“Ten years ago, there were relatively few treatment options [for bladder cancer] compared to what we have today which is a relative wealth of options,” said Debasish Sundi, MD, a James urologist and bladder cancer specialist. In this episode, Dr. Sundi explained the function of the bladder (storing urine), the different types of bladder cancer (contained within the bladder and metastatic), how new immunotherapies have led to better options and outcomes, and his research which focuses on identifying the biomarkers in a patient's bladder cancer by analyzing their urine, instead of a more invasive procedure using a scope. There are about 80,000 new cases of bladder cancer diagnosed ever year in the United States and “about 75 to 80 percent are in males,” Dr. Sundi said, adding “the number one cause is exposure to tobacco smoke and tobacco products.” The primary symptom is blood in the urine. “If you see blood in your urine, even if it's just pink, it is worthwhile to talk to your primary care doctor or see a urologist.” Clinical trials have led to the development and approval of several new immunotherapy treatments. “The challenge is we do not have any good biomarkers to tell us how our patients will respond,” Dr. Sundi said. “My lab is developing an assay [test] to non-invasively make an assessment. We've learned that if we look at the immune cells in the urine of someone with bladder cancer, they are similar to the immune cells in their tumor … And this could lead to significantly improving the therapeutic options and helping doctors pick the best medicine for their patients from the start.” Dr. Sundi said his research is motivated by his patients. “When I started in this field the experiences of patients with bladder cancer was something we could and should improve,” he said. “Working toward this is the fuel that is self-sustaining and there is so much excitement in terms of the innovation happening in the bladder-cancer field.”

New Ultra-Sensitive DNA Blood Test for Detecting Residual Cancer in B-cell Lymphoma Patients

BUFFALO, NY – May 19, 2025 – A new #research paper was #published in Volume 16 of Oncotarget on May 9, 2025, titled “Analytical validation of a circulating tumor DNA assay using PhasED-Seq technology for detecting residual disease in B-cell malignancies.” In this study, a team from Foresight Diagnostics led by first author Nina Klimova and corresponding author Laura Hyland validated a new DNA-based blood test designed to detect minimal residual disease (MRD) in patients with B-cell cancers. This assay uses a highly sensitive method called Phased Variant Enrichment and Detection Sequencing (PhasED-Seq) to find tiny fragments of tumor DNA in the blood. Its ultra-sensitive detection capabilities offer a powerful tool for early cancer detection, monitoring treatment response, and predicting cancer reappearance. B-cell lymphomas, such as diffuse large B-cell lymphoma (DLBCL), are among the most prevalent blood cancers. Although many patients respond to initial treatment, up to 40% relapse. Standard monitoring methods such as imaging scans often miss low levels of cancer cells, creating a need for more precise tools. This study introduces a non-invasive blood test that improves the detection of MRD, a critical factor in guiding follow-up care and early intervention. The test works by tracking unique groups of mutations known as phased variants in tumor DNA. These mutations are more specific to cancer and allow for highly accurate identification of tumor fragments in the bloodstream. The PhasED-Seq-based MRD assay was tested on three types of samples. First, blood plasma from healthy individuals was used to confirm the test does not give false positives. Second, researchers created controlled samples by mixing tumor DNA from lymphoma patients with healthy DNA to measure how sensitive and precise the test is. Finally, blood samples from patients with B-cell lymphoma were used to compare the new test to an existing method. Across all sample types, the PhasED-Seq-based MRD assay demonstrated exceptional performance—capable of detecting fewer than one cancer DNA molecule per million normal DNA fragments. It also demonstrated a very low false positive rate and over 96% reproducibility across different laboratory conditions. Compared to an existing method, the new PhasED-Seq assay showed more than 90% agreement in positive results and nearly 78% agreement in negative results. In cases where the tests disagreed, the new method aligned more closely with actual clinical outcomes, including whether patients relapsed or stayed in remission. “The background error rate of the PhasED-Seq-based MRD assay was 1.95E-08, or 1.95 mutant molecules in 100 million informative molecules.” The findings support the use of PhasED-Seq-based MRD assays in routine clinical practice. It could be especially useful for identifying patients who need additional treatment even when imaging results appear normal. This aligns with updated clinical guidelines that encourage the use of blood-based DNA tests to supplement traditional scans in lymphoma care. This study offers strong evidence that the PhasED-Seq-based MRD assay is a precise, reliable, and clinically relevant tool. By detecting signs of cancer earlier and more accurately, it may help clinicians tailor treatments to individual patients and improve long-term outcomes in B-cell malignancies. Eran Elhaik, from Lund University in Sweden, trained the AI tool using nearly 4,500 microbiome samples collected around the world from subway systems, soil and the oceans. He said they could identify the city source in 92 per cent of their urban samples, and in Hong Kong, where a lot of their data came from, they could identify the specific subway station samples were taken from with 82 per cent accuracy. The study was published in the journal Genome Biology and Evolution.

It's been a few months since wildfires devastated Los Angeles, and some people are just now starting the long process of repairing and rebuilding their homes. But mixed into the soot and ash can be some hidden dangers, including lead, asbestos, arsenic and lithium. These toxic materials were used to build those homes and got blown across LA. Scientists from the California Institute of Technology have been investigating this invisible danger, testing more than 50 homes for these contaminants. And one of those homes belongs to the lead scientist behind the study. Reporter Caroline Champlin has more.  And our series on Californians and resilience continues with twenty-five-year-old Brittianna Robinson. She experienced sexual abuse and trafficking as a child. Robinson credits her faith in God and support from mentors and her church for helping her find a path forward. Most AI safety conversations centre on alignment: ensuring AI systems share our values and goals. But despite progress, we're unlikely to know we've solved the problem before the arrival of human-level and superhuman systems in as little as three years.So some are developing a backup plan to safely deploy models we fear are actively scheming to harm us — so-called “AI control.” While this may sound mad, given the reluctance of AI companies to delay deploying anything they train, not developing such techniques is probably even crazier.Today's guest — Buck Shlegeris, CEO of Redwood Research — has spent the last few years developing control mechanisms, and for human-level systems they're more plausible than you might think. He argues that given companies' unwillingness to incur large costs for security, accepting the possibility of misalignment and designing robust safeguards might be one of our best remaining options.Links to learn more, highlights, video, and full transcript.As Buck puts it: "Five years ago I thought of misalignment risk from AIs as a really hard problem that you'd need some really galaxy-brained fundamental insights to resolve. Trauma and paediatrics are the themes for this month's collection of papers. Starting off is a review of the effectiveness of prehospital ultrasound in detecting lung injury, with some surprising statistics. Next is a letter on the topic of self-presentation by paediatric patients with major trauma, which is thankfully a rare occurrence. The third paper looks at prehospital testing of trauma patients for  low fibrinogen levels, a condition which can lead to worse bleeding, increased transfusions, and higher mortality. Because there are no symptoms in the early stages of pancreatic cancer “about 75 to 80 percent of patients are diagnosed in the later stages of the disease,” said Somashekar Krishna, MD, a James physician scientist who specializes in the early detection and screening of pancreatic cancer. Krishna and the team of pancreatic cancer experts at the James are using enhanced screening and artificial intelligence (AI) to better identify pre-cancerous cysts and cancerous tumors in the pancreas; and are the first in the world to use high-tech ablation techniques to attack and kill pancreatic cysts and tumors in a new clinical trial. Most pancreatic cancer tumors begin as pre-cancerous cysts and are most commonly found in people 60 and older. “For people in their 50s there is about a 10-percent prevalence, this doubles in people 60 to 70 to 20 percent and is 25 to 30 percent in people 70 to 80,” Krishna said, adding, “most are small and never grow and become cancerous.” Endoscopy procedures are used to better “see” these cysts and cancerous tumors, and the use of artificial intelligence is another tool to identify and determine the risk factor of pre-cancerous cysts. “The tip of the endoscopy probe can see even the tiniest structures in a very detailed manner, and we can pass a needle through the scope and do a biopsy and establish the risk,” Krishna said. Surgery is one option, but the procedure is quite invasive and not easily tolerated by older patient with other medical issues. “A new option is ablation, using heat, in a very precise and careful manner,” Krishna said. “We are the only ones doing this in the United States and have done this with nearly 30 patients in a clinical trial.” Krishna is determined to improve screening and treatment for pancreatic cancer and reduce the mortality rate. “Early in my career, 80 percent of patients diagnosed with pancreatic cancer did not make it more than a year,” he said. “We want to change these outcomes to intervene early and with better and better treatment options.”

Or at least... not officially.As today's guest — Jeffrey Lewis, founder of Arms Control Wonk and professor at the Middlebury Institute of International Studies — explains, in its official 'OPLANs' (military operation plans), the US is committed to 'dominating' in a nuclear war with Russia. How would they do that? "That is redacted."Rebroadcast: this episode was originally released in December 2022.Links to learn more, highlights, and full transcript.We invited Jeffrey to come on the show to lay out what we and our listeners are most likely to be misunderstanding about nuclear weapons, the nuclear posture of major powers, and his field as a whole, and he did not disappoint.As Jeffrey tells it, 'mutually assured destruction' was a slur used to criticise those who wanted to limit the 1960s arms buildup, and was never accepted as a matter of policy in any US administration. But isn't it still the de facto reality? Yes and no.Jeffrey is a specialist on the nuts and bolts of bureaucratic and military decision-making in real-life situations. He suspects that at the start of their term presidents get a briefing about the US' plan to prevail in a nuclear war and conclude that "it's freaking madness." They say to themselves that whatever these silly plans may say, they know a nuclear war cannot be won, so they just won't use the weapons.But Jeffrey thinks that's a big mistake. Yes, in a calm moment presidents can resist pressure from advisors and generals. But that idea of ‘winning' a nuclear war is in all the plans. Staff have been hired because they believe in those plans. It's what the generals and admirals have all prepared for.What matters is the 'not calm moment': the 3AM phone call to tell the president that ICBMs might hit the US in eight minutes — the same week Russia invades a neighbour or China invades Taiwan. Is it a false alarm? Should they retaliate before their land-based missile silos are hit? There's only minutes to decide.Jeffrey points out that in emergencies, presidents have repeatedly found themselves railroaded into actions they didn't want to take because of how information and options were processed and presented to them.
