Attention radiologists: Were you trained to look for Tarlov cysts when reading spine MRI? In this episode of Backtable MSK, interventional neuroradiologist Dr. Kieran Murphy joins the studio to discuss the serious issue of chronic pain related to Tarlov cysts, a condition often overlooked in both diagnosis and treatment. Tarlov cysts, also known as perineural cysts, are fluid-filled sacs that form due to the dilation of the subarachnoid space around spinal nerve roots, most commonly at the base of the spine. --- SYNPOSIS Dr. Murphy highlights the high risk of depression and suicide among patients, who are often misdiagnosed and overprescribed ineffective pain medications. He explains how to identify and treat Tarlov cysts through aspiration and fibrin sealing, addresses the historical dismissal of their significance, and underscores the need for a better understanding of CSF leaks. Additionally, Dr. Murphy emphasizes the importance of institutional responsibility in occupational radiation safety for interventionalists, advocating for improved lead protection and antioxidant use to mitigate radiation damage. The episode concludes with a humbling reminder of the implicit biases present in medical practice and the ongoing need for more inclusive and attentive patient care. --- TIMESTAMPS 00:00 - Introduction 01:24 - Challenges in Diagnosing and Treating Tarlov Cysts 04:05 - Patient Experiences and Misdiagnoses 06:06 - Cyst Aspiration Techniques 15:12 - Improved Diagnosis of CSF Abnormalities 22:10 - Allergic Reactions to Fibrin 25:30 - Implicit Bias in Medicine 34:50 - Radiation Safety in Radiology 43:47 - Final Thoughts and Reflections

In this episode, Dr. Mariam Hanna welcomes Dr. Edwin Kim, Division Chief of Pediatric Allergy and Immunology at the University of North Carolina, to dive into the science, practicality, and potential of SLIT. With a reputation for safety and ease of use, SLIT is gaining traction—but as Dr. Kim emphasizes, the evidence for other foods like milk, egg, and tree nuts remains extremely limited. On this episode:What is SLIT? A low-dose therapy held under the tongue, offering bite-proof protection with minimal side effects like mouth itch, making it a low-risk option for many patients. Why choose SLIT? Why some patients prefer SLIT's simplicity, including fewer restrictions on exercise and observation, compared to oral immunotherapy. Why peanuts? SLIT shows strong results for peanuts, but research on expanding to milk, egg, and tree nuts is still in its early stages, with unique challenges for each food.Who benefits most? Younger patients who thrive on routine and those seeking protection from accidental exposures without needing to eat large quantities of allergens. What lies ahead? Optimizing dosing for different foods, improving outcomes for patients with low thresholds, and expanding access to non-peanut allergens.Whether you're curious about incorporating SLIT into clinical practice or exploring its limits, this episode delivers evidence-based insights. FDA approves first nasal spray to treat dangerous allergic reactions

U.S. health officials approved a nasal spray to treat severe allergic reactions, the first needle-free alternative to shots like EpiPen. The Food and Drug Administration (FDA) said it approved the spray from drugmaker ARS Pharmaceuticals Inc. as an emergency treatment for adults and older children experiencing life-threatening allergic reactions known as anaphylaxis. Anaphylaxis occurs when the body's immune system develops a sudden, unexpected reaction to a foreign substance, such as food, insect stings or medications. Common symptoms include hives, swelling, itching, vomiting and difficulty breathing. The device, marketed as Neffy, could upend treatment for the 33 million to 45 million Americans with severe allergies to food and other triggers. Anaphylaxis sends more than 30,000 people to emergency rooms and results in more than 2,000 hospitalizations and more than 230 deaths in the U.S. each year. Of the 6 million prescriptions written for auto-injectors each year, more than 40% are never filled, Dr. Thomas Casale, an allergist at the University of South Florida, told an FDA advisory panel last year. Even when they are available to caregivers, many auto-injectors are used incorrectly, he said. "There's a real unmet medical need for a large portion of the population," he said. Neffy is intended for people who weigh at least 66 pounds. It is given in a single dose sprayed into one nostril. A second dose can be given if the person's symptoms don't improve. The new treatment could be life-changing for people with severe food allergies, said Dr. Kelly Cleary, a pediatrician and director at the Food Allergy Research & Education, a nonprofit advocacy group. Requiring an injection in an emergency is as scary to some children as the allergic reaction itself. Some parents have had to restrain thrashing children to inject them, sometimes causing cuts that require stitches. About 3,500 caregivers a year are injured when they accidentally inject themselves in the hands, ARS said. Neffy is designed to be easy to carry and easy to use, especially for children, said Richard Lowenthal, president and chief executive of San Diego-based ARS. "We don't want fear. There's no needle, there's no pain with this product," he said. "It's basically like spraying saline into your nose." This article was provided by The Associated Press.

Thank you for joining us for our 2nd Cabral HouseCall of the weekend! I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Marina: Hi Dr Cabral, There is so much confusing information out there about the foods to eat. Eat meat, don't eat meat it causes health issues, eat more plants, don't eat plants they are trying to kill you! I listen to vegetarian community and they make good arguments, I listen to carnivores and they make good arguments. What do we do?! Is golden middle the best? Some say the more variety of foods the better, others say there is no evidence. All you need is meat and fermented foods. Please share your thoughts.   Tonya: Hi Dr. Cabral. A few months ago my eye was swollen and then it was like I had a bump that moved in my upper eyelid and then it turned to a bruise and disappeared. A couple weeks later my other eye did this. And then again only this time it never bruised and the bump never went away and now I have 3 bumps in my left eyelid. They do not hurt at all. What could this possibly be from and how can I get rid of it? It has been a couple months now. Thank you   Page: Dear Dr. Cabral, My husband just experienced a severe allergic reaction to a fire ant bit that sent him into anaphylactic shock. While we were in the ER, I kept wondering if there was an underlying cause spurred this response. He has been bit before and never reacted in this way. We were told that subsequent bites/stings could lead to a more intense reaction. Is there anything we can do to reduce the body's response outside of carrying the epipen and being equipped with Benadryl? Thank you for your guidance.   Kavita: I just want to share that I am obsessed with your research and knowledge and listen to your podcasts. My daughter is 22 and she has been suffering from pain in her throat for the past 5-6 months. The doctors are unable to diagnose the issue. She has already been on antibiotics twice, and the pain goes away for a while but comes right back The ENT specialist told her that her tonsils might be inflamed and gave her antibiotics last month, which took care of the issue for a bit, but her pain is back again!!! She is so frustrated. She had asthma as a child and takes her maintenance medication for it daily, singular (5mg), and is on Allegra for her allergies. I am wondering whether these can be stopped as she has not had any episodes of asthma for a while. Any help will be appreciated.   Anonymous: Hi Dr. Cabral! As an IHP2 it is very hard dealing with our own personal health issues and feeling like there are no answers. I have been getting a blister like rash on different fingers for the last year. There was a few month gap where it did not happen. I cannot pinpoint a trigger. I did the CBO last year as an attempt to resolve it. I have researched and it could possibly be Dyshidrotic Eczema but I also get nerve pain with it. I also have HSVII and wondering if this is just moving throughout my body. I cannot afford lab work right now. I have done a HM detox + parasite detox + CBO and I do FM detoxes. I have done the entire virus protocol more than once, prior to having this rash. My main concern is what damage is this doing to my body if it is viral? What would you do to resolve?   Thank you for tuning into this weekend's Cabral HouseCalls and be sure to check back tomorrow for our Mindset & Motivation Monday show to get your week started off right! - - - Show Notes and Resources: StephenCabral.com/2865 - - - Get a FREE Copy of Dr. Cabral's Book: The Rain Barrel Effect - - - Join the Community & Get Your Questions Answered: CabralSupportGroup.com - - - Dr. Cabral's Most Popular At-Home Lab Tests: > Complete Minerals & Metals Test (Test for mineral imbalances & heavy metal toxicity) - - - > Complete Candida, Metabolic & Vitamins Test (Test for 75 biomarkers including yeast & bacterial gut overgrowth, as well as vitamin levels) - - - > Complete Stress, Mood & Metabolism Test (Discover your complete thyroid, adrenal, hormone, vitamin D & insulin levels) - - - > Complete Food Sensitivity Test (Find out your hidden food sensitivities) - - - > Complete Omega-3 & Inflammation Test (Discover your levels of inflammation related to your omega-6 to omega-3 levels) - - - Get Your Question Answered On An Upcoming HouseCall: StephenCabral.com/askcabral - - - Would You Take 30 Seconds To Rate & Review The Cabral Concept?
