Like anything else with the human body, nothing is as straightforward as you'd like it to be. So the theory is, and again, it depends on why you had your thyroid taken out. Did you have your thyroid taken out because you have Graves' disease? Did you have your thyroid taken out because you have Hashimoto's? Did you have it completely taken out or partially taken out? I'm assuming this question refers to completely taken out. And did you have it out because you had cancer? Okay. So basically let's go to Hashimoto's and Graves', and Graves' is a very small part of the population, so I'm going to talk mostly about Hashimoto's. So when you have your thyroid out, and I've watched some thyroid surgeries years ago and asked some questions, and it seems like a lot of times they just take out maybe part of the thyroid in cancer. But in Hashimoto's and in Graves', they're usually going to take the whole thing out. And so the theory is because of where the medical model is at this point in time in 2022, okay, they're still not addressing Hashimoto's as an autoimmune problem. They're still addressing it as a thyroid problem, which leads to the theory of I'm going to take it out and I'm going to be able to control your thyroid hormone levels by giving you the right thyroid medication. And that alone is a crapshoot, figuring out what is the right thyroid medication for that particular person. That's a whole other subject that I have a presentation on online. And so it gets a little complex because of their model. All right? So two things. One thing is they don't always get all the thyroid tissue out, and it's attached to certain structures where it can alter things if you mess up. And the other thing is they're treating it as though it's a thyroid problem and not an autoimmune problem. So if you're still not aware that there's 40 triggers or more now, and I know I've got one online that says 39, but there's more since then. So if you're not aware of that and you're not removing those triggers, those triggers are still there. Very seldom does a Hashimoto's patient have one autoimmune problem. They usually have Hashimoto's and autoimmune gastritis or Hashimoto's and arthritis or they have Hashimoto's and cerebellar antibodies and on and on and on and on and on. So in several different ways, the immune response is still a problem. And if you have thyroid tissue that is still remaining, you will have thyroid tissue that is still getting attacked. And have I had these patients come in? Oh, my God, probably a couple hundred over the period of time I've been doing this. And so, I mean, it's something I've seen a lot. What began as a routine physical for Kelley Pratt turned into a diagnosis of medullary thyroid cancer, a rare form of the disease. This week, the most high-yield topic is hyper and hypothyroidism. You’ll see a lot of your test questions coming from this subject.I’m gonna talk about hyperthyroidism first, and then I’ll cover hypothyroidism.So, What is hyperthyroidism? It’s when your body makes too much of the thyroid hormones T3 and T4. I’m not sure how much of the A&P you remember from forever ago, so to review how T3 & T4 are made, your hypothalamus in your brain releases Thyroid Releasing Hormone; this makes the Pituitary gland release TSH, or Thyroid Stimulating Hormone, and that makes the thyroid, the little butterfly-shaped gland on your neck, produce T3 and T4.In both hyper and hypothyroidism, the main causes are autoimmune disorders. The autoimmune disorder that causes hyperthyroidism is called Graves disease. With Graves disease, the patient will have a goiter, or an enlarged thyroid gland, that’s sometimes so big, you can see it from the door of their room. They’ll also have the characteristic bulging eyes, or exophthalmos. This will also be very noticeable, cause it’ll look like they are staring at you in a creepy way, and their eyes are protruding out of the sockets. Usually, our upper eyelids cover the top half of our iris. But in a patient with exophthalmos, you’ll be able to see the white of their eyeball above their iris, below their upper eyelid. So, if the patient has green eyes, you’d be able to see some white between the green and their upper eyelid. Sometimes the exophthalmos will be so bad that they can’t even close their eyelids all the way.Hypothyroidism is when the thyroid gland doesn’t make enough T3 and T4. So just the opposite of hyperthyroidism. The most common cause is the autoimmune disorder, Hashimoto’s thyroiditis. I remember that Hashimoto’s causes hypothyroidism like hypO, HashimOto. Hashimotos is when your body makes antibodies that destroy your thyroid gland, which decreases T3 and T4. This causes the gland to grow bigger to compensate, which creates a goiter. So you can have a goiter with both hyper and hypothyroidism. Another cause of hypothyroidism is simply not getting enough iodine in your diet, but this isn’t an issue here in the US, because we put iodine into our salt. Also, if someone is getting treated for hyperthyroidism, like they get a thyroidectomy, or radioactive iodine ablation, those can both overcorrect to lead to hypothyroidism. Even if they are just taking antithyroid medication, and they take too much.

Why do we do thyroidectomies? We're going to be talking about that tomorrow. But a thyroidectomy takes your thyroid out. The problem is that the thyroidectomy doesn't address the root cause of the problem. The root cause of the problem is an overactive immune response. It's an autoimmune problem. That's really the root cause of the problem. The thyroidectomy in this particular light usually occurs when you have driven your endocrinologist batty because they can't find a drug that works for you, and you're still having a significant number of symptoms. What I'm seeing today in the thyroid world is the doctors have now diagnosed you with Hashimoto's. They've tried all kinds of medications, none of them have worked. So it's like, "Let's just take it out, or radiate it." By the way, this could also go along with, does radiation a cure? And the answer is no. Not only does it not cure it, their position is, "We'll take it out. We'll take it out and we'll give you enough thyroid hormone. We'll fine tune it just right so that it works," which the vast majority of time it doesn't. You got number one, you have somebody who's giving you exogenous hormones. They're giving you levothyroxine or Synthroid, or maybe one of the natural ones, but still it's very difficult to dose that properly, number one. Number two, it is very frequent that because of the positioning of the thyroid, it isn't taken out completely. And so if you still have thyroid tissue there and the tissue was getting attacked before you got the surgery, you're still going to be getting thyroid attacks from the immune system after the surgery, and you're still going to end up putting out intermittently excessive amounts of T4. Not as much as if you had your whole thyroid but it's still there. It's still confusing. It's still not allowing that person to maybe dose your thyroid that they're going to give you the way that they think they're going to dose it. I think one day they're going to figure this out. But for right now there's a lot of thyroids coming out. There's a lot thyroids coming out. I'm amazed at how many people put up their hand and go, "No, give me radiation. Radiate me." And that's crazy. They walk into the room with a hazmat suit on and then they tell you don't go near anybody for the next several weeks or whatever it is. That's something that I would avoid. But it will not cure it. And there's another aspect to this too. Let's just even theorize that they got it all out and let's theorize that they could somehow figure out the absolute perfect dosing of the exact proper medication for you, which is no small task if can even be done. http://powerhealthtalk.com http://powerhealthreno.com Martin P. If you have had your thyroid removed with a thyroidectomy then there are 5 VERY important things that YOU should know! Once your thyroid is removed your body is changed forever and if you don't understand what that means for you then you may end up feeling poorly or symptomatic for a long time. This information ALSO applies to those people who have had their thyroid damaged from radioactive iodine ablation (by the way) and also those people who have had only HALF of their thyroid removed through something called a hemithyroidectomy. Here are 5 things to know if your thyroid has been removed: #1. You are now HYPOTHYROID. It doesn't matter WHY your thyroid was removed but once it has been removed you are now effectively and functionally HYPOTHYROID. This means that ALL of the information I talk about (directed at hypothyroidism) also applies to YOU! #2. T4 isn't enough by itself for you. Medical research has shown that T4 (such as Synthroid or levothyroxine) is not enough to NORMALIZE thyroid function in people who have had their thyroid removed. #3. You will probably gain weight and weight loss will be very difficult. On average, people who have had their thyroid removed gain 20 pounds. In addition, weight loss is more difficult afterward. #4. Thyroid supplements CAN and should be used. There is still a place for thyroid supplements (which can be helpful) whether you have a thyroid or not! #5. You will be on medication for LIFE. Once your thyroid is removed you will be on thyroid medication for the rest of your life! The only exception is if you've had half of your thyroid removed (it isn't always necessary in this case). I’m looking forward to sharing with you some of our community’s questions that have come in over the past few weeks… Let’s get started! Laura: Hello Dr. Cabral, I'm hoping you can help me. I get this lip rash every now and then, starting in 2016. Each time I was told it was contact dermatitis and given predisone, and I was fine. Since then, I've been very careful of what I put on my face. It would still flare up minimally in 2017, so I as prescribed Desonide cream, which I don't like putting on my lips. I have also been using hydrocoristone ointment as a lip balm since 2016, but not in the past few months. 2 weeks ago, this rash flared up badly again, and I refuse to take any sort of medication, or any topical steroids, as I want to get over it naturally. It's been going on for 2 weeks and I'm at a loss. I had a allergy patch testing done - no results. I am vegan (since 2017) and gluten free (since 2017). I don't eat refined sugar and I'm heavily plant based. My lips are incredibly itchy, almost unbearably, several times during the day. I think it starts overnight/in the morning, I always know it's going to be bad when I wake up with swollen lips. I haven't changed anything in months (skincare, detergent, etc., and everything I use is natural) so I'm pretty sure it's not a contact allergy. They're itchy and dry constantly, so much so that it's hard to eat, because they itch when anything, even food, touches them. The only thing that's been different in my life the last 2 weeks is that I've been taking it really easy in my daily Bikram yoga practice due to a shoulder overuse injury (nothing serious, but I've been unable to do the normal backbending, chest/neck stretching postures I normally do). But it definitely starts with the swollen lips in the morning, and sometimes I notice it in the middle of the night. It's not my pillowcase or laundry detergent. What could be causing my lips to swell overnight? Ashley: Hi Dr.Cabral, I have taken the OAT test twice and I've been working with a naturopath for the past year trying to get rid of all of my bacteria imbalances. I was diagnosed with Deep Infiltrating Endometriosis almost 1 year ago after trying for a baby for a year prior, and have this year undergone fertility treatment (2 x ICSI cycles -as my husband has 26% fragmented DNA in his sperm + 3 months of Zolodex) I'm now on a contraceptive pill with no breaks until we can (afford to) go ahead with embryo transfers (we have 4 day 5 blastocysts in the freezer). After trying for a while my hormones were tested on day 21 and the GP said my results were normal. This lead to a hycosy which showed I had clear fallopian tubes, which lead to an MRI that eventually showed i have DI Endo and involvement in the bowel. I was told by the first specialist to have surgery to have it all removed and I'd also need prt of my bowel removed. I saw a second professor who gave an alternate option without surgery for now. I have had acne skin problems forever and am realising a link between poor gut health and digestion issues and believe this is all linked to the fertility issues we've been having. I have always been a very bloated person with poor digestion and tummy issues (constipation, stomach sensitivity, cramping etc). I had a lot of antibiotics as a child and prolonged periods of antibiotics for skin treatment in my teens, recurrent tonsilitis and glandular fever and was always the sick kid. I also suffer from what I believe to be Myofascial Pain syndrome following an accident I had in 2015 which I have since treated with massage, regular physio dry needling (twice weekly) and clinical pilates every day. It affects my quality of life and ability to work. I went vegan at the start of the year and initially felt great but recently i feel so fatigued and flat and need to nap during the day and struggle to get out of bed before 9am - It's been hard to know if it's been the diet, or the medication I have been taking for fertility treatment but I don't feel like I'm thriving despite my healthy diet and want my body in a great place for when we go ahead with the transfers. I recently saw a naturopath and have gone ahead with a bacteria/culture test (stool) and a Ubiome gut test which I'm yet to get the results - but upon listening to your podcasts I'm not sure this is comprehensive enough for me to really be informed moving forward. What would you recommend I do to move forward? My goals are to reduce or eliminate my chronic pain, fall pregnant and feel energised. I know your big on testing and data so we can make informed decisions but I'm not sure which other tests are necessary for me. My naturopath put me on gluten free diet for my endo and curcumin tablets to reduce inflammation. I also take folic acid, Wellco super elixir green powder, wellco chocolate protein powder, the beauty chef glow powder and collagen tonic and their sleep powder. I have berries, hemp hearts, flax seeds, chia seeds, coconut oil, raspberries, blueberries, bananas, greens and generally a wide variety of nuts, seeds, pulses, vegetables and fruits in my diet. I'm also eliminating chemicals and toxins where possible, shop at the local farmers market, cleared all chemicals from my house, reducing plastics, use organic deodorant, toothpaste you name it! This video is all about why thyroidectomy patients need T3 thyroid medication. Thyroidectomy is a procedure in which your thyroid gland is completely removed. After you undergo this procedure you will be required to take thyroid medication for life. One problem that thyroidectomy patients face is persistent symptoms such as weight gain, brain fog, constipation, and fatigue after their procedure and even after taking thyroid medication. In this video, I highlight the importance of taking thyroid medication in this subgroup of patients and why the standard approach of using levothyroxine is not sufficient. There are new studies which show that approximately up to 20% of patients who don't have their thyroid is not able to achieve normal free T3 or free T4 levels by taking thyroid medication alone. And this makes sense considering that we know the thyroid produces both T4 and T3 when it is working. So why then do we only provide the body with T4 medication after it has been removed? Doctors believe that giving the body T4 is sufficient because they feel that the body will be able to activate T4 on its own. This study shows that this may not be the case for roughly 1 in 5 thyroidectomy patients. The net result is that you may need to take T3 medication in addition to your T4 medication if you want to normalize thyroid function. This video is all thyroid cancer treatments including what treatments are available if you have thyroid cancer, the consequences of these therapies on your life, and we'll also discuss some important factors which help determine your prognosis. It's important to realize, though, that the majority of thyroid cancer cases are caused by the least aggressive form of thyroid cancer. This, combined with the fact that the most common therapy to treat thyroid cancer is the removal of the thyroid gland has helped increase the 5-year survival rate of thyroid cancer to around 97 to 98%. When it comes to thyroid cancer you may not have a choice as to which therapy is ideal for you, but it still helps to understand each treatment and how it impacts your body. Treatment options available for thyroid cancer include: #1. Surgery: - By surgery, I am referring to a procedure known as a complete thyroidectomy. This surgery results in the entire removal of your thyroid gland (well close to the entire amount) and it is the most common treatment for thyroid cancer. Removing your thyroid, though, can sometimes cause side effects afterward such as weight gain or fatigue if you are not dosed appropriately. #2. Radioactive iodine ablation: - Radioactive iodine is sometimes combined with a thyroidectomy to help kill off any tissue which is left over after your surgery. Radioactive iodine is a procedure which specifically targets and destroys thyroid gland tissue. It's helpful to use in conjunction with surgery because sometimes thyroid gland tissue can exist outside of the normal anatomy. Using RAI can help kill off extra tissue especially if cancer has spread. #3. Chemotherapy and radiation: - Chemotherapy isn't used very often for thyroid cancer because most cancers can be treated with surgery and with RAI. But, some cases of aggressive thyroid cancer may require the use of chemotherapy and radiation therapy. #4. TSH Suppression: - Lastly, another technique used to treat thyroid cancer is the use of TSH suppressive therapy. The goal here is to take high enough doses of thyroid hormone to suppress the natural production of TSH or thyroid stimulating hormone. This hormone is stimulating to thyroid gland tissue and can theoretically stimulate thyroid cancer cells as well. You can suppress the normal production of TSH by taking high doses of thyroid medication and this is sometimes used for patients after surgery. Each person is different so the exact treatments that you need for your cancer will depend on the type of cancer you have, how aggressive it is, the stage at which it was diagnosed, your age and other important factors. Today is lesson #12 and it's all about what causes thyroid disease. Why does it matter what causes thyroid disease? If you can find the cause you can better target your treatment. Most doctors are only concerned with lab tests and not the causes of those problems, which means you may be unnecessarily treated for something that could be reversible. Thyroid Dysfunction vs Hypothyroidism: What's the difference? Thyroid dysfunction = thyroid hormone is not working properly at the cellular level but your body can still produce it. These are the top causes of thyroid dysfunction (remember they may be reversible): #1. Obesity - Just being overweight is enough to put strain on your thyroid (even as little as 10 pounds) #2. Calorie restriction or diets - as little as 1,200 calories per day x21 days #3. Nutrient deficiencies - Such as zinc, selenium and iodine - these are all required for your body to convert T4 into T3 #4. Insulin resistance/Leptin resistance - $5. Chronic illness & Infections - High blood pressure, chronic back pain, fibromyalgia, high cholesterol, heart disease and so on #6. Prescription medications - Beta blockers, diabetic mes, anti seizure meds, narcotics, antidepressants, heart medications and more (these are common medications!) #7. Inflammatory states - gut dysfunction, yeast or fungal overgrowth syndromes Thyroid dysfunction should be differentiated from hypothyroidism: Hypothyroidism = Your body is not producing ENOUGH thyroid hormone. The top causes of hypothyroidism include: #1. Hashimoto’s thyroiditis = probably main cause (triggers include stress, infections, gut dysfunction, etc. - see other video) #2. Treatment for hyperthyroidism - Thyroidectomy and RAI #3. Pituitary issues/hypothalamic issues - extreme stress, prolactinoma, trauma #4. Iodine deficiency #6. Pregnancy & Lactation = increased demand on thyroid hormone #7. Congenital = being born with issues How should you focus your treatment? Remember that thyroid dysfunction is usually reversible while hypothyroidism may or may not be. Both conditions may present with similar lab results (high TSH, low free t3, low free t4, high reverse T3) which can make diagnosis difficult. And both may benefit from thyroid medication (though those with thyroid dysfunction tend to only need it temporarily). Hey guys! Antonio Bianco, MD, is the Charles Arthur Weaver Professor of Cancer Research in the Department of Internal Medicine. He is the president of Rush University Medical Group and vice dean for clinical affairs in Rush Medical College. Bianco came to Rush from the University of Miami Health System, where he served as professor of medicine and chief of the Division of Endocrinology, Diabetes and Metabolism. He has more than 30 years of experience in the thyroid field. He has been recognized with a number of national and international awards and membership in prestigious medical societies. A well-rounded investigator in the field of thyroid disease, Bianco led two American Thyroid Association task forces: one charged with drafting guidelines for thyroid research (as chair) and another responsible for developing guidelines for the treatment of hypothyroidism (co-chair). Bianco’s research interests include the cellular and molecular physiology of the enzymes that control thyroid hormone action (the iodothyronine deiodinases). He has contributed approximately 250 papers, book chapters and review articles in this field, and has lectured extensively both nationally and internationally. Recently, he has focused on aspects of the deiodination pathway that interfere with treatment of hypothyroid patients, a disease that affects more than 10 million Americans. He directs an NIH-funded research laboratory where he has mentored almost 40 graduate students and postdoctoral fellows. This episode includes the following topics: Thyroid produces thyroxin of T4. Ideally 3 or 4 hours after taking the T3 tablet Hypothyroid-like symptoms could be depression There is greater likelihood of depression symptoms for those taking Nearly 5% of the U.S. population takes T4 or Levothyroxine, as revealed by the NHANE survey. This means 10 – 15 million Americans. In this episode, we visit with Dr. hypothyroidism, a pioneer in prescribing active surveillance in place of immediate surgery. Dr. Miyauchi is President and COO of Kuma Hospital, Center for Excellence in Thyroid Care, Kobe, Japan. He is an endocrine surgeon, especially interested in thyroid and parathyroid diseases. He earned his MD and PhD at Osaka University Medical School in 1970 and 1978, respectively. Thyroidectomy: complications; Thyroid surgery: RLN monitoring
Weight loss after thyroidectomy can be almost impossible unless you know exactly what to look for and how to treat it. It's considered "normal" to gain 20 pounds after thyroidectomy and Doctors tell patients that this is normal and in the same breath then say that the thyroid isn't causing the weight gain. How can this be? It is possible to lose weight after thyroidectomy but it takes a very different approach. Eating less and exercising more will NOT lead to significant weight loss if you've had a thyroidectomy or your thyroid ablated with radioactive iodine. Instead you need to be focusing on hormone imbalances that lead to weight gain. I've also included a diet guide to follow in addition to the following 5 tips which I explain in detail in the video. 5 Tips for weight loss after thyroidectomy: 1. Get on the right type and dose of thyroid medication - for most patients this means a combination of T3 and T4. So medications like cytomel or liothyronine in addition to levothyroxine/synthroid or the use of natural desiccated thyroid (NDT) will most likely be necessary. 2. Increase your free T3 - you need to be doing whatever is in your power to naturally increase your free T3. The more t3 you can in your system the higher your metabolism will be and the more weight you will lose. 3. Balance all other hormones in the body - I list several hormones, but the main ones I want you to focus on are insulin resistance and leptin resistance. Until these 2 are treated it will be almost impossible for you to lose weight. 4. Be strict with your diet but don't restrict calories - one of the worst things you can do is restrict your calories down to 1,000-1,200 calories per day. This will cause high reverse T3 levels and further reduce your metabolism. Instead be strict with WHAT you put in your mouth but don't restrict your calories. 5. When you have energy make sure you make exercise count - that means a combination of low intensity daily plus high intensity 1-3x per week. The surgery proceeds in a stepwise fashion, identifying the cancer-invaded nerve and verifying preservation of the contralateral only functioning nerve postoperatively. Total thyroidectomy, bilateral level VI and VII lymphadenectomies (bilateral paratracheal dissection), and nerve reanastomosis are demonstrated.
