Today we're diving into how nutrition intersects with the complicated world of pain. Today I have former guest and colleague Prof. Dr Deepak Ravindran to talk to us about how our immune system can impact our perception of pain, the different types of pain patients and how we can do better as medical professionals in the management of chronic pain. This topic covers IBS, pelvic pain, chronic cystitis, fibromyalgia, migraine, functional gut disorders, long covid and more. Pain cannot be explained by a singular pathway, and I think there is so much we could all learn from Deepak's insights on this topic. We discuss the different types of pain patients, how the immune system trains our responses to pain, how trauma in childhood can impact our responses to pain and why food plays a pivotal role.Deepak is Honorary Professor at Teesside University and has worked as an NHS Consultant in Pain management, he's author of the “Pain Free Mindset” a fantastic book that I recommend to both patients and medical colleagues. His new role is Director of Lifestyle Medicine for a primary care network (PCN) of GP practices in Reading, UK, working upstream to bring these insights early in the chain of pain management. We talk about the use of fasting, anti-inflammatory diets, the importance of gut supportive diets and key ingredients that could be useful for chronic pain sufferers.

Dr Ravindran highlights flawed thinking in regard to the nature of pain and the overpowering fear factors around pain as obstacles to a different style of management in which the provision of a safe space allows pain to subside naturally. Dr Deepak Ravindran, a NHS consultant in pain medicine, reveals how we need to completely rethink our views about pain. Pain, particularly long term intractable chronic pain, can be one of the most difficult medical conditions to live with, making normal work or life almost impossible. Globally around 20% of people suffer from the condition. And getting the help they need can be incredibly difficult. Ravindran explains how many of us totally misunderstand pain. The problem can be someone with chronic pain can have a battery of tests that all come back clear, which may lead some medical professionals to suggest that there is not actually anything wrong with these patients. But that is not the case. For the first time in 2022, the World Health Organisation updated the International Classification of Diseases to include chronic pain as a separate medical condition. And as Ravindran reveals, the biological mechanism of chronic pain, which can lead to an over-sensitised immune and nervous system is not the same as acute pain. That means a very different treatment is required, using not just drugs, but taking a whole body approach which includes diet, exercise, sleep and cognitive techniques. It's a technique that has had life-changing consequences for patients, who have often suffered for years with crippling chronic pain. In the podcast, Ravindran discusses the importance of being cautious about every drug that is given for pain management and recommends the following website: https://thennt.com/ The Pain-Free Mindset: 7 Steps to Taking Control and Overcoming Chronic Pain by Dr Deepak Ravindran is published by Vermilion The host of the podcast, Liz Tucker is an award winning medical journalist and former BBC producer and director.  You can follow Liz on Twitter at https://twitter.com/lizctucker and read her Substack newsletter about the podcast at https://liztucker.substack.com If you would like to support this podcast you can do so at patreon.com/whatyourgpdoesnttellyou or via PayPal at https://www.patreon.com/WhatYourGPDoesntTellYou What Your GP Doesn't Tell You has been selected by Feedspot as one of the top 20 UK Medical Podcasts https://blog.feedspot.com/uk_medical_podcasts/

I've been regularly podcasting now since 2019 and it's been a whirlwind. Whilst this podcast was initially focused on nutrition and medicine, the conversations have spanned a wide range of topics including happiness, motivation, purpose and finding joy and meaning during life's struggles.I'm obsessed with food, flavour and how we connect with each other through the shared experience of eating. And it's because of this appreciation for connection as to why it's felt quite natural for me to cast the net wider when it comes to the conversations I've had on the podcast. This pod isn't just about nutritional medicine, although that is the foundation of what started it. It's become just as much about finding meaning and sparking joy, and it just so happens that we use food as the metaphor for this.Whilst we'll always have food to focus on, I hope you will allow me to expand the conversations beyond the science of food to explore other elements of lifestyle that are as important from both a scientific lens and an emotional one.#124 Fix Your Pain with Deepak Ravindran#104 How To Live with Professor Robert Thomas#91 Solving Happiness with Mo Gawdat#49 Overcoming the odds with Alexandra Adams#26: Eating for Mental Health with Professor Felice Jacka#27 Life Lessons From A Brain Surgeon - Dr Rahul Jandial

My good friend Deepak is back on the podcast today speaking with Sakina. I had the most wonderful conversation with Deepak on the podcast before.Dr Deepak Ravindran is a boundary spanning ‘upstreamist' with a trauma informed approach to pain practice. He has over 20 years of experience in Pain Management and is Clinical Lead in Pain Medicine at the Royal Berkshire Hospital in the UK. He is also one of the very few consultants in the UK who holds a certification in musculoskeletal, pain medicine and is board certified in lifestyle medicine. Longcovid, Pain Medicine and Trauma informed practice are subjects that Deepak is passionate about and today Sakina chats to him about:DIY vs DWY medicineSleep and it's impact on chronic painSetting the circadian rhythm, sunlight and caffeineMaking complementary therapy more accessibleHe hosts the PainSpeak podcast and is the author of the Best Seller “The Pain Free Mindset” released in 2021 and it's by far one of my favouroite books. It's aimed at patients suffering with pain but it's also immensely useful for all healthcare professionals in understanding all the recent advances around pain management in a trauma informed manner.

How the symptoms of long COVID are being managed using group consultations and the many things long COVID has in common with other long-term conditions. Now that COVID has become a part of our day-to-day lives, so too has long COVID. So, with so much money now being directed towards researching treatments for long COVID, how might this impact the way we treat all manner of chronic conditions? In this episode of Airing Pain, Paul Evans interviews the team at the Berkshire Pain Clinic, who run a specific long COVID service, on treating people with the condition. Notably, there are many similarities between the symptoms of long COVID and the symptoms of other long-term conditions, such as fibromyalgia or chronic fatigue syndrome. Issues like managing pain, fatigue, sleep and pacing are not only common in those with long COVID, but those with chronic illness in general. This raises the possibility that the money being spent now on long COVID may have a far wider reaching benefit in the long term. The team at the Berkshire Pain Clinic have already found success in using group consultations to manage long COVID. Listen, or read the transcript, to find out how these consultations are run and why they are proving so beneficial. And I'm going to put this out there. If you know anyone who works in the NHS. A nurse, doctor, physio, dietitian, administrator, manager, literally anyone. Gift them this book: THE PAIN-FREE MINDSET. Not just because pain is a silent epidemic affecting over 25 million people in the UK alone, but because this book is just as much about how we change our approach to healthcare from one of purely patient service, to one of patient empowerment. And this eloquently written book describes how, with an open mind, how we all can create transformative change for patients and ourselves. Dr Deepak Ravindran is an NHS consultant in pain medicine and what he describes as an ‘upstreamist' with a trauma informed approach to pain practice. As you'll hear today, he takes a truly holistic, trauma informed and integrated approach to pain management, who has a deep appreciation for the role of lifestyle, nutrition and the overlap between pain.He has over 20 years of experience in Pain Management and is Clinical Lead in Pain Medicine at the Royal Berkshire Hospital in Reading, Berkshire, UK. He is also one of the very few consultants in the UK who holds triple certification in musculoskeletal, pain and lifestyle medicine.Today we're going to learn all about Pain. Welcome to a Pain Speak Special focussing on a new book on pain "The Painful Truth" by Oxford based junior doctor and academic scholar Dr Monty Lyman released on 15th July. This becomes the 7th book on pain released in the UK in just the 6 months of this year! We're currently experiencing a Renaissance in pain science and it is great to add Monty's book to this fantastic list of  resources for our patients and general public and for anyone interested in knowing more about pain science.Order your copy of the Painful Truth hereIn recent years our understanding of pain has altered so radically it's fair to say that everything we thought we knew about pain is wrong. I made the case in my book as well.  As Dr Monty Lyman reveals, we misunderstand pain - with harmful consequences. This is Dr Monty Lymans second book. His first one " the Remarkable life of the skin" was shortlisted for the Royal Society Science Book Prize". He is a remarkable story teller and in this book , The Painful Truth, he explores and presents cutting-edge research, encompassing phantom aches to persistent pain, and interviews survivors of torture as well as those who have never felt pain. It shows the myriad ways in which pain acts as a protector sometimes unnecessarily. He not only provides hope for reducing and managing pain, but takes us to a deeper understanding of what it means to be human. In the end he presents the hopefuly side of taking this knowledge and using it for improving our health and I am glad that i got a chance to be featured as well! This is the untold story of pain - our most elusive feeling and I am glad that I got a chance to be interviewed for the book and he has agreed to come on to this podcast. Trustee and Advisor to the charity Abscent. A chemist by training, she is based at the Flavour Centre and has had huge experience of work around olfaction and food smells. With previous experience of looking after people with smell disorders in other viral infections and brain injury, she is the right person to talk to us about taste and smell and the strange changes that happen in some people after COVID 19. Dr.Blackburn has been working with staff at the Royal Berkshire for many years and is a hugely respected national figure in Occupational Health and represented her profession at the highest levels. We had recently collaborated to write an article for the Journal Occupational Health@work. In the podcast episode, I wrongly mention it as an editorial so apologies for the error. Please find the link at https://www.atworkpartnership.co.uk/journal/issue/17_5/contents/long-covid-and-its-implications-at-workShe has been invaluable in providing a clear sounding board for many patients who havent been able to go back to work and this includes a large number of NHS workforce. There is a lot of confusion prevailing right now as to how return to work can be established when we are still learning about the long terms effects of Post Covid so I am glad that Blandina can inform us about the complexities and suggest a way forward for now. We discuss: -The kinds of symptoms and problems people are experiencing when they present to the Long Covid clinic -The finding that the vast majority - about 90% - of people with Long Covid are those who have had so-called "mild" disease, where they were not hospitalised. -Theories of the mechanisms underlying development and persistence of LC -Variability in how people respond to the virus -The controversy around use of the term "biopsychosocial" as some patients feel that this term has been mis-used -ME/CFS and other post-viral fatigue syndromes share many similarities with LC -The hope that the recent public interest in LC may positively effect research and future outcomes for people with with ME -In the year 2021, will anyone be diagnosed with ME/CFS, or will everyone presenting with these symptoms be diagnosed with Long Covid? -It's important to validate people's lived experience of Long Covid. At the same time, is there a possibility that exposure to a lot of content about Long Covid may create a "nocebo effect" for those who catch the virus? -Activism and its impact on funding -Pacing; what is it? This term means wildly different things to different people. -Physical and cognitive rest and "flight mode" Blogpost and video available at: getbetteronline.co.uk/whats-going-on-with-long-covid/
