In this healthcare podcast, I speak with Peter Hayes, who is president and CEO at the Healthcare Purchaser Alliance of Maine and a national presence in healthcare strategy, innovation, and a frequent keynote speaker. One thing, among many, that Peter said during our conversation struck me. He said it will take a village to fix what ails the healthcare industry in this country. There are too many interdependencies. This point obviously resonates around these parts because it's the rationale for the Relentless Health Value podcast. We started this show on the recognition that if you want to achieve anything in healthcare, you cannot do it without collaboration/cooperation/grudging acquiescence of other stakeholders in the patient journey or the payment journey. And when I say, “You can't do anything,” I mean you can't sell anything, you can't improve patient care, and, most relevant to this particular episode, you can't contain prices. If we're talking about health systems (for example, hospitals and the like), they are not going to curtail their price hikes or improve the value of care delivered or safety or infection control really unless patients and employers and CMS and others demand that they do—and unless employers and others do some of the five things that Peter Hayes mentions at the end of our conversation. Spoiler alert there. For context to this discussion, let's check in with some of the biggest, most powerful health systems in this country. If I limit this comment to the “nonprofit” ones—and I say “nonprofit” with air quotes because what does that mean exactly?—look, I know there are many health system execs that listen to this show, but there's some inalienable facts here. And let's talk about them with the intent of fixing them because nothing is going to get fixed that isn't talked about. It's not my nature to mince words, so I won't. Many hospitals are, by almost every account, pretty darn inefficient. And they don't do cost accounting, but then they'll scream and claim to be losing money when paid the exact same prices for certain services that other hospitals can get paid and make a fair profit. Crappy workflows cost money. Talk to anybody who has watched even the trailer to a Six Sigma course. Another thing that costs money is when all the burned-out doctors quit and you have to recruit new ones, but that's a topic for a different day. Listen the EP323 with Arshad Rahim, MD. But there's also inefficiencies in how many health systems purchase supplies. (Listen to EP281 with Rob Austin for more on that.) Further, paying the C-suite millions of dollars but maybe underpaying or understaffing nurses has consequences. There's complaints about Medicare payer mixes, but then somehow there's enough spare shekel to put a waterfall in the lobby. Nonprofit hospitals also don't pay any taxes, keep in mind, which is a huge financial windfall, especially when they provide vanishingly small amounts of charity care compared to revenue. Here's another point to ponder: Amongst the hundreds, thousands, of requests I get from PR firms pitching guests to come on this show, there are plenty from what appears to be a pretty large cottage industry that I had never heard of before. I'll call it the real estate for nonprofit hospitals cottage industry. From what I can gather by the promo copy, this involves buying up medical office buildings, not paying any real estate taxes, and then leasing out the space. I should have one of these guys come on the show just to shine some light on whatever this apparently pretty common shenanigan is. As Vikas Saini, MD, from the Lown Institute has said, “No margin, no mission” can become an excuse for all kinds of questionable behavior. So bottom line, we have employers, employees, taxpayers, cash-pay patients whose federal and/or state and/or local taxes are going to support these nonprofit hospitals—but then there's this double tax. Because they claim to be losing money on Medicare patients, they justify cost shifting some pretty big bucks onto the commercially insured patients, who are then paying, on average, some wildly inflated prices for healthcare services. This might be considered a double tax if you think about it: tax dollars going to the IRS directly and then after-tax dollars buying that knee replacement for $125,000 that should cost $25,000. Consider that a $100,000 double tax. But why should a hospital with a motive to maximize margins quit it with their questionable and secretive billing practices if employers just pay whatever the bill is no fuss no muss? Short answer: They won't. So, it's going to be up to someone else in the village to make it untenable to continue. It's going to be up to another party to slow that roll. In this conversation, Peter Hayes talks about the RAND Hospital Price Transparency Study. One last thing that may or may not be relevant here, but I can't resist a good sidebar. New catchphrase I have been hearing lately: the “deconstruction of hospitals.” Have you heard it, too? In fact, I was listening to Zeev Neuwirth's podcast recently that featured Raphael Rakowski. Raphael said that the average fixed cost of any given brick-and-mortar hospital is 65% of revenue. So, just having the building, the physical plant, and paying for all the things you need to pay for to run that physical plant is really high. I heard Jason Wells say in a HealthIMPACT forum the other day that it costs a million dollars to build a bed in California due to all the regulatory requirements. Add to that something Christin Deacon highlighted the other day on LinkedIn about how operating rooms are empty 30% of the time. So, it makes me wonder whether some of the issues that hospitals have when they claim that they are losing money on Medicaid or Medicare is because their fixed costs are out of whack. This potentially disproportionate situation, however, is one reason why hospitals really have to watch it for hospitals at home or virtual offerings. After all, this is exactly how Amazon ate everybody's lunch. Erase 65% of your costs, or even 50% of your costs, and that cost-plus profit threshold becomes a weapon of mass destruction. At the end of this podcast—the very end, so if you're in a rush, jump to 28 minutes or something [32:45]—Peter gives five ideas for employers to limit the ability for hospitals to take advantage. If you're a hospital exec that's listening, I would urge you to please help your local employers do these things. Let's all get on the same team here to improve the health of our communities with pricing and business models that are reasonable and fair. December 9, 2020 — The Board of Supervisors spent the whole morning yesterday on the novel coronavirus update, with reports from Dr. Coren and Dr. Doohan, high-level hospital staff, and the CEO of Redwood Community Services, who reported on the outbreak at Building Bridges, the homeless shelter and day program in Ukiah. Yesterday, 37 new cases were reported countywide, and the day before, another covid patient died on the South Coast, bringing the death count up to 24. Jason Wells, the president of Adventist Health in Mendocino County, assured the board that, with cross-trained staff and ventilators, the hospital could bring its current sixteen ICU beds up to 45. The state is only counting sixteen beds in its calculations of the county’s contribution to regional ICU capacity. Currently, 10% of the hospital’s workforce is out on medical leave, but Wells is confident that the 23-hospital system could rely on highly trained National Guard personnel if the surge demanded it. The best case scenario is that vaccines will arrive in the county in a week or two. Adventist Health Medical Officer Dr. Bessant Parker told the board that healthcare personnel caring for covid patients in the emergency department would be first in line for the new vaccine, followed by ICU staff, then those on the regular medical and surgical wards. Coren said the county has purchased one deep freezer that will be suitable for storing the Pfizer vaccine, but that there may be a delay in the delivery. In the meantime, he said the hospital is willing to share its refrigeration capacity with the county. Victoria Kelly, the CEO of Redwood Community Services, reported that staff and clients of Building Bridges are in quarantine or isolation. Eight clients who tested positive are isolating hotel rooms. Everyone else who was on the premises when the outbreak started has been determined to be a close contact, and has been quarantining on site since Thursday. Public health conducted PCR tests yesterday, and testing is being carried out twice a week until everyone tests negative. RCS and the Adventist Health Street Medicine team are still offering services, and Plowshares has been providing some of the meals. None of the staff has tested positive at this time. In an off-agenda item, Supervisor John McCowen and Supervisor Ted Williams brought forward a resolution to prioritize available county resources to assure effective enforcement of local health orders. Williams cited Facebook complaints about large gatherings, and McCowen relayed reports of people working out inside gyms. John Martire, the head of the Special Investigations Unit, said his office had gotten 23 calls for service in the month of November. He added that he regularly hears about violations days after the fact, and insisted that his office does prioritize complaints about large gatherings. The board also adopted a hazard mitigation plan for other disasters, which must be updated and approved every five years in order to receive FEMA hazard mitigation grants. The county, the four cities, and the Mendocino Office of Education participated in the plan, which identified the top hazards facing the area, and pointed the way to a variety of grant programs to preemptively reduce damage, largely by investing in infrastructure. Wildfire ranked highest on the risk matrix, squarely in the middle of the highly likely probability with catastrophic impact, followed closely by earthquake. The plan lays out mitigation actions, priorities, which agency is responsible, and whether the actions can be covered by the existing budget or if other sources of revenue should be sought out. The board also agreed unanimously to adopt an urgency ordinance to clean up fire debris and remove hazard trees from private properties that were damaged during the August Complex and Oak fires. Thirteen owners of damaged properties have signed right of entry agreements with a state-funded debris removal program, three have made private arrangements, and 14 have taken no action. The ordinance allows the county to declare those properties a nuisance and a health hazard and to abate the nuisance. The ordinance will appear on the consent calendar to be formally adopted next week, after county counsel fine tunes some language about the definition of hazard trees on private property.

Jason Wells is a guitar phenom who has been taking his music around the country for several years now. He's brought all that experience, along with three other superb musicians to his new release "Nine Hours". Wells intended for this album to sound like a spontaneous jam feel, and he hit the target. Varied in style, but never lacking intensity, 9 Hours is a piece of work the author can be proud of. We discussed the album-making experience and got a sense of Jason's intentions for the album. Episode 003 of Inside the Law features the second part of our interview with former United States Secret Service agent, Jason Wells. The book outlines how citizens, school and businesses can identify and assess threat-related behaviors The book was inspired by the firsthand experience of a close friend in the Washington Navy Yard shooting, in September, 2013. The attacker, a 34-year-old gunman named Aaron Alexis, entered the facility in the morning and began randomly shooting, with the intent to kill as many innocent people as possible. Jason Wells is also the President and Founder of The National Advancements for Proactive Safety (NAPS) , an educational non-profit organization committed to providing a safe community through intervention processes. Jason wrote the foreword for Protecting the President , written by former U.S. Secret Service agent and two-time New York Times bestselling author, Dan Bongino.

Rick is sponsoring the main stage at the Music Matters Festival in Darlington, IN on August 5-6 (details here). The fest features 11 bands over the two nights, some of whom have been on this podcast, and some we don't know. It's the brainchild of guitarist/songwriter/band leader Jason Wells, who is also the dad of Gibson Wells, a spectacular teen guitarist, and his not-to-miss band The Edgars. We also touched on some tips for putting your band's name out there, from social media to the proper press kit contents. What a night! The GLMLive Songwriting Contest was a huge success. Curt Yeoman's awesome Lafayette venue hosted 12 songwriters and their friends for a night of good original music. A tough one to judge for John Bower, Nicky Rood, and Jason Wells. Song subjects varied from walkin' blues, faith, death, and Pokemon Go (that's right: songwriter Cheryl Fletcher went there).The third place winner, Summer, is 16. The first place winner, Jay Thoennes, is 15! Makes you feel good about music's future. Second place finisher Jessica Collasto, a promising writer with an angelic voice, is resurrecting her music interest after some time out for parenting. Our good friends Joe Peters, Kenn Clark, and Brady Joseph brought good songs and their top-notch talents to the scene. A really good night, musicians, audience and venue. As our winner's tunes were mostly hot out of the oven, we'll wait to bring them to you once their published and recorded. So this time we conclude with an instrumental piece by me.

I've seen a lot of bands form, build up, chug on, burn out, fade out, and blow up (in the bad way) over the years. My favorites, of course, are the first two. Which is where this week's guest, Jason Wells falls. When I first had him on, it was promoting a few demos that he turned into his first album. While the music, I thought, was solid, the recordings were still lacking the depth and interplay that except in very rare instances, you only get with a full band. This is a guy you must go see play live to fully appreciate the depth of his talent and ability. To some extent, I think this comes across in the quick set he played live for the radio audience here in Studio B -- and now available for streaming on our -- even though this was only as a duo with guitarist J.T. Kidwell. As with most three-time guests, the interview process pretty much falls apart and becomes more a stream-of-conscious catching up between friends over a couple of beers. Still, we get to hear about the first fruits of his marketing efforts, as well as what didn't work out so well, and the popular "Random Odd-Ball Questions" segment, we get a bit of an insight into his influences and how his mind works. We're back! Did you miss us? It's been a pretty busy summer hiatus for us. Excellent, new day job (FINALLY), couple of new projects in the works (more on those later), and just getting lined up for the kickoff of the second century of Indy In-Tune shows, featuring some big names, some great underground talent, and the return of a few of old friends. Speaking of old friends, we're going to kick things off by revisiting a fan favorite, Mr. Jason Wells, who first appeared on the show about a year ago. Since then, Jason has built his home studio, completed a full length CD called "Stepping Out," and written a season of music for a television series -- and here I thought I was getting a lot done. Jason brings us some insights into independent music marketing, shares some stories about what's been happening with his music over the past year, and treats us to four new songs off his latest project, one of which is a new version of a track that will be familiar to Indy In-Tune listeners. You know, I've done a lot of these little intro/blog posts. So many that I've forgotten what I've said in half of them. Like the one that I wrote here, but just erased for because it was surprisingly similar to, and even borrowed a couple of jokes from, a post I made last year for a different band. So, sorry, Mike. There will be no contrived story or calculated essay loosely segued back into a description of you and your music. No carefully crafted joke that will probably fall flat anyway. No long self-indulgent story that people will merely skim over because I got too personal and deviated from the requisite 450-word limit the "best practices bloggers" unconsciously place on these things. See? I'm learning from you already. Some artists are storytellers, spinning legends out of characters and situations they've created. Other artists, are legends and stories unto themselves. In that analogy, I like to think of Junk Box Mike more as a portrait painter. The body of his work focuses around people, places, and themes in his life (by themes, I mean fishing) whose broad strokes are something we can all identify with. They seem real to us, because they are real to him. Sure, Bob Seger can do an entire album about laboring thanklessly for the man, but we know a day of work for him largely revolves around planning his trip to the mailbox to pick up his royalty check. When Junk Box Mike performs, you can be assured that, were you to walk up to him after the show and buy him a beer, you'd meet exactly the same guy you pictured in your head while you listened to his songs. Thoroughly confused by the numbering scheme of the episodes yet? Okay, it's about to get more confusing. This episode marks the second part of our two-part foray into the Frankfort Music Scene. That may sound like a joke, but they really do have some excellent players in that town, as you can hear. I only mention this because it references an episode featuring Brian Pitzer (#086), which won't be released for another three or four days due to some logistical problems, and I didn't want anybody thinking they might have missed one in there somewhere. Once we get that done, we only need Outer Sanctum to release their new CD so we can put out Show #081 and everything will be back to abnormal. While the downturn in the economy has caused us to lose no less than a dozen of my favorite live music venues here in Indianapolis, it has on the other hand, created some interesting opportunities for great players to become a little more serious about their art. Jason Wells is an excellent example of this, having turned the loss of a job and stable income into an opportunity to increase his musical production and live performances, as well as to branch into commercial recording as well, where his laid back, bluesy, percussive style of playing is finding a whole new group of fans, and a new home in very unlikely places. This just goes to show you that, no matter how bad things get, good music will survive.
