Break 1: 07.14.22 KIA MOG STORY 1: Lilburn's Mina Uddin Receives David Jemel Holloman Memorial Scholarship Mina Uddin of Lilburn was awarded the David Jemel Holloman Memorial Scholarship by Georgia State University's College of Education & Human Development. Established by alumnus Darryl Holloman in memory of his brother, the scholarship honors an outstanding undergraduate majoring in early childhood and elementary education. Uddin received the award at the CEHD Scholars Recognition Luncheon this spring. STORY 2: Mall of Georgia Shoplifting: 19 Arrested For Stealing More Than $5K Of Merchandise In late May, Gwinnett County Police, alongside 11 retailers and the Georgia Retailers Organized Crime Alliance, conducted a two-day retail theft operation at the Mall of Georgia. The effort led to 19 arrests, recovery of $5,313.75 in stolen goods, and the clearance of 24 outstanding warrants. Officers also issued 12 felony and 27 misdemeanor warrants. Among those arrested were Demesha Brown and Jasmine Harris, both with 12 active warrants, including a RICO charge for Brown. The operation involved 19 officers from multiple precincts. STORY 3: AROUND TOWN: Annandale Village To Name New Program Center In Honor Of Adam Pomeranz This month, Annandale Village in Suwanee will open the Adam Pomeranz Program Center, honoring its CEO for over 20 years. The $8 million facility, funded by the Keadle family, includes a gym, art and music therapy spaces, and a 300-seat auditorium. Meanwhile, Britt Ramroop was named CEO of the Gwinnett County Police Foundation, bringing 15+ years of nonprofit leadership to support officers and community programs. Former Rep. Farooq Mughal is launching his campaign to reclaim Georgia House District 105, with a kickoff event on June 7. Celebrating the South’s largest Korean population, the pass offers a self-guided cultural and dining experience. STORY 5: Jackson EMC Foundation awards $76K to agencies serving Gwinnett County residents The Jackson EMC Foundation awarded $101,529 in grants, with $76,693 benefiting Gwinnett County organizations. Notable recipients include Mending the Gap for senior food boxes, Ser Familia for Latino family counseling, and Mosaic Georgia for aftercare counseling for abuse survivors. Other grants supported programs like trauma counseling, addiction recovery, and youth therapy. Funded by Jackson EMC members through the Operation Round Up program, over $21.7 million has been donated to communities since 2005. Individuals and organizations in Jackson EMC’s 10-county service area can apply for grants online. Break 3: STORY 6: Kurt Eisele — Owner of Kurt's Bistro — Remembered For Bringing European Culinary Style to Atlanta, Gwinnett Kurt Eisele, founder of Kurt’s Bistro in Duluth and a pioneer of European cuisine in metro Atlanta, passed away on May 29 at 82, alongside his wife on their 60th wedding anniversary. A Stuttgart, Germany native, Eisele moved to Atlanta in 1965, opening Kurt’s in 1985 and mentoring many chefs throughout his career. Known for his dedication, he worked even on the day he passed. His legacy was celebrated with an elaborate 80th birthday event featuring dishes inspired by his life. The family plans to continue operating Kurt’s Bistro, honoring his passion for hospitality and service. STORY 7: Adidas Signs Grayson Basketball Star Caleb Holt to NIL Deal Grayson basketball star Caleb Holt has signed a lucrative NIL deal with Adidas, joining fellow Rams athlete Tyler Atkinson. Holt, a 6'5" five-star guard and top-10 national recruit for the Class of 2026, boasts an NIL valuation of $1.3 million. Scripture Passage 2 Corinthians 5:17 Worship Video Worship Audio Sermon Script The essence of the gospel is about change. Once I am in Christ, I am a new creation. The old has passed, the new has come. The person I am today is not the person I will be tomorrow. That is good news. In Christ, there is always the possibility of a new beginning. But many times, that change is not easy. We just came back from Sioux Valley yesterday. Thank you for your prayers and support. Sioux Valley is located in the southwest area of Manitoba. This year, seven members from London Korean Christian Church joined our team of twenty-seven, to make a total of thirty-four people. It was the second trip there for our church members. Joon Song and Sarah Park from the KSM went for many years, from 2002 to 2015 with their old church. Last year, we went for the first time. We had a great experience last year. We formed great bonds. But a whole year had passed. We weren't sure what it was going to be like this year. Would they remember us? This year, no one knew we were coming. So as soon as we arrived on Monday, we unpacked, had dinner, and immediately went out to knock door to door. When people opened the door, we introduced ourselves as the Koreans from Toronto, and that we were back. We were pleasantly surprised to see that many people remembered us! Their faces relaxed and they became friendly. Tuesday was our first day of programming. Over 100 people showed up – from kids all the way to adults. The seeds of love we planted last year bore some fruits of change. We recognized many of the kids – they had grown up so much! Things were off to a great start. But then we received some bad news: the main hall that we were using had to be vacated for a funeral on Thursday. So that meant we had to leave the hall by 3pm the following day, Wednesday. They asked us where we would stay. We were shocked! We said we don't have any other place to stay. They felt really bad and offered us a smaller space at their community center. But we had to move everything over to the community centre and the church, because the community center wasn't large enough by itself. Our kitchen team had to pack all the kitchen stuff and food. Our drivers and team members had to transport everything to the community center and the church. The kids and youth had their program in the hot non-air conditioned church on Wednesday. On Thursday they had to go outside to the park all afternoon where it was so hot. Meanwhile, everyone else had to re-pack everything to go back to the hall. It was very tiring. Some of our young people became sick. It seemed like a message from God saying that change doesn't come easily. The path to real change is never easy or smooth. Our challenges were so small compared to the challenges that people in Sioux Valley face. Everyday seems to bring new challenges for them. The very first day we were there, one of our close friends from Sioux Valley shared how her nephew just died last week from a car accident. He was a respected teacher at the Sioux Valley school. He knew the history and traditions of his people. And now they lost a respected role model for the youth. Our worship service that day became a space of mourning for her and she broke down during our prayer time. One of the boys who came was found to have lice in his hair. For the sake of the other children and our mission team members, I had to send him home. I felt so bad, because he had been having such a good time. He was playing and smiling. When I broke the news to him, I was expecting disappointment. But instead, all he said was ok with no reaction. That surprised me. He was around the same age as Joonie. If I had to send Joonie home when he was having the time of his life, he would have protested and been very upset. But this boy expressed no such thing. It made me wonder what he had gone through to have no expectation. It's almost like he expected nothing of life. At such a young age, he had faced one disappointment after another, to the point where he no longer expected anything of life. Another young man I met last year had a powerful spiritual experience last year. The message and prayers really hit him. I was very happy to see him again, and he joined our worship service. During that service, I got to speak with him at length. He too is going through so many struggles and challenges. He's trying to keep things together in his family. He's been one of the few to be on the straight and narrow path. He said that all of his peers his age succumbed to drug addictions or suicides, that not many were left. These are only a few stories. In our surroundings, these stories would be wild exceptions. But I'm not exaggerating in saying that these stories are everyone's stories in the community. It's not a large community – there are around 1400 people living on the reserve, and another 1400 who live outside of the reserve. And for those who live on or off the reserve, these stories run through every individual and family. Underneath these endless stories of despair, addictions, abandonment and death is the sense that their lives don't matter. That no one cares for them. Their hearts are broken and shattered. Even when they try to mend their hearts, more things are thrown at them that keeps their hearts broken. Do you know what the most significant result of a broken heart is? It's conflict and division in the community ( ). When your heart has been shattered by violence, you don't know how to love others. There is a lot of conflict and division in the community. For KSM: conflict and division in our community. This is the result of centuries of colonial policies that broke down the most important part of one's life – the family. Families were shattered and broken by residential schools and forced adoption of indigenous children into other families. They taught them that who they are is bad and that they don't matter. With the people of our KSM: what would have happened if Japan had stayed in power? One reason so many kids and youth came to us is because there are no other programs or camps for them in the community. When community leaders do try and organize something, it doesn't work out because no one attends. If one group comes, then those who don't like them won't come. Leaders are divided and in factions ( ). Families don't like other families. Even the Christians there are divided and don't get along with each other. Change can only come if the community comes together. If the community works together to figure out what to do about drugs and addictions. About the despair young people feel. That change can come only when people's hearts are healed. The change they need is healing that will bring their community together. My heart was heavy the whole time I was there. I wondered how change could come when there is such weight on everyone's hearts. Even going to school from junior high on up is such an ordeal. Kids have to wake up at 6am to get the bus to go into Brandon (half an hour away). Then they get bused back to school. It's hard enough for parents in our church to wake our kids up. At least we have parents to wake you up. But if your family doesn't care what you do, what motivation do they have? Only the power of God can heal hearts and bring change. That's the only conclusion I have. I don't know how, but only God can bring change through the power of love. One of our prayers for this trip was that we begin to make relationships with leaders in the community who are trying to help. Last year, we only built relationships with the children and families who came to us. There are leaders who care very deeply. Elders who worry about the youth. We wanted to connect with them. God opened up doors to new relationships. A few of us met the drug addiction workers in the community. I met the executive assistant of the Sioux Valley school in Brandon. One lady works for the family services department. She was so impressed by what we're doing. What we're doing is exactly what she was hoping to see in the community. So she first gave us hundreds of water bottles and juice boxes. She then said next year that her organization will give us $5000 for us to run and expand what we do there. These are the small ways that God is planting seeds for change. One burden on my heart from this trip is for the youth of Sioux Valley. That's the group that all the leaders and elders of the community feel the heaviest weight for. Right from the first evening there, our friend Dallas beckoned me over. She was with Rev. Dave and another two elders in the community. She said we need you to send Pastor Dave to be our full-time youth pastor in Sioux Valley . My prayer request is for a pastor, a shepherd, who can be there week in, week out with the youth of Sioux Valley. There were a group of guys who came in and out throughout the week. They look kind of intimidating. But on the last night, when everyone formed a circle to sing our last song, I made everyone, including them, join the circle. I prayed for them and hugged them. I told them that I want them to take good care of themselves over the next year, and that I want to see them again next year. I began to imagine the impact that a full-time youth ministry can have there. I've seen the impact our youth ministry has had on our members. And my prayer is that God may send someone to be with the youth. Change is so difficult. It doesn't come without a cost. The cost that God paid for change was the death of his son Jesus Christ. Because of Christ, we can be healed and changed I don't know what it will take for change to come to Sioux Valley. But I pray that God may bring change. The Karius Diagnostic Test: A Missing Puzzle Piece

Billions upon billions of microbial organisms inhabit the earth and our bodies. When we get sick because of modern-day medicine and technology developments, we can often pinpoint exactly which microbial organisms are the source of the problem. But how, exactly? Karius Chief Technical Officer Sivan Bercovici and Medical Director Dr. Sarah Park joined Host David Kemp to talk about the Karius diagnostic test and how it breaks barriers in antimicrobial research. Karius surfaced out of transplantation research at Stanford, where researchers learned DNA from microbes is sent to the bloodstream and can then be harvested and analyzed. This analysis has two aspects: chemistry, which is capturing as many molecules as possible, and machine learning, which makes sense of the data. “We all have microbes, and we have mixtures of microbes. And what's making very hard is that this mixture of microbes or self-DNA coming from these microbes, they can be similar to one another, they can be similar to other things,” explained Bercovici. "Machine learning had to kick into de-convolute that signal to tell you exactly which microbes you have and exactly what concentration of those microbes,” he continued. Bercovici added, “Unless you apply these algorithms, you end up with a pile of pieces of DNA that are impenetrable for any physician.”The immunocompromised population is just one example of why diagnostic tests are necessary. With this population, they could have multiple infections due to a lack of immunity. “As a clinician, you really have to think about ‘What test do I order?'” Dr. Park explained. This can be difficult with insurance, which may not agree to cover a test a physician deems necessary, and physicians often end up ordering multiple tests. “This is where tests like the Karius test can be very useful in conjunction with your other tests to be able to add potentially the answer to the question you're asking, which is: “What is causing the problem for my patient?'” Dr. Park stated.With the Karius test, the earlier you use it, the better the results will be, but the results still need to be interpreted in the context of each patient. The test result is not meaningful until put in the context of the patient—what medications they are on, their potential exposures, and other factors. The same microbe detected in patient A will not have the same effect in patient B if their clinical status is different. "It's like looking at pieces of a puzzle, and some of the pieces are together, and without the Karius test, we are still missing a huge part of the picture. And hopefully what the Karius test does is fill in more of that picture and make that picture more meaningful so that the clinician can actually use the appropriate antimicrobials, know how to manage the patient better, potentially have that chance to save the patient,” Dr. Park explained.For more on the Karius diagnostic test and its impact on antimicrobial research and clinical use, visit Spotify, Apple iTunes, YouTube, or www.marketscale.com.

Episode 90: Vaccines and Acne. Updates on pneumococcal and COVID-19 vaccines. We are not sponsored by the manufacturers of these vaccines. Terminology of pneumococcal vaccines: PCV13: Prevnar13®PPSV23: Pneumovax23®PCV15: Vaxneuvance® PCV20: Prevnar20®Tips about pneumococcal vaccines:-Prevnar13 is no longer used in adults. -Pneumovax23 is still being used in adults.-The two newer members of the pneumococcal vaccines are: Prevnar20® (PCV20) and Vaxneuvance® (PCV15). The following groups of patients are all adults 19-64 with underlying conditions OR >65 years old. Group A: Unknown or no prior doses of Prevnar13 or Pneumovax 23Option 1: Prevnar20 given as a single doseOption 2: Vaxneuvance followed by a dose of Pneumovax23 at least a year later (Consider >8 weeks in patients >19 at the highest risk)Group B: Previously received Pneumovax 23Give Prevnar20 or Vaxneuvance (at least 1 year since the last Pneumovax 23)Group C: Previously Received Prevnar13Give Pneumovax23 or Prevnar20 (if Pneumovax 23 is not available) >1 year since last dose of Prevnar13Group D: Previously completed series of Prevnar13 and Pneumovax23 in any orderNo additional doses are needed. Scenario 1: 68 yo M who has not previously received PCV or whose previous vaccination history is unknown (Group A). This patient should receive: 1 dose of Prevnar20 and be done, or Vaxneuvance followed by a dose of Pneumovax23. Scenario 2: 25 yo F with HIV not previously received PCV or whose vaccination history is unknown (Group A). This patient should receive: 1 dose of Prevnar20 and be done, or Vaxneuvance followed bya dose of Pneumovax 23 given 8 weeks later. This patient is in the highest risk group. Scenario 3: 50 yo M with chronic alcoholism who has not received any vaccine or unknown status (Group A). This patient should receive: 1 dose of Prevnar20 and be done, or Vaxneuvance followed by Pneumovax 23 one year later. Scenario 4: 43 yo M with previous Pneumovax 23 only (Group B). This patient should receive either: a single dose of Prevnar20 or Vaxneuvance and be done with either vaccine. Give either vaccine at least 1 year after Pneumovax 23. Scenario 5: 25 yo F with CSF leak and previously received Prevnar13 (Group C). This patient should receive Pneumovax23 or Prevnar 20 (if Pneumovax 23 is unavailable) at least one year after her las Pneumovax dose. Scenario 6: 35 yo M who previously completed Prevnar13 and Pneumovax in any order because he has a cochlear implant (Group D). This patient should NOT receive any additional dose. Research and MonitoringCDC and ACIP will continue to assess the safety of Vaxneuvance and Prevnar20 vaccines (the new kids on the block), monitor the impact of the implementation of new recommendations, and assess post-implementation effectiveness and recommendations as appropriate. Examples of risk factors to consider administration of pneumococcal vaccines: Chronic renal failure, HIV infection, alcoholism, cigarette smoking, chronic heart, liver, and lung disease. For a complete list of conditions, visit CDC.gov.___________________ A second booster shot of COVID-19 vaccines. By Hector Arreaza, MD.On March 29 and 30, 2022, CDC announced that a second booster dose of any mRNA COVID-19 vaccine may be given to certain individuals who are at risk of severe outcomes from COVID-19(1). Individuals who may choose to receive a second booster are: 1. People older than 12 years of age who have a moderate to severe immunocompromising condition. Remember, use Pfizer for older than 12 yo, and Moderna for older than 18 yo.2. People older than 50 years of age who are NOT moderately or severely immunocompromised.3. People 18-49 years of age who are NOT immunocompromised but received the J&J COVID-19 vaccine as both the primary and booster dose. When can you receive the second booster shot? At least 4 months after the first booster dose.Who is considered up to date? A person is considered up to date when he/she has received all recommended doses in their primary vaccine series, and a booster dose when eligible. A second booster dose is not required to be considered up to date at this time.Underlying medical conditions associated with higher risk for severe COVID-19 include: Cancer, obesity, cerebrovascular disease, diabetes mellitus, HIV, obesity, COPD, smokers, and chronic liver disease.Comment: Remember to give the second booster to your patients. ____________________Acne Treatment.  By Sarah Park, MS3, University of California Los Angeles. Discussed with Hector Arreaza, MD.  Definition: Acne vulgaris is a common inflammatory disorder of the pilosebaceous unit, which includes the hair follicle and sebaceous gland. It is characterized by chronic or recurrent development of papules, pustules, or nodules commonly on the face, chest, or upper back.(1,2) Acne affects nearly 50 million people in the U.S. per year and can cause significant psychological distress in those who are affected. It primarily begins at puberty when the production of androgens and/or sensitivity of androgen receptors increase, thereby commonly affecting adolescents and young adults.(2) Pathophysiology: The pathophysiology of acne involves four main processes: 1) sebum overproduction, 2) hyperkeratinization of the follicle, 3) bacterial colonization by Cutibacterium acnes, and 4) inflammation.(2,3) It can be classified as mild, moderate, or severe based on the extent and types of lesions.3Treatment: Treatment is selected based on the severity of the condition, patient preference, and tolerability. Acne treatment often requires long-term, consistent use of one or more medications.(3) The main objective of treatment is to decrease sebum production, get rid of extra keratin, treat infection and decrease inflammation. You can warn your patients that their skin may feel dryer and more scaly than usual, but that's part of the treatment. For mild and exclusively comedonal acne, topical retinoids like tretinoin are the treatment of choice(4), but topical retinoids can be used in any level of severity for maintenance. Examples: Adapelene, tazarotene, and tretinoin, For mild inflammatory papulopustular acne or mild mixed comedonal and papulopustular acne, topical retinoids may be used in combination with antimicrobial therapy (either combined with benzoyl peroxide or combined with benzoyl peroxide plus clindamycin or erythromycin). If patients cannot tolerate a topical retinoid, alternatives include salicylic acid and azelaic acid. Of note, oral or topical antibiotics should only be used in combination with benzoyl peroxide and retinoids for a maximum of 12 weeks. If unresponsive to these topical therapies, namely retinoids, benzoyl peroxide, and/or clindamycin, alternative therapies may be initiated. These include topical dapsone, minocycline, and clascosterone.Topical dapsone is an effective treatment for both inflammatory papulopustular and comedonal acne lesions. Topical minocycline is an alternative topical antibiotic used for specifically moderate to severe acne. And last but not least is topical clascosterone, a relatively new topical (specifically an androgen receptor inhibitor) approved by the FDA in 2020.(4)Treatment for moderate to severe acne: For moderate to severe acne vulgaris, management is systemic therapy. This includes oral antibiotics or hormonal therapies, often used in conjunction with topical therapy, or monotherapy with oral isotretinoin. 1. Oral antibiotics for acne vulgaris include doxycycline, minocycline, and sarecycline. Treatment should be limited to three to four months.(5)2. For female patients, hormonal therapy with oral contraceptives and/or spironolactone is also an option. A meta-analysis comparing oral contraceptive therapy and oral antibiotic therapy suggests similar efficacy for the treatment of acne. OCP treatment is often the first-line choice for hormonal therapy, especially for patients who desire the added benefit of contraception. Spironolactone is often used for patients who have contraindications to OCP therapy or prefer to avoid OCPs. Both methods work to inhibit acne by reducing the effects of androgen on the pilosebaceous unit.53. For severe, extensive, nodular acne vulgaris, oral isotretinoin is the drug of choice. It is given as a monotherapy and is often used when all other treatment modalities fail.  Oral isotretinoin is the only medication that can permanently affect the natural course of acne by affecting all four factors in acne pathogenesis. Isotretinoin is most notably known for its teratogenic adverse effects and so is contraindicated in pregnant women and pregnancy must be avoided during therapy by using two forms of birth control.(5)Comment about isotretinoin use: Although prescribing isotretinoin (brand name Accutane®) is within the scope of family medicine, many providers choose not to prescribe it because of lack of training, monitoring hassles, fear of side effects, especially due to concerns with teratogenicity. Isotretinoin is an effective treatment for a condition that can not only disfigure and scar the face but can also cause significant psychosocial dysfunction. Dr. Van Durme recommended when you prescribe isotretinoin, you should have a regular schedule of monthly laboratory tests (including pregnancy test), then office visit, and then prescription, in that order. This schedule will improve the likelihood that side effects are managed promptly and medication is taken appropriately(7). If you would like more information about prescribing isotretinoin, visit https://ipledgeprogram.com.Conclusion: Use topical retinoids alone for mild cases of acne; topical retinoids combined with benzoyl peroxide or topical clindamycin or erythromycin for moderate cases; and topical retinoids combined with benzoyl peroxide and oral antibiotics in severe cases. Remember that isotretinoin is an oral treatment reserved for severe inflammatory papules and pustules with nodules. Treating acne effectively can certainly improve the quality of life of your patients. Now we conclude Episode 90 “Vaccines and Acne”. We gave you an update on pneumococcal and COVID-19 vaccines. Prevnar 20 seems to be the new star in the show. PCV15 is also useful but it needs to be followed by a shot of Pneumovax 23. Regarding COVID-19 vaccines, a second shot may be given to patients older than 12 who are immunocompromised or patients older than 50 who are NOT immunocompromised. Then we finished with a discussion about acne and we learned that topical is usually enough for mild cases, but oral therapy may be needed in moderate to severe cases of acne. Hey y'all! In this episode, I am joined by two beautiful sisters in Christ. These two are fellow AAPI Christians and in my grade at the University of Georgia. Ally Murari (Indian/Music Major) and Sarah Park (Korean/Marketing) to share testimonies and words of encouragement. This episode focuses on Ally's story - specifically how she encountered Christ over quarantine (through Sarah), began listening to worship, and is now (almost a year later) a new born Christian!!! Worship is powerful. Though Ally primarily grew up Hindu, her temple was very multi-cultural/religious, and took part in various forms of worship (Christianity, Islam, Buddhism, Zoroastrianism, etc.). She shares her experience and the beliefs of her previous faith, and how she realized "there's something about Jesus". Powerful. We urge one another to truly search the scriptures for Truth (what does the Bible even say about the Person of Jesus?), to incline your ears away from "sweet, self-gratifying/health and wealth gospel, to test the spirits and weed out the false prophets. The state is reporting 18 new COVID-19 cases Thursday on Oahu, in the biggest single-day jump in two months. The spike comes after several days of smaller gains, but as public health officials continue to track clusters in Central Oahu, Honolulu and at a skilled nursing facility in Makiki. The new cases bring to 762 the number of people who have tested positive for COVID-19 since March 1. Of those, at least 640 have been released from isolation after recovering. The state Health Department said Thursday that the increase was “expected” as the economy continues to reopen. On Friday, several higher-risk businesses on Oahu ― including bars, movie theaters and gyms ― will be allowed to welcome customers back. “This spike in cases and other recent daily spikes have been expected as people begin to move around more freely and more businesses are reopening,” said Dr. Sarah Park, state epidemiologist, in a news release.
