Hey, hey, you guys! About 15%-25% of young children have some kind of communication disorder. Research has also found that approximately 13 percent of two-year-olds are late talkers. A Sacred Heart University professor just received a grant to do research on language and speech delay for toddlers. This week's guest is one of my absolute favourites Anna Dalziel, RSLP also known as Anna Dee SLP! Anna works primarily with 3-6 year olds with severe speech and language delays. Anna loves play based therapy, treating speech sound disorders, and working closely with families and school staff. We discuss late talkers, what SLPs assess other than word size in late talkers, pre-verbal skills, strategies to help late talkers and other kiddos in need of some SLP supports, how to differentiate between Autism and a late talker, and why Anna loves play-based therapy. We also talk about play milestones and how symbolic play relates to language development. Note: Two things we want to emphasize is (1) make sure to always get a hearing test in addition to an SLP assessment and (2) most late talkers still have difficulties by age five and therefore they still require SLP intervention. Research has shown us that Late Talkers (LT) follow one of these paths:- Between 20–30% of LT do not grow out of their language delay. These children have ongoing difficulties and need intervention to help them with their language and literacy skills. They may receive a diagnosis of language disorder.- Between 70–80% of LT seem to catch up to their peers by the time they enter school. Sometimes these children are called “late bloomers” because they eventually seem to catch up to other children their age. When their “general language skills” are tested, they score in the average range, which might tell us not to worry anymore. But research tells us that these children still have difficulties that we should not ignore.- LT who seem to catch up continue to have some weaknesses- LT who seem to “grow out of it” are working at a disadvantage when it comes to developing later language and literacy skills- While these differences might seem small, it means that LT who seem to “grow out of it” are working at a disadvantage when it comes to developing later language skills. “Late bloomers” who seem to catch up actually show weaknesses in these areas:- Some language & literacy skills – these include vocabulary, grammar, phonology, reading, understanding and creating stories, writing, and listening comprehension. These subtle weaknesses can continue through adolescence- Other skills that are related to language – these are skills that rely on language, such as social skills, behaviour, and executive function skills- How the brain processes speech – researchers have studied 3 to 5-year olds who had a history of late talking, and noticed that they don’t process the speech they hear as easily as other children do. Im Rahmen der Früherfassung müssen bei 24 Monate alten sprachlichen Risikokindern, den so genannten Late Talkers, neben den produktiven auch sprachrezeptive Fähigkeiten überprüft werden. Wenn zusätzliche rezeptive Verzögerungen vorhanden sind, ist dies ein negativer Prädiktor für die weitere Sprachentwicklung. Als standardisiertes und normiertes Verfahren zur frühen Untersuchung des Sprachverständnisses stehen im deutschsprachigen Raum Untertests des „Sprachentwicklungstests für zweijährige Kinder“ - SETK-2 (Grimm et al., 2000) zur Verfügung. Die Diagnostik sprachlicher Leistungen bei zweijährigen Kindern ist häufig problematisch, da in diesem Alter Testsituationen teilweise schwer herstellbar sind. Zudem ist die standardisierte Untersuchung mit dem SETK-2 nicht ausreichend, um eine umfassende qualitative Diagnose zu stellen. Diese soll den komplexen Sprachverstehensprozess in diesem jungen Alter abbilden sowie die Ableitung von Interventionszielen ermöglichen. Ziel dieser Arbeit war, zwei Möglichkeiten der Diagnostik des frühen Sprachverständnisses zu erproben. In einer ersten explorativen Untersuchung wurden bei 13 zweijährigen spät sprechenden Kindern Eltern-Kind-Spielinteraktionen systematisch analysiert. Dies geschah über eine videogestützte Verhaltensbeobachtung anhand festgelegter Kriterien zu deren Sprachverständnisleistungen. Durch einen Vergleich von Kindern, deren Leistungen im Untertest I „Verstehen Wörter“ des SETK-2 im unteren Normbereich lagen mit Kindern oberhalb des Normbereichs, konnte gezeigt werden, dass Sprachverständnisauffälligkeiten in einer Interaktionsbeobachtung tendenziell erfassbar sind. Die Ergebnisse des Vergleichs wiesen auf tendenzielle Unterschiede in einigen Beobachtungskriterien hin, z.B. im durchschnittlich häufigeren Einsatz von Aufmerksamkeits- und Reaktionssicherungsstrategien der Mütter oder in den häufigeren Ja-Antworten als auffällige Reaktionen auf Seiten der Kinder. In einer zweiten Untersuchung wurden Aufgaben zur Objektmanipulation entwickelt und an drei typisch entwickelten zweijährigen Kindern erprobt. Die Leistungen in diesem Verfahren wurden mit denen im SETK-2 verglichen. Für den Untertest II „Verstehen Sätze“ des SETK-2 zeigte sich eine hohe Korrelation mit den Ergebnissen des entwickelten Verfahrens. Die Ergebnisse dieser Arbeit unterstreichen die Komplexität der frühen Sprachverständnisdiagnostik und die Bedeutung der Kombination von Methoden und Verfahren. Bei beiden Untersuchungen wurde deutlich, dass für eine umfassende qualitative Sprachverständnisdiagnostik eine individuelle Analyse zur Ergänzung der standardisierten Diagnostik erforderlich ist.

Ziel der Studie war es, Aufschluss über die Prognose von sprachlich verzögerten Kindern im Alter von 24 Monaten (sog. Late Talkers) zu erhalten und Prädiktoren der sprachlichen Entwicklung zu identifizieren. Damit soll eine bessere Früherkennung von Kindern mit persistierenden Störungen der Sprachentwicklung ermöglicht und diese von sprachlichen Normvarianten unterschieden werden. Insgesamt 135 Kinder mit unterschiedlichen sprachlichen Fähigkeiten (58 Late Talkers, 46 Kontrollkinder sowie 31 sprachlichen Grenzfälle) wurden im Alter von 25 Monaten untersucht. 120 dieser Kinder konnten mit 37 Monaten erneut getestet werden. Zusätzlich erfolgte eine neurophysiologische Untersuchung mit einem MMN-Paradigma an einer Teilstichprobe von 28 Late Talkers und 26 Kontrollkindern, um auditiv sensorische Gedächtnisprozesse über eine Verlängerung der Interstimulusintervalle zu erfassen. Die Auswertungen zeigen, dass sich Late Talkers von Kontrollkindern nicht nur in sprachlichen, sondern auch in nichtsprachlichen und psychosozialen Dimensionen unterscheiden. Ein Drittel der Late Talkers zeigt mit drei Jahren eine völlig unauffällige Sprachentwicklung, während bei je einem weiteren Drittel noch sprachliche Schwächen bzw. eine spezifische Störung der Sprachentwicklung vorliegen. Schlechtere Leistungen im Sprachverständnis, eine geringere Schulbildung der Eltern, ein altersgerechter, aber niedrigerer nonverbaler Entwicklungsstand sowie externalisierende Verhaltensauffälligkeiten stehen in Zusammenhang mit einer schlechteren Prognose. Beim Vorliegen dieser Faktoren bei Late Talkers sollte eine frühe Förderung unbedingt angeregt werden, da die Wahrscheinlichkeit des Aufholens eher gering ist. Die Ergebnisse der neurophysiologischen Untersuchung könnten auf schneller verblassende Gedächtnisspuren für auditive Information bei den sprachlich verzögerten Kindern hindeuten und somit die Hypothese gestörter auditiver Gedächtnisfähigkeiten als pathogenetischen Faktor bestätigen. Außerdem scheinen spätere Verarbeitungsprozesse für auditive Informationen bei den Late Talkers auffällig. Für eine Früherkennung und Prognose im Einzelfall sind neurophysiologische Daten derzeit allerdings noch nicht nutzbar.
