Trauma is a universal experience, and our approach as health care providers to trauma should be universal as well. That's my main take-home point after learning from our three guests today when talking about trauma-informed care, an approach that highlights key principles including safety, trustworthiness, peer support, collaboration, empowerment, and cultural sensitivity. With that said, there is so much more that I learned from our guests for this trauma-informed care podcast. Our guests include Mariah Robertson, Kate Duchowny, and Ashwin Kotwal. Mariah discussed her JAGS paper on applying a trauma-informed approach to home visits. Kate and Ashwin talked about their research on the prevalence of lifetime trauma and its association with physical and psychosocial health among adults at the end of life. We also explored several questions with them, including how to define trauma, its prevalence in older adults, the impact of past traumatic experiences, the potential triggers of trauma screening, and the application of trauma-informed principles in clinical practice. A lot of things could go wrong as there are a lot of information breaks during the course of a discharge. There needs to be a broader action plan and a continual set of information which can be referred to in order to prevent mistakes happening down the line. Anything you can do from a product standpoint that helps alleviate some of the challenges of the day to day is valuable. Value based care is going to become more and more important over the next decade, as healthcare payment models need to continue to evolve to support the wave of care that's going to be required. The staffing shortage is caused by healthcare systems not being able to provide enough value such that payment models can start evolving and people could get paid more. Home-based healthcare is the stuff of tomorrow – literally. Tomorrow Health just closed a $60M Series B to grow their tech infrastructure biz into what CEO Vijay Kedar hopes will ultimately streamline and optimize how home health is ordered, delivered and paid for. This is the software that could be the thing that not only gets patients into home-based set-ups faster (vastly improving upon the up-to-90-minutes it currently takes providers to set-up home care for patients) but also creates a system for all stakeholders to track and monitor patient outcomes with an aim at the much larger, long-term opportunity: to realign incentives on value instead of fee-for-service. Vijay came out of Oscar Health, meaning there is definitely a payer slant to the way this software is designed and deployed. Payers are Tomorrow Health's clients, and it offers them a way to organize (or completely create, in some cases) home care networks out of the hundreds of different small, local market suppliers and providers that get medical equipment, skilled and unskilled services, and other in-home care elements to the doorsteps of the patients who need them. Their software alleviates a payer's pain of scaling this concept in every market while also providing a way to track what's happening with the patient and build a “bridge” back into the health system that's leading the patient care team With so many other players working in the home-health space – everyone from retail players like Walgreens/CareCentrix and Best Buy/Current Health to upstarts like Signify Health, Honor, and more – how will this tech stack approach play out against others that are one-stop-shops with frontline care and coordination layered on top? Some funding related to health care that was originally in the plan ended up on the cutting room floor.In particular, $400 billion in federal Medicaid funding for home and community-based long term care services were removed from the plan. The earmarked funds for Medicaid still could be passed through a budget reconciliation act but that remains to be seen. This has implications for the health and well-being of many Americans including low-income individuals 65 and older, people with disabilities, and home-based care workers. COVID-19 ushered in an overwhelmingly stronger preference for patients to receive care at home rather than in a facility. A national survey by Transcend Strategy Group revealed that 60 percent of family healthcare decision makers are now more confident that quality care can be provided at home versus 33 percent feeling more confident in the quality of care at a facility. This preference, along with the growing demand for care as the huge demographic of Baby Boomers continues to age, is opening new opportunities and challenges for providers of home-based care.In response, Bill Dombi – Doctor of Law and President of the National Association for Home Care & Hospice (NAHC) – has said that providers of home-based care must demonstrate “a willingness to not only embrace change, but to lead it.” Through this important discussion with host Stan Massey of Transcend, Bill offers his insights to three key areas of necessary change: 1) Widening the understanding of what home-based care can provide; 2) The role of technology and other innovations in meeting the demands of home-based care; and 3) Developing a bigger and better workforce to handle the growing volume of home-based patients.Bill has served at NAHC for more than 30 years, including his role as president since 2017. But we're asking the government to include us in development, so that we will be working together." - Rosemary Bemba language and English translation [English Translation] My name is Rosemary. When I first felt sick, it was 2013 and I went to Mungwi Baptist Clinic. When I reached Mungwi, I visited the clinic officer who tested me and found that I'm HIV positive. Then I came back from Mungwi and went straight to the Malole Clinic where Madam Marjorie attended to me. She started me on treatment on the 13th of March, 2013. I have received medicine in Malole from that time until now, 2019. We have an organization in my village that I am active in, we encourage other people within the community, those who fear being open, to be open. So I started encouraging people in the community because they are not open. From that time, I was accompanied by Mary Chileshe. We make sure that we also encourage other people who are already sick. That's how we continue encouraging other members. I'm also a member of Home Based Care, so it's easier for me to work in my village. We've continued that program from 2013 up to now, in 2019. We go around the village to make sure that we're with people. We started working with Clara on malnutrition because some children are not looking healthy. Then Clara went back to America and from there, we continued with Rebecca and the same program. After working together with Rebecca, then Sam came. Now he teaches us fish farming. But people are still feeling shy because of stigma. Some people are not attending the meetings because they are fearful. So we have continued with Home Based Care in the village. Today, Adam visited to help us learn more about fish farming which can help us, showing us how to care for fish. We are going to harvest in May or June. After harvesting our fish, we're going to continue, but we're asking the government to include us in development so that we will be working together. We need the government to make sure they're supporting us so that we can continue, because some of the children - their parents have died, so they are orphans. So they need to help. Some children have lost both parents, some one. They need to be helped. Mrs. Mary has continued to help us by collecting reports and taking them to Mrs. Chewe at the clinic and she supports those in the community who have fear. Ep. 105: Adam Gould is the Co-Founder and Executive Director of eduKenya (http://www.edukenya.org/), which seeks to empower children and their families with the opportunity to break the chronic cycle of poverty and transform their lives and communities.  Adam co-founded eduKenya with Bob Kikuyu in 2010 as an extension of the Kenya Children Project, an outreach to children and families in Mathare. The Kenya Children Project was established in 2006 as a partnership with Believers Centre Mathare Church and Life in Abundance to expand on the church’s childcare program by developing a fully functional primary school. The Kwa Watoto School – which means “for the children” in Swahili – was the result. From 2007 – 2010, the Kwa Watoto School rapidly expanded, adding a new grade level each year. The Skills Training and Home Based Care programs were initiated to benefit students’ parents and other community members by teaching income generating skills and providing support for people living with HIV/AIDS and other life threatening conditions. eduKenya was founded to build upon the success of the Kenya Children Project by replicating the model in other areas of the slum for the benefit of countless other uneducated children and families living in abject poverty.
