? Dr. David Goldbloom is a celebrated psychiatrist, professor and the senior medical advisor for Toronto's Centre for Addiction and Mental Health. In his new book, We Can Do Better: Urgent Innovations to Improve Mental Health Access and Care, Dr. Goldbloom suggests clear solutions to many of these problems. This week, he joins Medcan CEO Shaun Francis to discuss how we can reinvent our mental health care system and what we can do for the people in our own lives. INSIGHTS We all know that COVID-19 has been hard on our mental health — depression and anxiety, in particular, spiked earlier on in the pandemic. But Dr. Goldbloom, who also worked during SARS, says he has seen a difference with how we regard our mental health during crises: “There is greater awareness of the mental health implications when dealing with the spread of highly infectious, and even lethal, viruses through our community,” he says. “I err on the side of optimism, perhaps, in thinking that we're better attuned than we were.” Basically, the fact that we've been talking about our mental health during the pandemic is an improvement. [02:26] At this point, no one would blame you for being sick of only communicating with people over video chats, but virtual therapy can actually be highly effective. Dr. Goldbloom has used the technology to provide care to small communities in Northern Ontario for 20 years, and loves how convenient it is for people who may have difficulty accessing in-person therapy otherwise. There's also research to back up its usefulness: Dr. Goldbloom says it's likely you'll form just as great a connection with a psychiatrist virtually as you would in their office. That doesn't mean in-person therapy is going to disappear, but he does think digital alternatives should remain viable long after the pandemic is over. [04:47] “In Canada, our system of health care has been constructed around the idea that [it's] provided by doctors, and often in hospitals. In the reality of the 21st century, most health care is received outside of hospitals, and should be delivered by multiple disciplines—not just physicians—but it's hard for public funding to pivot as quickly as it needs to,” says Dr. Goldbloom. “That creates all kinds of bottlenecks and barriers for people who are seeking [care], so we've got a problem. When you say the system is broken, I'm not sure we could even describe it as a system. It's a loosely-woven fabric of different services and providers, and navigating it … is a pretty major challenge.” [15:45] So, if our “system” is in that much disrepair, what can be done? Dr. Goldbloom has a few innovative ideas. One is something that's already starting to be implemented around the world: Youth-focused community hubs. Ideally, these are places where adolescents can seek mental, and certain kinds of physical, health treatments in an easy environment. This will make accessing care much easier, especially as it will help them avoid unnecessary hospital visits. Another solution Dr. Goldbloom would like to see is the continued development of virtual-based cognitive behavioural therapy. By this, he doesn't mean more Zoom calls, but web-based tools where people can complete online assessments and receive a custom plan that is monitored by a licensed therapist. The goal behind innovations like this is to give people more flexibility in their treatment, and make progress at their own pace. [17:49] “We need to think of ways to expand the net of services that are covered,” says Dr. Goldbloom, pointing to the United Kingdom, which offers mental health care through the National Health Service, as an example of success. He also argues that the private sector can help, too. “When you treat people for mental health problems, you realize a return on investment,” he says, adding that Deloitte or PricewaterhouseCoopers have done studies that prove this. “It's good for the bottom line.” Employers can help by adapting their offered insurance programs, and by making changes in workplace policies and culture by adopting mental health care standards. [24:49] And if you have someone in your own life who may need help? You're not going to want to do a huge, Hollywood-style intervention, first of all. Instead, start by figuring out who they have the best connection to in their own life—maybe a parent, a friend, even a close colleague. “That person has to be prepared to be a bit of a pest,” says Dr. Goldbloom, as people who are struggling often tend to fend others off. This person has to be ready to stick with the process and make sure the other person knows you're not going anywhere. What they need is the time and space to feel heard. [29:23]

Microaggression is a term used for brief and commonplace daily verbal or behavioral indignities, whether intentional or unintentional, that communicate hostile, derogatory, or negative attitudes toward stigmatized or culturally marginalized groups. The fact is that you have likely been on both the giving and receiving end of microaggressions. When people speak of microaggressions most often they are talking about the subtle acts of exclusion that black people deal with on a day-to-day basis. The root of these acts feels like just another way that black people are subjected to racism. However, microaggressions aren't exclusive to any one race, gender, ethnicity, religion, or any other identity. This is a big, awkward, contentious and fiery subject. What we're talking about in this Quackcast is the current phenomenon of social marketing and how it fuels and drives the “culture war”, or “pop-culture war” to be more correct. Word-of-mouth and viral marketing are old hat now, Social Marketing is in. But what is it? Basically its word-of-mouth and viral marketing smashed together and weaponised: Marketing companies hijack hot-button social issues and hitch their client's brand to them in clever campaigns (“We can be better”, etc). The purpose isn't really to make a brand seem progressive, modern or new, rather it's another way of getting it trending on social media that's guaranteed to work, unlike the legion of hit or miss but mostly failed “Viral” campaigns. Whether people say negative or positive things about this issue is irrelevant to the marketer, as long as people are talking about the brand is all that matters. Free advertising is the goal, but it has a social cost. This has led to issues because the so called “culture wars” divide people in artificial ways. The way the social issues are presented in these campaigns is often overly simplified and so can sometimes be harmful to the issue itself. Fallout over community discussion can bring unwanted attention to the brand… People are random factors and can run with things in unexpected ways: actors can go off-message, people can get together and organise campaigns to review bomb things or even hound actors off-of social media, studios can get nervous and throw their weight around using their advertising budget (the threat of withholding it) as a bludgeon to kill websites… So much for the theory, what are examples of social marketing? A recent famous example was the “We can be better” video done for the razor company Gillette. The message was didactic and simplistically constructed, but what you'd expect from a marketing company. It was an enormous success though: it was fuel to the fire of the “culture-wars” and massively increased the profile of the Gillette brand. The campaign was trending on all social media, videos were made about it, blog posts, comments, rants, news articles and so on. The Gillette got far more than their money's worth with what they paid their marketing company. The social marketing of recent movies, the Ghostbusters reboot and the new Captain Marvel film caused problems (not the films themselves). Social marketers wanted us to think they were socially progressive and politically aware, promoting female empowerment, when in reality they're simply big budget, well-produced mainstream entertainment, the same as any other. The social marketing added to their profiles but also caused them to become pawns in the culture wars: by advertising love or hate of them people signal allegiance to a raft of other social issues and positions. Another early, but good example is the famous “Fearless Girl” This is a bronze sculpture that was commissioned by an advertising company, its purpose was to market a new female focussed investment fund on Wall street. As an aesthetic piece of art it's awful, looking like a Disney figurine; pure ugly kitsch. It also unfairly re-contextualised the older Charging Bull statue which it was put in front of. It looked like the girl was bravely facing it down which made the bull an evil villain. The bull sculpture had been created by an artist to celebrate the vitality of and dynamism of American business culture while this new sculpture was essentially fake art, a marketing gimmick created by an advertising company. However the unintentional result was that the sculpture became an icon of female empowerment, far beyond the small scope of the investment fund it was promoting. Bad, false art that it was, it actually became a successful “Good” art piece because it resonated and communicated so well with people everywhere, albeit with a far different meaning than was originally intended. It's now world famous. The problem with social marketing Companies ARE made up of individuals and they can support whatever ideals they like, as they should. This can genuinely be reflected in the products of those companies, this is perfectly ok. The problem comes when marketers insert issues as an artificial layer as part of a marketing campaign. Progressive issues are too important to be hijacked by marketers. The world is being divided more and more and the power of the individual is getting smaller, social marketing campaigns usually only exacerbate the problem. They hijack left-wing progressive issues and attempt to manipulate masses of people. They are not a genuine contribution to the cultural discussion, they aren't sincere, they are not organic, and they have more resources and a bigger voice so that they cause imbalance and contention. The web used to be a massive, chaotic, democratic morass, now it is dived amongst a much smaller number of media companies: Google, Apple, Facebook, Twitter etc, so that it's very easy for corporations to manipulate and attempt to control social sentiment while also shutting down the voice of the community. How can we combat social marketing? Calling out social marketing for what it is, that's a start: Recognise when something is attempting to manipulate you and instead of engaging with it (“This movie will be amazing because it's about this social issue!” or “This film is horrible because it's about this social issue!”) say what it's really doing: “This film is no more an advocate for that social issue than any other, I will see it because I like the content. The marketing campaign attempting to promote it as socially aware is false and manipulative”. But simply ignoring fake controversies the same way we do with viral ad campaigns is probably best. Progressive people should not be fooled into thinking a corporate entity properly represents issues and nor should those who are against them. No one should be tricked into doing the work of a marketing company, you are not being paid and they don't deserve the free help. We're on the verge of the most wonderful time of the year -- spooky season is almost upon us! With that said, we tackle a very serious topic in Episode 67: What is horror? Seriously, since everyone is debating whether "It" is a horror film, we dive into the meaning of the word and what types of horror entertainment we really find scary. Turns out Dan Aykroyd from "The Twilight Zone Movie" scarred Jack and me as children. Who'd have thought? NEWS: 8.6 million viewers tuned into Seth McFarlane’s “The Orville” on Sunday. AMC renewed "Ride with Norman Reedus" for a 3rd season before the Season 2’s Nov. 5 premiere. George R.R. Martin's "Nightflyers" has been picked up by Syfy. The series is “set in the future on the eve of Earth's destruction and follows a crew of explorers who journey on the most advanced ship in the galaxy, the Nightflyer, to intercept a mysterious alien spacecraft that might hold the key to their survival. As the crew nears their destination, they discover that the ship’s artificial intelligence and never-seen captain may be steering them into deadly and unspeakable horrors deep in the dark reaches of space.” Netflix is keeping fans in the dark regarding the premiere date for “The Punisher.” Instead of being exact, a recent press release promoting the series had the premiere date redacted from the email. Marvel will bring the show to NY Comic Con in October, so it’ll probably hit the streaming service soon after. HISTORY has ordered a sixth season of “Vikings.” The 20-episode season will begin shooting this fall. Season 5 will have a two-hour premiere on Wednesday, Nov. 29. Olivia de Havilland’s lawsuit against FX and Ryan Murphy for her depiction in “Feud” has moved forward. The defamation lawsuit will begin on Nov. 27. Phil Lord and Chris Miller have moved back to TV after being fired from the Han Solo movie. They are developing a series at ABC titled “We Can Do Better.” It’s about a “newly ‘woke’ soccer mom who must navigate life in the south as a parent, wife, American citizen, and daughter of hardcore conservative parents.” The pilot script was written by “Community” and “Last Man on Earth” alum, Liz Cackowski. With the 69th Annual Primetime Emmy Awards airing this weekend, we dig into some of our predictions. Who do you think should win? Tonight's SPECIAL GUEST is Kimberly Day of Every Child Matters, a coalition that seeks to end child abuse. Centered in Washington, DC, Kim coordinates for the Protect Our Kids Act & Petition, now before the House and Senate, which seeks to create a National Commission on Child Abuse Fatalities. 2,500 American children are killed every year as a result of child abuse and 80 percent of those killed are under the age of four. Almost half are under age one. Ms Day says, "All it takes is a minute of your time to click here and sign the petition asking your member of congress to support this act, then send a letter to your Congressmembers." Much more must be done. The rate of child abuse fatalities continues to rise. An overhaul of the nation's child protection system is long overdue.
