Bill Kanasky, Jr., Ph.D. shares ideas on how to upgrade your voir dire by applying measurement. One of the primary purposes of voir dire is to elicit information and Bill talks about ways to extract information from jurors to get accurate responses. Bill discusses how to handle oral questioning during voir dire, including the use of a 0-10 scale and asking jurors to give one reason for their number. After going over the scientific methodology for this approach to data collection, Bill talks about the art aspect of voir dire: the setup of the question, the answer set/options given to the jurors, and what to do with their answers. As an example, when you get negative information from a juror, you don't want to punish that juror with your reaction because you want to identify other jurors who share that same perspective so you can strike them. Reacting negatively to a juror's response could shut down other jurors who may feel the same way but want to avoid your negative reaction. In the first episode of a special daily series during the 2024 ASCO Annual Meeting, Dr. John Sweetenham shares highlights from Day 1, including exciting data on the CROWN trial in NSCLC, the ASC4First study in chronic myeloid leukemia, and the effects of high-deductible health plans on cancer survivorship. TRANSCRIPT Dr. John Sweetenham: I'm Dr. John Sweetenham, the host of the ASCO Daily News Podcast. I'm delighted to bring you a special series of daily episodes from the 2024 ASCO Annual Meeting and to share my top takeaways on selected abstracts.   Today, I'll be reviewing exciting new data in chronic myeloid leukemia, remarkable outcomes for patients with ALK-positive non-small cell lung cancer, and a compelling study on the effects of high deductible health plans on cancer survivorship.   My disclosures are available in the transcript of this episode.   LBA6500, the ASC4FIRST trial, is a phase 3 combination of asciminib with the current standard of care tyrosine kinase inhibitors, those being imatinib, nilotinib, dasatinib and bosutinib for the first line treatment of patients with chronic myeloid leukemia. The data from this large multinational study, conducted in 29 countries, were presented by Dr. Timothy Hughes from the Royal Adelaide Hospital in Australia. Some patients with chronic phase CML respond well to tyrosine kinase inhibitor therapy, and about one-third may eventually be able to stop therapy and will remain in remission, the so-called treatment free remission or TFR. Unfortunately, almost half of patients eventually need to change therapy due to resistance and intolerance, and most patients will need to remain on therapy for many years, possibly for life.  Asciminib is the first BCR-ABL1 inhibitor to specifically target the ABL myristate pocket or STAMP and was designed to be highly potent but also highly specific, thus minimizing side effects and toxicity. In this large trial, which is the first randomized head-to-head comparison of asciminib with other tyrosine kinase inhibitors, 405 patients were randomized 1:1 to receive either asciminib at a dose of 80 milligrams daily or another investigator-selected TKI. The groups were well balanced for all patient characteristics, including ELTS risk. The primary objectives of the study were to compare the major molecular response rate at 48 weeks with an additional analysis for the patients who received imatinib as the investigator-selected TKI. With median follow-up at 16.3 months for patients receiving asciminib and 15.7 months for those receiving the other TKIs, the 48-week MMR rates were 68% for asciminib compared with 49% for the other investigators-selected TKIs.  The rates of MR4 after 48 weeks, a deep molecular response which is a prerequisite to be considered for treatment free remission, were 39% for asciminib compared to 21% for the investigator-selected TKI. Tolerability and safety were excellent for asciminib, with only 5% discontinued due to toxicity compared to 10% for the other TKI arm. Frequently observed toxicities with asciminib included thrombocytopenia and neutropenia. The investigators concluded that asciminib is the only agent to show a statistically significant improvement in efficacy and toxicity in this patient group when compared with all other TKIs, and that asciminib has the potential to become the preferred standard of care for the first line treatment of CML. Follow-up on the study continues, but there is no question that these are exciting and probably practice-changing results.  The next exciting study, LBA8503, was presented by Dr. Benjamin Solomon from the Peter MacCallum Cancer Centre in Melbourne, Australia. This presentation was an update of the CROWN study for patients with previously untreated advanced ALK-positive non-small cell lung cancer. Lorlatinib is a third-generation brain-penetrating ALK inhibitor which was compared with crizotinib in the CROWN-3 study. This phase 3 study enrolled 296 patients randomly assigned to lorlatinib 100 milligrams once daily or crizotinib 250 milligrams twice daily. The interim results showed a 72% reduction in the risk for progression or death with lorlatinib compared with crizotinib and formed the basis for the March 2021 FDA approval of the drug for metastatic ALK positive non-small cell lung cancer. A subsequent post hoc analysis at three years showed continued progression free survival benefit with lorlatinib compared with crizotinib.  Earlier today, Dr. Solomon presented a further post hoc analysis of the study at 60.2 months of median follow-up. Among the entire patient population, the median PFS was not reached with lorlatinib compared with 9.1 months with crizotinib. At 60 months, the PFS rate was 60% with lorlatinib compared with only 8% with crizotinib. The PSF benefits with lorlatinib were seen across all patient subgroups. The improved control of central nervous system metastatic disease, which was observed in the earlier reports, has been confirmed in this recent analysis. Among those patients with baseline brain metastases, the median PFS with lorlatinib was not yet reached compared with six months with crizotinib. More than half of patients with baseline brain metastases were progression free at 60 months.   But the benefit of lorlatinib is certainly not confined to patients with brain metastases. Lorlatinib also significantly improved progression-free survival among patients without metastases. At 60 months, 63% of patients without baseline brain metastases assigned to lorlatinib were progression free, compared with only 10% of those assigned crizotinib. These are remarkable results. As Dr. Solomon stated in his conclusion, 60% of patients on lorlatinib are still progression free and 92% are progression free in the brain. No new safety signals were seen and the improved efficacy over crizotinib was seen across all risk groups. These results are unprecedented in patients with ALK-positive non-small cell lung cancer.   Concerning data were presented today by Dr. Justin Barnes from Washington University. Dr. Barnes presented results from a retrospective study in Abstract 11005 which showed whether a patient with cancer has high-deductible health insurance can play a role in their survival. Although previous studies have shown care disparities for those with high-deductible plans, this report focuses specifically on effects on survival and concludes that cancer survivors with high-deductible health plans had a greater risk of mortality both overall and from cancer. High-deductible insurance was defined as costing between $1,200 and $1,350 annually for individual insurance, or between $2,400 and $2,700 annually for a family plan. Investigators used data from the U.S. National Center for Health Statistics National Health Interview Survey and linked them to files from the National Death Index to determine mortality rates. Included were more than 147,000 respondents aged between 18 and 84 years who did not have Medicaid. Among these individuals, 5.9% were cancer survivors. The concern for cancer survivors with these plans is that in addition to recurrence that could require costly treatments, there might be issues related to survivorship. Investigators found that overall survival was worse for those with a cancer diagnosis coupled with high-deductible health insurance, with a hazard ratio of nearly 1.5.   But when the researchers reviewed data from the general population without a history of cancer, they didn't find any association between high-deductible health insurance and outcomes. According to Dr. Barnes, the leading hypothesis is that patients with cancer who have a high-deductible plan delay workup for a potential new or recurrent cancer diagnosis or postpone or avoid other care. The results also indicated that survival among certain subgroups, such as non-Hispanic white patients, patients with higher incomes, and patients with at least a college or high school education, was worse for those with a high-deductible health plan, not the groups who are typically impacted by care disparities. It is possible that these individuals are more likely to select high-deductible health plans and that having these plans might counteract what might otherwise be adequate access to care.  A key take-home from this analysis is that cancer patients and survivors, whatever their racial, ethnic, or socioeconomic status, should have access to health plans with low deductibles and should be informed of the potential risks of their long-term health and survival when covered by high-deductible plans.   Join me again tomorrow to hear more top takeaways from ASCO24. Les réactions politiques s'enchainent après la mort de l'opposant Yaya Dillo, tué le 28 février lors de l'assaut donné par l'armée contre le siège de son parti. Le porte-parole du gouvernement a affirmé ce vendredi sur RFI que ce sont les hommes de Yaya Dillo qui ont ouvert le feu et contraint à la riposte. Il assure que s'il s'était rendu, la situation n'aurait pas dégénéré. Les soutiens de l'opposant dénoncent un assassinat. La version officielle ne satisfait pas non plus Max Kemkoye, du groupe de concertation des acteurs politiques, une plateforme d'opposition radicale à la transition, politiquement proche de celle de Yaya Dillo. RFI : Yaya Dillo et vous, vous étiez des opposants résolus à la transition depuis le début, comment est-ce que vous avez pris ces événements tragiques de la semaine ? Max Kemkoye : J'ai été atterré, profondément bouleversé. On s'est vus avec Yaya deux jours avant. On s'est appelé 24 heures avant. Au petit matin, j'ai appelé sur ses deux numéros privés. Ça ne sonnait plus, je m'en suis inquiété. Les amis et quelques partenaires qui m'ont approché pour en savoir plus, je leur ai dit ‘moi non plus je n'ai pas la position de Yaya, je ne sais pas s'il est mort ou vivant cette nuit-là'. Quand quelqu'un m'a appelé pour me donner la confirmation de la mort de Yaya, je n'en suis pas revenu, je m'en suis difficilement remis et ça continue encore. Mais, pourquoi je suis réconforté ? Simplement parce qu'il faut honorer la mémoire d'un grand camarade de lutte, comme Yaya Dillo.  Selon les autorités, ce sont les forces de défense et de sécurité qui ont répondu à des tirs venant des gens du PSF. Qu'est-ce que vous pensez de cette version ? Le gouvernement et les autorités judiciaires se sont lancés dans un narratif, mais c'est un vernis qui cherche à donner un habillage à un gros mensonge. Tous les Tchadiens le savent, y compris les éléments des forces de défense et de sécurité le savent. Nous exigeons d'abord des clarifications, mais des clarifications nettes. Ce sont des narratifs bidons du ministre de la Communication, du procureur de la République qui ne convainc personne. Nous exigeons du Premier ministre ce texte opposant qui a permis que tout ça arrive, qu'il vienne nous donner des explications claires, c'est à lui, le chef du gouvernement et le président de transition. Ils doivent nous donner des clarifications nettes. Ensuite, nous exigeons une enquête indépendante. Mais cette enquête indépendante, ce ne sera pas une enquête locale qui a été toujours biaisée et enterrée, comme des précédentes enquêtes. Et c'est là-dessus que nous attendons les partenaires et amis du Tchad. C'est là-dessus que nous attendons les Nations unies. C'est là-dessus que nous attendons l'Union africaine. Sans quoi nous n'allons pas accepter.Le ministre dit que si Yaya Dillo s'était rendu, la situation ne serait pas arrivée à une telle extrémité. Totalement faux. Est-ce que Yaya Dillo a reçu régulièrement une convocation venant du procureur de la République ? Non. Première, deuxième, troisième convocation, comme se veut la procédure et qu'à l'issue, si Yaya refuse d'obtempérer, le procureur de la République a la possibilité – avec les officiers de la police judiciaire, la gendarmerie et la police – d'interpeller ou de prendre Yaya Dillo pour l'astreindre à une contrainte par corps et de l'amener, puisque Yaya Dillo est un civil, il n'est pas militaire. Le cocktail explosif, il est réuni mais ce que nous regrettons, c'est le facilitateur Felix Tshisekedi, qui s'est fendu dans un communiqué incompréhensible qui se permet de décrire une situation dont il n'a même pas les éléments d'appréciation. Pourquoi est-il allé si vite en besogne ? Mais ce n'était pas le cas, puisqu'il est l'appui inconditionnel de la junte au pouvoir. Il en fait usage pour produire son communiqué. Mais pour le reste, les Nations unies sont interpellées et avec en tête, les États-Unis qui apportent leur soutien inconditionnel à cette junte aujourd'hui qui tue les acteurs politiques, cette junte qui a tué déjà 300 jeunes Tchadiens qui sont sortis pour exercer leurs droits et dont on a accordé l'impunité, c'est à dire la licence de tirer pour tuer. Et nous pensons que les États-Unis d'Amérique, la France et au-delà l'ensemble de la communauté internationale incarnée par les Nations unies, ils ont la clé des solutions du problème du Tchad. Si la presse sénégalaise relaye cette date, annoncée hier jeudi par le président sénégalais, c'est plutôt celle du 2 avril, jour de la fin de son mandat qui préoccupe les journaux. « La date de mon départ reste absolument ferme », titre Sénégo qui cite Macky Sall.Et après, que peut-il se passer ? « Le brouillard plane sur l'avenir politique du Sénégal », nous dit Seneweb.Mais il n'y a pas pour autant 1 000 scénarios possibles. Rewmi cite le ministre de l'Intérieur : « Nous sommes dans une situation inédite, car la Constitution avait prévu trois cas de figure : l'empêchement, la démission et le décès. »Et ce sera au Conseil constitutionnel de trancher, poursuit Dakar actu : « Soit le conseil choisi que le président de la République ne peut pas partir, il reste en place pendant deux mois pour remettre la clé au président démocratiquement élu. Ou bien, le Conseil constitutionnel constate qu'il y a vacance de pouvoir et c'est le président de l'Assemblée nationale qui devient automatiquement président de la République. »Dans ce dernier cas, précise Dakar Matin : « Tout le processus électoral sera repris, il n'y a plus de droit acquis pour les 19 candidats, car le président de l'Assemblée national doit organiser de nouvelles élections. »Et puis il y a tout de même une dernière possibilité, relayée par l'avocat Abdoulaye Tine, membre de la coalition d'opposition F24 dans les colonnes de Dakar actu : « Si le président ne démissionne pas d'ici le 02 avril, l'opposition et les forces vives prendront leurs responsabilités car, elles ne comptent pas laisser le pouvoir sombrer. »Loi d'amnistie annoncée par Macky Sall lors du dialogue national« La loi d'amnistie ou le second assassinat des martyrs de 2021 à 2024 », titre Seneplus. Pour eux, « l'usage politicien de l'amnistie brouille toute perspective de réalisation d'un avenir collectif empreint de sérénité et d'apaisement ».Jeune Afrique constate que même au sein du camp présidentiel, l'idée ne fait pas l'unanimité. JA cite Samba Sy, ministre du Travail et secrétaire général du Parti de l'indépendance et du travail : « Je ne dis pas que nous ne devons pas nous réconcilier, mais n'oublions pas ce qu'il s'est passé pour que, demain, les mêmes causes ne produisent pas les mêmes effets. »Et le Monde Afrique cite un habitué du palais présidentiel qui assure que « Samba Sy a dit tout haut ce que pensent beaucoup d'élus de la majorité ». Et le quotidien constate que du côté de l'opposition, on voit dans la loi d'amnistie une manœuvre du président sénégalais : « Macky Sall et son équipe sont en réalité hantés par la perspective de devoir répondre de leurs actes, car le peuple ne pardonnera jamais », confie Thierno Alassane Sall, opposant et candidat à la présidentielle. La majorité présidentielle pourra également compter sur l'apport de voix des 24 élus du Parti démocratique sénégalais (PDS, de Karim Wade), son nouvel allié de circonstance au sein de l'opposition ».Mort de l'opposant Yaya Dillo au TchadTrès factuelle, la presse tchadienne se fait l'écho de l'annonce par le procureur de la république de la mort de Yaya Dillo lors de « l'assaut au siège du Parti socialiste sans frontières » ce mercredi 28 février.Tchad Vision relaye le bilan :« Une dizaine de morts, de nombreux blessés et 26 interpellations. »Le pure player qui ajoute que « d'après la version officielle, les forces de l'ordre se sont déployées en riposte, à l'attaque des locaux des services de renseignements ».Jeune Afrique précise les circonstances et les raisons de cette attaque : « Des éléments du PSF sont arrivés à bord de onze véhicules afin de libérer Abakar Torabi, le secrétaire chargé des finances de leur parti. Ce dernier est suspecté d'avoir fomenté, le 19 février, une tentative d'assassinat contre le président de la Cour suprême. »JA constate que « le flou demeure à Ndjamena, ou les réseaux téléphonique et internet sont largement perturbés ». A new build configuration flag, --disable-gil will be added to the configure script that will build CPython with support for running without the global interpreter lock. "In short, the SC accepts PEP 703, but with clear provisio: that the rollout be gradual and break as little as possible, that we can roll back any changes that turn out to be too disruptive – which includes potentially rolling back all of PEP 703 entirely if necessary (however unlikely or undesirable we expect that to be)."
