"The first line treatment for adolescents with anorexia now is family-based therapy typically, which involves helping the parents facilitate the refeeding of the adolescent. So, I was working with the patient in that way and found it to be helpful and useful, but was consistently struck by the neglect of the patient's inner life, and found, at least based on my experience with many patients, that while you could get some symptomatic relief, if you didn't, in some way, address the deeper dynamics, the aspects of the patient's personality organization that drove the disorder, that were implicated at the disorder, there was a way that the patient would snap back to their old behaviors over time, that deeper change and a deeper understanding of what was going on was really necessary; and so that's been kind of evolution from my work over the past ten years from  my first book, which was about anorexia in males, and tried to present a kind of Integrative understanding of that phenomena, increasingly over time I've become more and more interested in the deeper kind of analytic thinking that we can bring to bear on this kind of suffering.”    Episode Description:  We begin with a description of the common contertransferential pull to intervene behaviorally in the face of repetitive self-destructive eating disorder symptoms. This intention can inform but not compel the clinical decision as to the indicated treatment of choice for someone at any particular moment. Behavioral and pharmacologic treatments can be important in softening the pressure of eating disorder symptoms. They do not, however, give an individual access to their interoceptive life, from which these disturbing self-preoccupations emerge. We discuss the challenges of working with those who have limited capacities for mentalisation and as a result, live out their inner lives somatically and motorically. Immersive treatment leads the clinician to experience these proto-affects in one's own body and in one's own ruminations. Tom discusses alexithymia, typical family structures, and the presence of the 'abject' experience in the lives of these patients. He presents a disguised case of a patient who was able to work through both the early struggles and later neurotic aspects of these conflicts analytically. We close with his sharing with us his vision for the future which includes more integration between the dynamic and adynamic approaches to these challenging patients.   Our Guest: Tom Wooldridge, PsyD, is Chair in the Department of Psychology at Golden Gate University as well as a psychoanalyst and board-certified, licensed psychologist. His first book, Understanding Anorexia Nervosa in Males, was published in 2016. His second book, Psychoanalytic Treatment of Eating Disorders: When Words Fail and Bodies Speak, an edited volume in the Relational Perspectives Book Series, was published in 2018. His third book, Eating Disorders (New Introductions to Contemporary Psychoanalysis), was released in 2022.  His fourth book, co-edited with Burke, Michaels, and Muhr, is entitled Advancing Psychotherapy for the Next Generation: Rehumanizing Mental Health Policy and Practice. He has also written a novel about the process of psychotherapy, Ghosts of the Unremembered Past, additionally released as an audiobook. He is a Personal and Supervising Analyst at the Psychoanalytic Institute for Northern California and a Training Analyst at the San Francisco Center for Psychoanalysis. "When you're with a patient you take all that you know in your head, all the theory, and you throw it away. You have to listen to the patient and then maybe afterward something becomes clear - you use that ‘in-between' as a way maybe in the next session. But if you were sitting there and thinking: ‘Now the patient is in the paranoid/schizoid position…' that would be disastrous. You have to listen with your guts, your emotions, your intellect, and your body, in order to understand what is going on in a particular moment, in a particular session. Then later on you might be able to make sense of it through theory and through supervision."    Episode Description: We begin with considering the cultural and linguistic contributions to intrapsychic processes and the analytic encounter. Jeanne shares with us her life story involving her 'temporary' visit to California, which became a 37-year stay that included her becoming a psychoanalyst. We discuss the meaning to her and to her analysands of her being German and how she worked with that clinically. She moved to Vienna and began teaching and practicing analysis there, enabling her to compare the two psychoanalytic cultures and methods of practice. We also take up the importance of the German language as the vehicle through which Freud discovered the unconscious. So what in the WORLD do we do about it if we're not interested in having an eating disorder do the communication for us?    Tom is a psychologist and Certified Eating Disorder Specialist and Supervisor. He's a psychoanalyst and chairman of the Department of Psychology at Golden Gate University. Today's episode features an expert physician on Frontotemporal Dementia (FTD): Dr. Brad Dickerson. In this episode we cover: The science of FTD, how it is different from Alzheimer's, updates on research, and more. Brad Dickerson, MD is Professor of Neurology at Harvard Medical School and Tom Rickles Chair in Progressive Aphasia Research, Leader of the Neuroimaging Core of the MGH Alzheimer's Disease Research Center, and Director of the Frontotemporal Disorders Unit at the Massachusetts General Hospital, an integrated multidisciplinary unit dedicated to the highest level of care of patients with these conditions. Dr. Dickerson is also a behavioral neurologist in the MGH Memory Disorders Unit. Dr. Dickerson is an active clinical consultant in many aspects of cognitive and behavioral neurology of neurodegenerative and related disorders, including frontotemporal dementia, primary progressive aphasia, Alzheimer's, mild cognitive impairment, posterior cortical atrophy, and related conditions, and the use of neuroimaging and other diagnostic markers in neurodegenerative diseases. Dr. Dickerson runs a multidisciplinary team of 30 clinicians and scientists using advanced brain imaging and behavioral methods to study how memory, language, emotion, and social behaviors change in normal aging and in patients with neurodegenerative disease, with extensive funding from the National Institutes of Health. His team also studies new approaches to caregiving. He has published more than 195 articles in peer-reviewed scientific journals and has edited two books on dementia. He is active in mentoring trainees and in teaching, is Chair of the Medical and Scientific Advisory Council of the Massachusetts Alzheimer's Association and is Chair-Elect of the national Medical Advisory Council of the Association for FTD. A “very basic” type of gene therapy could potentially cure hemophilia, but a major hurdle has been the lack of an effective mode of delivery. Recent strides in using adeno-associated virus (AAV) vectors are changing that, and Glenn Pierce, MD, World Federation of Hemophilia Vice President, Medical, predicts approvals in the next 12-18 months. Dr. Pierce shared his personal experience with hemophilia and discussed his and others’ ongoing research on the use of AAV-mediated gene therapy with host David Henry, MD, in this episode. Hemophilia and AAV gene therapy key points: Hemophilia is caused by a monogenic defect and could, theoretically, be cured by gene replacement or augmentation, says Dr. Pierce, who notes that “it sounds disarmingly simple, but behind that simplicity is a very complex procedure.” The approach uses “gene addition,” which is a basic gene therapy involving the addition of a normal gene to the variant in an individual. This ultimately corrects the clotting factor deficiency. The complexity is in getting the normal gene into the body where it can have the intended therapeutic effect. After more than 20 years of working to overcome that barrier, Dr. Pierce and others are finding success with using AAVs. The approach has some similarities to that used in developing the mRNA COVID-19 vaccines but requires the use of DNA established within the virus (rather than mRNA) to provide a more stable effect. Questions about how long it will last are currently being investigated. Multiple phase 3 trials are underway or completed. Data from two of those have been released in recent months, and the results are very encouraging: “It’s a remarkable achievement – many patients are doing well and, for all intents and purposes, could be considered free of [hemophilia],” Dr. Pierce says, adding that he would “potentially … use the ‘C word’ – cured – for at least a period of time.” The therapy is generally well tolerated. Efforts are ongoing to identify the best ways to proactively and reactively use prednisone to manage side effects such as mild increases in transaminase levels. To date, the risk-benefit profile appears reasonable for patients with clotting factor IX deficiency, and it is likely that the therapy in that setting “will march toward the regulatory process to determine if it’s safe and effective for approval,” he said. Responses in those with clotting factor VIII deficiency have been more variable, with some patients requiring long-term prednisone use, which is problematic. More information is needed about this, but investigation is ongoing, he said. Registries are available and many companies are involved in clinical trials. Clinicians and patients can look for information at clinicaltrials.gov, wfh.org (which publishes trial results and conducts workshops and meetings), and at the US National Hemophilia Foundation (Hemophilia.org) and the Society of Thrombosis and Hemostasis (ISTH.org). He was awarded a Rotary Ambassadorial Scholarship (then known as a Rotary Foundation Fellowship) for post-doctoral work in pharmacology and neurology at the University of Michigan, USA to build on his PhD from The University of Sydney. In 2012 he was honoured internationally by Rotary International being the world wide winner of the ”Global Alumni Service to Humanity Award” for his contribution to greater understanding and peace through service to humanity. Only one other Australian has been so recognised – our former member Dr Alex Buchanan. John's Global Alumni Service award was made for two streams of contribution – contributions in research in health, agricultural and environmental sciences plus achievements as a leader of Australia's overseas development assistance (aid) program. While Deputy Secretary in the Victorian Government from 2009-2012 – overseeing emergency management, biosecurity, fisheries, and regional economic development through agribusiness in the former Department of Primary Industries, he was a member of Hampton Rotary Club. Effectively he remains an Honorary Member of that Club, as while he works in Canberra during the week, his weekend home is in Melbourne. He comes down for a number of weekend Rotary activities, including their popular monthly Farmers Markets. His wife, Amanda Hill is current President of the Rotary Club of Hampton. He has an extraordinarily broad range of experience in medical, agricultural and environmental policy as well as regulation, research management, technology application and commercialisation. In the 1990's he held senior management positions in CSIRO and Cooperative Research Centres. Dr Skerritt has been involved in international development assistance projects from the mid-1980s until the present, and from 1999-2009 he was Deputy CEO of the Australian Centre for International Agricultural Research, and had overall management of about $2 billion in international aid projects. He has significant experience on boards of international and national organisations. From 2005-2012 he was Board member and then Chair of the International Water Management Institute, and he is currently Vice Chair of the International Coalition of Medicines Regulatory Authorities and Chair of the Scientific Advisory Council of the Centre for Innovation in Regulatory Science. As well as his PhD in Pharmacology from Sydney University where he was the first University Medallist in that discipline in 150 years, he is an adjunct Professor of the Universities of Sydney, Queensland and Canberra – in medicine, agriculture/food and pharmacy respectively. He was elected a Fellow of the Academy of Technological Sciences and Engineering and a Fellow of the Institute of Public Administration (Victoria). With such an extraordinary range of skills, experience and plus his sustained connections to Rotary, we are privileged that John Skerritt is the Centenary Angus Mitchell Orator.

Amy Shapiro, MD, is Medical Director and CEO of the Indiana Hemophilia and Thrombosis Center in Indianapolis and Adjunct Senior Investigator, Clinical Track at the Blood Research Institute in Milwaukee, WI. After receiving her medical training at New York University School of Medicine in New York City, Dr. Shapiro completed her pediatric internship, residency, and fellowship in pediatric hematology/oncology at the University of Colorado Health Sciences Center in Denver. Author or co-author of more than 320 journal articles, abstracts, and textbook chapters, Dr. Shapiro is clinically focused on improving treatment for people with rare bleeding disorders. She has served on the National Hemophilia Foundation's Medical and Scientific Advisory Council and as well as several boards for the National Institutes of Health in Data Safety Monitoring and Clinical Trial Review. Dr. Shapiro currently serves on the FDA Blood Products Advisory Committee. As one of the founders of the American Thrombosis and Hemostasis Network (ATHN), she has served as Co-Chairman of the Board of Directors and remains active on various ATHN committees. Dr. Shapiro has been honored as the National Hemophilia Foundation Physician of the Year Award and most recently received their Leadership in Research Award. We are talking about brain health, and how to preserve yours. Our guest today is the amazing Dr. Annie Fenn, OB/GYN turned chef and brain health specialist. Annie explains how what we eat drastically affects the health of our brain as we age, and how good food choices can minimize our risk of Alzheimer's and other dementias. Listen on to hear what is good for your brain health and what you might want to avoid. Dr. Annie Fenn is a board-certified obstetrician/gynecologist specialized in menopausal health. After practicing medicine for more than 20 years in Jackson, WY, she realized that helping people change what they eat was just as effective a health-giving intervention as prescribing pills or procedures. She traded her stethoscope for an apron and went back to school to study culinary arts in Mexico, Italy, and at the Culinary Institute of America. In 2015, Dr. Fenn founded the Brain Health Kitchen, and evidence-based online resource about how and what to eat to resist cognitive decline. The Brain Health Kitchen Cooking School, founded in 2017, is the only school of its kind focused specifically on fending off Alzheimer's and other dementias. Students learn which foods are most neuroprotective, which ones accelerate cognitive decline, and how to prepare foods using brain-friendly cooking techniques. Dr. Fenn partners with the Aspen Brain Institute as the host and moderator of a Brain-Healthy Cooking series with leading neuroscientists and physicians in the brain health world. Dr. Fenn takes her cooking school on the road to provide classes throughout the country and abroad. Dr. Fenn lectures frequently about the impact of lifestyle and diet on Alzheimer's risk as part of the Sempre Sano Wellness Retreat in Tuscany, Italy, as a frequent Visiting Chef at Rancho La Puerta, and at diverse venues throughout the country—women's and men's groups, churches, schools, and wellness retreats. To date, more than one thousand participants have enrolled in the hands-on cooking school. Dr. Fenn serves on the Science Advisory Board for actor Seth Rogen and his wife Lauren's HFC (Hilarity For Charity) where she also creates content geared towards helping caregivers eat for brain health. She partners with Maria Shriver and the Women's Alzheimer's Movement to host events to fund grants for female-specific dementia research. She serves on the Scientific Advisory Council for the Alzheimer's Research and Prevention Foundation and is a Scientific Advisor for NeuroReserve. Dr. Fenn is currently under contract with Artisan Books for a science-based brain-healthy cookbook due out in 2022. About the Speaker: Professor Feld has been Chairman of the German Council of Economic Experts since March 2020 and is the current Director of the Walter Eucken Institute at the University of Freiburg where he lectures in economic and regulatory policy. Professor Feld is also a member of the Scientific Advisory Council to the Federal Ministry of Finance, the German National Academy of Natural Sciences Leopoldina, and the Independent Advisory Board to the German Stability Council. As an advisor to the German Ministry of Finance, he played a crucial role in embedding the debt brake in the German Constitution.

Ira Pastor Comments: Today we're honored to be joined by a true living legend of the regenerative biology world, Dr. Bruce M. Carlson, M.D., Ph.D., Professor Emeritus of Anatomy and Cell Biology at the University of Michigan. Dr. Carlson is also the author of one of the cornerstone texts of the world of regenerative biology, Principles of Regenerative Biology, a must read, encyclopedic text on the last 100 years of global science surrounding the discipline. Dr. Bruce M. Carlson: After receiving an M.S. in Ichthyology at Cornell University, Dr. Carlson completed his medical and doctoral studies at the University of Minnesota. For over 40 years, he was a faculty member in the Departments of Anatomy and Cell Biology and Biology at the University of Michigan. He also directed the Institute of Gerontology. His research has involved limb and muscle regeneration, limb embryology, and the biology of aging and denervated muscle. Along with 200 papers, Dr. Carlson has authored 13 books on regeneration, embryology and lake biology, and has edited another 15 symposium volumes and translations. He has received a number of awards, including the American Association for the Advancement of Science (AAAS) Newcomb-Cleveland Prize, the Henry Gray Award of the American Association of Anatomists, which he served as President, and membership in the Russian Academy of Natural Sciences. He has conducted research for extended periods in the former USSR, Czechoslovakia, the Netherlands, Finland and New Zealand. Dr. Carlson's Body of Work: In addition to the aforementioned Principles of Regenerative Biology, other books Dr. Carlson has been involved in authoring / co-authoring include: Stem Cell Anthology: From Stem Cell Biology, Tissue Engineering, Cloning, Regenerative Medicine and Biology, Human Embryology and Developmental Biology, The Human Body: Linking Structure and Function, and Beneath The Surface: A Natural History of a Fisherman's Lake. Dr. Carlson currently serves on the Scientific Advisory Council of the Fetzer Franklin Fund whose mission is to explore the frontiers of scientific knowledge and to advance breakthroughs towards scientific views of reality that are integrated and relational. In conducting its program of open exploration, the Fetzer Franklin Fund focuses on foundational questions at the frontiers of physics, biology, and consciousness research. In addition, the Fund supports work that re-examines the foundations of science, including scientific methodologies for both conventional and frontier research. On this show we will hear from Dr. Carlson: About his background, and scientific path from ichthyology, to anatomy, to regenerative biology. His regeneration research in the former Soviet Union, during the cold war, under the auspices of an exchange program between the Academies of Sciences of the USA and USSR. About his current views on research in 2020 related to regeneration, morphogenesis, aging, and cancer. This week we look forward to our interview with Eugene C. Cole DrPH. Dr. Gene Cole is Director of Research for LRC Indoor Testing & Research, Cary, NC; and formerly Professor of Environmental Health Sciences at Brigham Young University, Provo, UT. He has 35 years of research experience, with a primary focus on the ecology of indoor and work environments, with special emphasis on identification and reduction of pollutant reservoirs and sources, bioaerosols, human exposure assessment and control, product evaluation, cleaning and restoration, mold and sewage remediation, and biocides.   Since 2000, he has continued to conduct research on the relationship between the use of antibacterial cleaning and hygiene products in the home, and antibiotic resistance; as well as on the effectiveness of cleaning to reduce the transmission of disease agents in schools. He has also worked with national and international organizations to address environmental health and infectious disease concerns such as medical waste management in Central Europe and South East Asia, hygiene promotion in Africa, and healthy homes and buildings in the U.S. and Asia.   Dr. Cole is a member of the Scientific Advisory Council of the Cleaning Industry Research Institute (CIRI), and a Fellow of the American Industrial Hygiene Association. He holds a Master of Science in Public Health Microbiology and a Doctor of Public Health in Biohazard Science and Occupational Health, both from the University of North Carolina at Chapel Hill.

An award-winning teacher and researcher, Jahn also consults globally for business, governments, philanthropic organizations and others. During our 45 minute conversation we gain Dr. Jahn's perspective on what it'll take to address climate change on a planet of 7.6 billion people. We also discuss how our current approach in producing food and using water are in fact one of the most pressing National security concerns. Dr. Jahn shares how the Government shutdown at the end of '18 / early '19 is impacting real science which so vital in dealing with climate issues in a timely fashion. And, how that science is now losing traction under current governance. Dr. Molly Jahn has previously served as dean of the University of Wisconsin’s College of Agricultural and Life Sciences, and Director of the Wisconsin Agricultural Experiment Station.  From 2009-10, she served as Deputy and Acting Under Secretary of Research, Education, and Economics at the U.S. Department of Agriculture. Jahn has >100 peer-reviewed publications and >60 active commercial licenses.   She has numerous awards, fellowships and lectureships for her research, teaching and outreach.  In 2014, she was named the first Lilian Martin Fellow at the University of Oxford’s Martin School.  Her innovative approaches to inter-sector partnership, engagement with emerging institutions and integrated large projects focused on impact and technology transfer have been highlighted in a number of studies and books.  She has served on numerous boards and scientific advisory panels around the world including the US National Academies of Science Board on Agriculture and Natural Resources, NASA’s Applied Sciences Advisory Council. The first –Governing Cotton - focused on the challenges associated with cotton and poverty reduction across Sub-Saharan Africa. The second – Cotton, focuses on the global politics of this important commodity.
