Welcome to PsychEd, the psychiatry podcast for medical learners, by medical learners. This episode covers psychotherapy in youth with Dr. Laurence Katz, a professor of child and adolescent psychiatry at the University of Manitoba. Dr. Katz received his medical and adult psychiatric training at the University of Manitoba and his child and adolescent psychiatry training at the Albert Einstein College of Medicine, Bronx N.Y. He is an adjunct scientist at the Manitoba Centre for Health Policy and has published numerous papers using the population health administrative database in mental health outcomes. He has held and been part of numerous grants funded by CIHR, PHAC, and other national funding agencies related to work with First Nations communities. Dr. Katz is widely published in particular in the areas of suicide and suicidal behaviour. His other research interests include Dialectical Behaviour Therapy, pharmacoepidemiology, and implementation of complex interventions. Dr Deepak Ravindran, a NHS consultant in pain medicine, reveals how we need to completely rethink our views about pain. Pain, particularly long term intractable chronic pain, can be one of the most difficult medical conditions to live with, making normal work or life almost impossible. Globally around 20% of people suffer from the condition. And getting the help they need can be incredibly difficult. Ravindran explains how many of us totally misunderstand pain. The problem can be someone with chronic pain can have a battery of tests that all come back clear, which may lead some medical professionals to suggest that there is not actually anything wrong with these patients. But that is not the case. For the first time in 2022, the World Health Organisation updated the International Classification of Diseases to include chronic pain as a separate medical condition. And as Ravindran reveals, the biological mechanism of chronic pain, which can lead to an over-sensitised immune and nervous system is not the same as acute pain. That means a very different treatment is required, using not just drugs, but taking a whole body approach which includes diet, exercise, sleep and cognitive techniques. It's a technique that has had life-changing consequences for patients, who have often suffered for years with crippling chronic pain. I'm fascinated by the intersection of digital health and behavioral healthcare. After all, if more of our lives are moving online, wouldn't our therapy trend in that direction too? Vijay helps me navigate the implications of this through the lens of Floreo, the "first behavioral therapy metaverse." It's a powerful supplement to therapy for skills that can be hard - or of high consequence - to teach neurodiverse learners: crossing the street safely and police interactions to name a couple. Abstract: Disruption of neocortical circuitry and architecture in humans causes numerous neurodevelopmental disorders. Neocortical cytoarchitecture is orchestrated by various transcription factors such as Satb2 that control target genes during strict time windows. In humans, mutations of SATB2 cause SATB2 Associated Syndrome (SAS), a multisymptomatic syndrome involving intellectual disability, speech delay, epilepsy and craniofacial defects. We show that Satb2 controls neuronal migration and axonal outgrowth by inducing the expression of the GPI-anchored protein, Sema7A. We find that heterodimerization with Sema4D increases targeting of Sema4D to the membrane and is required for Sema7A function. Finally, we report that membrane localization and pos-translational modification of the Sema7A-Sema4D complex is disrupted by a novel de novo mutation in Sema4D (Q497P) that is associated with epilepsy in humans. Ravindran (Ravi) Balaraman is our guest for this episode (23 min. long). He is the Mindtree faculty fellow and a professor in the Department of Computer Science and Engineering at the Indian Institute of Technology Madras. And he also heads the Robert Bosch Centre for Data Science and Artificial Intelligence at IIT Madras, which is the leading interdisciplinary AI research center in India and India's first lab to join the Web Science Trust Network of laboratories from around the world. His research is pushing the boundaries of reinforcement learning, social network analysis, and data text mining. Long Covid is a term that's been adopted by those who have been struggling with a variety of symptoms having had an accurate Covid episode and is currently estimated to be experienced by 10% of the population. Dr Deepak Ravindran explains what Long Covid is, the ways it is being treated and the preventative measures that can be taken. Calming and supporting the immune system means re-evaluating your diet and assessing your levels of well-being to bring balance and in this episode packed with content you can find out how to get started. KEY TAKEAWAYS People can suffer from fluctuating symptoms and fatigue is a common presentation. In the short term (3-6months) respiratory symptoms, cardiac symptoms, neurological symptoms and ENT symptoms are seen. Beyond this period fatigue, pain and psychical issues become dominant. There is a hyperactivity of the immune system and that activation seems to be something the body can't control. On most occasions, the immune system achieves an equilibrium. We'll hear from a district attorney whose allowing 40 percent of his city's prisoners to ride out the epidemic at home. A Black scholar says Black kids are kicked out of class in obscene numbers because slavery and Jim Crow are alive and well in the nation's schools. And, we'll hear how racism was behind the coup that ousted Bolivia's first Native American president. llinois Governor J.B. Pritzker has promised not to bring any more inmates into his state's prison system due to the coronavirus epidemic. But Alan Mills, executive director of the Uptown People's Law Center, says the prisons are already infected, and the state needs to free many more inmates from being trapped in a cage with the disease. Chesa Boudine, the leftish District Attorney for San Francisco, has a personal interest in dramatically reducing the US prison population. His father is a 75 year old prison inmate serving time for his role in the 1981 Brinks armored car robbery – a political heist by white radicals and members of the Black Liberation Army. District Attorney Boudine told National Public Radio's Terry Gross that he's reduced the San Francisco jail's population by 40 percent -- both to fight the Covid-19 epidemic, and because this country puts too many people in jail. Dr. Justin Coles is a professor at the Fordham University Graduate School of Education with an emphasis on Urban Education and Critical Race Studies. Dr. Coles co-authored an article on mass suspensions of Black students, a long standing phenomenon that Cole says is a relic of slavery and Jim Crow. Race was the main force behind last year's coup that overthrew Evo Morales, the elected president of Bolivia, South America's most heavily indigenous nation. That's the assessment of Dr. TaTHAgatan RaVINdran, a professor of anthropology and sociology in Colombia who has done extensive research on Bolivia's Native American majority. Dr. RaVINdran says the United States and multinational corporations also had it in for Morales, but racism is what brought Bolivia's first Native president down.
