I just got a great question from a runner with a cuboid fracture. He signed up for four weekly coaching calls as he was returning to running.  He just wanted to make sure that he didn't make the fracture worse as he was actually getting back to activity.    He asked, “Once I start running outside, can I use the Alter-G treadmill to add miles and speed while lowering overall tissue stress?”   Can I use the Alter-G treadmill to add miles and speed while lowering overall tissue stress after recovering from an injury?    That's a great question and that's what we're talking about today on the Doc On The Run Podcast.

How to Make Everything Hard About Running Easier: Brad Miles Imagine being able to run and heal through an injury. Or imagine significantly increasing your weekly mileage while healthy, while also minimizing your chances for injury, increasing your speed, and improving your recovery. Wouldn’t that be great? Brad Miles, the founder of Lever Running says it’s not only possible, but scientifically proven.   Brad created his Lever system to deliver the benefits of body weight supported running in a convenient, portable package that costs a fraction of the AlterG. The what? The AlterG is basically a big, fancy, eye-poppingly expensive treadmill that takes some of your weight off as you run. It’s a bulky piece of equipment that NASA uses, not your local gym, and certainly not your average runner. And for years, it was the only player in the market for body weight supported running.   Enter Brad, who says that the Lever makes everything that’s hard about running easier. He shares use cases with Coach Claire about how injured athletes have benefitted from body weight supported running using his system, and also how healthy athletes have leveraged it to up their game. Tyler is an exercise physiologist with the University of Texas Medical Branch, collaborating with physical therapists and athletic trainers daily for late stage rehab progressions, rehabbing various orthopedic conditions, and return to sport protocols. Not to mention he is also an adjunct Kinesiology and Anatomy & Physiology instructor at Galveston College. Participating in several case studies regarding the Alter G anti gravity treadmill, exercise prescription, and working on one currently with Total knee replacements on diabetic patients. Certified through the ACSM and NSCA, masters degree with an emphasis in Strength & Conditioning, and seeking out doctoral programs in Kinesiology. Non Stanford has been involved in and surrounded by sport for her entire life. She was destined for big things on the track and was 3x Welsh Schools 1500m champion, before being mentored by Dame Kelly Holmes. But injuries took their toll and Non made a successful transition into triathlon. She moved to Leeds and in 2012, she was crowned U23 World Champion. Just a year later, she became ITU World Champion. But Non has had her fair share of ups and downs since. 2014 was wiped out by injury, in 2016, she finished 4th behind GB teammate Vicky Holland at the Rio Olympics. At the end of 2018 Non left the Leeds set up to join Joel Filliol’s squad. She won WTS Hamburg in 2019, before having knee surgery at the end of the year which put her hopes of a second Olympics in doubt. They had the concept of creating a virtual locker room and they called it “Kana Sports.” “We had this concept at the back of our minds, how Kana sports morphed into Kana communications, we had a great time building that, no regrets.” - Mark Gainey From B2B to B2C Mark was previously the CEO of Kana Communications which went public late 1999. With a market capital of 11 billion USD and an upcoming new CEO at that time, Mark found himself moving to a different chapter in his life after. “I distanced myself in a pretty meaningful way. Nick Knight Podiatrist talks to Dr. Justin Trosclair DC on A Doctor's Perspective Podcast. Deep dive into MSK sports podiatry with Nick Knight. What is plantar plate injury, how do you treat MSK as a podiatrist, clinical research, 3d gait analysis, zero gravity treadmills, and his love of field hockey. A fresh look at what you can specialize in. Podiatry was not his first idea for a profession but from being excluded from the military and physical therapy Nick picked it.  Luckily, there is a whole aspect/ specialty of sports based – MSK based podiatry and he just fell in love with that aspect of the profession and now wouldn't change his career. He spent a lot of time shadowing MSK doctors during university and purposing choose jobs that were musculoskeletal focused.  Knight and his wife are huge field hockey players so he would much rather treat sport injuries than tackle diabetic ulcers all day. When are custom orthotics appropriate compared to a High Quality over the counter version? How long does he typically have people wear orthotics? Nick Knight's most common condition is forefoot pain, particularly the plantar plate. I ask him to walk us through a typical treatment protocol. Most podiatrists focus on the plantar fascia and heel area so tune in to the forefoot answer.   Knight, DPM will also discuss how he is publishing his findings for conservative treatment for plantar plate injuries because for now the research is heavy on surgical interventions. His research is finding that the techniques used can get people back to running and not re-injuring the area. Problem is about 40% can have a plantar plate injury and not have symptoms. However 10 years later do they have issues like a hammer toe? Nick, sports podiatrist, gives us 3 powerful stories of how his treatment allowed patients to walk in their wedding after an Achilles' rupture, walk pain free on vacations and stroke recovery. Reasons to talk more functional improvements instead of pain focused with your patients. Realize that people expect to lose functional mobility as they age and that doesn't have to be the case. Also don't underestimate the need to handle realistic expectations. What are his thoughts on weightless AlterG zero gravity Treadmills?  Who benefits from zero gravity treadmills? A Few signs to look out for when getting a 3D foot and running gait analysis to know if you are getting a good or bad assessment.  Watching the foot vs the foot and knee, do they talk about over pronation, or do they immediately try to sell you an insert to straighten you up. Podiatry in the UK is working hard to produce more evidence based research, why is that? How do we get more research outside of the NHS because you miss a lot of types of patients if you don't? Once again we hear that hiring the right staff should focus on personal skills and how they line up with your brand because the clinical or office skills can all be taught. His marketing is highly geared to talking to other doctors in his area and facebook ads. In fact, he doesn't even track the return on investment of his facebook ads, find out why? For one running surface has nothing to do with the likelihood of developing a stress fracture. Also pronation vs. supination of the foot has nothing to with likelihood of developing a stress fracture either. What to expect; 6-8 weeks of recovery (pool, Alter-G treadmill, bike) Cost:$150-$3500 depending on insurance vs no insurance and if you shop around or not if you get an MRI. Lisa embraces a high-tech solution to her sore shin, going into running orbit on the Alter-G, and then produces a confidence building marathon-pace trial during the ANSW Sydney half-marathon to finish the week on a high. Over in Ballarat Brian mini-bonks during a long run, but recovers quickly to complete a good fartlek after his Park Run volunteering debut, and finishes the week on the up with a well-paced medium long run to record another 80km/50 mile week while staying ahead of an ITB niggle. Ankle syndesmosis injuries following forced ankle dorsiflexion and lateral rotation of the foot require very specific management and rehabilitation to achieve great treatment outcomes. If you approach management of ankle syndesmosis injuries in the same manner as a lateral ankle ligament injury it will likely result in long term pain and dysfunction for your patients. In the previous episode of the Physio Edge podcast - How to assess ankle syndesmosis injuries, Chris Morgan and David Pope explored how to assess and diagnose ankle syndesmosis injuries (ASI). In Houston we had a problem late in the game, but the Celtics nearly won a hard fought game against the league leading Rockets. This isn't horseshoes, but "close" might be good enough when you are on the road facing a club still VERY SALTY over the outcome of the last matchup in Boston. Justin had some criticism for Brad Stevens handling of the final minutes and the removal of the scorching hot Greg Monroe in favor of the scuffling Al Horford. Jon backed "Team Dad" Horford and saw Al as a necessary and nearly permanent fixture at the end of Celtics games. In the show's second half, Justin and Jon recorded from Hayward Island as they maintain their stance that Gordon Hayward will return to play for the Celtics this season. Justin is unconvinced the numbers associated with the Alter-G are all that helpful in understanding Hayward's recovery progress.
