This week we go back in time 2 years to review a recent paper from 2023 about outcomes of cardiac transplantation in the adult congenital heart patient (ACHD). Little has been written on this topic until this very robust and large scale report. How do single ventricle ACHD patients fair at transplant and how do they compare to non-ACHD heart transplant recipients? Why might 1 and 3 year outcomes not be a 'fair' method of assessing outcomes in this very high risk patient group? How do HLHS patient outcomes compare with other single ventricles? Are there better ways to measure risk in this patient group and how might the data in this work inform risk stratification and management of failing Fontan patients in whom transplantation is being considered? Pearls The current iteration of heart allocation listing is based on priority, with status 1 being the highest priority. The are multiple donor and recipient characteristics to consider when listing a patient for heart transplantation and accepting a heart offer. Desensitization is an option for patients who need heart transplantation but are highly sensitized. Protocols vary by center. Acceptance of DCD hearts is one of many efforts to expand the donor pool Notes Notes: Notes drafted by Dr. Rachel Goodman Once a patient is determined to be a candidate for heart transplantation, how is priority determined? The current iteration of heart listing statuses was implemented in 2018. Priority is determined by acuity, with higher statuses indicating higher acuity and given higher priority. Status 1 is the highest priority status, and Status 7 is inactive patients. (1,2) What criteria should be considered in organ selection when listing a patient for heart transplant? Once it is determined that a patient will be listed for heart transplantation, there are certain criteria that should be assessed. These factors may impact pre-transplant care and/or donor matching (3). (1) PVR (2) Height/weight (3) Milage listing criteria (4) Blood typing/cPRA/HLA typing What is desensitization and why would it be considered? Desensitization is an attempt to reduce or remove anti-HLA antibodies in the recipient. It is done to increase the donor pool. In general, desensitization is reserved for patients who are highly sensitized. Desensitization protocols vary by transplant center, and some may opt against it. When considering desensitization, it is important to note two key things: first, there is no promise that it will work, and second desensitization involves the use of immunosuppressive agents, thereby putting patients at increased risk of infection and cytopenia. (4) Can you explain DCD and DBD transplant? DBD: donor that have met the requirements for legal definition of brain death. DCD: donors that have not met the legal definition of brain death but have been determined to have circulatory death. Because the brain death criteria have not been met, organ recovery can only take place once death is confirmed based on cessation of circulatory and respiratory function. Life support is only withdrawn following declaration of circulatory death—once the heart has stopped beating and spontaneous respirations have stopped. ‘Be a Good Doctor, Not a Great Doctor'Dr Valluvan Jeevanandam, MD, is an internationally renowned expert in high-risk and transfusion-free (bloodless) cardiac surgery. Recognized as the world's most experienced active heart transplant surgeon, he has performed over 1,500 heart transplants.His team is known for taking on some of the most complex cardiac cases. In December 2018, under his leadership, the care team at the University of Chicago Medicine made medical history by performing two triple-organ transplants within 27 hours, replacing the failing hearts, livers, and kidneys of two 29-year-old patients. Globally, only 42 such procedures have ever been performed, and Dr Jeevanandam has completed the heart transplant operation in all 17 of those conducted at UChicago Medicine, where he serves as Chief of Cardiac Surgery and Director of the Heart and Vascular Center.Dr Jeevanandam has a deep commitment to treating patients who are often considered inoperable. Beyond his clinical accomplishments, Dr Jeevanandam is a highly active researcher, with over 250 publications in leading journals and more than 10,000 citations to his credit. His scholarly work, along with numerous presentations and abstracts at international conferences, continues to advance the field of cardiac surgery worldwide.In September 2025, Dr Jeevanandam visited Prasanthi Nilayam and addressed the delegates of the Sri Sathya Sai Global Cardiovascular Conference at the Sai Hira Convention Centre. Key Points Kyle Ring experienced a severe health crisis in November 2015 at age 36, starting with what appeared to be a cold that progressively worsened, leading to multiple emergency room visits where doctors initially misdiagnosed his condition.Medical professionals repeatedly gave Kyle fluids during his visits, which was counterproductive since his body was retaining water, and he experienced extreme fatigue where walking to the mailbox became exhausting.Kyle's condition deteriorated to the point where he passed out at the hospital, his body went septic with all organs shutting down, and his ejection fraction dropped to a critically low 8-12%.Doctors immediately blamed Kyle's condition on alcohol consumption after he honestly disclosed his drinking habits on medical forms, with three different doctors telling him the heart failure was his fault despite not knowing the actual cause.Medical team required Kyle to have a defibrillator surgically implanted and wouldn't allow him to leave the hospital without this life-saving device due to his high risk of sudden cardiac arrest.Kyle was sent home with a battery-powered IV system that delivered medication directly through a line from his arm into his heart, while doctors could only diagnose him with viral heart failure.Doctors informed Kyle he would need an LVAD mechanical heart pump and eventual heart transplant to survive, requiring him to abstain from all substances for six months before being listed for transplant.Kyle began using cannabis on April 20th, the same day he was placed on the heart transplant list, after multiple prescription sleep medications failed to address his severe insomnia caused by heart medications.Cannabis provided Kyle with the first quality sleep he had experienced in months, and he continued using it despite initially hiding it from his wife who gradually became more accepting as she saw the positive results.Beyond sleep improvement, cannabis helped Kyle with anxiety, exercise performance, and managing pump pain from the LVAD device in his chest, which he could feel constantly and caused significant discomfort.Medical team discovered Kyle's cannabis use through drug testing after 6-12 months and removed him from the heart transplant list when he refused to stop using it, stating it was the only treatment that effectively helped his symptoms.Kyle's ejection fraction began improving within 1-2 months after the LVAD installation, and he regained a pulse within three months, indicating his heart was gaining strength through cardiac rehabilitation and exercise.Kyle currently takes only Eloquis blood thinner medication and has lost approximately 50 pounds from his original weight of 215-220 pounds, now maintaining 178-179 pounds and describing himself as being in the best shape of his life.Kyle advises people facing hopeless medical diagnoses to not give up, do their own research, and question doctors' recommendations, emphasizing that cannabis likely saved his life by enabling the sleep necessary for his body to recover.
