In this episode, Dr. Jonathan Tarbox discusses an approach for addressing mental health issues in education called Acceptance Commitment Training, or ACT. ACT is a very practical but well-researched behavioral approach that can help students and educators better recognize and overcome internal and external challenges and move them towards their values. Jonathan Tarbox, PhD, BCBA-D, is the Program Director of the Master of Science in Applied Behavior Analysis program at the University of Southern California, as well as Director of Research at FirstSteps for Kids. Dr. Tarbox is the Editor-in-Chief of the journal Behavior Analysis in Practice and serves on the editorial boards of several scientific journals related to autism and behavior analysis. He has published four books on autism treatment, is the Series Editor of the Elsevier book series Critical Specialties in Treating Autism and Other Behavioral Challenges, and an author of well over 90 peer-reviewed journal articles and chapters in scientific texts. In this episode, our host Dr. Aaron Fritts interviews emergency medicine physician-entrepreneur Dr. Uli Chettipally about starting Sirica Therapeutics, a novel autism therapy technology that combines virtual reality, machine learning, and physical exercise. --- SHOW NOTES Dr. Chettipally begins by explaining how his daughter Siri was the original inspiration behind the invention. He had observed that routine physical exercise helped Siri cope with her autism and decreased her levels of anxiety and hyperactivity. As a result, the idea of “Cognitive Sensorimotor Therapy” was born— a treatment that combines appropriate mental and physical challenges in a therapeutic context for patients with autism spectrum disorder. Today, the Sirica Therapeutics prototype is a combined exercise machine, virtual reality headset, and machine learning software. All of these elements make the user feel as if they are operating a vehicle that is moving through different terrains. The program also responds to user motion and effort in order to identify and address deficiencies. The ultimate goal is to utilize neuroplasticity to enhance both cognitive and motor function. This device is currently in the pre-seed funding round. Dr. Chettipally shares his vision for this new therapy to be incorporated into clinical practice. Due to initial high costs of production, he expects the device to be first available in clinics where patients are already receiving other forms of therapy for autism. As the technology becomes more widely adopted, costs of production will decrease, making the device more accessible for patients to buy or lease for their own homes. In terms of market size and potential, we discuss the current prevalence of autism spectrum disorder (1/44 births) and the fact that there are few other long-term therapy options for adults. Finally, we discuss Dr. Chettipally's other entrepreneurial ventures. Welcome to part 2 of my series on the possibility of treating autistic symptoms with broccoli sprouts. This week we put theory into practice with some tips about how to prepare broccoli and broccoli sprouts in a form on the kitchen table that children with autism might eat. After all, even Pres. George H.W. Bush didn't like to eat his broccoli. But when it comes to sprouts, we can chop them finely or blend them, and in that way, hide them in the food. But before we get to hiding it in our food, it bears repeating that sulforaphane found in broccoli and other cruciferous vegetables is a potent anti-oxidant that appears to fight oxidative damage to our cells. There is strong evidence for sulforaphane battling rapidly growing cancer cells, preventing the aging of cells, and possibly fighting off whatever it is that is responsible for the symptoms of autism. There is a great article that gets into details about sulforaphane from broccoli sprouts in terms of its overall benefits. That article can be found here. This topic is near and dear to my heart because I happened to be one of those weird kids who no one seemed to have an explanation for. I didn't speak until I was about six years old. I wouldn't answer to my name, I ate only a tuna sandwich for lunch every day for two years, and I would often rock in a rocking chair for 3 or 4 straight hours. Now that I am more or less considered "normal", I've become obsessed about why I was the way I was. Is it possible that I am not alone? Probably. By helping kids with autism get better, we can all learn so much from them about how we can treat everyone on the autistic spectrum. If anyone ever told you to eat your broccoli, the proof of its benefits has finally arrived. Today we are discussing the multiple reasons why antioxidants, like those found in broccoli, would be a good idea. The National Institute of Health (NIH) has put out a few remarkable journal articles looking into the evidence that one particular substance called sulforaphane (from broccoli sprouts) can counteract the harmful effects of cellular damage. The article that sparked a lot of interest in sulforaphane particularly was put out by NIH and it can be found here. Another NIH article defines oxidative stress and lists the risk factors that cause this stress. Not surprisingly, it has a lot to do with the American diet and typical cell-damaging risk factors like smoking, x-rays, and excessive alcohol. That article can be found here. This NIH article connects the dots between large amounts of oxidative stress and the development of many chronic diseases, such as heart attacks, strokes, cancer, diabetes, and now autism. But help is on the way because scientists have now shown evidence that both cancer and autism have improved with daily broccoli sprouts. Broccoli sprouts just happened to have a large amount of a powerful antioxidant called sulforaphane. Could that be possible? It certainly could and right now it seems to be the case. Dr. Jonathan Tarbox is the Program Director of the Master of Science in Applied Behavior Analysis program at the University of Southern California, as well as Director of Research at FirstSteps for Kids. Dr. Tarbox is the Editor-in-Chief of the journal Behavior Analysis in Practice and serves on the editorial boards of several scientific journals related to autism and behavior analysis. He has published four books on autism treatment, is the Series Editor of the Elsevier book series Critical Specialties in Treating Autism and Other Behavioral Challenges, and an author of well over 80 peer-reviewed journal articles and chapters in scientific texts. His research focuses on behavioral interventions for teaching complex skills to individuals with autism, applications of Acceptance and Commitment Training (ACT) inside of applied behavior analysis, and applications of applied behavior analysis to issues of diversity and social justice. Dr. Rehfeldt holds a BA in psychology from the University of Puget Sound in Tacoma, WA (1993), and masters and Ph.D. degrees from the University of Nevada (1998). Dr. Rehfeldt is now serving as assistant chair and professor in the Applied Behavior Analysis program at the Chicago School of Professional Psychology Chicago campus. What would it mean to develop medical treatment for the core features of autism? Recent research has enriched our understanding of the biology of autism, as well as the social experiences of being autistic, but the core biological features of autism are still not well understood. Any medication for the core symptoms of autism is thus still some way off in the future. Nonetheless, much research is now looking for biological markers for which treatments might be developed. ‘EU-AIMS’ is a collaboration seeking new methods for the development of biological markers and drug treatments for autism. There is considerable support for such an endeavour, including from groups representing autistic people, and their families and allies. However, other autistic people, some autism self-advocacy groups, and some ethical and legal scholars, are concerned about autism coming to be defined only as a biological disorder in need of treatment – and have called for wide debate on the relationships between biology, treatment, and difference in autism. This public dialogue, organised by the Ethics Advisory Board of EU-AIMS, brings together autism advocates and self-advocates, psychiatrists, and ethicists, to discuss these issues. An estimated 1% of children worldwide are described as autistic. But what exactly does that mean? What is autism? And what are the promises, perils and politics of pharmaceutical intervention - could, should autism be treated? This podcast features some of the key points raised by autistic people and their families at an event held at Kings College, University of London in late October 2014 to discuss the EU AIMS project (European Autism Interventions - A Multicentre Study for Developing New Medications). The event was chaired by Sandy Starr, Progress Educational Trust Speakers were: Prof Ilina Singh, King’s College London Prof Declan Murphy, Institute of Psychiatry, King’s College London Prof Richard Ashcroft ,Queen Mary, University of London Virginia Bovell: Ambitious About Autism Russell Stronach: Autistic UK Importantly, as Ilina Singh and Sandy Starr say, autism has multiple claims on those involved in this event, both speakers and participants. Many are living with autism or are parents of children with autism. Autism affects how someone communicates with and relates to other people, it can also affect how they make sense of the world. But it is what is called a ‘spectrum condition’ – which means that different autistic people are affected in different ways depending where they are on the spectrum. And it’s a wide spectrum - some autistic people have above average intelligence and some have severe learning disabilities. So, autism can be described (although as Richard Ashcroft said at the event, 'when you have met one person with autism, you have met one person with autism') but almost nothing is known about why one in a hundred of us will have the diagnosis. Are the causes genetic, hormonal, social,or is it something to do with the wiring of the brain – or is it a combination of those things? No one knows. And that is why the EU AIMS project has been set up. It is the largest ever academic/industry collaboration aimed at understanding autism and finding new methods for developing drugs for the autism spectrum disorder. It is led by Kings college London (with partners in 13 other centres of academic excellence around Europe) and by Pharmaceutical company Roche, in partnership with other huge global pharma companies. Although it is clear that new drugs are the longer term aim of the project (and there is no evidence for the efficacy of any of the current treatments for the core symptoms of autism), the first phase is about understanding Autistic Spectrum Disorder (ASD). And there is much to understand. The EU AIMS project has brought together researchers from diverse backgrounds –genetics, neuroscience and cellular biology – and they are all beavering away looking for an answer to the question – what is autism? So we’ve got big pharma and we’ve got academic researchers. But where are the people with autism and their families? They live day in day out with autism, they must have crucial insights and experience to feed into the research programme…. Given the fact that the scientists don’t know what autism is, surely their knowledge and perspective is indispensable. That gap has been recognised - although perhaps a little late in the day given that EU AIMS is in its second year. This event was just one of a series of events around Europe planned for the next few months. Shawna Kemp is an occupational therapist that has worked with autistic children for over 13 years. We will discuss why autism is now affecting 1 in 54 males and 1 in 67 children (as of March 2013). We want to give you information about what signs to look for and treatment options. Autism can be completely turned around in many cases, but early intervention is key. Autism is the fastest-growing developmental disorder in the nation, according to Autism Speaks, an autism science and advocacy program. At present, about one in 67 children has the disorder. Most of them are male. According to The American Academy of Pediatrics, recognizing symptoms early is crucial, and checkups at 18 and 24 months should include “developmental screening for autism spectrum disorders for all children.” Knowing the signs can help identify a problem early. Those signs include lack of eye contact, repetitive behavior, including spinning, arm flapping or lining objects up; developmental delays, especially in speech or motor skills; a preference for playing alone; and odd eating behaviors, according to the Mayo Clinic. Parents should be observant of their children in case any of these signs emerge. Looking at typical developmental milestones and making sure your child is staying on track with motor, communication and cognitive development is an excellent idea. Begin your quest to figure our if there is a problem if your child is not speaking at the same time as other children. Do not rely on the often quipped saying, "Children just develop at different rate. Don't worry." Parents should request a developmental evaluation with a speech-language pathologist or an occupational therapist as soon as they suspect their child is not developing normally. Even though most doctors wait until a child is 3 to issue an official diagnosis, parents can help their children earlier. Shawna and I will help give you the information and tools you need to get your child evaluated for free by your county or city. The earlier you intervene, the better the prognosis for your child to develop normally and integrate into kindergarten or first grade with their peers. Do not wait.
