Today, surgeons will do everything in their power, not only to remove all of the cancer, but also to restore a woman's natural-looking breasts so she will have a cosmetic outcome that makes her feel and look great again.In many cases, a woman can have one reconstructive implant surgery, Dr. Stefani reports.The second option involves surgically implanting a tissue expander after the mastectomy, followed by a permanent implant at a later time. During the tissue expander surgery, the plastic surgeon expands the breast skin and muscle using a temporary tissue expander (an inflatable breast implant). A few months later, during the second surgery, the expander is removed and replaced by a breast implant. Dr. Stefani chooses the best option for his patients based on what he calls “mapping” of the breast to see what the tissue looks like after surgery, and to determine which procedure provides the safest and best results.During the podcast, Dr. Stefani also talked about animation deformity in breast reconstruction. To alleviate this problem, he performs a pre-pectoral reconstruction on top of the muscle instead of under it. This holds the implant in position better, there is no risk of the implant extruding through the incision and the outcome is “beautiful,” he reports. The breast looks natural, softer and it alleviates what he calls “animatory movement.”Dr. Stefani and Dr. Brown also discussed the importance of breast cancer patients quitting smoking at least six weeks prior to breast surgery. Smoking causes a tightening of the blood vessels that delays and decreases wound healing capabilities. The controversy regarding breast implant-associated anaplastic large cell lymphoma (BI-ALCL) also was a topic of discussion. Social media has created an unnecessarily alarming concern for women with breast implants, the doctors find. BI-ALCL is very rare, generally very slow-moving, and affects only a small number of women who primarily have received textured implants, which Dr. Stefani does not use. Fewer than 10 patients a year are diagnosed with BI-ALCL, while there are more than 10 million women globally who have breast implants. The doctors also talked about breast implant-associated illness. Prostate Cancer and Current Diagnostic Tools, Dr. Parra In American men, prostate cancer is only behind skin cancer as the most common form of cancer. Statistics showed in the U.S. there were about 164,690 new diagnoses of prostate cancer and about 29,430 death from prostate cancer in 2018. In their lifetime 1-9 men will be diagnosed with prostate cancer, and 1 in 41 of those diagnosed will succumb to complications related to the disease. The five-year survival rate for prostate cancer patients falls to 29% when prostate cancer metastasizes and spreads through the body. This staggering decline in survival rate highlights the serious disadvantages the NCCN recommended treatments have when trying to treat prostate cancer that has spread, become treatment-resistant, and recurrent. Prostate-specific antigen, or PSA, is a protein produced by normal, as well as malignant, cells of the prostate gland. The PSA test measures the level of PSA in a man’s blood. For this test, a blood sample is sent to a laboratory for analysis. Until about 2008, some doctors and professional organizations encouraged yearly PSA screening for men beginning at age 50. Some organizations recommended that men who are at higher risk of prostate cancer, including African American men and men whose father or brother had prostate cancer, begin screening at age 40 or 45. However, as more was learned about both the benefits and harms of prostate cancer screening, a number of organizations began to caution against routine population screening. Most organizations recommend that men who are considering PSA screening first discuss the risks and benefits with their doctors. PROSTATE IMAGING: a new horizon in early prostate cancer detection and treatment Recent advances in MRI, including Mp-MRI (“Multi-parametric” testing using a 3T scanner)imaging, have changed everything. The latest generation of MRI machines called 3 Tesla, or 3T machines, scan at a much higher resolution than the earlier machines. They enable radiologists to see all but the tiniest tumors. The tumors that they can’t see almost certainly don’t matter. MRI scanning for prostate cancer could also help a quarter of a million men, maybe up to half a million men a year, to avoid an unnecessary biopsy if the MRI is negative. Scanning the prostate in men with PSA elevation is a brand new approach that is more reliable than the old-fashioned method of using 12 random needle sticks. However, this claim is only accurate when using the very latest state-of-the-art MRI technology at approved centers. This technology is so new that finding doctors willing to abandon the old random needle biopsy approach is still a major challenge. The majority of men will be reassured they don’t have prostate cancer and importantly they may be able to avoid the harms of a biopsy, plus healthcare systems will be able to avoid the costs. PROSTATE IMAGING: a new horizon in early prostate cancer detection and treatment Recent advances in MRI, including Mp-MRI (“Multi-parametric” testing using a 3T scanner)imaging, have changed everything. The latest generation of MRI machines called 3 Tesla, or 3T machines, scan at a much higher resolution than the earlier machines. They enable radiologists to see all but the tiniest tumors. The tumors that they can’t see almost certainly don’t matter. MRI scanning for prostate cancer could also help a quarter of a million men, maybe up to half a million men a year, to avoid an unnecessary biopsy if the MRI is negative. Scanning the prostate in men with PSA elevation is a brand new approach that is more reliable than the old-fashioned method of using 12 random needle sticks. However, this claim is only accurate when using the very latest state-of-the-art MRI technology at approved centers. This technology is so new that finding doctors willing to abandon the old random needle biopsy approach is still a major challenge. The majority of men will be reassured they don’t have prostate cancer and importantly they may be able to avoid the harms of a biopsy, plus healthcare systems will be able to avoid the costs. We interviewed two of the leading orthopedic surgeons in the country, Dr. Nikhil Verma and Dr. Jorge Chahla of Midwest Orthopedics at Rush to discuss biologics. Biologics like PRP, stem cells, corticosteroid injections, and hyaluronic acid are all the rage in medicine right now – and for good reason. Instead of having to reconstruct or repair damaged or torn tissues in the body through surgery, what if we could help relieve pain and improve function with a minimal invasion biologic injection? That is the promise of biologic injections. While significantly more research needs to be done to better understand how to best deliver biologics and what patients they may benefit the most, the future is very promising. As physical therapists, we will always be advocates of more conservative measures like physical therapy or biologic injections prior to more invasive options like surgery when warranted to potentially give our patients the upper hand in their rehab. Gina Price Lundberg MD FACC is an Associate Professor of Medicine at Emory University and Clinical Director of the Emory Women's Heart Center. She is a Preventive Cardiologist and has practiced at Emory Saint Joseph's Hospital of Atlanta since 1998. She was invited to join the Emory University faculty in 2012 and led the development of the Emory Women's Heart program with six locations across the metro Atlanta area. Epilepsy is a medical condition in which nerve cells in the brain now and then send out excessive electrical discharges, causing the episodes that are called seizures. Epilepsy affects over 2 million Americans of all ages. Seizures can range from an odd sensation in the stomach and a vague feeling of confusion to whole-body stiffness and jerky motions, depending on the way that the excessive electrical impulses affect the brain activity. Once the seizure runs its course, usually within seconds or a few minutes, the brain enters a recovery period and then returns to its normal function. About 65 percent of new epilepsy cases have no obvious cause. Of the rest, the more common reasons include stroke, congenital abnormalities (those we are born with), brain tumors, trauma, and infection. Doctors routinely search for the cause with testing such as MRI or blood studies.
