Gary Taubes on Rethinking Diabetes: Diet, Insulin, and the History Behind Low-Carb Treatment: Journalist Gary Taubes is author of “Rethinking Diabetes: What Science Reveals About Diet, Insulin, and Successful Treatments.” The book traces diabetes treatment history and argues that carbohydrate restriction was standard care from 1797 through the early 20th century until insulin therapy shifted practice toward drug-centered management and higher-carbohydrate diets. Taubes explains how insulin's discovery changed dietary priorities, how later technology (radioimmunoassay) revealed that most diabetes is type 2 with insulin resistance and high insulin rather than deficiency, and why giving more insulin can worsen weight gain. They discuss major trials (including ACCORD, ADVANCE, and Look AHEAD) that failed to show benefits from intensive drug-based glucose control, the influence of low-fat guidelines, Richard Bernstein's role in blood-glucose self-monitoring and low-carb control, controversies about obesity models, ketosis vs ketoacidosis, GLP-1 drugs, and LDL increases on ketogenic diets.
Send us Fan MailHow dietary fatty acids regulate the initiation, propagation, and resolution of inflammation.TOPICS DISCUSSED:Parenteral nutrition history: IV nutrition evolved from soybean oil alone in the 1960s to complex blends including fish oil as the biology of ω-3s became understood.Three phases of inflammation: Initiation, propagation (driven by ω-6–derived prostaglandins and leukotrienes), and active resolution (driven by ω-3-derived resolvins and protectins).Pro-resolving mediators: Resolution is an active biological process, not passive; EPA and DHA generate resolvins and protectins that actively terminate inflammation.Aspirin's ω-3 mechanism: Aspirin inhibits arachidonic acid metabolism but allows EPA/DHA to access COX-2, upregulating pro-resolution mediators — a likely underappreciated mechanism of action.Cell membrane competition: EPA and DHA physically displace arachidonic acid in membranes, shifting the balance of mediators produced and improving resolution capacity.Linoleic acid & EPA synthesis: High linoleic acid intake saturates shared desaturase enzymes, inhibiting the body's conversion of ALA to EPA — meaning reducing ω-6 can raise ω-3 levels without supplementation.Dose & timing of ω-3 incorporation: ~2g/day of EPA+DHA shifts the arachidonic acid-to-EPA ratio enough to measurably reduce prostaglandin E2 production; white blood cells reflect changes within a month.ABOUT THE GUEST: Philip Calder, PhD is Professor of Nutritional Immunology in the Faculty of Medicine at the University of Southampton. His research focuses on how dietary fatty acids — particularly omega-3s — modulate immune responses and inflammatory processes across clinical and healthy populations.RELATED EPISODE:M&M 200: Dietary Fats & Seed Oils in Inflammation, Colon Cancer & Chronic Disease | Tim Yeatman & Ganesh HaladeSupport the showHealth Products by M&M Partners:AquaTru: Water filtration devices that remove microplastics, metals, bacteria, and more from your drinking water. Triacylglycerol, the major (90%) dietary fat is processed specifically by 3 distinct lipases with 3 distinct compartments, resulting in digestion, absorption, reassembly, transport and uptake by tissues. During fasting, lipoprotein lipase mobilizes stored triacylglycerol in adipose tissue, releasing fatty acids to the bloodstream and providing an alternative energy source for several tissues.

Send us a textHow seasonal changes in light and dietary unsaturated fats affect circadian rhythms in mammals.Topics Discussed:Evolutionary context of circadian rhythms: All organisms have adapted to Earth's 24-hour day for survival, with internal clocks slightly offset and adjusted by environmental cues.Molecular clock mechanism: Involves a feedback loop where proteins turn on/off genes, lasting ~24 hours, regulated by phosphorylation and degradation for timing precision.Genetic variations in sleep: Families with mutations in clock genes like PER2 cause extreme morning lark behavior, altering protein stability and period length by hours.Light entrainment: Morning light shortens human clocks (average 24.2 hours) to match 24-hour days; seasonal day length changes require gradual adjustments.Food & metabolic links: Seasonal food scarcity/abundance affects clock via glucose and fatty acids competing for protein modifications, as shown in diabetic mouse models.Role of unsaturated fats: Paper finds MUFA/PUFA ratios in diet alter phosphorylation of clock proteins, speeding or slowing adaptation to winter/summer light cycles in mice.Modern environmental impacts: Artificial light extends “daytime” signals, while constant food access erases seasonal patterns, contributing to obesity and diabetes risks.Jet lag & adaptations: Sudden time shifts mimic seasonal experiments; high-sugar/fat intake may phenocopy genetic effects to aid adjustment, though not recommended for health.Practical Takeaways:Expose yourself to morning natural light to help synchronize your internal clock and improve daily energy.Consume main meals during daylight hours and avoid late-night eating to align with natural metabolic rhythms.Limit evening screen time to reduce artificial blue light disrupting sleep onset.Consider varying diet seasonally, favoring diverse, whole foods to mimic natural availability patterns for better health.About the guest: Louis Ptacek, MD is a neurologist and professor at the University of California, San Francisco. Send us a textOverview and alternative interpretation to the mainstream view on how dietary fat and cholesterol relate to cardiovascular disease.Episode Summary: Tucker Goodrich is an engineer by training who has become a prominent independent researcher and blogger on nutrition and metabolic health, focusing on the harms of seed oils and polyunsaturated fats.About the guest: Nick Jikomes and Tucker Goodrich explore atherosclerosis and heart disease, critiquing the standard model that blames high LDL cholesterol while highlighting how oxidized LDL—driven by dietary linoleic acid from seed oils—plays a key role in plaque formation and inflammation; they discuss historical shifts in heart disease rates, genetic factors like familial hypercholesterolemia, the limitations of animal studies, and why reducing seed oil intake could prevent issues more effectively than just lowering cholesterol.Discussion Points:Atherosclerosis involves plaque buildup in arteries, often leading to heart attacks, but plaques contain oxidized fats and cholesterol, not just native cholesterol.Dietary cholesterol has little impact on blood levels or heart disease in humans, unlike in rabbits used in many studies.High LDL may not be inherently bad; oxidized LDL from polyunsaturated fatty acids (PUFAs) like linoleic acid causes macrophages to overeat and form harmful foam cells.Familial hypercholesterolemia patients only show higher heart disease rates in modern, industrial diets high in seed oils, not historically.Populations like the Kitavans and Tsimané have high apoB but no heart disease on traditional diets low in industrial foods.Fried foods are oxidized seed oils, explaining why they're unhealthy despite omega-6 fats being labeled "heart-healthy."Omega-3 fats can displace omega-6 in cells, reducing oxidation risk.Reference Papers:Witztum & Steinberg (1991)Boren et al. Partial versions are available elsewhere. Transcript and other information on Substack.Episode Summary: Dr. Matthew Budoff discusses cholesterol, heart disease, and his study on the ketogenic diet's impact on lean, metabolically healthy individuals with high LDL cholesterol. He explains LDL, HDL, and triglycerides, debunking myths about their direct link to heart disease, and emphasizes the importance of coronary calcium scans to assess plaque buildup. Budoff also covers statins, dietary cholesterol, and personalized heart health strategies.Key Takeaways:LDL cholesterol is not a definitive predictor of heart disease; plaque buildup, assessed via coronary calcium scans, is a better indicator.Lean metabolically healthy people on a ketogenic diet may have high LDL without increased plaque progression after one year.Coronary calcium scans, costing ~$100, are recommended for men around age 40 and women around 45-50 to evaluate heart disease risk.Statins effectively lower LDL and can reverse soft plaque, but may be overprescribed for those without plaque buildup.Dietary cholesterol has minimal impact on blood cholesterol, as the liver produces ~85% of it.Ketogenic diet can aid weight loss & diabetes control but may cause high LDL in some lean individuals, known as lean mass hyper-responders.Plaque progression depends more on existing plaque than LDL levels in metabolically healthy ketogenic diet followers.Heart health varies widely due to genetics and other unknown factors, underscoring the need for personalized assessments.Related episode:M&M #158: Ketosis & Ketogenic Diet: Brain & Mental Health, Metabolism, Diet & Exercise, Cancer, Diabetes | Dominic D'AgoSupport the showAll episodes, show notes, transcripts, etc. at the M&M Substack Affiliates: Lumen device to optimize your metabolism for weight loss or athletic performance. In the realm of nutrition science, few topics have sparked as much debate and evolution in understanding as the impact of dietary fat on cardiovascular health. Dr. Alice Lichtenstein, a renowned expert in nutrition and cardiovascular disease, brings clarity to this complex field. Her extensive research at several prestigious institutions has been pivotal in shaping contemporary dietary guidelines. Dr. Lichtenstein delves into the journey from early observational studies that linked high-cholesterol diets to cardiovascular risk to more nuanced insights emphasizing the types of fats consumed. The discussion highlights the critical shift from merely reducing total fat intake to replacing saturated fats with unsaturated fats, reflecting the latest evidence-based recommendations. Furthermore, the conversation addresses the variability in individual responses to dietary changes and the ongoing debate over the most accurate biomarkers for assessing cardiovascular risk. Dr. Lichtenstein also tackles the pervasive issue of nutrition misinformation and underscores the necessity of robust, evidence-based public health guidelines. This episode offers a comprehensive exploration of whole dietary patterns, the importance of examining food choices in context, and the role of public health policy in fostering healthier eating environments. Join us to gain a deeper understanding of how dietary fat influences health and the critical considerations for future nutrition research and recommendations. In episode 529 of Sigma Nutrition Radio, we discuss all things related to cholesterol, atherosclerosis, heart disease, and dietary fat. The discussion extensively covers the impact of LDL cholesterol and ApoB on atherosclerotic plaque progression, cardiovascular events, and the benefits of early LDL lowering interventions, particularly through the use of PCSK9 inhibitors alongside statins. We address misconceptions prevalent in the low-carb and carnivore communities, dissect claims regarding saturated fats, and elucidate why atherosclerosis predominantly affects arteries and not veins. The episode sheds light on the nuanced mechanisms of cholesterol metabolism, the significance of different lipid particles, and the limited dietary impact on LP(a) levels. Comprehensive evidence-based insights and practical recommendations for dietary patterns and cardiovascular health interventions are provided, offering clarity on these complex topics. Atherosclerosis, the build-up of plaque in arteries, is a key factor in the development of cardiovascular diseases, including heart attacks and strokes. Traditionally, dietary fat has been implicated in contributing to atherosclerosis, with a focus on reducing overall fat intake. However, recent studies have shifted the focus towards the quality of dietary fats rather than their quantity. Not all fats are created equal, and researchers are now paying closer attention to the types of fats consumed in the diet. Saturated fats, commonly found in animal products and some tropical oils, have long been associated with increased cholesterol levels and atherosclerosis. On the other hand, unsaturated fats, including monounsaturated and polyunsaturated fats found in olive oil, nuts, and fish, have been linked to potential cardiovascular benefits. Research suggests that replacing saturated fats with unsaturated fats may have a positive impact on blood lipid profiles and reduce the risk of atherosclerosis. Additionally, genetic factors and individual responses to different fats may play a role in how dietary fats impact atherosclerosis risk. In this episode, Dr. Jacob Christensen discusses the research in this area and some conclusions about whether we can say dietary fat quality causally increases atherosclerotic cardiovascular disease (ASCVD) risk. This includes looking at the relationship between low-density lipoprotein (LDL) particles and ASCVD, the link between dietary fat quality and LDL particles, and then finally the relationship between dietary fat quality, LDL particles, and ASCVD. About the Guest: Jacob J. Christensen is a clinical dietitian and researcher at University of Oslo. His research interests include cardiovascular diseases, lipid metabolism, nutrition, genomics and data science.

The MASScast Iron Culture episodes are back! Today Dr. Eric Trexler joins us to answer the murky question: “What is an evidence-based guideline for a minimum dietary fat intake?” It's easy to find guidelines and minimums for various circumstances and goals for protein, and while there is some debate, the same could be said for carbohydrate. However, a minimum intake for fat has been elusive. Fortunately, Dr. Trexler has been thinking deeply about this topic and wrote an extensive article on the topic in our upcoming issue of The MASS Research Review, and he's here to discuss his conclusions in depth with the CULT. Make sure to tune in, as this information can inform best practices in various situations, as the answer isn't a simple black and white one.
We have an exciting announcement at Iron Culture, we are combining forces with MASS, Monthly Applications in Strength Sport to bring you regular “MASScast” episodes where we will be joined by one or more of the expert MASS reviewers to talk science. In this inaugural MASScast episode we're joined by Dr. Eric Trexler to discuss surprising recent data that suggests basal metabolic rates have been decreasing at the population level over recent decades. Is this true? And if so, what's causing it? In addition, we discuss the continuing confusion regarding high versus low-carb diets. Where is the cut-off for being high or low carb, and when is each a good or bad approach? Listen in to learn what the latest data indicates and how to apply it!
