Ita O'Brien is the world renowned intimacy co-ordinator. She is behind the kisses, embraces and sex scenes in Normal People, I May Destroy You, It's A Sin and Gentlemen Jack, to name a few. She has used her expertise on set to inform her debut book, Intimacy, and joins Nuala McGovern to discuss it. A group of experts have highlighted that in global heart disease clinical trials, less than 30% of the people taking part are women. This is despite more than 30,000 women being admitted to hospital in the UK each year due to a heart attack. One of these experts, Vijay Kunadian who is Professor of Interventional Cardiology at Newcastle University, joins Nuala. An open letter organised by UK aid organisations has been delivered to 10 Downing Street today - signed by Sudanese activists, UK aid leaders and high profile figures demanding the UK government take urgent action to addresss the rapidly worsening crisis in Sudan. This comes as the charity Medecins Sans Frontières reported that of 659 survivors of sexual violence in South Darfur between January 2024 and March 2025 86% reported they were raped. Nuala speaks to Eva Khair, Campaigner and Founder of Women4Sudan and Sudan Transnational Consortium, one of the signatories of the letter. Flat racing jockey Hollie Doyle just surpassed the record set by her hero, Hayley Turner, who recently bowed out of the sport. At Ascot in May, Hollie had her 1,023rd win, becoming Britain's ‘most winning' female jockey of all time'. She joins Nuala. Model Hailey Bieber has sold her make-up company Rhode in a deal worth up to $1 billion. She joins a list of other celebrities earning millions from their cosmetic brand. In today's episode we will listen back to an episode from 2021 from the SCAI 2021 meeting in which we interviewed Dr. Mullins and Dr. Julie Vincent (then Professor of Pediatrics at Columbia University). Drs. Mullins and Vincent discuss their lives as interventional cardiologists and also speak about the role that mentorship played in their long careers. Dr. Aimee Armstrong of Nationwide Children's Hospital also participates in this conversation. Following this interview replay with Drs. Mullins and Vincent, we speak with Dr. Frank Ing who is the Director of Pediatric Cardiology at UC Davis and the former director of the catheterization lab at Texas Children's and a former trainee and close friend of Dr. Mullins and his family. Michael S. Chen graduated with a degree in investment banking. He worked at Hambrecht Quest, a smaller investment bank focusing on high tech and healthcare companies in New York City. After two years in investment banking, he decided to pursue medicine, inspired by his father's career as a cardiologist. He then went on to study internal medicine at UCSF, Cleveland Clinic, and then joined cardiac associates in Gaithersburg, Maryland. He has been with the practice for 18 years and is now the Chair of Cardiology at  Shady Grove Medical Center, and Medical Director of the Cardiac Catheterization Laboratory at White Oak Medical Center. Challenges Faced by Cardiac Catheterization Labs Michael discusses the challenges faced by cardiac catheterization labs in meeting performance measures and ensuring proper procedures. He highlights the need to balance equipment requests with limited budgets and prioritize patient care. He talks about the importance of cardiologist performance in heart attack patients, focusing on door to balloon time and other metrics like aspirin and blood thinners. He mentions the importance of assessing the overall performance of the hospital as a whole, focusing on reducing time spent on administrative tasks and ensuring patients are on the right medications.The conversation turns to steps patients can take to ensure they are prescribed the right medication. Interventional Cardiology Explained Michael goes on to explain what interventional cardiology is, what the new trends are, and he addresses common misconceptions. He discusses the learning process for new techniques and tools, such as fellowships and medical device reps. Michael notes that fellowships are the most effective method for learning new techniques, but it's important to stay updated and stay updated with medical device reps. Other methods include working with other doctors, attending training sessions, or learning online through YouTube videos. There are various ways to learn and improve in this field. Physicians and The Business of Healthcare Michael discusses the business of healthcare and the factors driving physicians to work with larger healthcare companies. He explains that larger practices have more bargaining power and can offer lower rates and financial security. However, there are drawbacks, such as the need to balance autonomy with financial security. Michael talks about the increasing trend of private equity investing in physician groups, such as anesthesia and cardiology. Private equity is seen as a way to ensure return on investment, which can sometimes be more expensive than providing the best care possible. Stressors for cardiologists include cash flow, meeting payroll, and meeting electronic medical records and government regulations. However, overall, the fit has been good and cardiologists have been happy with the situation.  Participating in the National Senior Games The conversation turns to fitness and sports. Michael keeps active in terms of exercise, running and working out. He recalls running a 5k five years ago and receiving a gift certificate from Panera for winning first place in his age division. He then qualified for the National Senior Games, (open to anybody 50 years and older) which features various events such as a 5k 10k, pickle ball, tennis, track and field, ping pong, basketball, and softball. Michael has competed in the National Senior Games in Fort Lauderdale and Pittsburgh. His mom competed in the 5K powerwalk and Michael competed in the 5K and 10 K road race. Michael came in 2nd in the 10K (and won a silver medal!) and 4th in the 5K in Pittsburgh in 2023, His mother placed 8th in the 5K Power walk in Fort Lauderdale in 2022. Michale mentions that he has a 5k coming up on Labor Day week, with his son running it with friends, and the National Senior Games.   Influential Harvard Professors and Courses Michael mentions Richard Taylor and TA Tom Roberts, and studying bipeds and quadruped to compare the energetics and mechanics. Part of the study included placing turkeys and chickens on a treadmill. They found that turkeys were quite stubborn and not smart, as they were not smart enough to run on a treadmill. They learned that turkeys were efficient at running and that their locomotion mechanics were efficient. They were fortunate to receive a Hoops Prize for their work, which ends up in the Lamont library. Dublin-based FIRE1 today announced that the first Irish patient has been successfully implanted with its FIRE1 System for remote heart failure monitoring. The innovative system has the potential to positively impact thousands of people living with heart failure in Ireland every year and alleviate the burden on hospital emergency rooms. The successful implantation took place at University Hospital Galway in the First in Human Clinical Investigation of the FIRE1 System in Heart Failure Patients (FUTURE-HF). The study will assess FIRE1's novel solution to improve outcomes for those living with heart failure. Irish medtech company FIRE1, whose CEO, Conor Hanley, was recently announced as a 2024 EY Entrepreneur of the Year finalist, seeks to transform heart failure management by enabling patients to monitor and control their fluid volume themselves with a device at home, similar to how continuous glucose monitoring transformed diabetes care. Fluid overload is a classic clinical feature of heart failure, which affects approximately 90,000 Irish people and is a leading cause of hospitalisations, including high admission and re-admission rates in Ireland. FIRE1 CEO and President Conor Hanley said, "I am especially pleased to announce our first patient in Ireland. This represents a true clinical research success for Ireland, being one of the first systems of its nature to be developed, manufactured, and now in a clinical trial in Ireland. It showcases Ireland's capability for groundbreaking medical innovation with the potential to change healthcare delivery globally. The FIRE1 team is steadfast in our mission to help millions of people around the world living with heart failure to get their normal lives back, but it is very special to be able to give access to people here at home. It is gratifying to see the physician interest in our novel technology. I would like to extend my thanks on behalf of the whole FIRE1 team to the fantastic staff at UHG, and most importantly to the patients who continue to put their trust in us and participate in this important research." The FIRE1 sensor is implanted into the body's largest vein, located in the abdomen, known as the inferior vena cava (IVC) and works by continuously measuring the size of the IVC, giving a marker of the amount of fluid in the body. High levels can increase the risk of breathing difficulties and a build-up of fluid in the lungs, which lead to an emergency hospital admission. The patient wears a belt reader around the abdomen for around a minute a day to take a reading from the sensor and the data is sent to the patient's clinical team at the hospital. The system is designed to alert whenever the patient's condition deteriorates. Professor of Interventional Cardiology and Translational Medicine at University Hospital Galway (UHG), Prof Faisal Sharif performed the implant. "We are delighted to bring this new, transformative technology to Ireland for the first time," said Prof Sharif. "The procedure was very straightforward, minimally invasive and the patient was confident using the FIRE1 system and taking the readings. The data we will get from this patient will give a new window into heart failure management and has the potential to dramatically improve our understanding of this patient's condition, and to enable proactive changes in medications that will keep them healthy and at home. We hope to implant further patients at UHG while recruitment is still active."

Pearls - ACHD: Interventional Cardiology The ductus arteriosus, which is formed from the distal portion of the left sixth arch, is key to fetal circulation because it allows blood to bypass Transcatheter pulmonic valve replacement (TPVR) is a treatment for many ACHD patients that can spare them repeat sternotomies. This is important as many ACHD patients hava already undergone multiple surgeries in their childhood.   Before any ACHD cardiology intervention, appropriate imaging (TEE, TTE, Cardiac MRI, Cardiac CTA, and/or 3D printing) is imperative to understanding the relevant anatomy and hemodynamics to guide procedural indication and planning.    As with other structural interventions, consider a SENTINEL device (cerebral embolic protection system) to provide embolic protection in procedures that could lead to debris/embolic dislodgement when appropriate. Sinus venosus defects can be repaired via a transcatheter approach with a covered stent in the superior vena cava (SVC).   Consider using 3D printing or 3D digital imaging when preparing for complex ACHD interventions.    Notes- ACHD: Interventional Cardiology 1. When considering a patient for TPVR there are 3 types of landing zones for pulmonic valves in ACHD patients:    Pulmonary conduits or homografts. These are typically seen in patients with TOF or prior Ross or Rastelli procedure. These may be calcified and stenotic and so pre-dilatation is often needed before valve replacement.   Bioprosthetic Valves. (Valve in Valve TPVR) Native outflow tract 2. What are some of the more severe complications to consider when talking to an ACHD patient about a TPVR?   Coronary artery compression Conduit rupture Vessel injury (including the pulmonary bed) Valve embolization Endocarditis 3. What are some of the hemodynamic measurements one would want to pay attention to in a patient with a Fontan heart?    You will see higher CVPs in patients with a Fontan palliation.

Together, they explore a complex case of STEMI that was further complicated by ventricular free wall rupture. Additionally, Dr. Ajar Kochar, Program Director for Interventional Cardiology at Brigham and Women's Hospital, provides an insightful "ECPR" segment, adding a unique perspective to the case. This is the case of a patient who presented with STEMI and was found to have a moderate pericardial effusion with echogenic material within the pericardial space concerning for thrombus. Urgent CTA/CT surgery was engaged due to concern for dissection, but no evidence of dissection, rupture or intramural hematoma was found. The patient underwent an urgent pericardiocentesis which yielded 350cc of hemorrhagic fluid, leading to an improvement in hemodynamic status. A coronary angiogram was performed which showed a 100% thrombotic occlusion of OM 1, the culprit lesion for the STEMI. Due to the possibility of a delayed STEMI and high suspicion for mechanical complication of MI, aspirin and IV cangrelor were chosen as the preferred antiplatelet strategy. However, cangrelor was held and cardiac surgery was consulted, as LV free wall rupture was suspected. The patient underwent urgent repair of the LV free wall rupture, with an uneventful post-op recovery and discharge on day 8 to cardiac rehab.
