Tooth decay is a preventable yet common chronic childhood disease that affects children's nutritional intake and their ability to speak, play, concentrate, and succeed in school. Pediatric nurses have the opportunity to promote children's oral health by educating parents and providing anticipatory guidance on dental caries prevention when caring for children in schools, clinics, and acute care settings. Acute-on-chronic pain and tolerance are very complex issues. The bedside nurse plays a critical role in pain management, including medication reconciliation and making sure home medications are continued in the hospital. Patient education is imperative. Managing the patient’s expectation is key, but setting realistic goals is a must.MEDSURG Nursing routinely publishes articles on pain management. In this episode, MEDSURG Nursing Editorial Board Member Susan Bohnenkamp cites two such articles recently published in the journal and talks with Dr. Heather Carlisle, a Clinical Associate Professor at the University of Arizona College of Nursing. Dr. Carlisle is certified in pain management and palliative care and has published extensively on the topic of opioid safety and patient monitoring in acute care settings. Prior to joining the faculty, she worked as a nurse practitioner on an acute pain service at a level one trauma center, where she consulted on patients with complex pain needs, particularly acute-on-chronic pain. The nursing community has a unique opportunity to inform the public and policy makers of the vital role nurse practitioners play in advancing health access, equity, and quality care. Recently, Becker’s Hospital Review published a notice stating about 15 physicians at Edward Elmhurst Hospital were to be terminated and replaced by nurse practitioners. The implications made in response from local physician groups were that nurse practitioners are viewed as mid-level providers, and this practice could put patients in danger.In response, in December 2019, nurses from Rush University College of Nursing and Rush Health in Chicago submitted a Letter to the Editor to Becker’s Hospital Review, expressing dismay at the description of nurse practitioners as mid-level providers. The Nursing Economic$ Editorial Board believes all nurses have an ethical obligation to correct inaccurate descriptions of advanced practice colleagues, and in support of the nurses from Rush University and Rush Health, submitted a follow-up Letter to the Editor to Becker’s Hospital Review. Today’s discussion, led by Nursing Economic$ Editorial Board member Dr. Therese Fitzpatrick, focuses on the events at Edward Elmhurst Hospital as reported in Becker’s Hospital Review and elaborates on why advanced practice registered nurses and nurse practitioners must become informants to policy makers and the public. Dr. Bond provides information regarding why respiratory compromise is of concern for medsurg nurses; the importance of awareness of respiratory compromise to help reduce adverse events and patient deaths; how medsurg nurses can increase their awareness through recommendations, clinical education, and training; policies that should be put in place to guide nursing practice; and the role of medsurg nurses in treating patients with respiratory compromise. Donna Bond, DNP, RN, CCNS, AE-C, CTTS, is a Clinical Nurse Specialist also at Carilion Clinic in Roanoke, VA; and is an active member in several nursing organizations, including the Virginia Chapter of Clinical Nurse Specialists; the Respiratory Nursing Society Interprofessional Collaborative, and the Respiratory Compromise Institute. Other organizations where she has contributed her time are Sigma Theta Tau, as a founding member of Tau Phi; and the American Association of Critical Care Nurses. She currently serves on the national board of the Respiratory Compromise Institute representing the Clinical Nurse Specialist role as one of two nurses on the board, and represented the Clinical Nurse Specialist role/Respiratory Nursing Society on the national CONCERT workshop to determine priorities for patients with Chronic Obstructive Pulmonary Disease. She carries certification as a Critical Care Clinical Nurse Specialist, Certified Asthma Educator, and Certified Tobacco Treatment Specialist. Recently she was inducted into the first class of fellows at the 2019 Annual National Clinical Nurse Specialist conference. Donna has presented at many national conferences as both a speaker and poster presenter. The newest fad for youth is dangerous and addictive. While the prevalence of smoking tobacco by teens has decreased significantly over the years, nicotine delivery has resurfaced in a new format. Whether referred to as vaping or juuling, the use of electronic cigarettes has implications for the pediatric nurse as well as for the health of youth. Pediatric Nursing, a peer-reviewed clinical journal for pediatric nurses, published a feature article on vaping in its January/February 2019 issue. The article, entitled, “Vaping: It’s All a Smokescreen,” was authored by Dr. Janice Selekman, Professor Emeritus from the University of Delaware in Wilmington, Delaware. In this episode, Pediatric Nursing Journal Editorial Board member Bernice Mowery, a Nursing Research Scientist from the Inova Health System in Northern Virginia, interviews Dr. Selekman, about the use of e-cigarettes and vaping in teens, the serious health risks associated with vaping, and the important role pediatric nurses can play in preventing or decreasing this practice.Dr. Janice Selekman is Professor Emeritus from the University of Delaware, in Wilmington, Delaware. She is a leader and national expert on multiple topics related to children and adolescents, most notably care of children in the school setting, children with chronic conditions, high-risk behaviors, and mental health. While visiting San Juan, Puerto Rico, in January 2019, Nursing Economic$ Editor Dr. Donna Nickitas talked with Senator Eduardo Bhatia, the current Minority Leader of the Puerto Rico Senate. Their conversation focused on climate change and Hurricane Maria’s impact on Puerto Rico. Having assumed office in 2017, Senator Eduardo Bhatia is the current Minority Leader of the Puerto Rico Senate and the former President of the Puerto Rico Senate. Senator Bhatia holds a degree in Government and Public Policy from the Woodrow Wilson School of Public and International Affairs at Princeton University, where he later was also a member of the Princeton University Democrats and the Princeton University Council. Senator Bhatia’s career in public policy is extensive, including in 2014 authoring the most comprehensive energy reform law in Puerto Rico’s history, as well as his roles as Chair of the Eastern Regional Conference of the United States Council of State Governments and President of the National Hispanic Caucus of State Legislators, as well as his membership with the Council of State Government, the National Association of Latino Elected Officials, and the National Hispanic Leadership Agenda. Dr. Shaffer discusses his decision to pursue nursing as a career at a time when the nursing profession was predominantly a woman’s profession, and his ongoing work as a prominent nurse leader. He also provides insight surrounding the similarities and differences in the workplace between male and female nurses, as well as diversity among foreign nurses, stating he believes at the soul of every nurse, nurses are more similar than they are different.Franklin A. Shaffer, EdD, RN, FAAN, is the President and Chief Executive Officer, CGFNS International, located in Philadelphia, Pennsylvania. He earned his doctorate in nursing administration and nursing education at Columbia University and has 50 years of progressive and varied nursing experience which includes administration, education, clinical and research.
