What are the origins of HRT and estrogen therapy? Historian Alison Li takes us back to 1930s Canada, and the ‘endocrine gold rush', when estrogen was extracted from hogs, then human placentas, and finally horses - to create the drug Premarin (the name comes from PREgnant MAre urINE). The making and marketing of estrogen therapy would shape how we think about menopause in particular, for decades to come.More on Alison Li and her work https://alisonli.com/-------------------------------------------------------------------------------------------------------A word from our sponsor:This episode is sponsored by Tell Every Amazing Lady®Whether you've had an ovarian cancer diagnosis or have questions about general women's health, like your first visit to the OBGYN or you're going through menopause, Tell Every Amazing Lady® is here to help!Tell Every Amazing Lady® supports ovarian cancer survivors and their families, provides ovarian cancer awareness, offers education on women's health, and funds medical research. Hormone modulation therapy is a growing area of research in cardiovascular science related to a number of factors, such as menopause and andropause, cancers in hormone-producing organs, as well as gender-affirming hormone therapy. So how do scientists and clinicians measure, monitor, and balance applications of hormone therapy? In our latest podcast, Associate Dr. Keith Brunt interviews authors Dr. Chantal Rytz (University of Calgary) and Dr. Sofia Ahmed (University of Alberta), along with expert Dr. Nina Stachenfeld (John B. Pierce Laboratory/Yale School of Medicine) about the recent article by Rytz et al. The authors review the use of estrogen in adult transgender, non-binary and gender diverse individuals who are medically managed with hormone modulation therapy for gender affirmation. Their findings indicate that the rate at which serum estrogen concentrations change may be important in order to optimize cardiovascular health and manage risk factors for obesity, hyperlipidemia, and elevated blood pressure. How should scientists account for and explore the hormone state of individuals within a cardiovascular context to improve the foundation of evidence for hormone modulation therapies for clinicians? In this episode, Dr. Shawn Tassone discusses a recent study published in the Journal of the North American Menopause Society that challenges the previous negative views on hormone replacement therapy (HRT) for women over the age of 65. The study found that HRT, specifically estrogen therapy, beyond the age of 65 is associated with significant risk reductions in mortality, breast cancer, lung cancer, colorectal cancer, congestive heart failure, venous thromboembolism, atrial fibrillation, heart attacks, and dementia. The study also highlighted the importance of individualized hormone therapy, lower doses, and non-oral routes of administration for optimal health outcomes. In the third episode of Tit Talk, Rosalina guides us through a study featured in JAMA Oncology in November 2023. Examining data from nearly 50,000 women in the UK, the study investigates the potential link between vaginal estrogen therapy and breast cancer-related mortality. The study's findings reveal no evidence of an elevated risk. As always, Rosalina encourages listeners to stay informed, seek advice from healthcare professionals, and feel empowered to make decisions that align with their well-being.In this third season of the podcast, we had a dedicated episode on sexual health, where we spoke to medical oncologist and sexual health expert Dr. Laila Agrawal on a misconception about hormone-positive breast cancer and the use of vaginal estrogen. The number one reason women give for not taking estrogen is fear of breast cancer. This fear is based on the 2002 study which has been reevaluated and shown to be flawed in a number of ways. Despite solid proof that estrogen therapy does not cause breast cancer and other serious illness, most doctors continue to exaggerate the risks and undersell the benefits of hormone therapy. The goal of this episode is not to convince you to take estrogen. The goal is to present the data so that you can make an informed decision. In this episode, Dr. Streicher discusses: The original PURPOSE of the 2002 Women's Health Initiative (WHI) and how the study was designed. The INITIAL RESULTS of the WHI study Why the design of the study was PROBLEMATIC Why the results of the WHI were NOT REPRESENTATIVE of the women who take hormone therapy today The results of the RE-ANALYSIS of the WHI How the age of women in the study, the timing of therapy, and the use of statins altered CARDIOVASCULAR data Why the use of PROGESTIN and the kind of progestin in the study changed the results. Why TRANSDERMAL estrogens have less risk and who should use one The FACTS about the relationship of estrogen and BREAST CANCER The risk of hormone therapy COMPARED to other known risks for breast cancer The medical impact of women NOT taking HT GUIDELINES for taking hormone therapy safely For more information on this topic: Hot Flash Hell: A Gynecologist's Guide to Turning Down the Heat has a more complete discussion of the results of the WHI and includes pros and cons of every type of estrogen currently on the market.
