Author David Weill discusses his new book, All That Really Matters. In this novel, the main character, Dr. Joe Bosco, is a rising star in the heart transplant field, and he's feeling all the pressures that come with the job. His professional and personal issues lead him into trouble, and he struggles to find redemption. David knows all about being a transplant surgeon, and he wrote this story to give readers an idea of the kind of people who work in the field and what it feels like to that job. While the book is a work of fiction, it incorporates real-world personalities and situations in an effort to humanize doctors and explore the high stakes world of medical transplants.

My guest this week is Dr. David Weill, the former director of the Center for Advanced Lung Disease and Lung and Heart-Lung Transplant Program at Stanford University Medical Center. He's currently the principal of the Weill Consulting Group, which focuses on improving the delivery of pulmonary, ICU, and transplant care. He's also the author of the new novel, All That Really Matters, which follows his memoir, Exhale: Hope, Healing, and a Life in Transplant. David talks with me about how he made the shift from surgery into writing, how healthcare workers have historically been encouraged to deny the emotional reality of their work as a defense mechanism, and how that is finally shifting; the harsh and sometimes cruel realities of the transplant world; how fiction allows greater latitude in telling the truth about those realities; why transplant patients often like to go sky diving after their surgeries; and more. Episode breakdown: 00:00 Introduction 01:31 Transplant professional documented experiences, produced two books. 05:19 Hid emotions in career, later embraced vulnerability. 06:53 Health care work strains minds, career change needed. 12:14 Interesting emotional arc, unwise career decision impacts protagonist. 16:51 Medicine's learning curve: years of motivating terror. 18:01 Mentor's patient admiration influenced his demeanor. 22:32 Some should leave jobs earlier, unlike most. 26:47 Healthcare struggles balancing patient care and profitability. 29:34 Moral injury from value misalignment in healthcare. 31:57 Left clinical role; now consulting for solutions. 36:49 Saving young lives is my most memorable work. 40:27 Enjoyed writing fiction; eager to continue exploring. 42:31 Organ allocation ethics and financial incentives examined. 45:40 Talented, flawed protagonist falls and redeems self. 50:55 Next book explores questions of identity. Dr. Wilner would love your feedback! Click here to send a text! Thanks! Many thanks to David Weill, MD, former Director of the Center for Advanced Lung Diseases and the Lung and Heart-Lung Transplant Program at Stanford University Medical Center. Dr. Weill is the author of the new novel “All That Really Matters” and the memoir “Exhale”. He's working on a 3rd book to be published next year. I read “All That Really Matters” and couldn't put it down. Although I am not a transplant surgeon like the protagonist, we definitely shared some similar experiences during our training and early careers. The novel leads us through the complex world of organ transplants, with the author as an expert guide. Even those without a medical background will have no difficulty following the plot and may perhaps identify with a character or two. Dr. Weill told me that the novel was a vehicle that allowed him explore the moral and ethical issues involved in human organ transplant. During our 30-minute discussion, Dr. Weill filled me in on his 20 years of life-saving organ transplant work. Although immensely gratifying, this 24/7 activity wreaked havoc on his work/life balance. Finally, Dr. Weill left his position as Director of the Transplant Program at Stanford to become Principal of the Weill Consulting Group, which focuses on improving the delivery of pulmonary, ICU, and transplant care. This transition allowed him to move back to his hometown of New Orleans, continue to help improve the transplant system, and provided an opportunity for him to write. During our interview, we also discussed the practicalities and rewards of writing fiction, his long-term goals, and how he gets the writing done. It was indeed a pleasure interviewing David Weill! Through the lens of his own emotional and physical exhaustion, he explores the underbelly of hospital systems and the clinical, administrative, and financial issues plaguing the specialty. He is an advocate of shifting the blame of burnout away from the individual and placing it on the macro problems that contribute to physician dissatisfaction.

Dr. David Weill joins Tim to talk about those life-saving transplant surgeries, the patients, the system for care and the challenges it faces, and what it's like to be a doctor of second chances. Dr. Weill was the Director of the Center for Advanced Lung Disease, and the Lung Transplant Program at Stanford. Today he operates the Weill Consulting Group, where he focuses on improving the delivery of transplant care. This episode was first released December 13, 2021. https://traffic.libsyn.com/forcedn/shapingopinion/Encore_-_A_Transplant_Surgeon_Story.mp3 The first time doctors were able to transplant a human organ happened in 1954. That's when a kidney was transplanted successfully. In the decades to come, medical pioneers would master the medical art of transplanting livers, lungs, hearts, pancreases and other vital organs. While these surgeries are never described as routine, they are no longer uncommon. In the early years, individual hospitals and certain organizations managed everything from organ recovery, to transport, to transplantation. In between, they had to learn how to allocate valuable, life-saving organs for the most viable patients. In some respects, not much has changed, and that's a problem. Dr. David Weill has spent his career in organ transplantation with a focus on lungs helping those with severe lung disease. He ran the Lung Transplant Program at Stanford, and he wrote a book called “Exhale: Hope, Healing, and a Life in Transplant.” Links Weill Consulting Group (website) Dr. David Weill, Tulane Medicine website Supply Isn't the Problem with Organ Transplants, Wall Street Journal Exhale: Hope, Healing, and a Life in Transplant, by Dr. David Weill (Barnes & Noble) Opinion: Of course unvaccinated people should be barred from receiving transplant organs, Washington Post About this Episode's Guest Dr. David Weill Dr. David Weill has been in the forefront of developing and running some of the most successful lung transplant programs in the country. He served as Director of the Lung and Heart-Lung Transplant Program at Stanford University Medical Center from 2005-2016. He also developed the Stanford Center for Advanced Lung Disease which provides care for hundreds of patients with cystic fibrosis, interstitial fibrosis, pulmonary hypertension, and emphysema. During this period, he also directed a rebuilding effort of the lung transplant program, producing some of the best outcomes in the country, while increasing the transplant program volume more than three-fold. In addition to his medical practice, Dr. Weill has testified before the U.S. Congress regarding occupational lung diseases. He has also published extensively in the medical literature regarding lung transplantation, occupational lung disease and advanced lung disease. He and his wife Jackie recently moved to his hometown of New Orleans with their two daughters.

Some people may have heard of COPD. That's essentially the same thing.” (07:43) (Dr. David) “There's a vast array of chemicals in a vape and what goes into the lung. But even more than that, it's thermal energy. So, in other words, it produces very high heat in the lung, and the lung is quite sensitive to very high temperatures. The damage can not only come from the chemicals that are contained in the vape oil, but also from the heat that's generated. It's much hotter than cigarette smoke.” (10:52) (Dr. David) “As marijuana increasingly becomes more mainstream in our society, we're seeing more and more research that it's not quite the benign drug that we thought it was maybe early on, mainly because there wasn't much research on it when it was illegal.” (18:19) (Dr. David) “It was very difficult for me to separate how much I cared from accepting an outcome that was not good. And, I don't think I ever really learned how to do that well over the course of my career.” (25:25) (Dr. David) “What I try to do now is coach the teams through everything and try to figure out a way to get on the same page. That's probably the number one value that I can bring to programs now.” (31:46) (Dr. David) Resources Mentioned Dr. David's Website Dr. David's Book Dr. David's Instagram Dr. David's Twitter Dr. David's Facebook Dr. David's LinkedIn DM Dr. Brian your questions and he will respond back with answers Dr. Brian's Website Dr. Brian's TikTok Dr. Brian's Instagram Please remember, Dr. Brian is a doctor, but he is not your doctor. “My religion was ambition and achievement. It took a long time for me to realize I wasn't the one saving their lives. I was a small part of that. I was the hand of a greater being.” – Dr. David Weill For 20 years, Dr. David Weill kept with him a list of names of people who were in dire need of lung transplants. As the Director of the Center for Advanced Lung Disease and Lung and Heart-Lung Transplant Program at Stanford University Medical Center, he led a team of 55-60 people and was in charge of the care plan for transplant patients, assessing their psychological chances for survival and matching them with organs as they became available. Life and death seemed to be in his hands and losing a patient seemed to him like a personal failure. In this episode Dr. Weill tells the story of how he rode the emotional rollercoaster and evolved from arrogance to authenticity as he loosened his hold on control and awakened to the presence of higher power working through him. When he left the front lines of the hospital, he started Weill Consulting Group, which focuses on improving the delivery of pulmonary, ICU, and transplant care. His riveting memoir, Exhale: Hope, Healing, and a Life in Transplant, rawly and authentically depicts his fascinating life journey. Here are some of the things we talked about: • The job of a transplant director • Learning from transplant patients • The ego's addiction to control (i.e. ‘control enthusiasts) • Insights gained from reading the Bible cover to cover • The humanity of doctors and the loss of the soul in medicine • The prevalence of post-covid confusion • The need for systemic change (government, healthcare, etc.) to come from the grassroots level • Writing as a way to process life • Telling the truth and not pretending • The importance of connection © the spiritual forum 2023

A young father with a rare form of lung cancer has been turned down for a transplant by several hospitals - a kid who was considered not 'smart enough' to be worthy of a transplant - a young mother dying on the waiting list in front of her two small children - the nights waiting for donor lungs to become available, understanding that someone needed to die so that another patient could live - These are some of the stories in Exhale, a memoir about Dr. David Weill's ten years spent directing the lung transplant program at Stanford. Today he operates the Weill Consulting Group, where he focuses on improving the delivery of transplant care. https://traffic.libsyn.com/secure/shapingopinion/Doctor_of_Second_Chances.mp3 The first time doctors were able to transplant a human organ happened in 1954. "Continuing to do this work the way I was doing it was not sustainable. I needed to get off the merry-go-round. I just didn't know how. I didn't think I could stop myself—transplant was my duty, my responsibility, and what I was trained and programmed to do. It was hard to imagine leaving my post, but I needed to find a graceful exit, an elegant off-ramp that would satisfy my need for a tidy conclusion. One way or another, I wanted off the roller coaster, so I could never again be crushed by a patient like Tina. That had been my life for nearly twenty years. That was enough. I felt yanked around by the ups and downs of the job, my emotions seesawed, and all the while I had to make rational decisions for my patients. But for me, the job was no longer a pure and simple exercise in rationality. I had mastered the mechanics of being a transplant doctor— that was the easy part. But now, from an emotional standpoint, every wife was my wife, every child was my child, and every father was my father. And I needed to save them all." Exhale: Hope, Healing, and a Life in Transplant (Post Hill Press, 2021) is the riveting memoir of a top transplant doctor who rode the emotional rollercoaster of saving and losing lives—until it was time to step back and reassess his own life. A young father with a rare form of lung cancer who has been turned down for a transplant by several hospitals. A kid who was considered not “smart enough” to be worthy of a transplant. A young mother dying on the waiting list in front of her two small children. A father losing his oldest daughter after a transplant goes awry. The nights waiting for donor lungs to become available, understanding that someone needed to die so that another patient could live. These are some of the stories in Exhale, a memoir about Dr. Weill's ten years spent directing the lung transplant program at Stanford. Through these stories, he shows not only the miracle of transplantation, but also how it is a very human endeavor performed by people with strengths and weaknesses, powerful attributes, and profound flaws. Exhale is an inside look at the world of high-stakes medicine, complete with the decisions that are confronted, the mistakes that are made, and the story of a transplant doctor's slow recognition that he needed to step away from the front lines. This book is an exploration of holding on too tight, of losing one's way, and of the power of another kind of decision—to leave behind everything for a fresh start. Exhale: Hope, Healing, and a Life in Transplant (Post Hill Press, 2021) is the riveting memoir of a top transplant doctor who rode the emotional rollercoaster of saving and losing lives—until it was time to step back and reassess his own life. On s'y croirait !
