On January 31, Film at Lincoln Center opened a landmark new retrospective titled Frederick Wiseman: An American Institution. The series showcases new 4K restorations of over thirty of the filmmaker's works, which together form a monumental survey of modern American life—with a frequent focus on the intersections of individuals and institutions. Wiseman just turned 95 on New Year's Day, and the FLC series comes on the heels of similar retrospectives in Chicago, Portland, Maine, and Vancouver—with more planned for Paris, Los Angeles, St. Louis, Boston, and beyond. How does one even begin to consider a body of work so sprawling, so rigorous, and so significant? For today's episode, Film Comment hosted a “Wiseman Potluck,” where each guest was tasked with bringing one film that especially resonates with them to the discussion. Film Comment Editors Clinton Krute and Devika Girish were joined by Andrew Katzenstein, the author of a terrific new essay on Wiseman for the New York Review of Books; Genevieve Yue, who interviewed the legendary filmmaker for the Film Comment Letter in 2022; and FC‘s very own Michael Blair. This week we're excited to present a conversation with writer/director Robert Eggers who recently joined us for a Q&A following a screening of his highly anticipated new feature Nosferatu. Across four intensely stylish, powerfully atmospheric and richly detailed feature films, Robert Eggers has established himself as one of contemporary cinema's most singular auteurs. His work looks to different historical periods, folkloric traditions, and the abject and the arcane alike to craft enigmatic and utterly gripping parables about madness, the antagonism between man and nature, and desire as all-consuming compulsion. But his films, while deeply researched and steeped in worlds that themselves predate the advent of cinema, are nevertheless plainly the output of a passionate cinephile, an artist both in conversation with film history and in conversation with the the history of the occult. This is particularly evident in his latest, Nosferatu, which takes up the challenge of reinventing the story of Dracula after the seminal treatments by F.W. Murnau, Tod Browning, Werner Herzog, and Francis Ford Coppola, to name a few. This February, Film at Lincoln Center is excited to present Conjuring Nosferatu: Robert Eggers Presents, a special series made up of the films that inspired Eggers's spellbinding new take on fiction's most famous monster, an eclectic can't-miss array of gothic Hollywood deep cuts, rare works of Eastern European folk horror, and captivating evocations of 18th-century England, as well as a special screening on 35mm of his own Nosferatu. His searing drama The Seed of the Sacred Fig won a Special Prize from the jury and three other awards on its premiere at the Cannes Film Festival. The film is every bit as urgent and gripping as its real-life backstory would portend: longtime government worker Iman (Missagh Zareh) has just received a major promotion to the role of judge's investigator, to the hopeful delight of his wife Najmeh (Soheila Golestani); at the same moment, a series of student protests against the government have exploded in the streets, stoking the sympathies of their independent-minded daughters Rezvan (Mahsa Rostami) and Sana (Setareh Maleki). The growing wedge between progressive children and traditional parents intensifies through a series of unsettling events that put Iman's future in jeopardy. Headed by Palestinian activist Basel Adra and Israeli journalist Yuval Abraham (also two of the film's directors), the collective commits itself to filming and protesting the demolitions of homes and schools and the resulting displacement of their inhabitants, which were carried out to make way for Israeli military training ground. American filmmaker Julia Loktev (The Loneliest Planet, NYFF49), born in the Soviet Union, returned to Moscow in 2021 to make a documentary on the persistence of independent journalism in Putin's Russia—just months, as it turned out, before the country's full-scale invasion of Ukraine. With her friend Anna Nemzer, a talk show journalist for TV Rain, Russia's last remaining independent news channel, Loktev ends up immersing herself with a group of young women fighting to ensure the vocalization of dissent and outspoken criticism of the country—even as they are branded by the government as “foreign agents,” their careers and lives increasingly at risk as the country creeps toward war. Structured in five chapters, Loktev's film, the climactic days of which were filmed in Moscow during the first week of the invasion, when most independent journalists fled the country, is an extraordinary vérité document of a moment of immense change and anxiety, as well as a vital depiction of the eternal hope that so many in Russia hold for living in a democratic state. On October 29, 2019, Michelle Xue and her climbing partner, Jenny Shedden, set off to climb Red Slate Mountain in the Sierra Nevada when they were struck by rockfall and killed. This story is told through the lens of her dear friend, Artem.While all genres of climbing can be dangerous, alpine climbing is likely the most. These stories of loss pay tribute to loved ones, but beyond that, the life of an alpine climber is complex. They say, “What's done in love, is done well.” But the passion for this sport creates a catch-22.The cold paradox of alpine climbing leaves loved ones behind, pondering the timeworn question: Why do we climb? Why choose to continue a sport when the risks encountered could mean death? Everyone will grieve in this lifetime. And everyone will experience loss. But it doesn't really work that way because the system of measurement that dictates the “good mom” / “bad mom” status is fundamentally flawed.When Tracy became a newly-minted mom, she was determined to find a way to stick with climbing while she navigated all the new hurdles motherhood brings. The following question refers to Section 7.8 of the 2022 AHA/ACC/HFSA Guideline for the Management of Heart Failure.The question is asked by Stony Brook University Hospital medicine resident and CardioNerds Intern Dr. Chelsea Tweneboah, answered first by Mayo Clinic Cardiology Fellow and CardioNerds Academy Chief Dr. Teodora Donisan, and then by expert faculty Dr. Michelle Kittleson.The Decipher the Guidelines: 2022 AHA / ACC / HFSA Guideline for The Management of Heart Failure series was developed by the CardioNerds and created in collaboration with the American Heart Association and the Heart Failure Society of America. Question #29 A 69-year-old man was referred to the cardiology clinic after being found to have a reduced left ventricular ejection fraction and left ventricular hypertrophy. For the last several months he has been experiencing progressively worsening fatigue and shortness of breath while getting to the 2nd floor in his house. He has a history of bilateral carpal tunnel syndrome and chronic low back pain. He takes no medications. On exam, his heart rate is 82 bpm, blood pressure is 86/60 mmHg, O2 saturation is 97% breathing ambient air, and BMI is 29 kg/m2. He has a regular rate and rhythm with normal S1 and S2, bibasilar pulmonary rales, and 1+ pitting edema in both legs. EKG shows normal sinus rhythm with a first-degree AV delay and low voltages. Transthoracic echocardiogram shows a moderately depressed LVEF of 35-39%, severe concentric hypertrophy with a left ventricular posterior wall thickness of 1.5 cm and strain imaging showing globally reduced longitudinal strain with apical sparring. There is also biatrial enlargement and a small pericardial effusion. A pharmacologic nuclear stress test did not reveal any perfusion defects. A gammopathy panel including SPEP, UPEP, serum and urine immunofixation studies, and serum free light chains are unrevealing. A 99mTc-Pyrophosphate scan was positive with grade 3 uptake. In addition to starting diuretics, what is the next most appropriate step for managing for this patient? A Start metoprolol succinate B Start sacubitril/valsartan C Perform genetic sequencing of the TTR gene D Perform endomyocardial biopsy Answer #29 Explanation The correct answer is C – perform genetic sequencing of the TTR gene.   This patient has findings which raise suspicion for cardiac amyloidosis. There are both cardiac (low voltages on EKG and echocardiogram showing marked LVH with biatrial enlargement and small pericardial effusion as well as a characteristic strain pattern) and extra-cardiac (bilateral carpal tunnel syndrome and low back pain) features to suggest amyloidosis. The diagnosis of cardiac amyloidosis requires a high index of suspicion and most commonly occurs due to a deposition of monoclonal immunoglobulin light chains (AL-CM) or transthyretin (ATTR-CM). ATTR may cause cardiac amyloidosis as either a pathogenic variant (ATTRv) or as a wild-type protein (ATTRwt).   Patients for whom there is a clinical suspicion for cardiac amyloidosis should have screening for serum and urine monoclonal light chains with serum and urine immunofixation electrophoresis and serum free light chains (Class 1, LOE B-NR). Immunofixation electrophoresis (IFE) is preferred because serum or urine plasma electrophoresis (SPEP or UPEP) are less sensitive. Together, measurement of serum IFE, urine IFE, and serum FLC is >99% sensitive for AL amyloidosis.
