Dr. Vertrees is a general surgeon and certified coach. She is an Army veteran and was deployed 3 times to Afghanistan and Iraq. She is the Founder of 'BOSS' (the 'Business of Surgery Series') with includes a popular podcast and coaching program where she teaches the lessons not taught in residency. Dr. Vertrees realized that medical school and residency helped her become a great technical surgeon, but she soon learned that there were several career aspects that residency did not cover. She trained in the military and, once her commitment was up, she realized she had no idea how to find a job, negotiate her contract, or code notes and cases. She created the 'BOSS Business of Surgery Series' as a way to address those topics, because if she needed to know it, others probably needed to as well. Over time, she realized that even more topics were necessary. Dealing with a difficult partner, managing thoughts about complications, realizing that everything is a negotiation, and learning how to manage a successful clinic were topics also not covered in residency. The BOSS series was her answer to these problems. She has used all of the strategies she teaches in her own career, which allowed her to open and run a successful private practice as a general surgeon. “We are in an epidemic of gaslighting in medicine. The best definition I heard of gaslighting is transferring your authority on yourself from you to someone else. We give up our authority on ourselves to everyone else but ourselves. We leave our feelings of work ethic to our current culture: we are lazy if we don't step in line and do everything asked of us. We are told we aren't worth our salaries as physicians are replaced with less expensive nurse practitioners and physician assistants. We are told our education is not important as the public ignores our expertise, and the judicial and legislative system tells us they know better for our patients. And it is somehow our fault, or at least we are left to deal with it. Show up and work hard to pay outrageous student loans gathered in pursuit of the cause. Dr. Amy Vertrees is a general surgeon in Columbia and is affiliated with Maury Regional Hospital. She is the founder of a wonderful company called Boss, and the host of the Boss Business and Surgery Series podcast. Dr. Vertrees received her medical degree from Uniformed Services University of Health Sciences F.Edward Hebert School of Medicine and has been practicing for over 10 years.Dr. Vertrees got all her training in the military, at a military Medical School in Bethesda, Maryland. In 2017, she chose not to stay in the military but become a civilian instead. When she was first employed, she felt restless, as if she was missing out on something. On this episode, Dr. Amy Vertrees talks with Dr. Arianna Gianakos, an orthopedic surgeon and advocate on abuse during surgical resident training. She recently published a study on bullying, discrimination, harassment, sexual harassment, and the fear of retaliation during surgical residency training: a systematic review. The paper noted some shocking statistics: 37% of trainees experienced burnout 33% experienced anxiety or depression 71% decided not to report abusing behavior with over half concerned that they would experience retaliation 56% reported that they were retaliated against for reporting Women were more likely to experience this behavior, which could prevent diversity in surgical specialties. She is a physician advocate in Physician Just Equity providing peer-support to physicians who experience workplace conflicts, through education, research, empowerment and advocacy while facilitating institutional culture change. "The problem with feeling like an imposter is thinking there's something wrong with it. I know this is different from any way you have thought about it, but humor me. What happens when we are offered an opportunity but we feel like an imposter? We hide. We overwork to compensate. We look around at our colleagues and resent them. Because why do they get to feel so secure? Why aren't they feeling insecure? They don't even spend time worrying about it, and that leads to more resentment! They are not working as hard as you, and now they are not stressing about the opportunity like you are. What is the result of those actions? We spend lots of time in indecision, overwhelm, fear, resentment, and overwork. We might take the opportunity, do well even, but hate the whole process and add it to the bad feelings we already feel about ourselves. What is behind feeling like an imposter? It's the thoughts that we tell ourselves. It helps to understand the motivation behind these thoughts. We were wired to survive." "We can move from being a reactor to creator. Creators think of the future and make it start happening today. Reactors are just dealing with the crisis at hand, not progressing, feeling stuck, feeling like life is unfair. Reactors are people-pleasing, reacting to everything said, constantly worrying about how to win the approval of others. That's the trouble with people-pleasing: You are always reacting. We think we are making others happy, but we despise ourselves and resent others. We should start taking the time to please ourselves. It is only from this space that we can start helping others. If you think this will be a waste, or you can't take a break because there is work to do, your brain will find ways to prove this thought true. If you tell yourself this is stupid and a waste of time, then you will squander the day and prove yourself right. If you tell yourself they can't live without out you (and your confidence depends on them wanting you), you will allow the interruptions. If you find yourself saying you really can't take the time off, then you have a non-sustainable job. Give yourself permission. Start to develop strong boundaries. Understanding what we can and cannot do in a day. Setting aside days for us to reflect, listen deeply to our inner thoughts, and strategize the path we are meant to live are keys to a successful life.
