Recorded live from the Providence St. Vincent Medical Center strike line on 1/25/25, strike day 16. Special thanks to the Oregon Nurses Association. On January 10, 2025, nearly 5,000 healthcare professionals began an open-ended strike, calling on Providence Health & Services to put their patients before profits by offering a fair contract to frontline caregivers. The strike includes doctors, nurses, advanced practice providers, and midwives from eight Providence Oregon hospitals and six clinics. At issue: Providence's alleged unfair labor practices, dangerously understaffed hospitals, the ability to spend more time with patients, employee healthcare, and competitive wages and benefits to be able to recruit and retain more caregivers. Medical aid in dying is currently legal in ten states and Washington, D.C.We've brought together an activist and a doctor to dive into the topic:Dr. Ira Byock is a physician, author, and advocate for palliative care — the medical practice of treating people with serious, complex, and terminal illnesses. Ira is the founder of the Providence St. Joseph Health Institute for Human Caring and is an emeritus professor of medicine and professor of community health and family medicine at the Geisel School of Medicine at Dartmouth College.Kim Callinan is an end-of-life leader and advocate in the field of medical aid in dying. Listen in as we discuss promoting Vaginal Nulliparous Term Singleton Vertex (NTSV) birth to decrease the risk of maternal morbidity and mortality. Dr. Lagrew agrees with a past president of the Society for Maternal-Fetal Medicine, Vincenzo Berghella, who said, “Physicians do need to balance risks and benefits, and for some clinical conditions, cesarean is definitely the best mode of delivery. But for most (NTSV) pregnancies that are low risk, a cesarean delivery may pose greater risk than vaginal delivery, especially risks related to future pregnancies.” Working to decrease the number of cesareans that are not strictly for clinical conditions is something we can all work on. Educators, nurses, community health workers, and others can use the information Dr. Lagrew shares to help people avoid the first cesarean. This will not only lower unintended outcomes of the initial cesarean like hemorrhage and infection but also prevent problems in subsequent pregnancies. Dr. Lagrew is a maternal-fetal medicine specialist and physician informaticist with a particular interest in maternal quality improvement. He recently retired as Medical Director of Women's and Children's services for Providence St. Joseph-Southern California and as a Providence System Provider Informaticist in Obstetrics. He has started his new role as Chief of Service, Maternal Fetal Medicine at Hoag Healthcare. He holds triple board certification in Obstetrics/Gynecology, Maternal Fetal Medicine, and Clinical Informatics. In addition to these administrative and leadership roles, he continues to actively author numerous peer-reviewed publications and book chapters. He is a clinical professor in the Department of Obstetrics and Gynecology at UC Irvine. He is the immediate past chair of the Society of Maternal-Fetal Medicine's Clinical Informatics Committee. Her desire to serve led her to the role of executive vice president and chief strategy officer at Henry Ford Health System in July 2020, where she is responsible for providing executive counsel and leadership for the system's strategic planning efforts and partnership ventures, business development, and transformation initiatives, government affairs, as well as Henry Ford Innovations, the health system's multi-disciplinary team responsible for product design and commercialization, technology transfer, licensing agreements and international programs. Before joining Henry Ford Health System, Carladenise served as EVP of Strategy at Providence St. Joseph Health, where she was promoted from Chief Administrative Officer for Population Health to SVP of Contracting to Chief Strategy Officer. Giving back to the community is a top priority of Dr. Edwards, who has started two endowed scholarships for African-American students, one at the University of Pennsylvania's School of Nursing and the other at Johnson C. Smith University. She serves on several non-profit boards, including Heluna Health, College Success Foundation, American Board of Internal Medicine, Rand Health, e-Health Initiative, and Connect 313. She has a long history of serving as a strategic advisor to start-ups, technology, and healthcare companies, currently serving as an advisor to Apatics and Parsley Health. As a military brat, Carladenise imagined settling down in one place, but the universe had different plans. She graduated from high school in San Diego, CA, then received her BA and MS.Ed from the University of Pennsylvania, followed by a trip down south to Gator country, where she received her Ph.D. from the University of Florida. She studied neuroscience at the University of Leuven in Belgium and gerontology at the University of California Los Angeles and has worked in Florida, Georgia, California, Washington, and now Michigan.
