Dr. Jonathan Harned, PT, DPT, earned his Bachelor of Science in Health Sciences and Doctorate of Physical Therapy from Lebanon Valley College in Annville, Pennsylvania. There he played Division III baseball for four years. Jonathan has had the unique opportunity to complete clinical rotations all across the country including with the Cleveland Indians, a professional baseball organization in the MLB. While with the Indians, he was able to work with other medical professionals in designing and implementing rehab and injury prevention programs for professional baseball players. In addition, Jonathan has obtained valuable experiences in outpatient orthopedics and the acute care setting. Jonathan holds certifications in Personalized Blood Flow Restriction through Owens Recovery Science, Suspension Training through TRX, and has his Emergency Medical Response for the Athlete (ERA) certification. In addition, he is a Certified Strength and Conditioning Specialist (CSCS) through the National Strength and Conditioning Association and a Functional Range Conditioning Master Specialist. Kyle Kimbrell, PT, MPT joins Kory Zimney and JJ Thomas to discuss using Blood Flow Restriction (BFR) in the clinic. Kimbrell was first exposed to BFR through research and Johnny Owens' twitter and now works as an instructor for Owens Recovery Science in the West Coast Division. Here are some of the highlights: The clinical reasoning behind using BFR with the right patients comes down to a better understanding of the muscle and how it responds to load. One of the best cases for BFR are people that have an osteoarthritic knee. You can make the quadricep work hard and the knee doesn't have to  Getting your patients discharged quickly doesn't mean that you were successful with your patient. Many patients need capacity building and biology building takes time, and BFR helps with building that biology. In building muscles, load isn't as important as making the muscle tired. Convincing your patients that are scared of BFR can be as easy as just telling them that the pressure can be tight, but it will only be for a few seconds. Then, you can underload and give them the highest pressure just to let them see how the muscle feels and be familiar with it. BFR can reduce the time under tension, because it makes your muscles fatigue faster. When your patients starts counting all by themselves, you have the right load for your patient. Even the brands/model  ....safety bar, ramps, hex bar, etc. Answer: To improve movement options, you don't really need a whole lot of equipment. You just have to ensure that you can challenge your body to get into positions it normally struggles with. The hope is then that you are crushing life once that happens. That said, the right piece of equipment can help quite a bit at enhancing both your movement and fitness. Here is a list of stuff I would look into, depending on what your budget is: The home gym essentials Heel Ramp - These are essential to improving your squat depth, which is a critical motion to improve your movement options throughout the body. A heel ramp makes it easier to vertically displace the pelvis. I like the one that my boy Levi Kirkpatrick makes get large. Olympic lifting shoes can also be an alternative, to which The Nike Romaleos are the best. If you want to save some buck, use books PVC pipe - These are useful to do supported lower body activities and can incorporate rotation through the trunk. Dowel rods work great as well. Kettlebells - These are quite versatile and offer way more loading options than dumbbells. They are much easier to hold for squats as well. Power blocks - These are adjustable dumbbells that can save you space. Superbands - these are great bands that provide versatility in loading for pulldowns and chops. Cook bands - These bands are great for jumping, chops and lifts, unloading exercises. They rock! Airex pads - These make any kneeling or inversion-based activities go WAY better. BFR cuffs - These can help provide a great training effect at a fraction of the workload. My favorite brands are Occlusion Cuff, Edge, and Owens Recovery Science. Weighted vest - When you get too strong for bodyweight stuff, this is a must. Wooden plyo box - I love these for step-ups and more! Gym equipment for a bigger space and a larger budget Adjustable bench - Having decline capabilities is a must, as the slight inversion is great for improving your movement options. Trap bar - A must. It's a great middle-ground between a squat and hinge. Power rack - If you are going to be lifting heavy weights, this is a must. If you get a retractable wall mount version, it'll save you space. DC blocks - These are useful for adjusting the height of your pulls but are also great for grading step-ups, jumps, and everything. Barbells - A classic to get jacked. Slideboard - A great piece of equipment for improving frontal plane mobility, pronation, and gives you tons of variety. You can do hamstring curls, body saws, the possibilities are endless. Sled - Use for pushes, drags, pulls, all types of awesome stuff. TRX - The amount of variety you can get to do with bodyweight stuff is endless with this piece of equipment. Plyo boxes - You want something with a hard surface to land on and is adjustable. If you are doing any jumping, this is a must. Mini hurdles - Once you've nailed box jumps, hurdle jumps are the next logical progression. Adjustable cable system - You want something that you can move in multiple directions. Glute ham raise - Great for nordic hamstrings and side hangs. Specialty bars - I don't use often because you can accomplish most things with the regular stuff. If I had my choice, the spider bar, safety bar, and neutral press bar would be my top choices. The transformer bar also sounds appealing but I haven't played with it. Raptor - Great for resisted sprints, backpedals, and more. The best part is your hands can stay free and move! The best cardio machines that also improve your movement assault bike - I love it because you can alternate pushing, pulling, and rotation! Elliptical - Another great way to drive rotation. Versaclimber - One of the best options to drive frontal plane conditioning. Your ribcage is going to be opened up like cray cray. Jacobs ladder - Resisted crawling for conditioning? Drivinng all types of thorax rotation? I'm sold. Treadmill - Although the free-running treadmills are dope, I like the classic ones to drive high incline walks and to run on when they are turned off. If you got the money and space Buy machines. Straight bodybuilder stuff. I love these for the following populations: geriatrics, obese, deconditioned, post-surgical, and variety with peeps who do a lot of free weight work. It's just fun.

Pam Bartlo is a clinical associate professor at D'Youville College, Vice President of the Cardiopulm Section of APTA, and a Cardio pulmonary PT at Mount St Mary's Hospital. We take a look at an interdisciplinary study she did with 3 registered dietitians about online nutrition information for people with type 2 diabetes. They looked at the quality of websites providing nutrition information for people with diabetes and then the accuracy of that information. Here's what they found: * Overall there weren't a lot of websites that came up with diabetes nutrition search terms - 34 sites total to analyze.* 85% of the sites analyzed were quality websites based on Healthy People 2020 quality website crtieria.* None of the sites had >75% accuracy of the nutrition info provided based on the American Diabetes Association's Medical Nutrition Therapy Guidelines. Why does this matter to Physical Therapy? As PTs, especially in outpatient or cardiac care, we should be providing at least basic nutrition information to patients with diabetes. It is important to know whether we can rely on websites to help augment or complete that education. Based on these results, Professional Organization websites could help augment that nutrition education, but will not be accurate enough for full education. Therefore, we should work with our Registered Dietician or licensed Nutritionist colleagues to complete nutrition for individuals with diabetes. * Many people look online for information about nutrition and it may not be accurate.* Professional organization websites were the most accurate.* It is most important for patients to have in person consultations for nutrition education for diabetes. These quality ratings are used to quickly guide users to trials that are more likely to be valid and to contain sufficient information to guide clinical practice. These trials were nominated by PEDro users, and an independent panel of international trialists judged the nominations received. It’s not about clothes or looking good… thank goodness or I’d be WELL out of my element, but this Fashion UK trial’s title was: Hip arthroscopy versus best conservative care for the treatment of femoroacetabular impingement syndrome (UK FASHIoN): a multicentre randomised controlled trial - if you had scrabble letters you’d find FASHION UK in there somewhere and let’s be honest, FASHIO UK was easier to remember than…. Wait what was that title again? Heads 'N Tales Podcast - Talking Sports Injury Rehab, Prevention, Perseverance, Concussions & Athlete Transition

Rebecca Schumer Lee is a Sports Physical Therapist at MedStar National Rehabilitation Network and was a dual sport (soccer and softball) division-1 athlete at The George Washington University. Currently, Rebecca primarily works with athletes of all levels and patients with orthopedic injuries and/or post-surgical recovery at the US Lacrosse Headquarters in Sparks, Maryland.  Rebecca reached out to me after coming across our Instagram account. She was understandably intrigued by our podcast after having 23 surgeries of her own throughout her athletic career.  Rebecca believes her injury-proneness stemmed from the natural hyper-mobility in her joints. During our interview we learn about how although mobility and range of motion are important for both performance and in reducing the risk of injury, but joint stability is also important.  Additionally, Rebecca feels her competitive drive and not knowing when to stop was also a contributing factor of her high injury rate.  I can certainly relate to Rebecca on that one!  Rebecca wishes she knew more about the importance of active recovery during her athletic career, which is a topic she touches on more than one occasion throughout the episode.  Sixteen of the 23 surgeries were on her left foot after suffering a Lisfranc injury on October 6th, 2004 during her Junior year soccer campaign at GW.  Rebecca remembers this day vividly and recalls her athletic trainer encouraging her to find a hobby, knowing her injury would required surgery. Easier said than done for such a competitive athlete who saw no life outside of sports.  During our conversation she described her left foot as looking like a bag of screws (see picture below). I think that is a pretty accurate claim. Although she admits that it has taken time to come to grips with her various injuries and the resultant limitations those have placed on her active lifestyle, she has learned to make light of the situation (see below).  "Pain is your body's warning signal that something is not right" With all these injuries, Rebecca talks about the struggle of having the injury prone tag.  She remembered having both a walking boot and an arm sling at one point while still in college.  Rebecca gives advice in this episode on how to use your injury to your advantage.  This isn't referring to the use of elevator services and getting sympathy from your friends, teachers and family.  There is a lot that can be learned from watching from the sideline. Believe it or not, since high school Rebecca knew she wanted to be a physical therapist even before all the injuries. Below, she is pictured treating one of her patients with dry needling. As Rebecca states in her Instagram caption (above), there is not one tool that is going to solve everything.  During our conversation, I asked Rebecca about some things injured athletes and their families should look for when choosing a PT facility.  Not all PT practices are created equal. What to look for in a PT facility: What is the population? Athletes or hip replacements? Take a tour of the facility to see their space and equipment options Rebecca's new favorite tool is the Owens Recovery Science blood flow restriction device featured in episode 84.   The most commonly treated injury in Rebecca's practice are ACL injuries Are you progressing? Are your exercises always the same? Are you always off on your own or working with an aide? If any of those are true, you should look into finding a new facility. Previously, Johnny spent 10 years as the Chief of Human Performance Optimization at the Center for the Intrepid (See Video Below).  Here he treated service members, including Ryan Miller from Episode 79, who suffered severe musculoskeletal trauma. I heard Johnny speak at the AMSSM conference this past May in San Diego and was amazed by the incredible knowledge he was sharing on Blood Flow Restriction (BFR) and the implications it has for athletes recovering from injury.  Johnny has been published extensively for his work with BFR in the peer reviewed literature and his work has been featured on 60 Minutes, Time Magazine, NPR, Discovery Channel and ESPN.  I have no doubt many of the athletes who listen to this episode can benefit one way or another by adding BFR to their rehab protocol or training regimen. As many of you know, I have been rehabbing from an Osteochonral Autograft Transfer (OATs) surgery I elected to have back in January of 2016.  I have been struggling with moderate to severe kneecap pain and atrophy  since 6 months out from surgery.  The pain has diminished greatly since working with Dr. Danielle Clarke over at Parabolic Performance and Rehab in Little Falls, NJ. However, when I heard Johnny talk about BFR at the AMSSM conference I first thought, I need to get this guy on the podcast and then thought, this could be a potential solution to my lingering knee pain! Johnny told me he thought I would be a good candidate for BFR training. For athletes like me who can't build muscle through lifting heavy loads because it is painful, the anaerobic properties of BFR training have been found to stimulate a similar biological response in the muscle with much lighter weights.  In the above picture, I am performing a step-up with a slow eccentric descent.  Trust me, you have never felt muscle fatigue until you have tried BFR.  It's a deep burn (Insert Ron Burgundy voice).  The single leg squat on the Total Gym is by far the most difficult and uncomfortable, but after having limited use of my quad for over a year, it's also glorious. Since recording this episode with Johnny I have been doing BFR on my leg for the past 3 weeks at Edge Physical Therapy & Sports Medici

Since working with Lawrence and Johanns at Edge, I can definitely feel a big difference in the strength of my left leg. Normally when I go out for a ride on my road bike or do some intervals on an Assault Bike, my non-surgically repaired right leg feels like it is doing all the work.  However, I feel much more symmetrical in the last couple rides and workouts I have done. I am excited to see the progress in the coming weeks.  Keep an eye out for some videos on social media of my BFR training. Below you will find notes on my interview with the BFR legend himself, Johnny Owens: BFR is an individualized tourniquet system and is meant to reduce blood flow into an extremity. If you are able to exercise at a low blood flow, you can use lighter weights to get similar results that you would expect from lifting heavy weights. BFR isn't as new of an idea as you might think. Early physiology studies from decades ago look into anaerobic muscle response. Yoshiaki Sato noticed bigger pump effect with BFR. Population Perfect for person recovering from injury because those patients cant handle heavy loads. Geriatric population. Battling sarcopenia after 50, body doesn’t respond to muscle protein synthesis. Athletes in season, won’t beat up the body. Dwight Howard video Both lower tourniquet pressures have an effect and high pressures have an effect. Why not just use a tourniquet? Clinically you can’t ignore the tourniquet literature. Wide and tapered devices to avoid pressure gradients. Contraindications list: Cardiovascular compromised Cancer “Sick person List” Young pediatrics Certification process Over 1000 certified providers Medical License is required PT, OT, ATC, MD Teaching how to use tourniquets. The science: Who wins?  lifting light or lifting light with a tourniquet? Tourniquet Johnny believes lifting heavy still wins overall, but not everyone can. It was hard to ever feel bad for yourself at the center for the intrepid. Johnny's story of 3 ACL injuries: Grew up in west Texas and was the son of a mechanic (That just sounds tough). Played High school football in the same district as Permian High School from the Book "Friday Night Lights". Eventually gravitated to soccer: Blew his knee out playing soccer on a non-contact play in the rain. Eventually re-tore the repaired knee. Johnny talks about the evolution in ACL surgeries and treatments since his initial injury. Got hit by a car and tore it a third time while at The University of Texas at Austin. The third injury was an eye opener for Johnny, because he realized he couldn’t rely on becoming a pro soccer player. Johnny's advice to athletes going through the transition to life after sports is, "what you do does not define who you are". Why staying at the Center for the Intrepid was “Fail better” and how athletes should seek out non-health-hazardous opportunities that makes them uncomfortable.
