Hi Jimmy and Dr. Cole, I really enjoy your podcast and appreciate your diligent work. My question is about sports performance. I've been doing a lower carb diet for 8 months now, always under 150g carbs and mostly under 100g. For the past 6 weeks, I've been strictly following a keto diet with net carbs under 30, many days around 20g. I'm also limiting my calories and doing intermittent fasting with calories coming in between 1200-1500. Once or twice a week I cycle up to about 2000 calories. I just returned home from a snowboard trip where I experienced a scary problem and I'd like to learn more about the physiology of what may have happened. The first two days were challenging conditions with a lot of powder. This takes more effort so I'd consider it somewhat demanding exercise, but still aerobic. This was my first trip since going strictly keto and I was still adhering to the diet strictly. On the third day after a breakfast of Canadian bacon and eggs, we got up and headed up the mountain. On my first short run of the day, the trouble began. My legs literally would not work and I had trouble getting up on my board after falling. This happened a few times and I started to worry I may be having a panic attack even though this is not something I've ever experienced on the slopes. I was definitely a little worried about what I was experiencing but I just chalked it up to nerves, calmed myself, and eventually made it down. I assumed resting on the lift would be all I needed. I got up to the top and the same thing happened although initially I was fine for a few minutes. It was a real challenge to both my legs and my brain and then I started having the same symptoms again about halfway down. These were steep, challenging runs and I began to worry I had just finally freaked myself out to the limit. Then it occurred to me that I may be "bonking"...I've run one marathon and experienced something like that, however, this was very different. I seriously couldn't get my legs to work, as in laying on my back trying to flip over and couldn't even pull my knees up to my chest. I ended up making it down safely, albeit so ungracefully and very slowly and decided I should probably eat some quick absorbing carbs if I wanted to continue. I slowly ate half of a very large chocolate chip cookie, monitoring how I felt. Incidentally, this didn't even taste good to me and didn't create any cravings in the days after. After a rest, I went back out and was totally back to "normal." I know I'm relatively fat-adapted at this point since I can fast 18 hours easily. However, I've also read it can take months for metabolic processes to fully adapt. I'm guessing what happened is that the demands of the activity outpaced my body's ability to access fat, and being keto, I had fully depleted my glycogen stores. Is that what happened to me? That's pretty simplistic, but I'd love to hear a more detailed scientific explanation of what may have happened. Thanks again for producing a great show! Hi Jimmy and Will, I’m one month and two days in to my new lifestyle, but I still have diarrhea and it’s always yellow. Is this normal? Thank you! Hello you guys, It looks like my wife is having low platelets and a possible diagnosis of ITP (Idiopathic Thrombocytopenia) and we are working with doctors to find out. Jimmy and Will, I have been on keto since August and have lost about 50 pounds. I have been testing blood ketones and I am usually at 1.0 mmol+. Over the holidays I had three times when I went out of ketosis and each time I had a bad reaction. About 2-4 hours after consuming carbohydrates, I would have what I think is a hypoglycemic reaction with terrible nausea, diarrhea, sweating, and shaking. I was unable to test my blood to see what my glucose reading was because I felt so bad. After a few hours of resting I would start feeling normal again and eating carbohydrates after that was fine. But each of the three times I was getting out of ketosis I had this reaction. Any thoughts on why this is happening or ways to prevent it (besides the obvious of just staying in ketosis)? One more bit of info, the third time it happened I was trying to prevent it and instead of eating a big meal I kept snacking to try and keep my blood sugar up and that didn't work. Hello Jimmy and Will, Thank you for your work. It is a pleasure to listen to Keto Talk. I have an observation about high-fat/low-carb that you and other listeners may find interesting. I sustained a very bad sports concussion in 2011 and this injury knocked me out (literally!) of school, all physical activity, and nearly all social interaction for upwards of six months. Most of my symptoms subsided within a year, but I had several longer-lasting cognitive effects, including math and language impairment, lack of focus, and increased irritability. One of the more prominent personality changes I noticed after the concussion was the sudden development of OCD, especially compulsion, and heightened anxiety and paranoia. In the years since, I have learned to live with this and work around my "new self." I have even been to counseling, which was somewhat helpful. When I first became concussed, my medical team recommended I take omega-3 fats in the form of flaxseed oil supplements, as well as an anti-seizure medication. This past summer, I adopted a Paleo diet which quickly transitioned to Paleo ketogenic and I recently began implementing periods of fasting. I am doing all of this for health/longevity/vanity benefits, unrelated to my concussion. Since introducing fasting to my ketogenic diet, I am becoming my "old self" again. I have experienced all the benefits of ketosis and fasting—mental clarity, emotional stability, physical changes, physical performance PR’s, and more. But I never expected that the effects of my concussion would be reversed. They are not gone completely, but every fasting cycle I see huge improvements. After doing a quick search of Dr. Google and looking on PubMed, I found out that this is a thing. People are using ketosis and fasting to heal traumatic brain injury. And it makes perfect sense- the neuroprotective aspects of fasting and a ketogenic diet should theoretically also help heal. Can you talk more about the mechanism of this on your podcast? My concussion was relatively minor, but there are people with CTE and servicemen coming back from overseas with PTSD and very serious traumatic brain injuries. However, there were a some side effects which I wonder if you could help with like sore, dry eyes which feels like I’ve drank too much alcohol. What’s going on with this? Liver issues? Dehydration? Lack of sleep? When I switched back over to a “normal” diet during Christmas, my eyes got a lot better. Since the beginning of the year I’ve cut my carbs again (but not fully keto) and the eye pain is back again. I’ve tried supplementing daily with magnesium, potassium, fish oil, a multivitamin, 5-HTP, a probiotic, and butterbur (which is supposed to be good for headaches). After slowly gaining back all the weight after getting frustrated with doing that, I then got diagnosed as prediabetic in June 2016 and it terrified me since my older brother has Type 2 diabetes. Fortunately I found the work of Tim Ferriss which led to me the cyclical ketogenic diet and then into full-time keto which helped me lose over 100 pounds. So far so good, right? I recently went to the doctor and got some bad news about my prolactin levels indicative of the health of my pituitary gland. Normal range is 2.1-17.7, but mine came in at 63.9! Is this increase in prolactin a result of eating keto or is it something completely unrelated to my diet? My doctor is going to run an MRI to see if he can find out more about it. Hello guys, I am incredibly frustrated with my keto experience so far and hoping you can help. I am a 5'3" female and weigh 155 pounds. While I have gotten better now, it seems to be a bumpy ride so far. Hey Jimmy and Will, Saturated fat and cholesterol are the primary concerns that my vegan and vegetarian (and even my SAD dieter) friends point to when it comes to the keto diet. Even when I point out that plant-based food sources such as coconut and olive oil both have more saturated fat than most animal-based foods, they argue that this way of eating will raise cholesterol and saturated fat levels in the blood which will inevitably lead to atherosclerosis and heart disease. I’ve asked them to read your book Cholesterol Clarity, Jimmy, but my very intelligent and mostly skeptical friends want resources from “unbiased” sources since you are a prominent figure in the keto space. I really need some third party resources to share with them about the healthy role of saturated fat, cholesterol, and why a ketogenic diet is a positive thing for the body. I eat low-carb keto and my blood sugars stay in tight control. My thyroid levels are very good thanks to the Nature-throid medication I use. But here’s my problem—I can never get into a state of nutritional ketosis. Is it because I’m on an insulin pump and there is a constant flow of basal insulin being injected into my body? I've been a lot like you, Jimmy, trying all different kinds of diets and reducing calories—but nothing seemingly works! I know I got too hypocaloric at one point and increased my calories again. I’m a Type 1 diabetic on an insulin pump and don’t need very much insulin because I keep carbs below 20g, 70-80g protein, and fats from eggs, avocados, butter, cheese, and fatty meats. My blood ketones are in the healthy range of 0.7-1.9. As much as I believe in keto, I can’t deal with gaining weight. The only time I’ve seen the scale move is when I’ve cut calories to 1100 a day. But I know that’s not a healthy level for me to be consuming long-term. Why is this so hard for me? It feels like all of the low-calorie, low-fat diets I’ve always suffered with. It has truly inspired me in my own journey. I have been eating keto since the beginning of the new year and I’ve noticed this way of eating makes fasting for upwards of 24 hours very easy to do. I tried my first 72-hour fast this week to see how I’d do and the first 24 hours was a cinch. Day two was a challenge, but I already knew that from hearing Jimmy talk about this so much. I was anticipating the “euphoria” of day three, but at hour 58 I woke up feeling absolutely horrible. I took some salt, drank coffee, pounded water, and did my best just to walk on the treadmill for a mile that day. After the workout I felt so bad that I knew it was time to end the fast because hunger pangs and food cravings at that point were just too much to bear. I feel like I did everything right and saw blood ketones in the 2.0-3.5 range and blood sugar in the sixties feeling great. There is one monkey wrench in this story I haven’t shared yet, but on the night of day two of this 72-hour fast, I had to take an antibiotic. Is it possible that is the culprit in my hunger and symptoms that forced me to quit the fast? Is there a way to safely and effectively get the benefits of fasting if you are taking an antibiotic? With lots of prayer and commitment to my own health, I was able to lose around 600 of those pounds. Today, I am a very active person and have been eating low-carb for the past nine years. In late 2018, I decided to shift my nutritional intake over to the ketogenic diet. I love to move and do an hour plus of cardio six days a week as well as strength training for 20 minutes 3-4 times per week. I have a standing desk at work, and I attempt to move as much as I can throughout the day - sometimes even doubling up on the cardio sessions! My question for you guys is this: What impact does all this exercise that I do have on my cholesterol levels while eating a ketogenic diet? I started to get melasma when I was 30 years old and I’m convinced that this is from my hormones going haywire. I am no longer taking plaquenil for pain and inflammation since going keto three months ago and I haven’t felt this good in years. Hi Jimmy and Will, I started keto three weeks ago and recently had some blood work done that shocked me. I had a huge spike in triglycerides—up to 700! I have had traditionally high triglycerides 150-200 for years. I’ve increased my fat intake and consumed very low carbs (< 30 grams), yet it scared me to see this large jump in triglycerides. It is noteworthy that I am recently diagnosed with Type 2 diabetes and have stage 3 kidney disease. I also recently stopped my statin and beta blocker medications. So my questions for you guys is this: 1. Why did I see a spike in my triglycerides to 700 just three weeks into the diet ? 2. Is a ketogenic diet suitable for someone with stage 3 kidney disease? I’m 5’6” tall and weigh 179 pounds as a 61 years old woman and have been keto for a couple of years. I started eating this way when my family doctor suggested I may be pre-diabetic with a fasting blood glucose of 101. My blood ketones remain in the 1.5-2.0 range, so I know I’m burning fat. Initially I lose 55 pounds, but for some strange reason I gained back 10 pounds. Although I have a function medicine medical doctor, she’s no fan of me trying longer fasts because of my hypothyroid issues which I take Armour thyroid for. I’ve tried changing things up in the way I’m doing keto, including a protein sparing modified fast with significantly less fat than I typically eat and carnivore. These did help me lose some weight, but both gave me problems and the weight I lost came right back on with my regular keto diet. I am currently preparing for a women's mini-triathlon (1.5 mile kayak, 11-mile bike, and 3.1 mile walk or run) which takes place in February. I did this last year in a fasted state and did great! I feel like that's not going to be the case this time around. I'd like to at least remain keto but I’m thinking being mostly carnivore is where I feel my best. I will add that my doctor prescribed me Gabapentin 100mg for me to take at night to help with pain that interferes with my sleep. Do I need to add in additional carbs to what I’m doing to help with these issues or should I be concerned? In the earlier episodes I was listening to, Dr. Nally mentioned that Long QT is a reason to be monitored by your doctor when starting keto. This piqued my interest since I deal with this condition. I have an implanted heart monitor that checks for abnormal rhythms 24/7, so I’m assuming that would catch any issues. Are there any specific precautions I should be taking with long QT and keto (mine is the incredibly rare genetic variant, Long QT type 10, which has to do with a sodium channel pathway and is most similar to Type 3 of the more commonly known types). Hi Jimmy and Will, I am a naturopath in Australia and I have been keto for a few months. I had a fairly easy adaptation but my partner is now having a lot of nausea. He started a couple of weeks ago and got into ketosis in about 48 hours with blood ketones around 1.2. He was doing so well until I made the mistake of giving him some cake I had made with almond and coconut flour. He almost immediately started feeling sluggish and experienced mental fatigue and fogginess as his blood ketones crash to under 0.5. He was able to get back into ketosis again, but the nausea and other side effects have been relentless ever since. We’ve been adding in more salt to try to help, but nothing is helping so far. I feel like I broke the momentum he had on his keto and would love any suggestions about how to help him get back on track again. He came to keto from a mostly fruit-based diet for a year prior to this. I feel my partner has so much to gain from eating this way coming from a history of chronic fatigue! Will ketosis compromise my body’s ability to retain the fat that is transferred as it needs to establish a new blood supply to be able to live in the new location. I am currently at my goal weight and am no longer losing weight. My initial thought is that since ketosis is a healthy state for the body to be in, it should not be a problem. But what say you?
Alex is back in the country which means Alex and Mike are both back with another episode of The Atlantic Files! While away, Jimmy Butler came to Philadelphia, Caris LeVert suffered an ugly injury, and the Atlantic Division continued to look like the best division in the East. Jimmy Butler was shipped out of Minnesota for Dario Saric and Robert Covington among some other pieces. Alex was sad to see those two go but is more than excited to see the big three of Butler, Joel Embiid, and Ben Simmons. Mike and Alex both agree that this finally gives Philly someone to perform in the clutch. Caris LeVert suffered a very ugly injury that, unfortunately, put a halt to such a great season that he was putting together. The good news is that the damage to his ankle wasn't as serious as everyone thought. We may see him later this season, and finally, Mike can breathe. Hello Jimmy and Dr. Cole, I am a 38-year old male with Type 2 diabetes. I am 6'3" and currently weigh 307 pounds. I started keto at the beginning of the year and have lost 25 pounds so far. I stopped taking all my diabetes medications on am only on blood pressure medication at the moment. My blood glucose numbers run between 155-200 no matter what time of day I check it. I have been implementing intermittent fasting with keto for the past month. So my question for you guys is this: Did I stop taking my medications for controlling Type 2 diabetes too soon? I was taking 1000mg metformin twice a day, 20mg glipizide twice a day, 10 units of lantis per day. I feel so amazing on keto and never want to go back to eating any other way. Do I need to go back on the medications for a short period of time until I heal a little more or will my body heal with a ketogenic diet and lifestyle changes over time? Most shocking to me when when my doctor told me I’d be on medication for the rest of my life and that diet and lifestyle don’t make a difference with Type 2 diabetes. In fact, one of his nurses told me that I was not insulin resistant. How the heck can a Type 2 diabetic NOT be insulin resistant? The idea of going back on these prescriptions that force me to eat more carbohydrates to counteract their effects is frustrating to me. Can you help me figure this out? But a goofy thought ran through my head and it made me wonder if dietary fat could have the exact same effect in a fat-adapted person. I know that when I eat fat I always feel really good and happy. So does that mean I had a spike in dopamine? And if so, what are the ramifications for a ketogenic dieter if that is indeed happening? Hey guys, My daughter is a big believer in keto and she really wants to do the diet for her health. But she has a terrible time with eating fat, even the healthy fats, because of a bad bile reflux condition where she produces too much bile. Foods like guacamole, sausage, and even eggs make her queasy and nauseated. The standard medical advice for this condition is to go on a low-fat diet--something she knows from her research and experience is not a good thing. Hello my friends, I am new to keto and had to deal with this condition last week known as legionnaires' disease which is a severe form of pneumonia from the lung inflammation usually caused by an infection. I felt like I was basically starving to death. I consume carbs and sugar the last couple of days of dealing with this because I felt like my body needed them since I was in a mentally dark place. This condition left me crying often and feeling absolutely awful. Did the sugar and carbohydrates I consumed dealing with this make the infection worse or are they somehow a good thing when dealing with legionnaires' disease? Thank you for your insights. Catalina 3. Why are my fasting blood glucose levels rising since giving up wine when I went keto and will they normalize over time? Hi Jimmy and Dr. Cole, I've been following a low-carb diet for years and recently shifted over to keto. I love drinking wine every single day and I realized I probably was drinking too much of it. BOO! So I decided to give it up altogether to see if it would boost my ketogenic efforts to lose weight and improve my health markers. I lost 35 pounds a couple of years ago eating low-carb, but I haven’t lost anything more since. One thing I’ve always hung my hat on despite the lack of weight loss is my impeccable blood sugar control as well as other health markers I regularly track. I take a medication called Atacand Plus for hypertension but no other medications. I’m sure dropping the remaining 35 pounds of weight from my body will enable me to come off that medication for good. So here’s my issue: since going keto and coming off of my beloved wine, my fasting blood glucose levels have risen. I thought coming off the wine would be a good thing, but it seems my blood sugar has gone in the opposite direction. What’s causing this to happen and will it eventually normalize over time? In August, I began struggling with regular bouts of diarrhea on a daily basis. I can't remember the last solid stool that I had. I no longer track my macros since I'm at my goal weight. I typically have a Bulletproof coffee with MCT oil powder and butter in the morning and eat leftovers for lunch, an afternoon snack of a cheese stick or almond butter, and then dinner with meat and veggies. I've been playing around with my carbs, including adding back in healthy carbs like sweet potato and checking my blood ketones to confirm I'm always in ketosis. I drink lots of water and I'm good about getting in my electrolytes. I've even cut back on magnesium to see if that would help. It has not. My stomach isn't upset and overall I feel a lot better than I did before keto. Any clues on what I can do to decrease the diarrhea? Thank you for your help! What is causing my poop to be watery and yellow when I’m eating keto and extended fasting? Hello Jimmy and Dr. Cole, I’ve been eating keto with intermittent fasting since mid-April and have been slowly healing my gut from a lot of issues I was experiencing. But here is the issue: my poop is watery and yellowish in color (my liver levels were tested and it came back as optimal on the lab results). I especially see this yellowish loose stool when I fast for longer than 16 hours. My MCT oil consumption is 1 Tbs in my coffee and I did that without issues at the beginning of keto. It kicked in at the three- month mark. My food intake hasn’t changed much since going keto, but I have noticed that when I eat off plan even a little I get the runs hard and furious. One other issue that might be connected to this is I’m dealing with hair loss from thyroid issues as well as high estrogen levels. Can you help me figure this out? Thanks so much for your podcast! The plan is very limiting allowing only about 600 calories a day. It is also very low in fat and only includes 11 ounces of protein daily. So, after much thought and my sad attempt at researching this online, I thought I would contact you to ask this important question: Does limiting the amount of food you eat on a plan like this equal what a bariatric surgery patient would consume put the body into a more effective state of ketosis than following a more traditional ketogenic diet as described in your book? Hi Jimmy and Will, I recently began working with a nutritional therapy practitioner and have been practicing the GAPS diet for about a week now to begin healing my gut and digestive system as well as to detoxify. Before this diet, I was following a ketogenic approach along with intermittent fasting for 16:8 naturally and then practicing 42-hour fasts three times a week and losing weight fairly easily. Now my weight has gone up some and plateaued. My NTP has asked me to stop fasting since it might deprive my body of the nutrients it needs to heal. At the first stage of the elimination portion of the GAPS diet, you eat animal meat boiled in bone broth with specifically approved vegetables like squash, broccoli, onion or cauliflower that are also boiled in bone broth. I am becoming convinced my plateau has to do with the higher amount of protein in this approach. I am only able to add animal fat to the stews and soups that are GAPS-approved and realize it's not the larger amount of fat I was eating on keto. So it seems I’m now eating a high-protein, moderate fat, low-carb diet. Do you have any insight whether or not it’s the protein that is stalling my weight loss and even gaining weight again? I have been eating a ketogenic diet for over a year after starting my nutritional health journey with a primal/Paleo diet. The pounds on the scale simply are not moving. My doctor had said according to my health markers that there is nothing wrong with my metabolism. My thyroid and adrenals were good and my A1C is 5.0 showing my blood sugar to be well-controlled. According to my doctor, my sex hormone levels indicate I am still fertile at the age of 46. When I test blood ketones, I am .5 or higher and even seeing as high as 2.2 mmol. Prior to eating clean, my cycles were becoming irregular, but after 2 1/2 years of eating well my cycles are now back on track. I am having cyclical symptoms of fatigue, moodiness, and joint pain that is not in correlation with my cycle. It's seems to be happening every 3-4 months and I am no long losing inches while gaining scale weight. Could these symptoms be perimenopausal even with a regular cycle? I was very sad to let go of it. In my research on the effects of the lap band had on my body and eating so very little for so long, I suspect that it has taken a toll on my body and I’m now struggling to lose weight. When I consulted a medical weight loss clinic, they put me on a 1200 calorie, 120g carb diet and I gained weight in the two weeks I did it. That’s what led me to the ketogenic diet to help stave off the weight gain. Do you have any clinical experience or know about any research that looks at the use of keto for former lap banders looking for a way to keep their weight under control? I am definitely in ketosis when I test for them and I’m trying to avoid what my mother just went through with a double knee replacement. Does a ketogenic diet lead to abnormally high liver function test results? Is my liver overstressed by having to metabolize the extra dietary fat? Hi Jimmy and The Doc, A few years back, I had abnormally high liver function test results and my primary care physician was concerned that my numbers got worse because of keto. My AST and ALT were around 100 and they’re supposed to be no higher than 37. At the time she advised me to start eating more carbs and all my numbers that were bad came within the normal range. I started keto and fasting again in 2016 and lost 64 pounds after my third pregnancy. I’m concerned when I go back to see my doctor next month that my liver tests will be bad again. I’m hypothyroid, but I don’t have any known liver or gallbladder issues. Is it possible my liver was overstressed by having to metabolize the extra fat in my ketogenic diet? Hey Jimmy and Adam, I’ve been doing a ketogenic diet for three months as a physically active 27-year old man with 10% body fat. I’ve noticed that I feel so much better when I’m barely in ketosis at around 0.5 mmol—that’s my sweet spot where I have better mental clarity and energy. Anytime I’ve tried pushing blood ketones up by moderating protein more, my ketones rise to 2.0-5.0 mmol, but I don’t feel quite the same mental benefits as I do in the lower level of ketosis. So my question for you guys is this—should I strive for higher ketones and hope these kick in at some point to give me the benefits I desire or should I listen to my body and return to the lower level where I seem to thrive? Hi guys, I'm a recovered diabetic thanks to eating a low carb (but not necessarily ketogenic) diet. I used to be on the maximum daily Metformin dosage and took 130 units a day of long-acting insulin. I’m off all of those medications now and have an A1c of 5.3. I lost 60 pounds fairly easily on low-carb, but now it’s stalled out. My insulin levels are still very high, so I assume I’m still quite insulin resistant which is making the weight loss stubborn. I’ve added in some intermittent fasting of 18/6 as well as a few 24-hour fasts on the weekend where my blood ketone levels have reached about 1.0 mmol. My question for you guys is this: Does your experience show that a truly ketogenic diet lowers insulin levels? Some experts I’ve heard have said that research shows that insulin resistance is pretty much genetic which means you may not be able to drop it below some arbitrary level no matter what you do. Hey Guys, We love your podcast and anxiously await the new episodes each week. My wife and I have been low-carb for two years and keto most of the time for the past year. I travel about 18 days a month so it’s hard to maintain a state of nutritional ketosis all the time. We are 60 and 61 years old and we keep our blood ketone levels between 0.5 to 1.0 most of the time. Our question is about rising blood sugar levels after getting a Vitamin C IV from our functional medical doctor. My wife and I have both been getting this treatment lately because our bloodwork showed that we are low on Vitamin B and C even though we take supplements for both of them. After the first Vitamin C IV treatment, my blood glucose level rose to 172 and my wife’s went to 284 about 1 hour after the treatment. Our normal fasting blood glucose levels are between 77-95 depending on how we ate the day before. Needless to say, we became concerned this would have a negative impact on our insulin resistance. When we asked our doctor about it thinking that there must be some glucose in the IV bag, she said that it was just a reaction to the high dose vitamin C and that there was no glucose in it. We had already paid for two treatments each, so we did it again this week and this time our blood sugar went up to 144 and 128 respectively an hour later. Have you ever heard of this kind of reaction or do you think that there was some glucose in the IV? Our doctor knows that we practice low-carb and she is monitoring us because she’s still quite skeptical. I grew up in Orlando and ate a typical diet throughout the first 25 years of my life. I did cut out soda/pop in my teens and noticed that I lost some weight without any change to my physical activity but I was still overweight with a poor body composition. I stayed up late, ate crap junk food and drank in university and grad school. My weight peaked while on holiday in Germany as a graduation trip, 235 pounds at 6'2". I noticed new stretch marks, a marked drop in my energy levels and my clothes were tight! I was starting to get concerned about my future health and what could happen given my family medical history of diabetes, high blood pressure, heart attack, and Alzheimer's. I'm a scientist and I enjoy researching, so I went to work. With the limited nutritional knowledge I had and novice workout experience, I started reading various sources online. There are some great fitness subreddits that really helped me to understand what I needed to do. I started tracking my calories, lowered my fat intake, upped the protein and began a weight training and cardio regiment. After about 9 months of exercise and diet, my weight was about 220 pounds and I was significantly stronger in the gym, but I still had plenty of fat. This is where my story changes, I met Megan Christie (Ramos). After we had been dating for a few months, treating every trip to see each other as a vacation and eating junk, Megan explained the IDM program to me in detail and I was blown away. I began to see that by eating tons of processed/refined carbohydrates, I was only hurting myself. I also began fasting when I started eating an LCHF diet. Our cavemen ancestors certainly didn't get lucky and have a feast every day, so why do I need 3 square meals a day. I started out doing 24-hour fasts from dinner to dinner and was quickly surprised that it was much easier than I thought. After about a week, my body wasn't angry at me if at 1 pm I haven't eaten anything. I began to see weight fall off over the course of a few months. There’s no way I could get over 1,000 calories in one meal. Hey Jimmy and Megan, I have water fasted for 21 days and many shorter ones for spiritual reasons. I know what it's like to have zero hunger after 3 days of no food. I have plenty of energy to walk and lift light things, but I’m not ready for eight hours of hard labor during a water fast. When I’m in nutritional ketosis, I also have no hunger but I’m able to do heavy lifting and do a full eight hours of hard labor work without trouble. I'm guessing the body fat burns better when we eat fat and protein on eating days than when we water fast. But we have plenty of stored body fat to fuel us when we fast. So my question is this: why can't I do my normal routine that includes a very physically demanding job on just a water fast and have the same amount of energy as I experience in nutritional ketosis? It seems they are accomplishing the same thing and should work equally. What say you? Steve 2. Does the autophagy that fasting produces cause any pain? And can that process prevent weight loss? Hi Jimmy and Megan, I’m currently in Day 7 of my fast and for the past three days my lower body, including my calves, quads, glutes, hips, and lower back, has been so sore that it has prevented me from getting adequate sleep. Full disclosure, these areas in particular are stressed in my daily life already. But fasting seems to make it worse. Could this pain be associated with the autophagy that is happening while I’m fasting? Could this be why I haven’t lost any weight during my fast? I’m consuming clear homemade vegetable broth made from carrots, onions, parsley, and celery over the past couple of days, so maybe there in an insulin response to consuming this. Hi Jimmy and Megan, I had a gastric sleeve put on three years ago and I have been LCHF off and on for most of that time shifting to keto three months ago. I have been playing around with intermittent fasting since 2015 but decided to give an extended fast a go in 2017. My goal was to fast for as long as I could, but by Day 4 I started to feel nauseous and threw up after taking my salt in the morning. By Day 5 it got worse and I broke the fast. I actually felt so horrible that I called my doctor who recommended taking pain medications and rest. My blood pressure got very high during the fast and I felt so uncomfortable from this experience. Within a few days I felt better and was back to normal again. I don’t know why I reacted so badly, but was wondering if my gastric sleeve was the reason why my extended fasting attempt was an abysmal failure. Have you seen any patients in my situation who have had a similar experience? I really want to do extended fasts to experience the benefits that come from doing it. Thanks for you help!
