Dr. Josh Mandel says his first love was software. But on a whim, while studying computer science and software engineering at MIT, he took a course that opened his eyes to the world of medicine and genetics. It changed the trajectory of his career away from software – but only temporarily. He entered medical school after earning a bachelor's degree in computer science and began rotating through Boston-area hospitals at the same time Meaningful Use accelerated adoption of electronic health records. With a background in computer science and training as a physician, Josh understood the promise of EHRs, how medical professionals would actually use them, and how to make them better. Based on his unique combination of expertise, Josh took it upon himself to begin making improvements to the systems at the hospital where he worked.Nearly two decades later, Josh is now Chief Architect for Health at Microsoft Research. In this role, he focuses on developing an ecosystem for health apps with access to clinical and research data, leading standards development for data access, authorization, and app integration.For the third and last episode in this Healthcare is Hard series, Keith Figlioli spoke to Josh about data interoperability and emerging technologies. This conversation follows previous episodes with Epic's head of R&D, Seth Hain in Part 1, and the Interoperability Practice Lead at HTD Health, Brendan Keeler – also known as the “Health API Guy” – in Part 2.Some of the topics Keith and Josh discussed include:The standards landscape. At Keith's request to explain the evolution of health IT standards as if he were talking to a seven-year-old, Josh breaks it down in simple terms. He outlines how structured data related to things like allergies, medications, and vital signs are well standardized today, while newer data types like genomics and imaging remain fragmented. He also explains the role of HL7, FHIR, and the Argonaut Project in shaping interoperability.How AI flips the script on standards. Josh says generative AI changed the way he thinks about engaging with the standards community. After getting an early preview of GPT-4 a few years ago, he realized that it would dramatically reduce the value of detailed data structure standards over time. He says that as AI becomes better at interpreting unstructured data, the focus will shift from formatting to governance – who can access what, and under what conditions. He described the concept of “language first interoperability” as one initiative he's working on where automated agents query each other in the equivalent of an email or chat thread. Instead of exposing extensive details upfront, agents that can access unstructured data and understand things like medical necessity and other guardrails can send messages to each other until they make a conclusion about a specific task. This technology will increase the value of standards for data access and privacy, while reducing the focus on interoperability.Advice for startups. In a fast-moving landscape, Josh urges startups to “build and explore.” He emphasizes the importance of staying close to customers, iterating quickly, and leveraging today's best models while keeping an eye on what's coming next. Joshua Mandel, a 46-year-old film production accountant from Woodland Hills, California, has been indicted on six counts of wire fraud for allegedly embezzling over $1.9 million from independent film productions between 2019 and 2023. Mandel, who owned and operated First J Productions Inc. as its CEO and CFO, was entrusted with managing cash flow, payroll, expenses, and bank accounts for various indie films. He also controlled prepaid debit cards issued by CASHét Card, a tool commonly used in the film industry for quick payments. Prosecutors allege that Mandel exploited this access by writing unauthorized checks, executing illicit wire transfers, and funneling money into a personal CASHét Card account whimsically named "Fun Fun Fun."To mask his activities, Mandel reportedly shuffled funds between different production companies, creating a web of transfers that temporarily covered deficits in one entity's books with stolen money from another—a tactic reminiscent of a Ponzi-like shell game, though formally charged as wire fraud rather than a traditional investment scam. This scheme allowed him to sustain personal extravagances while delaying detection. The embezzled funds fueled a lavish lifestyle, including payments totaling hundreds of thousands of dollars to various young women, such as pornographic actresses, and over $129,000 specifically to one woman he connected with via a "sugar daddy" website. Additional expenditures included more than $24,000 on Las Vegas hotels, clubs, and shows, as well as over $12,000 on luxury goods from Louis Vuitton. If convicted on all counts, Mandel faces up to 20 years in federal prison per charge, underscoring the severity of the alleged financial betrayal in an industry already fraught with tight budgets and creative risks.The case highlights vulnerabilities in film production accounting, where trusted individuals like Mandel hold significant financial authority over transient projects. The FBI is leading the investigation, with Assistant U.S. Attorney Alexander B. Schwab prosecuting. Mandel is presumed innocent until proven guilty and is scheduled for arraignment on September 10, 2025, in U.S. District Court in Los Angeles.### Additional Information on the CaseThis indictment was announced by the U.S. Department of Justice on August 8, 2025, just days before the current date. Today, Ali and Asif, after a digression about Paris restaurants and Reddit trolls, discuss the career of Bill Burr (7:33). They discuss his early life, starting out in comedy, his Philly rant and his podcast. They then discuss his television cartoon ‘F is for Family' and his acting roles, his film ‘Old Dads'  including his recent broadway turn in ‘Glengarry Glen Ross'. They then discuss Burr's standup style, whether comedians can be “happy and funny”, and Burr's recent special ‘Drop Dead Years'.  They then (under duress) discuss the recent interview which Howie Mandel conducted with Bill Burr, where Mandel brings out Billy Corgan (Smashing Pumpkins) who is allegedly Burr's half-brother.    Based on some topics brought up in his special, the guys discuss sudden cardiac death (SCD) (50:44). Asif starts off by differentiating between heart attacks, sudden cardiac arrest and sudden cardiac death. He then goes over how common it is and the major causes. Asif differentiates how the causes of SCD differ between young people and middle aged people. It seems as though the 12 bar blues structure is an immovable object, an obelisk of sacred, ritualistic, totemic authority. But the form contains multitudes of variety. Here are two examples of the plasticity of that container, which holds the magic blues elixir and can pour it smoothly, or have it gush out, uncontrollably, like a tidal wave. So, be careful when you put your lips to the chalice … the edges can be sharp, and the liquid it delivers may be too intoxicating! Up first, we have Harvey Mandel, the “Snake”- a guitar wizard, brandishing and slashing his ax-cutlass like a pirate, as he boards equilibrium's frigate docked inside your ear canal, with his own composition, 4pm; then, JB Hutto and the Hawks stagger through “Too Much Alcohol” like drunkards in search of another, much needed pint of medicine - with his razor sharp slide work JB illustrates those sensations perfectly, with an insistent, whimpering pulse.HARVEY MANDELIn 1967, Samuel Charters, for Vanguard records, in an early effort to reveal and showcase the second generation of Chicago's blues scene,  produced harmonica master, Charley Musselwhite's first album STAND BACK! HERE COMES CHARLEY MUSSELWHITE'S SOUTHSIDE BAND, and caused a literal big bang in the hearts and minds of American youth. This was Harvey Mandel's first recording, and launched the notable guitar slinger for hire's 60 year career, going on to join Canned Heat, John Mayall's Bluesbreakers, recording many solo efforts, and landing one step away from becoming a Rolling Stone.   Known as “the King of Sustain,” our track, 4pm, showcases the longest sustained electric guitar note at age 21, a feat copied by Jimi Hendrix two years later. Harvey's inventive style is trademarked, and you can hear those clarion notes ringing out like a buzz saw, transitioning from one amazing improvisation to the next. JB HUTTO AND THE HAWKSThe next cut, TOO MUCH ALCOHOL, also on the Vanguard label - featured on 1966's Chicago / Blues / Today! Sounds almost like it could have been recorded in the previous century, when, fresh off the farm, players were captured in their first big city outings on lo-fi recordings - it's that raw and spiky. And, yet, both Mandel and Hutto were breathing the same mid-60s Chicago air. JB's slide style was influenced by Elmore James, of DUST MY BROOM acclaim, and he carried that torch forward. The son of a preacher from South Carolina, Hutto made his way to Chicago after his father's death in the early 50s, but ended up working as a janitor for 11 years, until, in the mid 60s, he was discovered and unleashed on the world by Vanguard.
