Send us a message with this link, we would love to hear from you. Standard message rates may apply.Managing allergic rhinitis requires realistic expectations and a strategic approach to medication. Though no treatment will completely eliminate symptoms, the right combination of therapies can significantly reduce discomfort and improve quality of life during allergy season.• Nasal corticosteroid sprays are first-line treatments that reduce inflammation in nasal passages• Proper technique matters: point spray toward outside of nostril and sniff gently "like smelling a flower"• Non-sedating antihistamines (Zyrtec, Claritin, Allegra) relieve itching, sneezing, and runny nose without drowsiness• Nasal antihistamine sprays provide targeted relief and can start working within minutes• Oral decongestants raise concerns about blood pressure and heart rate increases• Avoid nasal decongestant sprays (like Afrin) beyond 2-3 days to prevent rebound congestion• Combination therapies often work better than single medications• Consider allergen immunotherapy when standard treatments don't provide adequate reliefThank you for coming back to another episode of Your Checkup. This week on Conflicted, Ronnie Hamada is back with Thomas to describe his experiences as a Kurdish civilian during the Syrian Civil War, as we continue our retrospective on that most complex of conflicts. And this week, after setting up some Kurdish history last time, Ronnie takes us through what it was like to live through the Arab Spring protests as a university student, where he saw his classmates attacked by Assad backed thugs, and as violence increased around him by the day. In a gripping conversation, Ronnie recounts some harrowing experiences, detailing a terrifying robbery at an internet cafe, the escalating violence in Aleppo, and the eventual decision to flee to Afrin. This week on Conflicted, we begin in earnest our retrospective on the Syrian Civil War, where we will be inviting actual Syrians from a multitude of different perspectives, to tell us about their experiences of the war.  After setting the scene with our old friend Wassim Nasr last time, over our upcoming episodes, we'll be creating a kind of narrative history for you, our listeners, as we make sense of this most complex of conflicts from as many angles as we can. In this episode, Thomas invites on Syrian Kurd Ronnie Hamada to delve into the complexities of the Syrian Civil War, focusing on the Kurdish experience and setting the scene with their historical identity.  Ronnie has been working in data and analysis of conflicts in Syria and the wider Middle East for the past 10 years, working alongside journalists and NGOs to map conflicts and combat misinformation, and his story of life under siege in Aleppo and Afrin is an extraordinary one… They explore the historical context of Kurdish nationalism, the impact of Arab nationalism, and the political dynamics within Kurdish communities in Syria. Ronnie shares his personal story and insights, highlighting the challenges faced by Kurds, including statelessness and cultural repression and the intricate dynamics of Kurdish identity and politics in Syria, particularly in the context of the Arab Spring. Hamas lideri İsmail Heniyye'nin şehadeti sonrası Türkiye'de 1 günlük yas ilan edildi, bütün camilerde gıyabi cenaze namazı kılındı, çağrısına icabet eden milyonlar meydanlarda toplandı. Bazı kesimlerdeki Hamas'a yönelik hazımsızlık da doruk noktasına çıktı. 3.

The edited volume is unique in its focus and engagement of the veil as it appears in various literary, artistic, and popular cultures, such as of historical Algeria and contemporary Iranian television series, Bollywood films, and street art in Europe. The book locates these critical discussions and theoretical interventions within both a postcolonial and neocolonial critique of how the veil was orientalised and fetishized historically, for example in the Victorian transmissions of the 1001 Nights, and how the veil, specifically the burka, is deployed in reductive and harmful ways in contemporary state projects today, as seen in France, and even in popular liberal discourses (such as in the United States). The essays in this collection are sharp and accessibly written and will be useful as a teaching tool in various undergraduate courses. The book will also be of interest to those who work on literature, popular culture, gender, Islam, and much more. Learn more about your ad choices. In this episode of BackTable ENT, Dr. Shah interviews rhinologist Dr. Jeffrey Suh (UCLA) about cryotherapy as a new treatment for chronic rhinitis. --- CHECK OUT OUR SPONSOR Stryker ENT https://ent.stryker.com --- SHOW NOTES First, the doctors discuss the typical presentation of a patient with rhinitis. Although all rhinitis patients present with a chief complaint of a runny nose, there are different subtypes of rhinitis, such as allergic rhinitis, gustatory rhinitis, and exercise-induced. Environmental exposures, duration of symptoms, and the presence of other symptoms should be elicited, as differentiating between allergic and non-allergic rhinitis changes treatment options. Next, Dr. Suh explains how he performs a physical exam in rhinitis patients. He uses a rigid endoscope to make sure there is not another cause of rhinitis. He is sure to premedicate patients with a lidocaine and Afrin nasal spray before using the endoscope. CT scans of the sinuses may be helpful if patients have drainage, polyps, or notable inflammation. In general, Dr. Suh will pursue medical treatment options before considering procedural options. For allergic rhinitis patients, he educates patients about understanding their triggers for allergies and prescribes nasal steroid sprays and antihistamines. In non-allergic rhinitis patients, he recommends ipratropium bromide, which can also be used as a trial to see if cryotherapy would resolve their symptoms. He notes that sprays need to be used for at least 1 month before they show effects and that they should be used 30 minutes before trigger exposure for allergic rhinitis patients. He does not think that saline rinses help much with rhinitis, as they contain no medications to reduce swelling and dampen nerve stimulation. Next, Dr. Suh explains two procedural options for chronic rhinitis: radioablation therapy and cryotherapy. Both procedures have similar success rates, but he prefers to use cryotherapy in patients with non-allergic rhinitis who have a positive response to ipratropium bromide. The procedure involves inserting balloon a into the middle meatus of the nasal cavity and in order to deliver liquid nitrogen to the nasal mucosa overlying the posterior nasal nerve. He explains different considerations for performing cryotherapy in the office versus in the OR. He also explains his anesthesia protocol and how to mitigate the common post-operative complaint of an “ice cream headache”, which he prophylactically treats with gabapentin. Radiofrequency ablation with the RhinAer procedure also targets the posterior nasal nerve, but it has other complications, like damaging other nerves that allow for the tearing of eyes. He chooses the cryotherapy option more often because it does not require as good visibility as the radiofrequency ablation therapy does. Finally, he explains his postoperative care regimen which includes starting the patient back on their usual nasal sprays. His patients take 2 weeks to see results, and cryotherapy is effective in 80% of his patients. He notes that patients with recurrent rhinitis may need recurrent treatments. --- RESOURCES Stryker ENT https://www.stryker.com/us/en/portfolios/medical-surgical-equipment/ent.html Stryker ClariFix Cryotherapy https://clarifix.com/ RhinAer Radiofrequency Ablation https://rhinaer.com/

Hundreds Injured After Two New Quakes Felt Across Türkiye and Syria

Two new earthquakes have hit southern Turkiye's Hatay province, reigniting the horrors millions felt on February 6, when a pair of quakes killed more than 41,000 people. A magnitude 6.4 earthquake struck Monday evening, and was followed just minutes later by another 5.8 tremor. Several people have died, with eyewitnesses reporting damaged buildings collapsing. Just a few dozen kilometres away in Syria, the situtation was also grim, with monitoring groups saying several hundred had been injured. Having already suffered more than a decade of war, large parts of northern Syria saw renewed destruction with the February 6 earthquakes. The twin tremors, centered in Kahramanmaras, struck just 100km from the Syrian border. More than a thousand buildings in Jindris and the Afrin region were destroyed. In the immediate aftermath, aid supplies fell dangerously short, as international assistance found it hard to enter Syria's last opposition stronghold. What impact will this month's quakes have on the country's ongoing conflict, and has the quake created an opening for the opposition and the regime? While everyone's attention is on Ukraine, key developments in Syria have gone under the radar. Recent reports put the spotlight on Hayat Tahrir al-Sham (HTS), the jihadi faction that runs the northwestern province of Idlib, which had moved into Afrin, the Kurdish-majority enclave that was occupied by Turkey in 2018. This move, and the broader trend we are seeing of gradual HTS expansion and consolidation have raised questions about where Turkey stands in this power grab as it's an open secret that Turkey and HTS collaborate on the ground. Amberin Zaman, a senior correspondent reporting from the Middle East, North Africa and Europe exclusively for Al-Monitor, joins Thanos Davelis to look at what's at stake in northern Syria. Read Amberin Zaman's piece in Al Monitor here: Are Syria's HTS jihadis Turkey's new friends? You can read the articles we discuss on our podcast here:Zelenskyy thanks Greece for tanks, EU bid, solidarityErdoğan gears up for huge pre-election spending spree in TurkeyTurkey set to increase defense spending

Ep. 68 In-Office Procedures for Nasal Valve Obstruction with Dr. Mary Ashmead

In this episode of BackTable ENT, Dr. Ashley Agan and Dr. Gopi Shah speak with Dr. Mary Ashmead, a Dallas/Fort Worth based rhinologist (Texas Ear, Nose, & Throat Specialists) about in-office procedures for nasal valve obstruction. --- CHECK OUT OUR SPONSOR Aerin Medical https://aerinmedical.com/ --- SHOW NOTES First, Dr. Ashmead describes her typical work up for a patient presenting with nasal valve obstruction in the clinic. She explains that “congestion” is a common but ambiguous chief complaint that patients use when they are unable to be specific about their nasal problems. She emphasizes the importance of doing a thorough ENT review of systems and asking about nasal obstruction for patients with chief complaints other than nasal valve obstruction as well because one-fifth of patients with severe nasal valve obstruction are missed. Before she observes her patients, all of them will complete a NOSE score as well. During the physical exam, Dr. Ashmead observes the nasal bones and external nasal valves first and then uses a rigid scope to examine the internal valve, turbinates, septum, and nasal mucosa. She does not routinely give her patients a nasal decongestant before doing her primary scope exam, as she wishes to examine the patient's nose in its native state. Then, she will administer a nasal decongestant and go through the second scope exam again. Miscellaneous issues she will look for include nasal polyps and sinusitis. For conservative treatments, she generally prescribes nasal steroid spray, topical antihistamines, Afrin at night, nasal dilators, Breathe Right strips, and nose cones. Then, Dr. Ashmead delves into the different in-office procedures she employs for nasal valve obstruction patients. The type of therapy she embarks on will depend on the kind of patients she sees. Some of her patients will want to fix everything at once up front and others will want multiple procedures in smaller steps. Additionally, some patients will choose the operating room setting over the office setting. Finally, other details to consider are the patient's desire to keep the same appearance of their nose, the patient's skin thickness, and recovery time. Dr. Ashmead recommends the VivAer procedure for patients with a positive modified Cottle maneuver test, dissatisfied patients post-septoplasty/turbinate reduction, rhinoplasty patients with a narrowed internal valve, and snorers. The VivAer procedure uses bipolar radiofrequency energy to shrink tissue and allow the surgeon to remodel the internal nasal valve, turbinates, and swell body. There are different cooling and heating cycles that take a total of 6-7 minutes to complete. During this procedure, Dr. Ashmead uses a scope to visualize where to place the small paddles. She notes that rebound swelling, nasal tip tension, and the development of scabs are three common minor consequences of this procedure. Next, she discusses the Latera nasal implant, which can only be used in dynamic valve collapse. She does not use this synthetic implant often, as it can get infected and many patients do not want an implant in their noses. A third option she mentions is a septal rhinoplasty, a procedure that she often refers to her facial plastics colleagues. Finally, Dr. Ashmead goes into detail about her anesthesia procedure and the role of anxiolytics in her practice. Her patients will take either 0.125 mg of halcion or triazolam one hour before they arrive at her office for the procedure. In longer cases, she will prescribe her patients Valium. She emphasizes the importance of thorough topical numbing; she usually uses a 4% topical compounded tetracaine/lidocaine gel. Generally, she avoids administering epinephrine in the office, as adrenaline can aggravate an already nervous patient. Finally, she does everything she can to soothe the patient, such as having separate procedure rooms with soothing music and dimmed lights. --- RESOURCES VivAer Procedure: https://vivaer.com/hcp/

Today on the Rarified Heir Podcast, we talk to Julie Nimoy and husband David Knight about Julie's dad, actor and raconteur Leonard Nimoy. It was great to connect with Julie about her father and what it was like being the daughter of a pop culture icon. The thing is, Leonard Nimoy was so much more than just Mr. Spock.  He was a photographer, a poet, a singer, a director, a voice over artist, a pitchman and more. Did you know he directed the top grossing movie of 1987 which was NOT a Star Trek film? Did you know he was such a fan of Vincent Van Gogh that he wrote a one man show, starred in it and had it taped for PBS? Nimoy was also the host of In Search of….where he showed off his amazing collection of houndstooth jackets and turtleneck shirts. He replaced Peter Graves in Mission Impossible. He stared inNight Gallery, Deathwatch, an independent film directed by pal Vic Morrow, a TV movie A Woman Called Golda which got him an Emmy nomination, The Outer Limits and much more. Nimoy was also interested in the paranormal, science (and cars) which led him to Baffled!which is synonymous for our host Josh Mills with Afrin and fever dreams, he recorded albums for Dot Records such as The Touch of Leonard Nimoy, did summer stock, owned a single engine plane he flew, a boat at Lake Mead…it's like they based the film The Adventures of Buckaroo Bonzai Across The 8th Dimension on Leonard Nimoy the man. So it was great talk to Julie and David about their PBS documentary Remembering LeonardNimoy about his battle with COPD as well as their upcoming documentary Wilder! on comedian Gene Wilder & his battle with Alzheimer's.  Did you know who directed Wilder in the film Funny About Love? You guessed it. And yes, we talk more about University High School, LA restaurants, the Century City Mall and much more on the next episode of the Rarified Heir Podcast. Take a Listen.

John is on the lam at Beaver Island so we recruited long time Hartland pal Jason Welichko to step in for him. Turns out Jason loves Beaver Island just like Scott. Do you remember Grumpy Old Men? Jack Lemmon and Walter Matthau movies of the 1990s become the focus for the moment. We're having a heat wave! Are you using somebody else's HBO Max account at that moment? Mike has been exploiting Luke's Netflix for Seinfeld. We owe friend of the show Sean a long segment on his email that we forgot to finish. So we dive into his curiosities about that old news Roe V. Wade and the future motivation of the Republican party now that they have scored some epic wins in the courts. Jason gets honest and admits the thought that Biden might actually follow through on a few things. He admits though, that he wanted anybody but Trump in so he voted for Biden. Paul Ryan used to work out to Rage Against The Machine and we wonder if Bernie Sanders does the same while he stays sharp? The Supreme Court and the punk band MXPX have a commonality in Bremerton. Both have to do with Christianity as well. Wild! Hopefully the right supports the Church of Satan as much as they do Christianity. Maybe Bernie Sanders needs some Vivarin? Or maybe it was the dude with the giant nose from the Afrin commercial? Luke learned from a friend about the bizarre behavior going on in Afghanistan when the women have to wear burkas from head to toe. If you're into sociology you will find it curious. Our next email from Mr. Pink continues into the same serious territory about the exploitation of women and children by famous people. Did you know Pete Townshend was arrested in 2003? Can you still enjoy the music of Michael Jackson knowing what you've heard about him in the past? What about Deshaun Watson and his sexual misconduct? Would you root for him on the field? In the end everything is about money. The answer to all of your questions is money. Finally, Danny calls us squids as he wants to know what grade we give ourselves at the halftime moment of our lives? Great question Danny! We close the show with The Song We Used To Call "Wasting Time" by the Riverboat Gamblers. If any of this nonsense inspires you for better or for worse, email the show at isitsafepod@gmail.com
