Send us a message! For many women, hair is more than just an appearance—it's tied to identity, femininity, and confidence. Hair loss can be difficult to navigate, especially with hormonal changes from pregnancy, perimenopause, and menopause. Many women, however, are left unsure of the cause and how to address it.I've been there too—brushing off hair shedding as "just stress" until a dermatologist friend pointed out that it was actually female-pattern hair loss. It's easy to overlook the signs, but ignoring the issue can end up making it worse. In this episode, I'm joined by Dr. Nancy Echefu, a hair transplant surgeon, who breaks down the different types of hair loss, including scarring and non-scarring alopecia, and explains how to spot the signs early. She also provides a simple, actionable guide to help you start addressing hair loss before seeing a doctor—because awareness and early intervention are key. Tune in now to learn how you can start taking control of your hair health today.ReferencesConnect with Dr. EchefuWebsiteInstagramFacebookConnect with me:TikTokInstagramEastside Menopause & MetabolismAudio Stamps01:35 - Dr. Echefu shares her background and expertise and what pushed her towards pursuing her career in hair restoration. 07:00 - Dr. Patil-Sisodia shares her own experience of hair shedding and hair loss.08:33 - Dr. Echefu explains the two major classifications of hair loss: scarring and non-scarring alopecia.13:18 - CCCA, a type of scarring alopecia, is identified through symptoms like itching and burning, with biopsies used to confirm a diagnosis when needed.15:27 - We learn the difference between hair loss and hair shedding and how much hair shedding is considered normal.21:45 - Dr. Echefu customizes treatment based on history, symptoms, and prior care, tracking progress over a year with in-office and at-home therapies.26:48 - Dr. Echefu combines medical treatments with PRP, microneedling, or laser therapy, tailoring care to patient needs and referring to specialists when needed.32:18 - Trichotillomania is often linked to anxiety. Hair loss is a deeply personal and often emotional experience, especially when it happens unexpectedly. For many women, hair thinning begins during perimenopause or menopause, yet the topic remains widely overlooked. In this special Women's History Month episode of HairPod, we sit down with Angela Fields, founder of a curly hair product line, to discuss her personal journey with hair thinning, her eventual diagnosis of CCCA alopecia, and the solutions she discovered. Angela's experience sheds light on the challenges of hormonal hair loss and the steps women can take to reclaim both their hair and confidence. How Vitamin Deficiencies and Hormones Affect Hair Loss Angela first noticed her hair thinning in 2017 when her part began to widen, revealing more scalp than before. This early sign of hair loss led her to her doctor, where lab tests revealed she had a significant vitamin D deficiency—something she hadn't previously considered. Vitamin D plays a crucial role in hair follicle health, and a deficiency can contribute to both hair loss and fatigue. Supplementing with vitamin D helped restore some of her hair's fullness, but as she entered menopause, new challenges emerged. Her hair loss became more pronounced, leading her to seek the expertise of a dermatologist, who diagnosed her with CCCA alopecia, a condition that causes scarring of the scalp and makes regrowth more difficult. Effective Hair Regrowth Treatments and Lifestyle Changes Finding a treatment plan that worked required time, patience, and a comprehensive approach. Angela began by incorporating key supplements such as Nutrafol, folic acid, vitamin C, and iron to support hair regrowth. But supplementation alone wasn't enough—because of the scarring caused by CCCA alopecia, her dermatologist recommended a series of scalp injections to break down scar tissue and allow new hair to push through. This process took months, but the results were encouraging. Daily Habits for Hair Regrowth Beyond medical treatments, Angela also adjusted her daily habits. She emphasizes the importance of maintaining a clean scalp by shampooing regularly, as product buildup and excess oil can contribute to hair thinning. She also warns against wearing tight or glued-in hairstyles, which can put stress on the hair follicles and lead to long-term damage. Managing stress is another key factor, as high cortisol levels can accelerate hair loss. Through exercise, proper sleep, and self-care, Angela found ways to support her overall well-being while working toward healthier hair. Finding Support and Breaking the Stigma of Hair Loss Hair loss can feel isolating, but Angela stresses that no one has to go through it alone. She encourages women to seek support through online communities, Facebook groups, and even local meetups dedicated to hair loss. Speaking with a knowledgeable dermatologist who understands different hair types and traditional styling methods is another crucial step in finding the right treatment. Dr. Donovan discusses the potential of topical metformin as a second-line treatment option alongside oral metformin, PRP, and JAK inhibitors, while emphasizing the importance of first-line treatments such as steroid injections and topical steroids. He also addresses the challenges of compounding metformin in various formulations, which can affect its efficacy. Supporting hair growth through chemical burns, scarring alopecia and menopause. Angela Fields is from Florida. She first experienced issues with her hair when she suffered from chemical burns, following the use of relaxer to straighten her hair. She then began to experience thinning in perimenopause, and discovered that she had a deficiency in Vitamin D. After these issues were resolved, she then developed a scarring condition, central centrifugal cicatricial alopecia, known as CCCA, and has had to undergo scalp injections and biopsies to help to identify and treat this . Hair health at this point became extremely important to Angela, but, as she has an allergy to fragrances, she found that she was unable to use 96- 98% of haircare on the market that contains fragrance. She decided to make something for herself in her kitchen, but after being laid off from her job in the corporate world she was pushed to take the product range to market. She now works to help & support those with hair health concerns, particularly leading up to the menopause, and has created a supportive community where she offers education and advice. To quote Patricia Blocksom: “Equality is a robust concept but a fragile reality.” Fortunately lawyers are good at remaining constantly vigilant and feminism is nothing to be afraid of, quite the opposite: by the end of this our second episode on the Touchstones Report, you will be very grateful you have chosen to let these wonderful women, these wonderful feminists, into your life for a while. Thanks for listening, reach out to us anytime at podcasts@cba.org! Touchstones Report Executive Summary:The CBA's Task Force on Gender Equality's groundbreaking 1993 Report titled “Touchstones for Change: Equality, Diversity, and Accountability” addresses a range of issues related to equality and discrimination in the legal profession. It focuses on barriers to entry into the profession, including law school admission policies, access to articling positions, and the articling experience. The Report also highlights the unique challenges faced by women in the legal profession, such as employment opportunities, career advancement, and the lack of accommodation for family responsibilities. Additionally, the Report explores the dynamics within private practice, government legal departments, and corporate legal departments, including issues of employment equality, career opportunities, and instances of sexual harassment. Further, the Report delves into the challenges faced by faculties of law and administrative tribunals, examining issues such as representation, career advancement, balancing career and family responsibilities, and sexual harassment. It also addresses the judiciary and the need for a fair and diverse appointment process, judicial education, and tackling discrimination within the judiciary. The practice of family law is discussed in detail, highlighting issues related to representation, career advancement, funding for legal aid, and challenges with the substantive law. The Report also emphasizes the role of Law Societies and the CBA in promoting equality within the legal profession. It explores the need for non-discrimination, monitoring, disclosure requirements, alternative discipline processes, and the importance of implementing the Task Force's recommendations. Finally, it touches upon the need for gender equality in substantive law and procedure, examining areas such as family law, intellectual property law, criminal justice, and pensions and benefits. Overall, the Touchstones for Change Report provides a comprehensive overview of the challenges and areas requiring improvement in achieving equality and addressing discrimination within the legal profession in Canada. Reviewing the Summary below, one will see that many of these recommendations have been achieved; however, there is still much to be done to make them all a reality. Summary of Recommendations Chapter Two addresses barriers to entry into the legal profession and recommends a range of measures, including consultation mechanisms between law schools and law societies, outreach programs to promote diversity in the profession, education equity initiatives, affirmative action recruitment in law schools, admissions policy improvements, support for part-time studies, child care services on campus, increased funding for scholarships, gender-inclusive language and sensitivity in classrooms, curriculum reforms, potential Indigenous law school, appointment of Equity Officers and establishment of safe spaces in law schools, combating harassment and bias, and inclusion of gender-related courses in the Bar Admission program. Chapter Five addresses issues within law firm and recommends adopting interview guidelines, implementing employment equity programs, monitoring hiring processes, tracking internal referrals to detect systemic discrimination, supporting female lawyers facing client discrimination, banning sexist activities in client promotion, reviewing promotion practices, establishing mentoring programs for women lawyers, evaluating partnership decision-making processes to eliminate bias, reporting demographic information and retention rates to law societies, establishing parental leave policies, offering child care support, implementing policies to address sexual harassment, promoting workplace equity, conducting educational initiatives and collaborating with relevant organizations to implement and finance these initiatives. In Chapter Six, the Task Force presents recommendations to address gender equality issues within Government Legal Departments, including ensuring fair allocation of work and gender balance on committees, ensuring representation of minority women in the public service and gender equality in management and promotion processes, endorsing membership in professional associations, reviewing recruitment procedures, providing mechanisms for reviewing discretionary decisions related to family responsibilities, accommodating family responsibilities without hindering career advancement, providing anti-discrimination training, establishing advisory committees on gender issues and equal opportunities, and referring legal work to law firms with inclusive employment equity programs. Chapter Seven of the Report recommends that corporate legal departments actively promote employment equity by hiring women from minority groups, referring legal work to law firms with strong employment equity programs that include representation of all women, establishing a Subcommittee on Gender Issues within the Canadian Corporate Counsel Association (CCCA) to develop strategies and monitor progress, and utilizing the CCCA as a resource for corporate policies on parental leave, flexible work arrangements, and programs addressing sexual harassment. Chapter Eight suggests that law schools should prioritize recruiting faculty members from minority groups, regularly review recruitment practices for gender parity and minority representation, ensure affirmative action recruits are recognized as fully qualified, promote women's representation in committees and leadership roles, adopt workplace equity policies, consider women and minority faculty for dean appointments, eliminate gender-based salary differences, address salary inequities, provide reduced teaching loads and sabbaticals, and develop materials and seminars to address gender and minority challenges in teaching, with the Canadian Council of Law Deans playing a supportive role. Chapter Nine advises governments to actively recruit women and under-represented groups for administrative tribunal positions, establish a re-appointment protocol without term limits, consider tribunal lawyers for other government and senior civil service roles, view tribunal experience as valuable for judicial appointments, review adjudicators' salary levels for potential discrimination, and encourage administrative tribunals to implement flexibility policies and suspend appointments during maternity and parental leave. Chapter Ten proposes that the CBA create objective criteria for judicial appointments and establish committees for their development, and advocate for an affirmative action policy to appoint women and minority women, while also addressing discrimination in the appointments process; it recommends mandatory sensitivity courses for judges, fair workload allocation for women judges, and the creation of a Commission on Judicial Conduct with public hearings and equal representation of judges, lawyers, and the public. In Chapter Eleven, the Task Force recommends increasing the emphasis on family law in legal education, improving its status within the legal profession, supporting family law practitioners, and ensuring sufficient funding and judicial resources for family law cases at both federal and provincial levels. In Chapter Twelve, the Task Force recommends that law societies prioritize diversity and enforce non-discrimination rules. The Task Force also suggests establishing internal procedures within law firms to handle gender discrimination complaints and appointing "safe counsel" to investigate such complaints. Additionally, they propose monitoring the progress of women and minority groups within firms, ensuring compliance with human rights legislation, and developing educational initiatives to eliminate discrimination and promote equality. Law societies are also urged to gather statistics on the representation of minorities and conduct studies on racism in the profession. In Chapter Thirteen of the Report, the Task Force recommends that the CBA amend its constitution and mission statement to address discrimination, conduct an inquiry into racism, promote accessibility and inclusion, gather statistics on underrepresented groups, establish committees, support lawyers with family responsibilities, advocate for gender equality, and adopt gender-inclusive language policies. Lastly, the Task Force recommends in Chapter Fourteen that the CBA offer legal education programs on gender bias, develop a national strategy to eliminate gender bias in the justice system, advocate for access to justice measures for women, establish gender issues sub-committees, consult the National Equality Committee on law reform initiatives, and review submissions to address gender-related issues.touchstonesForChange.pdf (cba.org)
Our special guest is Zarik Khan, Founder and Managing Director of Finsolute Advisors. Congress is now considering whether to enact the CCCA, a bill that would require credit card issuers with assets over $100 billion to include at least two possible network processors on each card. We first look at the roles of the various parties involved in a typical transaction in which a consumer makes a purchase from a merchant using a credit card. We then discuss how the CCCA requirement for at least two networks would operate, its potential implications for smaller banks and credit unions, and its implications for each of the parties involved in a typical credit card transaction. After discussing the bill's political prospects, we assess the principal arguments made in support of or opposition to the bill, including the arguments made by the bill's opponents that the CCCA will reduce access to consumer benefits such as rewards programs and zero fraud liability protection. Alan Kaplinsky, Senior Counsel in Ballard Spahr's Consumer Financial Services Group, hosts the conversation.

In this Q&A Karen answers listeners' questions on all things hormones and weight loss. maymoon_design When you refer to HRT are these natural or synthetic hormones? I am having a very hard time with hot flushes. janelleminto Can HRT cause/ contribute to a thickening in the lining of the uterus? I recently had surgery to remove this and wondered if the hormones could be a cause. lcavazza66 If I struggle with progesterone replacement, is it ok to just use estrogen and testosterone? I can't sleep, gain weight and struggle with progesterone. I had severe PMDD when I was younger prior to partial hysterectomy. Labs show estrogen dominance but adding progesterone makes me feel worse. Kimdixonwiley How do you know how much HRT to prescribe when Canadian blood work only gives Estrogen and Progesterone not actual numbers? Carol What serology test should I get if I have been in menopause for 2 years. Estrogen will be low but what are the levels we are hoping to achieve for both estrogen and progesterone? If supplementing how often should we check this - serology and with the Dutch? Md say levels will fluctuate so they don't want to do testing - how can we make our case to them to get it done? How do I know a progesterone is bioidentical? Jennifer Hi Karen, I love your podcast as you are so informative! I'm 46 and have been on my current hrt for 6 months. I experienced a lot of side effects at the beginning of my hrt. I had migraines and felt depressed and extremely emotional. This went on for months and was debilitating. So my provider put me on the following treatment to assist with the symptoms stating I was really estrogen dominant and needed more progesterone. I have noticed I feel better when I'm not taking any progesterone during my period. Is it possible to have too much progesterone and then have adverse effects? I don't feel like I'm reaping any of the benefits so many others have with progesterone. Pam Hi Karen! I'm 59 and was diagnosed with Hashimoto's in my early 20s after my 1st child. I'm just wondering if something is off b/c it seems like my hair is really thinning. I'm taking Armour Thyroid 90 mg daily. My doctor says everything is normal. I'm also struggling with weight loss resistance. I know this is a lot but would really appreciate any insight you may have. I love listening to your podcasts, I learn so much! Thanks for all you do and know that you're making a difference in the lives of many! KK Hi Karen! After listening to your podcast a couple of years ago, I got my hormones checked and found a Functional Practitioner to go over my health - she confirmed I was in Menopause and that my Progesterone was flat-lining, my Testosterone and Estrogen were low. My question is, Should I be getting better results? I still struggle with quality sleep and maintaining lubrication and holding onto menopausal weight, I have still been having DHEA (50mg per day). My practitioner believes that the DHEA is better to boost than Progesterone or Testosterone. Should I expect better results or am I being ridiculous? Luhu Hi Karen, Your number one fan here :) Studying with FDN to become a practitioner, very well-versed in gut health, hormones, etc but cannot figure out my own body since menopause! When my estrogen, progesterone, and testosterone all went very low—6 for E and T, I gained weight all over and can't lose it. Also developed chronic gut issues in menopause despite no gluten, no dairy, no sugar, (no fun!). SO Q: I started on the estrogen patch applying one half of the .025 dose, next day I added the next half, felt so much more energy and felt like I started losing lbs but quickly developed heartburn/reflux and felt a little overstimulated (maybe an adhesive ingredient in the patch? I'm very sensitive), I also felt some discomfort in my liver/gallbladder area even though I support both. I waited 3 weeks before adding progesterone due to past issues (ok'd by DR), then applied Quicksilver Scientific's topical progesterone 8mg and all hell broke loose. Within 2 days I retained so much water, and looked like I'd gained another 10lbs, belly got so swollen from diaphragm down and I felt weird pinching-like gas pains in ribs under right breast and then it moved to under left breast and the heartburn/reflux got worse. I had to stop both hormones :(. Please help, what do you think is going on with my body? Jean clark I'm almost 54 (bday in May) my local naturopath prescribed bio-identical hormone replacement without having me do a hormone test first. Should I be taking a hormone test before just going ahead with a prescription? It is all so confusing. Thanks!! Anonymous What is your morning routine? What time do you eat breakfast and what do you eat? Do you drink coffee? Could not eating breakfast actually raise your blood sugar? Anonymous Hi Karen, I was wondering how your experience with Fully Vital hair growth system has been? Thank you, Gail Anonymous How does one determine when it's appropriate to use 50/50 Biest or use only estradiol? (I'm referring to your Nov 18/22 podcast with Dr. Felice Gersh.) Thank you! Milly Hi Karen, Thanks for taking my question. I started transdermal testosterone to optimize my levels last November, we're still trying to find that sweet spot as it's still plummeting per my January report where it was at 8. I'm due to go at the end of February to check on testosterone levels again, I'm sure (I hope) my doctor is going to check my other hormone levels to see where I'm at. Also, per my last lipid panels (Sep 2022) my total cholesterol and LDL levels have gone out of range for the first time. Taking all this into consideration, what would be the best day of the cycle to test ALL my hormones assuming I get my cycle eventually? Thank you, Milly Hi Karen, thanks so much for all the amazing information you share. My question is about heart palpitations. I'm 40 and the last few months they have been getting pretty intense the last half of my cycle so I'm pretty sure they are hormone related. I recently had a Dutch test in December and my estrone and estradiol were above range, estriol was in range. Estrogen is favoring the 2OH pathway but my methylation activity is low. Progesterone was in range. I've been using oral and the bio labs progesterone cream for more than two years now and it has helped with many symptoms, especially sleep. Besides the heart palpitations, I also get irritable around my cycle and again after ovulation. Feel pretty good besides that. Any ideas as to what's causing the sudden increase in heart palpitations? Thanks so much for your help! Gates 44 I guess my question is should mild symptoms be addressed? I have hot flashes that are bad for a few months then subside for a month or two. I have dry skin but I live in Canada so dry harsh winters, have had dry skin for years. I do have a low sex drive, but again I never had a high one either. Yes I do have more trouble sleeping for sure, but not sure that is enough to go on a drug. So please answer me should I do something or not? I am 56 years old and had an ablation done almost 17 years ago, so have not had a period since I was 39. Ira What is the window to start hrt and how long can you stay on hrt to get the full benefits? Do you believe by doing extended fasting (24+hrs) you do lose lean muscle tissue? Kathie Hi Karen, In trying to balance hormones, I've had some occasional bleeding. A recent biopsy was clear of anything being an issue. Is it normal to have a “period” with hrt? My regime has been a .25/1 mg estradiol/testosterone troche in the a.m., and 200mg progesterone in pm for about two months. In that time, I've had a period two times as well. Thank you! Sally I'm 49 years old and recently had a hysterectomy sparing my ovaries. I've suffered from extremely heavy periods for years and even through a year of chemo for brain cancer. My OB/GYN wanted to put me on birth control or the Mirena but I refused as I've attempted BC over the years with poor results. I had a terrible episode of anxiety/depression and cognition issues so she sent me to a psychiatrist and they put me on antidepressants which have helped some. Finally, she ordered a D&C due to the uncontrollable bleeding and precancerous cells were found hence the hysterectomy in late December. I spoke with her about hormone replacement and she said I'd probably be better off without it because my hormones made me so miserable with the fluctuations throughout my lifetime I went to my PCP because I couldn't sleep and she gave me Trazadone and said just wait and my hormones would settle. Then I talked to the Psychiatrist and told her about the temperature irregularities (inability to control my body temperature) and she increased my Effexor saying it had a vasomotor response. I'm a little fed up with all of the women doctors (all in their midlife and my OB is part of NAMS) giving me the runaround. I'm not crazy. I've medically been through a lot but this should be easy. I thought at first that the multiple brain surgeries had messed up my mind but after listening to you I think it's probably just my hormones. Thank you, Karen Anonymous Lots of talk about hormone replacement therapy at the beginning of transferring through perimenopause to menopause. My question to you is, what are the options for an almost 60-year-old who is past 10 years of being in menopause? We are still concerned about the future health of the heart, bones, brain, etc. Also, where can I find published current studies or research proving that bio-identical hormone replacement theory is safe? I would like to present to my doctor as she is a nonbeliever. Thank you! Beth Hi Karen, I have been on Estrogen replacement since age 36 when I had a total hysterectomy with bilateral ovaries removed due to aggressive endometriosis at the time. I'm now 55, and I've struggled with 15 pounds of too much weight ever since. My libido sucks, Intercourse is painful, I'm a professional insomniac and have been dependent on magnesium supplements and Trazadone to help me sleep for 10 years! and I also have low energy. I'm thinking I need to address my hormone therapy, currently, it's an Estradiol patch of 0.037mg changed every 3 days. I really would love your help. I have been eating a green Mediterranean diet and avoided sweets and processed foods for the last 3 months, cut alcohol
