Related Report: Failure to Follow a Consult Process Resulting in Undocumented Patient Care at the Chillicothe VA Medical Center in OhioThe VA OIG conducted a healthcare inspection for 10 allegations related to the quality and management of patient care and the availability of resources within the Urgent Care Center at the Chillicothe VA Medical Center in Ohio.One allegation involved an urgent care provider sending a patient with a T12 vertebrae compression fracture to have chiropractic care at the Complementary and Alternative Medicine (CAM) clinic. The patient returned a week later with a T12 burst fracture and rib fractures.The OIG found that an urgent care provider verbally referred a patient for pain management and not for chiropractic care. However, the OIG found that the urgent care provider did not enter a CAM consult until eight days after seeing the patient. Due to this delay, the chiropractor and clinical massage therapist failed to review the consult prior to seeing the patient. Additionally, the chiropractor and massage therapist could not link documentation to the consult and had no other process to complete the documentation resulting in the failure to document care provided within the medical record.The patient returned to the Urgent Care Center eight days later where a computerized tomography scan showed an acute burst fracture and acute rib fractures. Because of the lack of documentation and provider recall, the OIG could not conclusively determine the relationship between the actions taken by the chiropractor and clinical massage therapist and the patient's bone fractures.The OIG found the nine additional allegations to be unsupported and lacked merit.The OIG made two recommendations to the Facility Director related to education of providers, chiropractors, and clinical massage therapists on the use of consults and timely documentation, and conducting an internal review of the CAM program processes related to patient care, reviewing consults, scheduling appointments, checking-in patients, and documentation.

One of the questions I get most often is. . . how do you do your hair? With all the sweats and workouts I do, people are surprised that I can go from gym to date night within a few hours with a fully styled head of hair. This episode was inspired by an Instagram DM question I received and it sparked me to record a show all about this question. SMSgt Griffin-Greer discusses struggling with survivor's guilt, invisible wounds, and giving 100% to the Air Force and her family. Senior Master Sergeant Griffin-Greer completed seven years First Sergeant duty at 37 TRW, 37 TRG, JBSA-Lackland Texas at the 37 TRG Training Support Squadron (TRSS), 344th Training Squadron, 342nd Training Squadron and Seymour Johnson's 4th Fighter Wing's Wing Staff Agency and 4th Operations group. Prior to First Sergeant duty, she was assigned as the Senior Enlisted Leader of the Surgical Technology Course at the Medical Education & Training Campus, at Fort Sam Houston, Texas where she provided supervision and instruction to over 780 students annually in a 368-hour entry-level surgical technician course. She was born in Alexandria Virginia, raised in Raleigh North Carolina and is married to William DeShon Greer and has 2 children: Aaron-26, and Cristian-22. After graduating high school, she attended Draughons Medical College in Atlanta Ga. where she studied to become a Medical Assistant. Upon graduation, she was employed at Planned Parenthood of Greater Raleigh; Dr's Urgent Care Center and served a brief stint at the local health department while awaiting admission to nursing school. She decided to enlist in the Air Force in 1994 and became a surgical technician. SMSgt Griffin-Greer also served a total of seven and ½ years as a recruiter at the 337th Recruiting Squadron as both an Enlisted Accessions recruiter and as an Operations NCO, Prior Service liaison. She has completed, Airman Leadership School (ALS), Non-Commissioned Officer Academy, (NCOA) the Basic Instructor Course (BIC), the Coast Guard's Chief Petty Officer Senior Enlisted Academy (CPOA), the Instructor Supervisor Training Course and received her Surgical Services Associates of Applied Science and Instructor of Technology and Military Science Degree and Human Resource Management degrees via CCAF in Jun 2007, 2008 and 2012. She has also received the Occupational Instructor and Professional Manager and earned the title of Master Instructor. She has completed Senior Enlisted Joint Professional Military Education (SEJPME), her Bachelor's degree in Organizational Leadership and is currently enrolled in Liberty University's Masters in Theological Studies program. SMSgt Griffin-Greer has also performed as a certified Mediator and Conflict Management Coach for the Alamo Federal Executive Board's (AFEB) Alternate Dispute Resolution Program for more than five years.   One of the most expensive ways to receive care in the US health care system is through the emergency room. The use of urgent care centers is an alternative that is much less expensive. Unfortunately, one of the problems with urgent care centers is that the care is often not very good. It is especially poor when it comes to treating children. As an example, the average urgent care refers 40% of its patients to the emergency room even though about half receive no special treatment at the ER.   My guest today is pediatrician Dr. Corey Fish who saw the importance of urgent care but felt ones that specialize in kids could do better. His ER, Brave Care, averages referrals to the ER of around 1-2% meaning that most kids can get seen and treated for 10% of the cost of an ER trip and saving time for the parents as well.   Dr. Fish describes the concept of Brave Care, a startup pediatric urgent care company looking to expand into markets all across the United States. Does Medicare Pay For Emergency Care While Traveling?

Hey Medicare Nation! www.TheMedicareNation.com It's almost Spring time! For many people, this has been a terrible winter. Many Medicare Nation listeners have been emailing me to find out if Medicare covers "Emergencies" while traveling across the U.S. or abroad. That's a great question! Original Medicare and Medicare Advantage Plans Do cover "Emergency Care" AND  Urgent Care ANYWHERE in the United States and it's Territories. An "Emergency" is Life-Threatening. An example would be if you were having chest pain and you believed you were having a heart attack. In this situation.....you would go to the nearest hospital to seek emergency care. Even if it turns out you were diagnosed with "heart burn," Original Medicare AND Medicare Advantage plans will cover the medically necessary treatment for this situation because you believed you were in a "life-threatening" situation.  "Urgent Care" is defined by Medicare as: Care that you get outside of your Medicare health plan's service area for a sudden illness or injury that needs medical care right away but isn’t life threatening. If it’s not safe to wait until you get home to get care from a plan doctor, the health plan must pay for the care. An "Urgent Care" example would be if you were walking in St. Peter's Square at the Vatican, and you slipped on a banana peel and fell onto the ground, breaking your ankle. That's an injury that isn't life threatening, but requires immediate medical care. So......if you are traveling ANYWHERE in the U.S. or it's Territories, AND you have an Urgent Care or Emergency situation.....you can go to the nearest hospital or Urgent Care Center to receive care AND it will be covered by Original Medicare and Medicare Advantage Plans. If you are traveling outside of the U.S. Medicare generally DOES NOT cover emergencies or urgent care needs. There are a few circumstances where Original Medicare WILL cover Emergency Care AND Urgent Care. www.TheMedicareNation.com 1.  If you are on a CRUISE and you require EMERGENCY care from a doctor who is stationed on the ship while the ship is in a U.S. port.....Your Emergency Care WILL be covered by Original Medicare. 2. If the ship is Departing or Arriving to/from a U.S. port within 6 hours and you have a medical emergency and require to be treated by the ship's doctor......Your Emergency Care will be covered by Original Medicare. 3. If you are in Alaska and you are traveling directly to another State without unreasonable delay, and you require Emergency Care at a hospital in Canada, because it was the closest hospital at the time of the emergency......Your Emergency Care will be covered by Original Medicare. If you have a "Medicare Advantage Plan," you may have coverage for Emergency and/or Urgent Care Coverage on your plan. You must do your own due dilligence to understand the benefits of your plan while you are traveling abroad. Some Medicare Advantage Plans have a deductible for emergency care outside the U.S. Some Medicare Advantage Plans have a deductible and a co-pay for emergency care outside the U.S. There is an annual maximum out-of-pocket amount for your plan. Some are around $1,500.00 all the way up to $6,700.00  READ Your Plan's EVIDENCE OF COVERAGE Booklet. Some of you have "Medi-Gap" or Supplement to Original Medicare Plans. Plans "C" through "G" and also plan "M" and "N" have coverage for Emergency Care while traveling abroad. Some Medi-Gap plans have a deductible. Some plans have "Maximum Lifetime Amounts." It is important to READ your Medi-Gap Policy to determine coverage while traveling abroad. Travel Insurance www.TheMedicareNation.com   I always recommend purchasing "Travel Insurance," while traveling abroad. I use these different websites to look for policies: 1. www.TravelGuard.com 2. www.AllianzTravelInsurance.com 3. www.TravelInsurance.com Cost will depend on - a. Total Cost of the Trip b. Your Age c. What country you're visiting d. Types of coverage you're adding (ex: Air evacuation, cancel for any reason etc.) If you have ANY questions, and I can answer your question in ONE paragrapn, send them to me by email. When it comes to choosing the best care for you and your loved ones, it's important to know whether you can opt for a visit to the local Urgent Care Center or if it's best to head straight to the Emergency Room. Janice Edmonds, RN, talks with program host Robin Sills, RN, about Saint Mary's Urgent Care Centers, which are equipped to handle urgent medical concerns, from sore throats and ear aches to something more serious that might require stitches or an X-ray. Saint Mary's Urgent Care Centers are staffed by physicians and other providers who are trained in emergency medicine, and they're super quick and convenient. But for more serious or life-threatening injuries or illnesses, like chest pain, suspected stroke, head injury, extreme burns (with blistering), suspected drug overdose, uncontrollable bleeding, or broken bones that you can see, your best bet is still the ER.
