Dr. Touchinsky is creating a direct primary care functional medicine practice that is independent of insurance, and while it is breaking new ground, it is very tough going. He is making a difference in his small-town Pennsylvania community. Listen in for counsel on showing up even in the hard times. This is a special interview with Sachin's friend, Dr. Buddy Touchinsky. Sachin and Dr. Buddy went to chiropractic school together. They are also both functional medicine practitioners and entrepreneurs trying to innovate in the healthcare space! Today, they talk about mentorship!   [3:00] When Dr. Buddy Touchinsky came out of chiropractic college, he didn't know what he didn't know. Coaching at the time consisted of expensive practice management companies with rigid programs.   [4:14] At first, Sachin wanted to do things his way. Asking for help was seen as a sign of weakness. Sachin recalls how his mother would just assume he knew how to do everything. Now, he believes it's always great to get help from people who are just a few steps ahead of us.   [5:54] Sachin hired Dr. David Singer, went to one of his trainings, and was hooked on the idea of being held accountable. If it wasn't for training on how to charge for a package of care, Dr. Patel might still be charging visit-by-visit. That tool changed his belief in himself.   [6:59] Dr. Touchinsky tells of his first experiences charging for a package of care. He discounted his services to get more patients. He had some success and noticed that the patients were there to get better. They had buy-in.   [8:25] It was a big milestone for Sachin to be able to attract the right person, who is investing in themselves, and trusting and investing in you to get them where they want to go with their health.   [9:02] Dr. Touchinsky came out of chiropractic college ready to work on sports injuries, back injuries, and spinal manipulation. He noticed that patients with back pain usually had more health issues. Before he heard of functional medicine, he started to dabble in it. He picked up Integrative Orthopedics: Integrative, Nutritional, Botanical and Manipulative Therapeutics, by Dr. Alex Vasquez and studied the science in it.   [10:25] About five years into his practice, Dr. Touchinsky saw he was overweight and felt out of shape so he did a metabolic reset. In 21 days he lost 21 pounds, was sleeping better, thinking more clearly, getting his files done on time, and feeling better.   [11:21] Dr. Touchinsky decided to test out the reset on a small group of patients. He picked a cohort of five people that he put on a program. They all got great results, too. He wanted to get this program to more people, so he dove into functional medicine, and signed up for FMU. About halfway through, he still wanted to know how to spread his influence to the masses.   [11:59] Then Dr. Touchinsky saw an old classmate, Dr. Patel!. Sachin was talking about all the things Dr. Touchinsky wanted to hear about and that people needed to hear. Dr. Touchinsky got together with Dr. Patel to learn more and became an active member and now, a coach, in Dr. Patel's community.   [13:21] Others have helped Dr. Touchinsky along the way, including local Rotary and other group meetings. He finds that everybody he meets has got something going on. You see all the smiles on social media, but meeting people, you see into their real lives. Nobody's perfect and we all work through our stuff.   [17:13] Sachin says joining groups is always a great reminder that everyone's human. We all go through stuff, and we can help each other, either to prepare for going through it or because we've been through it. Never struggle alone. Join a group like Perfect Practice or another group of like-minded people.   [18:26] Sachin thinks of a mentorship or a coaching program as a time machine. Join a group of six 29-year-olds and you've got 180 years of life experience and thousands of experiences these people have been through. You're tapping into that. With all the wisdom others have poured into them, you have essentially an infinite reservoir of knowledge and wisdom.   [20:15] Dr. Touchinsky also has a health food store, and a metabolic reset group. Now he is shifting into direct primary care. COVID-19 shut down the functional medicine and chiropractic practice for a few weeks and then they opened up slowly.   [21:51] The shutdown gave them more time to think about alternative ways of helping people if you can't see them in person. Dr. Touchinsky tried James Maskell's The Community Cure, and a test group in 2020 went well. Groups are powerful. Dr. Touchinsky wants to get more people back in person.   [24:02] Dr. Touchinsky started a functional medicine family practice that is more proactive care, building healthy habits, than reactive and community-centred.   [27:32] Dr. Touchinsky hired a consultant to help him set up a direct primary care functional medicine practice but this is a new business model! Dr. Touchinsky is creating his own thing. Having a coach and mentors helped. Without them and his self-care, he may not have made it. Now he can move forward to better things and grow his practice.   [30:01] Sachin compares a mentor to a personal trainer. The trainer spots you while you lift, but you have to do the work and lift the weight, but the trainer gives you the 5% lift to make it. Carrying a bigger load is where the growth takes place. Mentors and the community give an emotional lift.   [31:39] If you don't show up, even when it's toughest and you don't want to show up, you don't get the support and you won't make it through. Sachin says vulnerability is a superpower! It's the stepping stone to the next level of your personal character development.   [33:59] Sachin cites Simon Sinek, “You will be shocked at who and how many people will come to your rescue if you simply ask for help.” People are waiting in the woodwork to jump in and be supportive of others. People don't aspire to be perfect, they aspire to be human and vulnerability is part of the human experience.   [36:57] Advice to any who are struggling and feel alone: Step back and look at the big picture. Remember the people you've helped. Be with your family. Keep showing up and plugging into your groups, and get help. Self-care. Keep investing in yourself. Keep going to the sauna and exercising. Get your sleep. Stay hydrated. Eat healthily.   [41:48] Sachin has taken a cold shower every day this year, and every day, he still dreads it but he feels good afterward. In the cold shower, Sachin practices slow, steady, box breathing, and rotating in the shower with each four-second breath in, hold, breath out, and hold.   [43:05] Dr. Touchinsky's professional goal is to prove that there's a model for healthcare that is fun, exciting, and relaxing for the providers, and that benefits the patients as well. He explains how patients access his direct primary care model that does not rely on or use insurance.   [45:22] Dr. Touchinsky's personal goal is to set these practices up in a way where he has plenty of time for his family and himself. He and his wife are going to Italy soon, and he'd like to travel more often and invest more money in experiences with his family rather than material things.   [46:39] Sachin thanks Dr. Touchinsky for his time today and being a member, council member, and coach at Perfect Practice. He invites Dr. Touchinsky to share his closing thoughts.   [47:23] Dr. Touchinsky shares his blue-collar background and says none of this would be possible without seeing what others are doing for him through groups and mentorship. Bridget was late to her very first meetup and ended up having to sit up front, and when it was over, she just left - “don’t do that!” ;)Jason had a similar experience, but he likes sitting in the front row! His first meetup had nothing to do with WordPress. He threw a bunch of search terms at Meetup.com and the Adobe Software meetup kept coming up so he went! Met a guy the first time there who encouraged him to speak at the very next meetup!Attending your first meetup:Be on time (or early!)Have a 10-sec intro or “elevator pitch” available to introduce yourself - don’t try to sound clever, just say your name and some quick things about who you are - “If your grandma doesn’t know what you do by reading your twitter bio…”Sit where you can hear/see easilyMeetup.com is a good resource for finding things you likeIf you’re a ‘doer’ and want to get involved in your meetup, sit back for a bit and see what people are NOT doing - then offer to do that!Take notes and share them! It’s a great way to help.Use a photo of yourself (that actually looks like you) on meetup.com (or on social)If you raise your hand with a question, make it a “question”, not a statement. You might get a great speaker!It’s not that people aren’t experienced or don’t have the needed gifts, but many people just need to be asked. They either don’t want to brag about their abilities or they may even struggle with ‘imposter syndrome’.Ask “what are you working on? let’s see it!” It’s a great way to get everyone involved and possibly brainstorm new topics. You and your fellow organizers can speak every month but if you ask and seek input / topics / speakers / from your members, everyone can feel included.If you can get people to talk, even if it’s just answering questions, eventually they’ll become more comfortable with speaking and may even be encouraged to speak at a future meetupThe way you present or comment on a tool/plugin should not reflect on how you view the user. Don’t make it sound like you’re the only one who is smart because you use “X” plugin instead of “Y” plugin. Share pro/cons and thoughts without being judgy! Try to be host/plugin agnostic as a leader of a meetup. Our job as facilitators is to create a psychologically safe place.Be careful with gender-specific meetups - be gender-inclusive, not exclusive. You can focus your content and style at women, but you shouldn’t exclude men from attending. We can all learn from each other - men and women do things differently but all have something to offer.We have a lot of seasoned WordPress professionals who no longer want to attend meetups because it’s too basic for them. We also have very new beginners who feel it’s too advanced. We want to get bigger jobs, so we should partner up with each other!We’re more than just what we do for a living - we have unique interests/skills outside of WordPress - like yoga!When Matt Cromwell went to WP Las Vegas a few years ago, he went to a beginner’s workshop (even though he’s an advanced user) and got to hear so much for new users that he was then able to take back with him and say “I’m going to do this differently from now on”.We go through phases where we are advanced at some things and beginners at other things. It’s important to be a lifelong learner, there’s always something new you can learn and people you can learn from.Try not to always talk about only the commercial plugin options and the pricier hosting options, you have to remember sometimes your audience is Joe, the cat-blogger! Not everyone works for a WordPress agency!Promoting and blogging about your meetup is really important. Even over the holidays - that is often when people are the loneliest and need connection with others the most. Try to promote at least 2 weeks out and use more than just meetup.com to advertise.Taking photos during your events can be super helpful - especially if you use those photos to promote the group on social media in between eventsIf you’re going to have your meetup on a week night, it can be tricky for people to get dinner before your meeting, so maybe provide cheap eats (pizza, etc) or try to meet up beforehand for a quick bite together.In your meetup event page, comment & reply to people when they say they can’t attend or when they give feedback on the meeting - be interactive!If you have a facebook page, try to schedule facebook events as reminders, and if it’s possible, even have someone record it and share it via facebook live during the meetup. (Can even be done with a cellphone)Andrea Middleton: WordCamps are a celebration of the meetups that happen the whole year before!Try not to have all of the meetup/WordCamp organizers sit together at events, it can be intimidating and exclusionary - it also makes it harder to focus on meeting new members and getting to know each other better.Try to do your staff/organizer meetups not at the meetup event. But because sometimes the symptoms can be confusing, people fail to seek immediate treatment for autoimmunity. My guest today, Dr. Jared Seigler has an amazing story on how his journey led him to help people address autoimmunity conditions. Stumbled By Accident Dr. Jared Seigler relates that he stumbled into functional medicine by accident. It started when his wife was struggling to conceive for years because she was suffering from PCOS. After almost five years of searching for answers, they were even contemplating on adopting a child. But Dr. Jared Seigler refused to give up mainly since his wife was just 23 years old at the time. He was even subjected to a stool test wherein they found parasites so Dr. Jared Seigler also underwent treatment. After years of searching for answers and countless treatments, the couple was finally blessed with a daughter who is now five years old. Helminth Parasites Helminths are parasitic worms, and parasites are known manipulators of the immune system. The first time Dr. Jared Seigler got exposed to it was six years ago when one of his lupus patients called him. That patient claimed to feel better because of her dog’s licks. She was not taking any drugs nor any supplements.  “What good parasites can do, is act like a probiotic for the parasite. As they manipulate the immune system to hide from it, so the immune system doesn’t attack it, they also suppress the immune system that is causing autoimmunity,” Dr. Jared Seigler said.  He adds, “I’m not anti-parasite. When I look autoimmunity and probiotics research, it’s a hit or miss. There is even research showing that for example, Crohn’s is due to the immune system in some cases, losing tolerance to normal bacteria.” But Dr. Jared Seigler clarifies that when we see someone being given a beneficial parasite consistently, that can take away a lot of autoimmune symptoms. He says dead bacteria behaves utterly different than a live bacteria. Some of the most potent stimulators of inflammation in the gut are dead bacteria that they use in the modern dairy industry.  The Truth About Milk Dr. Jared Seigler believes the gut has to quit getting colostrum from mother’s milk. Hence, a mom has to stop breastfeeding for the gut to grow. “The premise that we need milk for health has been purely built on BS. The research goes the other way,” explains Dr. Jared Seigler. “Technically, when you look at it on a big enough scale, 100% of the world is lactose-intolerant. It takes five gallons of milk a day or a sip to be able to reset intolerance of sugars in the milk.” Understanding Autoimmunity Dr. Jared Seigler readily shares that he has autoimmunity as well as his wife. So, he clearly knows he can’t go back to eating the way he used to eat. Dr. Jared Seigler is also aware that he is not perfect just because he has changed his lifestyle to a healthier one. But he loves being a work in progress. That is why his passion lies in also helping people get their bodies to a place where they can heal. “With the whole premises of autoimmunity, there’s the genetic susceptibility component. I truly don’t believe the immune system makes a mistake with autoimmunity,” said Dr. Jared Seigler. “I believe the immune system is being tricked and overwhelmed in that scenario.”  He adds, “Once the immune system recognizes it as a problem, we have to think about what is driving certain things for autoimmunity like molecular mimicry. For molecular mimicry, the immune system initially recognizes something bad like identifying characteristics. Once it progresses, it can start epitope spreading.” Human Microbiome Project The Human Microbiome Project tries to figure out all the bacteria in our gut, and skin, as well as figuring out what all these bacteria do and how they influence our genes. Dr. Jared Seigler says bacteria pitifully outnumber us. “One of the things that the Microbiome Project started to make clearer is that it is really about the diversity of bacteria more than anything,” said Dr. Jared Seigler. Apparently, there about ten times more viruses inside the gut than there are bacteria. So, there’s a whole concept called the microbiome. Plus, there’s a microbiome where we can vividly see worms. “Someone’s microbiome is like a thumbprint. I think a lot of these companies are data-mining people’s microbiomes,” Dr. Jared Seigler said. “They are giving away these costly tests for relatively nothing. For the 23andMe, they were selling people’s genetic data to pharma companies.” Cold Showers Dr. Jared Seigler reveals that he is also a big fan of cold showers. What it does as far as glutathione production, neurotransmitter production, and microbiome change, it that we weren’t designed to be 72 degrees year-round. Just like we weren’t intended to eat the same food. He says taking cold showers changes how mitochondria functions. It also changes antioxidants, changes how our body utilizes and breaks down a lot of things as well as making people warm.  Eating Real Food Dr. Jared Seigler says as far as figuring out the cause of autoimmunity; he never blames food as long as it is real food. He believes that in the long run, probably one of the worse things someone can do is restrict a lot of their food diversity. Dr. Jared Seigler also believes that doing a three-day fast can be beneficial.  And as far as recommending the best prebiotics, he advises thinking of prebiotics as fiber. Think of fiber like fruits and vegetables. Eat different fruits and vegetables if you want different prebiotics. On the other hand, I have had Chinese medicine practitioner Sarica Cernohous on the show who attested to the benefits of eating fermented foods. Shifting her family to eat at least one fermented food a day eventually strengthened their gut. “I would rather somebody eat fermented foods than try to take it in a capsule. Because traditionally, that’s how we would get probiotics from the fermentation process,” said Dr. Jared Seigler. “It might not necessarily beat all the bacteria that’s in there, but as the bacteria is eating that food, their poop is what we’re after. They change genetic expression.” Gluten Sensitivity According to Dr. Jared Seigler, gluten sensitivity is the genetic predisposition. That is why we see such a huge correlation between autoimmunity and gluten sensitivity. Dr. Jared Seigler also mentions that he took Dr. Tom O’ Bryan and Dr. Peter Osbone’s gluten certification course. He took the classes after deciding to go gluten-free when his wife was diagnosed with celiac. “My energy and depression got better, and my anxiety and brain fog disappeared. I see people on so many different spectrums. Some people don’t even know what gluten is,” said Dr. Jared Seigler. Dr. Jared Seigler says there are even case studies of ALS being gluten sensitive with a 100% resolution upon going on a gluten-free diet. So, it eliminates the inflammation. We dramatically see that when we start to remove problematic foods. Functional Neurology Dr. Jared Seigler explains that from 0 to 6 years old, brain waves of a child is similar to how we dream or store long-term memories. It is similar to dreaming while they are awake. As soon as it happens inside of a child’s brain, their subconscious now operates on an entirely different level. And this is where you can measure some things like heart rate variability. “I love doing a thing called Functional Neurology. Most people know about functional medicine, but functional neurology is like a personal trainer for your brain. You can exercise different parts of the brain to increase its function,” said Dr. Jared Seigler. The Power Of Thoughts Dr. Jared Seigler explains that we always think food may be poisoning our autoimmune disease. So, if you’re only eating 4 to 5 times a day, you have the potential to poison yourself 4 to 5 times a day. But we have anywhere around 30,000 thoughts a day and only 1% may be bad. That’s still hundreds of times a day we poison ourselves with a thought. Technically, a toxin can be a thought or a relationship. “So, if we know that these bad thoughts have negative effects, you can feel your blood pressure rising,” Dr. Jared Seigler explains. “That’s your stress response shifting into high gear. You change your physiology with thoughts. Generosity and gratitude are the two strongest positive emotions.” A positive mindset is powerful. Dr. Jared Seigler usually advises people suffering from addiction to write on a journal and focus on what they are grateful for. “You can get over addiction by diluting it until it is socially acceptable. With an addiction to food, in particular, we can tell when somebody is ready to make a change to the question they are asking. Ultimately, the brain wants to feel normal,” said Dr. Jared Seigler. “The body has never made a mistake. If everyone is doing something a certain way, and the same way – how do you break that mold?It starts with the knowledge of who you are and then you start to build a culture around that. Pick something and start with it, you can change it up later and build on it – the truer you are to yourself, the more you are attracting the right kind of customers/people to youYou become so passionate about what it is that you’re doing that every part of you bleeds itEveryday, you’re scrolling past the people you could be reaching and you’re trying to come up with smart, clever marketing to hook people… but it’s the real moments, the connection with people, that matter more than anything elseUrban Southern desperately needed a new website – they hired Alex Vasquez … they had many references, but they chose Alex because she met him at her 1st WordCamp and told her she was “a really intimidating but fascinating women” and it felt so cool and authentic and she remembered him – He had established a ‘moment’ with herIn the beginning Urban Southern had no engagement with their customers – they’ve worked on it a ton and now spend quite a bit of time connecting and replying and commentingYou need to make your social accounts & interactions something worth sticking around forWhen she was with WannaPixel (??) they used a lot of clever social talk in the beginning but it wasn’t really connecting with people. On Colorado Gives Day – they would be really intentional about interacting with hashtags …. that day she went on their facebook and commented very specifically and intentionally on the people who were raising money – it made an impression and someone decided that day, they were going to use WannaPixelIf you’re starting and you have a young agency… plan to be in it for the long game, it takes work and it takes time to build a brand and to build up relationships – there is no “Growth Hack”Being authentic vs. being rudeWe all have challenging interactions with clients – if you can authentically but gracefully express the struggles you are dealing with (take time to process and calm yourself down first) – it can become a relatable lesson for your followers. Don’t make it super personal. Vent, but “vent with value!!”Try to take the finger that you’re tempted to point at someone and turn it back to yourself and frame the issue around your response and what you learned from it – try to turn a negative experience positiveThe painting on her wall (behind her on video) …if you look from afar, it’s interesting, but  if you go up close, there is still a whole world more to discover that’s interesting.  Think about that in your marketingDid I do at least 2 things today to make an impact and move my business forward?Everyone should have at least 3 ‘traction strategies’ going at a time (email newsletter, social media campaign, etc.)45 sales in first year to over 3000 products sold this year… over 9000 people on email listRead a book on Amazon called “Traction” … implement strategies and keep developing new ones.  Don’t stop the old ones, add to them!
