In the article Dr. Short talks about Acute to Chronic Workload Ratio (ACWR) and how it can help coaches plan their workload. Today on the podcast, is a lecture that I gave to all of the College Gymnastics Coaches last year when our research paper on Achilles studies, Achilles tears, and NCAA gymnastics was being published.    I wasn't able to share this as a podcast because we had not yet published our paper, but now that it's out, and many people have read it and have talked about it, I can share this lecture.    Obviously, there is a huge issue with Achilles tears in NCAA gymnastics and this is a problem that has been going on for the last four to five seasons.    Before COVID in 2018, there were about 20 Achilles tears in the first couple of months of the season, which was absolutely devastating to watch.    This lecture gives an outline of my ideas, shares conversations with world experts on Achilles' tendons, and summarizes and presents the research in a way for people to think about them.   As well as provide some options for what we can do, both before and during College Gymnastics to possibly start reducing the risk of tears happening.     I DISCUSS; The elephant in the room.   What are the possible risk factors?   Why you can't rupture a tendon if you don't have pathology.   NCAA Achilles Paper finding.   What can be done Pre and during NCAA to help reduce Achilles injuries.   And…   Confusing questions answered.   RELATED EPISODES: PROFESSOR JILL COOK ON ACHILLES TENDON INJURIES, ACHILLES TEAR RISK FACTORS, RISK REDUCTION  How To Train Harder and Smarter Using Workloads with Tim Gabbett and Scott Willgress

In this episode we were delighted to welcome Dr Tim Gabbett who consults across numerous elite sporting domains worldwide and talk about load management. He talks about what load actually is, how people respond to it and how it can be progressed safely. A great analogy of 'beer tolerance' is discussed, along with what the acute to chronic workload ratio is and how it can be calculated. The biggest application of this for clinicians is clearly how it should influence their history taking of injured atheltes/patients (asking about previous weeks load along with psyhosocial factors that may influence load capacity), and how they can advise their patients monitor their own load in a free and easy way (by multiplying their weekly intensity by volume and frequency). We also touched on the limitations of the "10% rule"

Tim continues to work as a Sports Scientist and Coaching Consultant for several high performance teams around the world. Some of the teams being the biggest sporting franchises on the planet including the Chicago Bulls, Liverpool Football Club, Chelsea and Arsenal Football Club, Manchester United, Australian Rugby Union, San Francisco Giants, British Athletics, USA Olympic Committee and the list goes on and on. During this episode Dr Gabbett shares why training harder might actually be training smarter. We explore some of the factors that determine what training load you can handle at any given point in time. Tim shares a great analogy with our capacity building as athletes that involves the floor, the ceiling and the basement. This week, Dr. Tim Gabbett joins me on the Healthy Wealthy and Smart podcast to talk about how he prepares athletes to perform at high levels of competition and mitigate injury risk during critical periods of play. Tim is an applied sports scientist who consults with elite international athletes and has authored numerous research papers that benefit the sports performance community. In this episode, we discuss: -Sport specific external and internal training loads -How chronic training load history impacts training guidelines and injury prevention -The mathematical relationship between fitness and fatigue and its effect on physical performance -Why building trust with athletes supports the mental component of competition -And so much more!   Tim stresses that it is more common for players to be undertrained than over trained when it comes to injury risk. He suggests, “High chronic load is protective against injury and gives you the physical qualities that allow you to compete.”   Tim also develops strong relationships with his athletes to help facilitate the training regimen. He emphasizes, “They need to know that I have their best interest at heart. I'm looking to keep them injury free and make sure they can compete as hard as possible. They know at the end of it I'm not asking them to do anything that will put them at risk, but it will prepare them better and keep them injury free.”   Our role as coaches and physical therapists is to guide our athletes during times of difficulty within training sessions for ultimate success in competition. “Winning games comes back to how often they have learned to win the session. High performance demands heavy workloads but loading increases the risk of ‘overuse’ injury. How can clinicians and strength & conditioning (S&C) coaches find the happy medium? Or is it time to take a different view? Perhaps INCREASING training load will make tissues more resilient and injury LESS likely. Dr Tim Gabbett (www.gabbettperformance.com) bridges sports medicine and S&C as a performance consultant and a recognized applied researcher. In the BJSM spotlight he shares what physiotherapists can learn from S&C coaches and argues for the concept of the ‘performance physiotherapist’. Similarly, he translates S&C secrets for physios – insights to progressing sport specific game demands within the rehabilitation program using a rugby example. We ask him about the high-tech and low-tech equipment to gauge how much players are doing and how to titrate their training dose. The latter part of the podcast focuses on how greater training loads can make players less likely to suffer injury but match demands must be considered. “High training loads are not necessarily the problem, it is how you get there that can damage players”. The internationally-recognized Gabbett closes by outlining the concept of ‘training stress balance’ – the net difference between fitness (long-term training load) and fatigue (short-term training load). When the player is in ‘positive’ balance, (i.e, the short-term load is lower than the longer term level of load the player is adapted to) injury is very unlikely. However, when this pattern is reversed, such as when a player is attempting to rapidly ‘gain fitness’ after a layoff, injury is almost inevitable.
