After fulfilling the role of the Perinatal Nurse Manager, Yvette became the Nurse Manager and then Director for a 21 high-risk OB/Antepartum unit at Henrico, with oversight of the Perinatal Navigator Program in the Women's Hospital. The most recent role before transitioning to the Capital Division as Associate Vice President of Academic Affairs, Yvette was the Director of Oncology, Progressive Care Neurological Unit, and the Central Decision Unit which was the unit known to administer some of the first infusions in the outpatient setting during the pandemic for the MAB infusions in efforts of helping those diagnosed with COVID. Yvette also has a passion for nursing academia where she spent over 5 years as lead instructor for medical surgical, women and newborn health, and high-fidelity simulation in a hybrid program for both licensed practical nursing students and registered nursing students. Nancy Boykin is the Chief Nursing Officer/Chief Operating Officer at HCA Virginia's Retreat Doctors' Hospital. I have been a nurse for over 26 years and started my nursing career at Henrico Doctor's Hospital. My clinical background includes Medical/Surgical, Oncology, Telemetry, and Intensive Care Unit. My leadership career started as a Nurse Manager of a NeuroVascular Intensive Care Unit and a Nursing Director for several units including the NeuroVascular Intensive Care unit, NeuroVascular step-down unit, Medical Oncology, Surgical Oncology, Cardiac Medical Intensive Care Unit, Interventional Unit, and the Progressive Care Unit. My leadership has been identified by Corporate HCA and is one of a small group of people with particularly high performance and strong leadership skills to be included in the CNO Executive Development Program for 2015 where I then transitioned to the Associate Chief Nursing Officer position at Chippenham Hospital. We have increasing awareness of the multiple deficiencies in the U.S. medical system, particularly in hospitals. Here with an inside look at efforts to solve this, is Dianna Goodwin, a registered nurse with 28 years of varied experience. Speaking as a manager, she shares her experience in providing the best possible support for nurses in terms of training on all levels. Dianna emphasizes the importance of having a mix of new and experienced nurses. New nurses need mentoring, and too many in need of this assistance place a burden on available resources. Nurses especially need training in high-risk, low-frequency procedures, which can occur in any area of the hospital. Dianne explains that any nurse who hasn't done such procedures in a long time may need supervision. Cardiovascular events make up some of the most dangerous and time-sensitive conditions that get treated in the emergency room and hospital at large. For this reason, they make up a significant proportion of malpractice suits concerning cardiac care An LNC needs to be deeply familiar with all aspects of cardiovascular and treatment. She or he must know what results are unavoidable and which are preventable. Here to explain some of the most critical elements of cardiovascular care is Lailen Baua, an LNC with broad experience in many aspects of clinical care. Time is a critical factor. In some cases, delays are beyond a hospital team's control, such as those caused by distance from a well-equipped hospital or traffic delays. Lailan says that ideally it should be no more than two hours before a cardiovascular patient is receiving appropriate treatment, such as a balloon angioplasty. Errors in the administration of these procedures can also be potential sources for malpractice. Misdiagnosis is also a factor in disability and death from a cardiac event. Therese (Tez) Bryars brings a wealth of experience to her LNC practice. Initially, most of her work was with defense cases, and she worked as an inhouse LNC. During this time span, she gained a great deal of familiarity with nursing home cases, which is one of the main topics of this podcast. Since then, she has gone independent and both deepened and broadened her experience. Nursing home records can often be much more complex than hospital records, and, because they may cover a much longer time span, the LNC is faced with many more records. Tez gives a step-by-step approach to economically review the records appropriate to the case. She also describes how she has come to master electronic records and details her sources for necessary research, making the point that checking the date pertinent documents were written must match the time span of the case. Tez, because of her experience as both an in-house LNC and an independent one, brings a high level of understanding of the differences between these two ways of working. She particularly recommends that those starting out in this field give strong consideration to an in-house setting. She also describes how, in her independent role, she devotes considerable time to calling attorneys' offices, and explains how she focuses on nurturing relationships with the paralegals when they answer the phone. So, today we’re gonna be talking about one of the most challenging courses for nursing students, Medical Surgical (or Med Surg since the medical and nursing fields abbreviate everything.) It’s not just the content it's the sheer amount of content that you’re expected to learn in a short period of time in addition to all the other classes, clinical, work, and family…. It can be overwhelming especially if you don’t know what and how to study. At the start of every semester I walk my students through this 11 step process that I developed when I was in nursing school. Not only did it reduce my reading time but I was in the top 5% of my class which was no small feat considering I was a single mom working full time night shift. In my seven years of teaching I’ve never had a student that followed this process fail theory. Now, I’m not gonna lie. It’s work. This often leads to increased levels of anxiety in nursing environments, which can be a challenge for nurses. One calming technique is coloring pre-drawn geometric designs, such as mandalas.The May/June 2020 issue of MEDSURG Nursing features the article entitled, “The Effect of Coloring Mandalas on the Anxiety of Medical-Surgical Nurses and Nursing Support Staff,” authored by Patricia Maguire, Ann Coughlan, Deborah Drayton, Hannah Lacko, Jessie Reich, and Linda Hatfield. As the healthcare system begins addressing an overhaul of its costs, processes, accountabilities, etc., one area available for closer examination is medical-surgical units and the costs associated with recovery at the hospital vs a long term care facility. As the government looks for ways to reduce costs and complexity, this is perhaps one aspect where long term care facilities may realize opportunities for growth as these are migrated away from more costly hospital stays.
