      In S 5 E 11 I am delighted welcome Dr Bethan Richards to the podcast.   Dr Richards is a Staff Specialist Rheumatologist at Royal Prince Alfred Hospital, Sydney and Deputy Director of the Institute for Musculoskeletal Health. In 2019, following her return from Stanford, Bethan had the honour of being appointed Chief Medical Wellness Officer in Sydney Local Health District – an Australian first. She is also the District's inaugural MDOK Centre Director.            Throughout her career, Dr Richards has had extensive experience and a passion for designing and implementing mentoring, teaching, leadership and wellbeing programs. Following a successful pilot in junior medical staff, her evidence based BPTOK wellbeing program is now being rolled out to over 14,000 staff in Sydney Local Health District as MyDistrictOK (MDOK). Dr Richards is a national thought leader and researcher in the field of workplace wellbeing and has advised and mentored local, national and international organisational leaders on how to improve healthcare staff burnout and promote professional fulfilment.        This is a conversation I have wanted to have on the podcast for some time but feel we have reached a fitting and opportune juncture to have now.       It is hard to go first. This podcast celebrates the trailblazers and change makers in healthcare, business academia, sport and beyond. This week we had such an interesting conversation with Dr. Erica Barnell. Dr. Barnell is the Co-Founder and Chief Medical and Science officer for Geneoscopy, Inc! Geneoscopy is a life-sciences start-up company that leverages eukaryotic biomarkers to non-invasively diagnose, monitor, and treat gastrointestinal disease. The idea for Geneoscopy was conceived during Dr. Barnell's first clinical rotation while earning her MD/Ph.D. at Washington University School of Medicine in St. Louis. Specifically, Erica encountered a woman with Stage IV colorectal cancer (CRC) who had never undergone a colonoscopy, citing the inconvenience and burden of attending a traditional colonoscopy as key barriers. Dr. Barnell developed an easy non-invasive stool test to screen for colon cancer and she is now expanding it to the IBD space. She and her team are working to be able to use this RNA screening tool to allow for better prediction of treatment outcomes, disease monitoring and more. We talked to her about developing her initial test, going through the FDA approval process, and then developing a company while still finishing her MD/PhD. We also talked to her about starting a company with her brother and all of the many life changes she went through during it's initial stage as well. We discussed RNA technology and she gave us a quick lesson on it, and finally we discussed her participation in the Crohn's & Colitis Foundation's IBD Innovate Conference earlier this year and how it helped her premier her product to the IBD space and to explore possible partnerships and funding opportunities. Robin and I are big geeks for research and innovation and this conversation with Erica was so fun. In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Dr. Robert Coleman to discuss overall survival of SORAYA trial. Dr. Coleman completed his Obstetrics & Gynecology residency at Northwestern University Medical Center in Chicago, Illinois, and completed his fellowship at The University of Texas MD Anderson Cancer Center in 1993. From 1993-1996, he served as Assistant Professor at Creighton University followed by service as Vice-Chairman, Department of Obstetrics and Gynecology at the University of Texas, Southwestern Medical Center. Dr. Coleman joined as Faculty at MD Anderson Cancer center in 2004 and served as Professor and Executive Director for Cancer Network Research, holding the Ann Rife Cox Chair in Gynecology. In March 2020, he joined US Oncology Research (USOR) as Chief Scientific Officer and served briefly as Chief Medical Officer for Sarah Cannon Research Institute (SCRI). He currently serves as Chief Medical officer for Vaniam Group. Dr. Coleman has authored or coauthored over 750 scientific publications, including over 450 peer-reviewed articles, along with numerous book chapters, monographs, invited articles, and textbooks. He was the 2019 APGO-CREOG awardee for Excellence in Teaching. He serves as Vice President of GOG-Foundation, Inc. He served as President of SGO (2015-2016) and is the immediate Past-President of IGCS. He was recently inducted into MJH Life Sciences™ 2020 class of “Giants in Cancer Care®.”   Highlights: Mirvetuximab soravtansine (MIRV) is a first-in-class antibody-drug conjugate comprising an  FRα-binding antibody, cleavable linker, and maytansinoid DM4, a potent tubulin-targeting agent Early clinical data suggested efficacy in recurrent platinum-resistant ovarian cancer (PROC) prompting a larger trial to confirm both safety and efficacy in patients with PROC In this SORAYA trial MIRV demonstrated clinically meaningful antitumor activity in patients with FRα-high platinum-resistant ovarian cancer with an ORR: 32.4% and a median DOR: 6.9 months Remarkably the median OS was 15 months; 37% patients alive at 24 months Efficacy of MIRV was further evaluated with respect to when it was administered (as first treatment for PROC) and in patients receiving prior bevacizumab.  An ORR of 34.8% was documented in the formers, and 31.5% in the latter. The ocular toxicity was new for many treating physicians when the drug first became available. However with mitigating strategies as used in the study the events were predictable, low-grade, and rarely (n=1 patient) led to treatment discontinuation Now with MIRASOL confirming these data and demonstrating efficacy over standard of care for response, PFS, and unprecedently, OS, the agent is a staple of contemporary management.

"Task overload", or the related concepts of "information overload" and "task saturation", is a term developed to describe what happens when there is simply too much for our brains to do--a common occurrence in airline emergencies, spacecraft emergencies, and, unfortunately, everyday work loads in our modern medical systems. The consequences of task saturation are well understood,  lead to very poor decision making and leave very little room for interpersonal engagement, as the overwhelmed brain looks inwards for answers and shuts out further external engagement. Fountain Life claims their data-driven approach enables them to find cancer, cardiac, metabolic, and neurodegenerative disease early, before they can cause harm. Dr. Messier received a Bachelor of Science and a PhD in Molecular Immunology from the University of Alberta. She completed post-doctoral studies at the La Jolla Institute for Allergy and Immunology in San Diego, CA and received her MD from the University of Calgary. She's board certified in family medicine. In addition to leading the company's clinical and scientific programs, Dr. Messier will establish Fountain Life University to train Fountain Life certified practitioners and lead the effort to gain insights and publish findings from Fountain Life's vast amount of data. In addition to being related to the Hall of Fame hockey player Mark Messier, she found out on this episode that she's about to be related to the McCaffreys.
