With the Government pledging to overhaul the way food is sourced for public institutions like hospitals, schools, prisons, and army bases, Sheila Dillon explores how these changes could be implemented and why they are deemed essential by many.Sheila visits St Peter's Hospital in Chertsey, Surrey, where chef Raouf Mansour has transformed the canteen for staff and visitors. After bringing the operation in-house post-Covid, the hospital began collaborating with local suppliers to provide fresh, seasonal produce. Raouf emphasises that retraining chefs to prepare nutritious, mezze-style meals has been crucial in encouraging staff to dine at the restaurant. The hospital is also working on plans to revamp patient meals, which are all prepared off-site, by working with smaller local caterers who can better meet some of the specific needs of patients there.Following her visit, Sheila returns to the studio to discuss whether the changes in Chertsey could signal a broader trend. She is joined by:Kevin Morgan, Professor of Governance and Development at Cardiff University and author of "Serving the Public: The Good Food Revolution in Schools, Hospitals, and Prisons" Kath Dalmeny, Chief Executive of Sustain Katie Palmer from Food Sense Wales, who is working on the Welsh Veg in Schools Project Derek Wright from Blackpool Catering Services, which has expanded its school meal provision over the past five years, with on-site chefs and locally sourced produce.Presented by Sheila Dillon Produced in Bristol for BBC Audio by Natalie Donovan

In reality, it does not happen often enough - it's just inefficient assumptions on both sides.Always communicate, and don't make assumptions about "the average" participant or plan sponsor. Treat every plan sponsor individually, survey their culture, employee tenure, and what success looks like for their 401(k) plan. Look at your plan through a window, and not a mirror. Understand the specifics, don't treat it as a mirror of yourself. Select only the target date funds that are suited to your participants' risk tolerances and auto-escalation. A small difference in savings can drastically impact someone's dignity and quality of life in retirement. That's why we must ensure people have enough wealth to live retirement on their own terms after working so hard.“Your job is to look at your company and your plan through a window and not through a mirror.” - Kevin MorganConnect with Kevin Morgan:LinkedIn: https://www.linkedin.com/in/kevin-morgan-0a95bba/ Connect with Eric Dyson: Website: https://90northllc.com/Phone: 940-248-4800Email: contact@90northllc.com LinkedIn: https://www.linkedin.com/in/401kguy/ The information contained herein is general in nature and is provided solely for educational and informational purposes. It is not intended to provide a specific recommendation for any type of product or service discussed in this presentation or to provide any warranties, financial advice, or legal advice. The specific facts and circumstances of all qualified plans can vary and the information contained in this podcast may or may not apply to your individual circumstances or to your plan or client plan-specific circumstances.

Show co-host Kym McNicholas is dedicating this how to her Mom Marilyn Lou Baum McNicholas who transitioned earlier this year due to an undetected abdominal aortic aneurysm that ultimately ruptured. Marilyn's cardiologist told Kym that there's still so much doctors don't know about aneurysms and because of that, there are gaps in care, which lead to premature death. Marilyn was diagnosed with having a 4cm thoracic aneurysm six months prior to her transition. Her cardiologist would only say it's not at a size to worry about since a surgeon typically does not treat an aneurysm until it's at least 5cm. Her cardiologist never discussed how to preventthe aneurysm from growing, what symptoms to look for, or her increased risk of having an abdominal aortic aneurysm because of it. So, not further testing was done despite textbook symptoms of symptoms of increasing erratic blood pressure, tachycardia, a pulsating feeling around her belly button, an abdominal mass, and severe radiating pain in her back. Her cardiologist, a vascular surgeon, and an emergency room doctor told her there was nothing life-threatening with her symptoms. They told her to take muscle relaxers and prescribed physical therapy, along with traction. The autopsy revealed what doctors missed.    Should Marilyn's 4cm thoracic aneurysm have been taken more seriously? Should she have been tested for an abdominal aortic aneurysm after being diagnosed with a thoracic aneurysm? Should her symptoms have been a red flag for doctors? In this episode, co-host Dr. John Phillips leads the discussion with guest Cardiac Surgeon Dr. Arnaoutakis, about specifically thoracic and thoraco abdominal aneurysms. For those who aren't familiar with what an aneurysm is and how it forms, they will go through the basics. But the primary discussion will be around the current gaps in care, new innovative approaches to diagnosis and treatment, as well as critical questions to discuss with your healthcare team.  Also, Retired Veterinarian, triathlete, and author Dr. Kevin Morgan shares his personal story about how he diagnosed his own aortic aneurysm, found the right treatment, and continues to compete in running and cycling events.

How Doctors Stay on The Cutting-Edge

Kym McNicholas On Innovation

Play Episode Listen Later Jun 28, 2022 54:21

Why is it that some doctors are on the cutting-edge with new treatments while others still use the tools and techniques they learned from medical school despite advancements in medicine?  That's the topic Emmy Award-winning Co-host Kym McNicholas and Interventional Cardiologist Dr. John Phillips are tackling in this week's The Heart of Innovation show. Kym is LIVE from Italy on the heels of CLI-C, one of the most prestigious conferences in the world for treating blocked arteries in the legs, an issue which impacts one in five people over age 60, known as Peripheral Artery Disease (P.A.D.). Without proper treatment, P.A.D. can lead to amputation, stroke, and even heart attack. Only 10% of vascular specialists, whether it's a vascular surgeon, interventional radiologist, or interventional cardiologist, are trained in advanced treatment tools and techniques to save limbs for the most complex cases of P.A.D., which we see in patients who have Critical Limb Ischemia (CLI), limb threatening artery blockages.  That's because innovation around the treatment of these patients is moving at the speed of light and physicians may not be aware of the opportunities to learn what's new and next. You might wonder, “How is that possible that physicians aren't aware of what's new and next?” The answer is that it depends on where the physician decides to pursue their continuing medical education activities (CME). Physicians are required to participate in continuing medical education (CME) activities to meet requirements of state medical boards, medical specialty societies, etc. for licensing and accreditation. The purpose of requiring CME credits, is to encourage physicians to main, develop, or increase the knowledge, and skills. They can earn these credits by attending live meetings, working online, or watching course recordings on a variety of topics related to patient care, community health or public health, preventive medicine, quality assurance or improvement, risk management, and more. Physicians are not required to attend specific sessions covering cutting-edge treatments A lot of factors play into the CME activities of any physician such as facility, society guidelines for a specific practice which may either encourage or limit interest in certain treatment approaches, interest in new ideas, resistance to new ideas that may interfere with their standardized approach, family obligations, networking opportunities, and more. So, while they all may have the title of doctor, a specific specialty, fancy pedigree, work in a prestigious facility, it doesn't necessarily reflect their current knowledge and ability as compared to their peers. In this episode, Kym and Dr. Phillips use the CLI-C conference as an example of a CME activity that offers vascular specialists the ability to not only hear about new tools and techniques to treat complex artery blockages in the legs, but to see them put into practice with multiple live cases that allow attendees to interact with physicians in hospitals who are treating a patient in real-time. Dr. Mariano Palena and Dr. Marco Manzi were the leading physicians for this particular course in Padua, Italy this past week. They share the importance of coming to these events to discuss and even debate new treatment options for patients. The greatest value, they say, is in the open discussions that transpire between physicians with a variety of different perspectives and then seeing a physician during the LIVE case implement the best ideas as well as the patient outcomes. They encourage patients to use online resources such as The Way To My Heart to learn the most advanced treatment options for P.A.D., then search online to see what conferences physicians can attend to learn them, and see if their physician attends those conferences or ones like them to ensure they're on the cutting-edge. Some key questions discussed during the show, which patients can ask their physician about when it comes to continuing include: What are the most advanced tools and techniques available for my issue, even if you don't offer them? What CME activities do you attend to learn the most advanced tools and techniques? How often do you attend them? What was the last one you attended and what did you learn which you might apply in my case? Are you on the faculty of any advanced meetings or conferences and if so, which ones? Do you have any cases, studies, or publications you've presented at recent meetings or conferences? Tune into the show to learn more about why it's important for physicians to broaden their scope and attend a variety of different CME activities, as well as how a patient can determine if their physician is on the cutting edge. Also, you don't want to miss this week's Save My Piggies segment as author and triathlete Kevin Morgan shares his story which drives home the importance of advanced medical education as he was forced to self-diagnose when his physicians failed to diagnose an abdominal aneurysm that was about to rupture. His advanced research skills as a veterinarian and pathologist led him to attribute his symptoms to an aneurysm. He independently got an abdominal ultrasound and sought a second opinion using his scans to prove his own initial diagnosis. Successful treatment for the aneurysm has kept him on his feet competing as a triathlete and inspiring others through his blog at www.athletewithstent.com.

There is some exciting news to share from Clearfield, and they could not wait to spread the word: they just completed a new 100K square foot facility for warehouse and logistics space in Brooklyn Park just down the road from their headquarters. This facility will support multi-state regional telco providers and operators. To shed light on this project and how it reflects significant shifts in the fiber market, Kevin Morgan, Chief Marketing Officer at Clearfield, joined Daniel J. Litwin on this latest episode of Better Broadband. This facility fits in with Clearfield's multi-year plan to provide a platform to support the fiber deployments the industry expects over the course of the next decade. “The company's been built to simultaneously capture the market share while we're establishing a modular product line that can scale and grow with the needs of the market,” Morgan said. “Our ability to do that this year has been challenged in terms of the significant demand we saw for our product.” To meet those demands, Clearfield opened a new distribution center in Minnesota during their fiscal Q2, which effectively doubled their U.S. footprint. Providing fiber products to their over 700 providers in many under-served communities across the United States is a point of pride for Clearfield. And, as Morgan pointed out, now is a critical time for fiber-fed broadband. “We estimate the broadband providers will reach more homes with fiber in the next five years more than the combined total number of homes passed through the entire industry through 2020.” It is a time of enormous opportunity, and facilities like the one in Brooklyn Park will ensure Clearfield can meet the needs of the industry today, tomorrow, and in the future. Morgan noted partner providers were experiencing heavy demand and running into difficulties getting the product to support their needs. So, to ensure product availability for their client base, Clearfield is investing to increase its footprint and strengthen the supply chain to make these broadband initiatives a success.

Directed by future Ghostbusters head honcho Ivan Reitman, it was an animated film based on the comic book anthology magazine of the same name. Most of the scenes in the movie were based on short stories originally published in the magazine tied together in this plot revolving around the sinister Loc-Nar. Greenfields, brownfields, overbuilds and Multi-Dwelling Units all present a significant opportunity to bring more customers on net but not without a cost. When evaluating new fiber opportunities, service providers are faced with any number of deployment variables as no network build is the same. With over 70% of the cost of fiber connectivity tied up in labor, time over-runs and return trips to solve deployment challenges, not having product that alleviates that burden is a common but avoidable oversight. During this Fiber for Breakfast session, we will look at some time and cost saving techniques learned through case studies of deployments in all types of networks, through all types of terrain and to every type of “network deployment". We will outline a path to creating a Labor Light deployment strategy that can significantly reduce both the time and cost to deploy.

While broadband fiber may not be something the average consumer thinks much about, it has a great impact on all of our day-to-day lives. Unfortunately, high-speed internet is something many of us take for granted. No one knows this more than Kevin Morgan, Chief Marketing Officer for Clearfield. Host Shelby Skrhak had the opportunity to pick Morgan's brain on a “state of the union” of sorts for fiber deployment across the country and, more specifically, rural broadband fiber. For almost a decade, Morgan has been a seasoned advocate for broadband infrastructure and initiatives. While Morgan has always been thrilled to work at Clearfield, where he focuses on removing the barriers to fiber deployment, he feels that the tragic events of 2020 really showed the importance of the problems he is trying to solve.“This year in particular, in 2020, has been the most bizarre, challenging year in history in terms of communications. The entire economy is shut down, we came back, we've been in quarantine, and, all along the way, we've seen evidence of what a broadband connection really means to people in their homes and their businesses,” Morgan said.He went on to explain that the broadband used today is much different than it was even three years ago.“We were anticipating that this demand, this amount of broadband that was needed by the public, was going to come over the period of a number of years. When March hit in 2020, all of that demand accelerated right up to the present time,” Morgan said.He explained that, because so many people utilized broadband connections to work, for distance learning and to receive medical care via telehealth, 2020 really illustrated the importance of breaking down broadband barriers.“The ability to be diagnosed and talk to your doctor remotely over a video link, something that was far-fetched just a few years ago is reality today. So, these things make a huge difference, and the role that fiber optics plays in providing those broadband connections is undeniably the best of all the types of technologies out there,” Morgan said. For deeper insight into the current state and future of fiber-fed broadband, tune in to the latest episode of Better Broadband.

You must play the hand you are dealt. This has advantages and disadvantages as one might imagine as compared to the civilian sector. Kevin reflects on his time at Coast Station Golden Gate located in Sausalito, CA, and the dynamic strengths of many who served with Kevin and worked for Kevin. He explains how much he loved his job in San Francisco and how that kept him at work for long hours by choice; not by force. This was a fun interview. In this interview, Kevin’s views, ideas, and opinions on life and leadership are his and are not supported nor endorsed by the U.S. Coast Guard.

The 365 Days of Astronomy, the daily podcast of the International Year of Astronomy 2009

The Discussion: Our live recording of our monthly astronomy show to provide a bit of extra entertainment and interactivity while people are cooped up at home sitting out the coronavirus.   We discuss a burgeoning love-hate relationship with Starlink, Jeni being the BBC’s go to person for Starlink and meteor showers, and Apollo 13 filling up Twitter timelines and giving us a bit of a respite from coronavirus.   The News: Rounding up the astronomy news this month we have: - A round up of astronomy-based April fools gags found in research papers. - Hubble marks its 30th birthday. - Bye bye Comet Atlas. - Fomalhaut b might not be a planet after all. - Centaurs might well be asteroids from other star systems. And... - Pluto looks to have had a ‘hot start’   Main News story: Earth 2.0 found in old ignored data.   The Sky Guide: This month we’re taking a look at the constellation of Ursa Major with a guide to its history, how to find it, a couple of deep sky objects and a round up of the solar system views on offer in May.   A guide to the electromagnetic spectrum: In this series we take a look at the electromagnetic spectrum, what, it is, what is shows us and why it’s so important to astronomers. This month we explain the sub-millimetre and far infrared part of the spectrum and its relevance to astronomy.   Q&A: How do scientists work out the trajectories for putting satellites in orbit around other bodies, or on trajectories that take them past numerous objects? From our good friend Kevin Morgan in the UK.   http://www.awesomeastronomy.com   Bio: Awesome Astronomy is a podcast beamed direct from an underground bunker on Mars to promote science, space and astronomy (and enslave Earth if all goes well).   We've added a new way to donate to 365 Days of Astronomy to support editing, hosting, and production costs. In February 2017, the Cabinet Secretary for Communities and Children announced that the Communities First Programme would be phased out by March 2018. In the announcement he conceded that whilst “…Communities First has been successful in its impact on many individuals in poverty, …it has not had an impact on overall poverty levels in Wales, which remain stubbornly high.” While some legacy funding has been agreed, the absence of any other plans to replace the Communities First Programme has led some to question if the Welsh Government has given up on tackling poverty altogether. Dr Mark Porter finds out why insomnia can often go untreated by the NHS despite there being a treatment that not only works but also doesn't involve drugs. There are nearly 11 million prescriptions for sleeping tablets in the UK every year but their effect isn't long lasting and people can find it hard to come off the tablets. Cognitive behavioural therapy has consistently been shown to be very effective at improving sleep in the long term but few people have access to it. Mark is joined by Colin Espie, professor of Sleep Medicine at the University of Oxford, and by professor Kevin Morgan, director of the Clinical Sleep Research Unit at Loughborough University, to discuss why insomnia is so neglected, and to talk about the success of methods to deliver CBT online using mobile and web technology. Also in the programme, Mark talks to Peter Goadsby, professor of neurology at King's College Hospital London, to find out what cluster headaches are, why they're so painful and why they can occur when the clocks change. The NBN scam -already an abject failure

Is there anything the Gillard government cannot handle competently? That socialist monopoly, the NBN, was born on the back of an envelope when communications minister Stephen Conroy managed to obtain an audience with Kevin Rudd on the prime ministerial jet . There is no cost benefit study and the billions - over $40 billion - are all off budget. They could pour $100 billion into the NBN and it wouldn't appear in the budget. It would have no effect on the surplus or deficit. What an appalling scam. The initial NBN plan was released in December 2010 by Julia Gillard. Kevin Morgan, who was the ACTU member Kim Beazley's advisory committee on telecommunications, says that plan is now in tatters. At the time, corporate advisers Greenhill Caliburn reported that “that, taken as a whole, the corporate plan for the development of the NBN is reasonable." That report cost $1.1 million. But the NBN has achieved only 9 per cent of its rollout target for homes passed by fibre and 3 per cent of the planned connections where customers are hooked up to broadband. Based on its initial estimates, by June this year 317,000 households should have been passed with fibre and 137,000 homes actually connected to a broadband service. Writing in The Australian on 7 August 2012 Kevin Morgan points out that fewer than 25,000 homes had been passed and fewer than 4000 connected. He says this is not just a debacle. Its an abject failure by NBN Co, especially in new (greenfield) housing estates. Lessr than 20 months after predicting that 172,000 greenfield premises would be passed and 132,000 connected, 0.6 per cent of the coverage target and less than 0.2 per cent of the active service target have been met. Kevin Morgan says many homeowners will have a long wait for new telephone services. But if NBN Co is failing dismally on delivering the fibre promised to 93 per cent of homes, he says it is at least making progress on its wireless and satellite services for the other 7 per cent. That's because NBN Co has little active involvement in building either. These have been fully contracted out, albeit at considerable cost. The average capital cost of the wireless and satellite services will be about $14,000. Given each service will yield less than $300 a year in revenues; these rural services will require ongoing annual subsidies of at least $3000 a year per service. In contrast, Optus and Telstra are building far faster new-generation mobile networks at a cost in the hundreds of dollars, not thousands, for each customer. To repeat, is there anything the Gillard government cannot handle competently?
