He received his undergraduate degree from Kansas State University in Manhattan, KS and his MD from the University of Texas Southwestern in Dallas, TX. He completed his residency at the University of Kansas Medical Center and completed spine fellowships at The Texas Back Institute in Plano, TX and at Thomas Jefferson University in Philadelphia, PA. In 2003 he was awarded the Marc and Elinor Asher Spine Professorship. Dr. Burton's clinical practice is focused on the diagnosis and treatment of complex spinal disorders in both pediatric and adult patients. In addition to his primary practice location at the University of Kansas Hospital, he also travels to both Hutchinson and Salina, KS for a monthly outreach clinic to provide health care to patients in rural parts of Kansas. He is a member of the American Academy of Orthopaedic Surgeons, the American Orthopaedic Association, the Interurban Orthopaedic Society, the North American Spine Society and the Scoliosis Research Society, where he is the incoming Chair-Elect of the Research Council and member of the Board of Directors. He served as President of the Federation of Spine Associations from 2018 to 2019. In 2019, The American Association of Neurological Surgeons and the American Academy of Orthopaedic Surgeons announced a new partnership, the American Spine Registry (ASR), which will be jointly owned and developed by both organizations. Dr. Burton currently serves as co-chair of the Data Use Committee for the ASR. His research interests include the development of disease specific health related quality of life instruments and the study of complications and outcomes associated with spinal deformity surgery. He has authored or co-authored over 236 peer reviewed publications and serves as a Deputy Editor of Spine Deformity, the official journal of the Scoliosis Research Society. In 2006 he helped found and remains on the Executive Council of the International Spine Study Group. This is a consortium of spinal deformity surgeons and researchers at over 15 top academic centers in the United States and Canada with collaborators in Europe and Japan. Dr. Byron Schneider is currently an associate professor with the Department of Physical Medicine and Rehabilitation at Vanderbilt University Medical Center and serves as the Director of the Interventional Spine and Musculoskeletal Medicine Fellowship. Previously, he completed his residency and interventional spine fellowship at Stanford University. He has nearly 100 publications, with a research focus on the safety and outcomes of interventional spine procedures. He has given over 100 lectures at national and international meetings. He currently is on the Spine Intervention Society Board of Directors as the Chair of Research, and within the North American Spine Society is Chair of the Interventional Spine and Musculoskeletal Section as well as Co-Chair for the Coverage Committee. In Part 2 of his presentation, he indicated that the study by Wolf and his group was observational and retrospective, so there are some missing data. They enrolled patients based on provocation discography, which you hope would result in better outcomes. He stated that this number, 50 percent of people saying that they are 50 percent better is very common in pain literature. He wouldn't say it is favorable. Over and over, these are the numbers we see that turn out to be dead ends. These are non-compelling data unless we are able to show they are non-placebo. You need RCTs to do that. He is a huge proponent of observational studies. They can give you very meaningful clinical information, but unfortunately for a new technology like this, we need at least some evidence that these things are doing something beyond placebo. Next, he indicated the discussion in his presentation would shift to discussing some RCTs that have been published more recently. As of right now, however, the totality of evidence because of the negative RCTs in the research done today, stem cells do not work as a treatment for disc-related low back pain. He then described four new RCTs that came out in the last two years that will shed some new light. The first study involved a comparison with saline treatment. Unwanted side effects, such as infections and other complications have occurred as a result of the treatments in the four studies. Safety continues to be a concern in developing effective treatments using stem cell and PRP approaches.

He currently is on the Spine Intervention Society Board of Directors as the Chair of Research, and within the North American Spine Society is Chair of the Interventional Spine and Musculoskeletal Section as well as Co-Chair for the Coverage Committee. In Part 1 of his presentation, the focus was on a systematic review that was done of how stem cells and PRP pertain to back pain. He began by going over the history and regulations. Traditionally as it pertains to all biologics, they were largely exempt from the pathway of the FDA. Because they did not go through that process, these medicines were allowed to be done, but really did not have any insurance coverage. They became a cash cow for those offering fee-for-service treatment for things that did not have a lot of evidence behind them yet. It led to much public confusion, but that gap has shrunk in recent years. The FDA began issuing more restrictive language in 2020 regarding treatments that were not approved by that agency. The primary purpose of the aforementioned review was to look at 50 or more percent relief of low back pain at a six-month outcome. Based on those criteria, the number of citations was whittled down from 3,000 citations reviewed to 37, then finally down to only 12 that actually met the criteria. Those results should be alarming given that hundreds of clinics in the U.S. were providing treatments for cash payments that were based on only 12 research papers. Only one paper was on PRPs and one on stem cell treatment. He then described a study on PRP in 2015. He also provided summaries of other investigations that entailed PRP and stem cell treatments.

In this episode, we chat with Louis DiGiovine PT, who is a clinically talented orthopedic manual therapist with 41 years of experience. Lou graduated from the very first class at The University of Medicine and Dentistry of New Jersey (UMDNJ) in June 1980 giving him the distinction of being one of the first eighteen physical therapists graduating in the State of New Jersey. He continued that “pioneering” spirit when he took his first position as a staff therapist at the busy and reputable Christ Hospital in Jersey City, NJ. Working and training in an urban General Hospital provided him with a strong background in medical and surgical knowledge.  In 1983 he became a co-founder of Hudson Heights Physical Therapy (one of the first few private practices owned by physical therapists in New Jersey).  Through that organization, he performed contract-hospital work at St. Mary's Hospital in Hoboken, Palisades General Hospital in North Bergen, Meadowlands Hospital in Secaucus, Essex County Geriatric Facility in Belleville, The Industrial Medicine Center in Secaucus, Union City Visiting Nurse Association, and Christ Hospital  Homecare Agency for more than five years. During those early years, he attended many extensive training courses, workshops, seminars, in-services, study groups, and conferences throughout the country. Lou learned all of this extensive training to become the best physical therapist that he could be and provide his patients with the best care possible. Throughout his early professional years, his primary area of interest and goals was to become an orthopedic manual therapist with a specialty in spine care. In time, Lou became known as a “spine therapist” and as a result received many more referrals from spine care physicians, also helping him to accelerate his knowledge base in the treatment of spinal injuries, conditions, and surgeries. It was through the discovery process of learning spine care and treating many patients that Lou became acutely aware of the relevance and close interrelationship of back and neck problems, TMJ dysfunction, headaches, upper quarter dysfunction, shoulder problems, posture and pain, orthotics, body mechanics, ergonomics and lifestyle management. All of which he has received specific training and education. He has lectured on these topics to study groups, in-services, seminars, physical therapy students, industrial work sites, corporations, athletic organizations, and varied community groups. Throughout the past twenty-nine years, Lou has participated in various treatment programs and clinics including sports medicine clinics, amputee clinics, prosthetics and orthotics clinics, arthritis clinics, work hardening, worksite ergonomic assessments, pre-employment screens, and pre-season athletic screenings. In 1996 Lou founded Excellent Physical Therapy Back and Neck Care an orthopedic physical therapy private practice dedicated to the treatment of spine care. Within the practice, he developed and implemented administrative and treatment standards of care through policies, procedures, and protocols.  The interweaving of professional knowledge and experience with tested business practices is what made Excellent Physical Therapy a very unique facility where dedication to excellence was evident. Lou DiGiovine PT continued his training and education by establishing a working relationship with two recognized spine surgeons and performing rounds with the physicians and their patients six hours per week for eight years. This once-in-a-lifetime experience allowed him to gain an understanding of the complete spectrum of spine care treatment including medicine, pain control methods, epidural injections, spinal diagnostics, indications, and contraindications of spine surgery, types of surgeries alternative care, and spine surgery rehab. Lou is an active participating clinician of the New Jersey Spine Institute, North American Spine Society, and The American Physical Therapy Association. He is licensed by the New Jersey State Board of Physical Therapy. He has inspired others to follow his lead and with the addition of experienced and qualified staff over the years, he has expanded the scope of Excellent Physical Therapy to include all orthopedic musculoskeletal conditions including the treatment of knees, shoulders, fractures, joint replacement, pediatric and geriatrics.   (Since recording this interview, Lou's practice has been acquired by Jag-One Physical Therapy.  Lou continues to practice here and is also the Clinical Director.) Dr. Schuler is a pioneer in the advancement of disc arthroplasty, minimally invasive spine surgery, and regenerative therapies. He has revolutionized spinal health care across the Washington, DC metropolitan area and the nation. Dr. Schuler is the Physician Assistant Fellowship Program Director at the Virginia Spine Institute. Dr. Schuler is a distinguished fellow in both the American College of Spine Surgery and the American College of Surgeons. They discussed Cervical Spondylotic Myelopathy, the symptoms, the causes, and the surgical approach to treatment. Dr. Sandro LaRocca is director and founder of the New Jersey Neck and Back Institute and is a fellowship trained orthopaedic spinal surgeon who is board certified by American Board of Orthopaedic Surgery. He is a member of the American Academy of Orthopaedic Surgeons, the North American Spine Society, the New Jersey Spine Society, the Society for Minimally Invasive Spine Surgery, The Orthopaedic Surgeons of New Jersey, and the American Medical Association. Dr. LaRocca has been in private practice since 2001. He has extensive experience in minimally invasive and open techniques of spinal reconstructions, stabilization, and nerve (spinal cord) decompression of the cervical (neck), thoracic (mid-back), and lumbar (low back) spine. Dr. Marco Funiciello, DO, is a board certified physician specializing in the non-operative care of spine, joint, muscle and nerve pain. Dr. Funiciello is the Co-Director of the Interventional Spine Program at Princeton Spine and Joint Center. After graduating from the University of Scranton, Dr. Funiciello received his medical degree from UMDNJ-School of Osteopathic Medicine. Dr. Funiciello then received his specialty training in Physical Medicine and Rehabilitation from Saint Vincent’s Hospital in Manhattan where he was honored to serve as Chief Resident. Dr. Funiciello has extensive additional training in Osteopathic Manual Medicine and often utilizes this skill set to complement his conservative and holistic approach to his patients. Dr. Funiciello has authored numerous chapters and papers and lectures widely. He is consistently rated as one of the best doctors in the country by his peers and by his patients.

Dr. Ted Dreisinger (tdreisinger@therapyadvisors [dot] com) graduated from Fairmont State University with a degree in physical education and language arts. He earned a Master’s Degree at the University of Wisconsin-LaCrosse and a PhD at the University of Missouri. Dr Dreisinger’s background spans nearly 40 years of clinical experience in rehabilitation, orthopedics and physical medicine. He has published articles in biomedical journals including Spine, Orthopedics, Journal of Rheumatology, Sports Medicine, Paraplegia, and the Journal of Cardiac Rehabilitation. Dreisinger is a member of the North American Spine Society, fellow in the American Back Society and the American College of Sports Medicine. He is a member of the Board of Trustees for the McKenzie Institute International, the Boards of the Vert Mooney Foundation and the North American Spine Foundation – a non-profit arm of the North American Spine Society. Dr. Dreisinger is President of Therapy Advisors, a consulting firm that provides solutions for the management of chronic back pain and web-based clinical outcomes for medical practices. On the personal side, he has published three books of life observations accumulated over the many years of professional traveling. He lives with his wife Molly in Oro Valley, Arizona.
