Immunic Inc chief medical officer Dr Andreas Muehler speaks to Proactive's Stephen Gunnion about the start of an investigator-sponsored Phase 2 clinical trial focused on treating Post COVID Syndrome (PCS) with its lead compound, vidofludimus calcium. This drug is currently in Phase 3 trials for multiple sclerosis and has shown potential as a broad-spectrum antiviral. Muehler explained that the trial, initiated by a group of German investigators, aims to investigate the role of viral reactivation in fatigue symptoms associated with PCS. The study builds on Immunic's earlier work during the COVID-19 pandemic, which hinted at the drug's ability to reduce fatigue in hospitalized patients. Muehler highlighted the potential connection between Epstein-Barr virus reactivation and fatigue, a key area of interest that may also have implications for multiple sclerosis treatment. “We are hopeful that this trial will provide supportive data linking virus reactivation and fatigue,” said Muehler. In this episode of our Tea Room long covid series we summarise the latest long covid research presented at last week's Australasian covid-19 conference.Professor Anthony Kelleher, director of the Kirby Institute, is seeking the cause of long covid.What his team has found is persistent immune system dysregulation.“There is ongoing activation of the immune system, in a range of compartments,” he tells The Tea Room. “The immune system is still producing the cytokines that it normally produces early in infection against the virus, that would normally turn off within a couple of days to a couple of weeks.”Associate Professor Kari Lancaster, from the University of NSW, says “long covid” is possibly the first patient-made term in medicine, and explains how learning from patient experiences is central to effective treatment.“The effects of long covid are likely to have uneven effects across different groups of disadvantage,” she says. “So, talking about what other kinds of community support and social support and social care might be available, alongside clinical care, is really important.”

You might hear it referred to as “Long COVID” or “Post-COVID Syndrome” and the people who have it, “Long Haulers.” Johns Hopkins Medicine describes it as, “lingering health problems” people who were diagnosed with COVID-19 have, “even when they have recovered from the acute phase of the illness.” Some of them may have been hospitalized with COVID, while others had mild cases, but what these 10-30% of COVID survivors have in common now may be breathing issues, heart problems, kidney damage, gastrointestinal upset, lost or distorted senses of smell or taste, trouble with circulation, neurological effects and/or mental health disorders. However, there has been an optimistic report from Yale where Akiko Iwasaki has been studying the effect the COVID vaccine has on people with post-COVID syndrome: As many as 30 to 40% of those who get the vaccine have reported improvements to their symptoms.
