Está previsto decorrer na Guiné-Bissau um projecto clínico que está a suscitar polémica e a levantar muitas questões. Trata-se de um estudo, financiado por fundos americanos - 1,600 milhões de dólares pagos pelo Centro de Controlo das Doenças americano (CDC) - sobre os efeitos não específicos da vacina contra a Hepatite B. Em entrevista à RFI, a antiga ministra da Saúde da Guiné-Bissau, Magda Robalo, manifesta sérias reservas em relação ao estudo, alerta para fragilidades éticas, científicas e comunicacionais. O estudo, liderado pelo Projecto de Saúde de Bandim, pretende acompanhar 14.000 recém-nascidos ao longo de cinco anos, comparando dois grupos: um que recebe a vacina contra a Hepatite B nas primeiras 24 horas de vida e outro que segue a prática actual do país, recebendo a vacina apenas a partir das seis semanas de idade. E assim estudar os efeitos associados à vacinação contra a Hepatite B em função da data de administração da vacina. As recomendações da Organização Mundial da Saúde são claras: a vacina contra a Hepatite B é segura, sem efeitos adversos, e é mais eficaz quando administrada à nascença, sobretudo num país como a Guiné-Bissau, onde a prevalência da Hepatite B é elevada (18% da população está infectada). Actualmente, a Guiné-Bissau não administra a vacina contra a Hepatite B à nascença. No entanto, o Governo decidiu adoptar a vacinação universal de recém-nascidos a partir de 2028, em linha com as recomendações da Organização Mundial da Saúde (OMS). Segundo os investigadores do Projecto de Saúde de Bandim, esta transição cria uma “janela de oportunidade” para comparar a política actual com a futura política já decidida, mas ainda não implementada. Durante o ensaio, todas as crianças recebem a vacina contra a Hepatite B às 6, 10 e 14 semanas, respeitando o plano nacional de vacinação. Metade, 7.000 recém-nascidos, vão receber uma dose adicional nas primeiras 24h de vida. Para alguns elementos da comunidade médica internacional, o interesse deste ensaio clínico é mais político do que sanitário, como demonstra o financiamento americano de 1,6 milhões de dólares, numa altura em que o secretário norte-americano da Saúde, Robert Kennedy, é conhecido pelas suas posições antivacinação. O Centro Africano de Controlo das Doenças anunciou, na semana passada, o cancelamento do ensaio clínico com base nas questões éticas. Todavia, a equipa que se encontra a liderar a investigação, contactada pela RFI, desmente o cancelamento ou a suspensão. Em entrevista à RFI, a antiga ministra da Saúde da Guiné-Bissau, Magda Robalo, manifesta sérias reservas em relação ao estudo proposto pelo Projecto de Saúde de Bandim, alertando para fragilidades éticas, científicas e comunicacionais que podem ter consequências negativas duradouras na confiança da população nas vacinas. Segundo a ex-governante, o Comité de Ética da Guiné Bissau terá analisado o protocolo “com a maior das boas vontades”, mas condicionado por uma relação histórica de proximidade com o Projecto de Saúde de Bandim, que actua no país há cerca de 45 anos. Essa proximidade poderá, na sua opinião, ter limitado uma avaliação mais crítica e abrangente do impacto do estudo. Para Magda Robalo, faltou ao Comité de Ética uma análise da “dimensão externa” do protocolo, nomeadamente do seu alcance internacional e da real necessidade científica de testar uma vacina amplamente utilizada. “Estamos a falar de uma vacina contra a Hepatite B que é utilizada há 40 anos e que não precisa de ser testada nos recém-nascidos da Guiné-Bissau”, afirma. A antiga ministra acrescenta que a eficácia da vacina administrada à nascença está solidamente comprovada e é recomendada pela Organização Mundial de Saúde. Num país onde se estima que 18% da população esteja infectada com Hepatite B e onde cerca de 11% das crianças já estejam infectadas aos 18 meses, Magda Robalo considera que o estudo pode ter um efeito contraproducente. “O grande efeito nefasto deste estudo é exactamente criar na mente das populações, nomeadamente das mães, a desconfiança relativamente a esta vacina.” A ex-ministra relata que já ouviu no terreno perguntas que revelam esse receio: “Mas porque é que nós vamos introduzir uma vacina que nunca foi testada na Guiné-Bissau?”. A resposta é clara: “Várias vacinas, se não todas, que estão a ser administradas na Guiné-Bissau, nunca foram testadas particularmente na Guiné-Bissau. Não precisam de ser testadas na Guiné-Bissau, porque foram testadas em vários outros países e em populações muito maiores.” Magda Robalo insiste que as recomendações da OMS resultam sempre de estudos alargados e da avaliação de múltiplos peritos, antes de serem integradas nas políticas de saúde pública. Ainda assim, alerta para o perigo de as mães se sentirem usadas como “cobaias”, o que pode levar à rejeição da vacina. “É o risco que nós corremos, que as mães pensem que não se deve dar esta vacina às crianças”, afirma. Outra questão que se levanta prende-se com o processo de consentimento informado, num contexto de fraca literacia em saúde. “Na Guiné-Bissau, o consentimento informado é problemático”, garante Magda Robalo, lembrando que 14 mil mães teriam de ser esclarecidas no âmbito do estudo. Na sua opinião, é pouco provável que a maioria compreenda plenamente as diferenças entre os grupos e as implicações da decisão. “Posso arriscar a dizer que a maior parte das mães não irá perceber exactamente o que é que se passa”, esclarece. Em resposta escrita à RFI, Isaquel da Silva e Frederik Schaltz-Buchholzer, investigadores responsáveis pelo estudo no Projecto de Saúde de Bandim, salientam que o ensaio “não retira vacinas dos participantes, pelo contrário, metade dos participantes receberá uma vacina que, de outra forma, não teria recebido”. Explicam que se trata de “um ensaio clínico para avaliar os efeitos não específicos das vacinas administradas no período neonatal. (...) O estudo não questiona a eficácia da vacina contra Hepatite B na prevenção da doença. A lacuna científica está nos efeitos não específicos das vacinas - impactos sobre mortalidade geral, resistência a outras infecções e saúde global da criança. Há evidências de que o momento da vacinação pode influenciar esses efeitos, mas faltam dados robustos em contextos africanos.” Os investigadores acrescentam que “o ensaio resultará num maior número de recém-nascidos vacinados (e protegidos) contra a Hepatite B” e ressalvam que “o estudo segue protocolos éticos e científicos, foi aprovado pelo Comissão de Ética na Saúde de Guiné-Bissau, e vai responder perguntas importantes antes da implementação universal”. Questionados sobre as críticas éticas levantadas por alguns especialistas, explicam que essas críticas estão baseadas “mal-entendidos e em informação incompleta.” Apesar do Centro Africano de Controlo das Doenças ter anunciado o cancelamento do ensaio clínico, a equipa do Projecto de Saúde de Bandim desmente o cancelamento ou suspensão e reitera que os preparativos continuam e que será feito um anúncio aquando do arranque do projecto.
