There are two guests on this week's episode of The Chauncey DeVega Show. Dr. Justin Frank is a former professor of psychiatry at George Washington University. He is the author of several books including Trump on the Couch: Inside the Mind of the President. Dr. Frank reflects on how the American people and their collective sense of normal, safety, and security has been dramatically disrupted by Trump's return to power and his  “shock and awe” campaign against American democracy and society. Dr. Frank counsels that feelings of dread, fear, and anxiety are totally normal responses to such a threat – but the question then becomes what to do with those feelings. Dr. Frank also explains the psychological dynamics at work in why so many Americans were in denial, immature, and refused to believe the many warnings, over almost a decade, that Trump's return to power would be a disaster for the country and the world. Dr. Lance Dodes is a former clinical professor of psychiatry at Harvard Medical School and a training and supervising analyst emeritus at the Boston Psychoanalytic Society and Institute. Dr. Dodes explains why and how so many Americans and others in this time of crisis are increasing their unhealthy use of alcohol, drugs, and engaging in other addictive behavior as a coping mechanism. He also explains the important differences between compulsion and bad habits and their importance for treating alcoholism and other diseases. I was working with a client this week and he said something that really struck me.   He said, the work that we had been doing made him think that he was less depressed, more hopeful, no longer part of the addict class. This got me thinking.  When I was attending 12 step meetings and saying, “Hi, my name is Zach and I'm a porn addict” the message of being an addict was clear.  Once a porn addict, always a porn addict.  I don't know why this ideology grew up in the 12 step system.  No matter why it's there, being a forever addict seems to be the way that we are encouraged to think about our struggles when it comes to pornography.  It baffles me a little because it would seem to be contrary to everything that the 12 step program would be working toward.   This seemingly contradictory message of being a forever addict while simultaneously participating in a group that has, as its core mission, eliminating an addictive behavior from your life is, I believe one of the reasons 12 step groups fail to succeed and most people fail to actually eliminate their problems within those groups.   Dr. Lance Dodes, author of The Sober Truth, details the evidence in his book, that about 5% of people who participate in 12 step groups are successful at actually eliminating their pornography addiction.  That is, by the way, the same percentage that occurs when you just do the work on your own and just decide to quit.  People who do it without any program, succeed about 5% of the time.  So, in terms of evidence based analysis of successful systems of intervention, 12 step programs are about as likely to help you overcome pornography for good as if you went to no one and did it on your own.   By contrast, Acceptance and Commitment coaching has a 95% success rate at 30 days and 80% at 6 months.  But back to the idea that Once a porn addict, always a porn addict has always been something that I bristled at, even when I was deep in the 12 step ethos.   Why?   I think there are 3 reasons why being a forever addict is not only a bad idea, but it is actually holding you back and keeping you from succeeding at overcoming pornography.  Being an addict reduces your responsibility over your actions. When we use addiction language we are often putting off responsibility for our actions to something outside of us.  For instance, the phrase “I slipped this week and looked at porn.” Which is a phrase I've said and heard countless times.  It sets the locus of control for the actions outside of the person.  It is even self contradictory.  The first part, I slipped, conjures up a sense that some unseen force like gravity pulled me in a direction that I did not choose.  The same as when we say, I slipped on some Ice or I slipped down the mountain.   Gravity, regardless of your choices is taking you in a direction that you are not choosing to go.   But then we say, I looked at porn, which looking is an action we take.   I often ask people, when you view pornography how does it happen?  They tell me things like how they go on their computer or phone, they start with some website or another and after a few minutes they are engaged in viewing pornography.  I have never once, had anyone tell me that someone showed up to their house and chained them to a chair, and made them watch pornography.   We talk like we are out of control, but the reality is that we are making the choices even though we regret them later.   That isn't to say that there isn't compulsion or urging in that direction, because, for most people there is.  But compulsion isn't the same as being out of control and unable to choose.  It is that there are real, strong, and compelling reasons to view...

But these last few weeks have been a great deep dive into how you can really begin to move past pornography by not just focusing on pornography, but focusing on how you can become the person you want to be. Many of you know that I am all about an evidence based approach to Overcoming Pornography. You've probably listened to my episode about how pornography is not an addiction and the book by Dr. Lance Dodes, you may know Cam Staley, who is a PhD researcher out of the University of Idaho, a really great guy, quite like him. You are probably thinking, okay, yes, we know that you like evidence. So not only do I have the evidence from my own journey that I've shared with you, one of the components of my coaching to Overcoming Pornography is based in Acceptance and Commitment coaching, I call it ACC, which comes from Acceptance and Commitment Therapy. Now, I'm not a therapist, but I have studied the techniques and processes and systems that have helped 1000s of men and women and their pornography struggle. And I've done it myself, like, this is not pie in the sky. This is not, hey, you know what, let me tell you a theory, and I will help you figure it out. That's not it. This is real life. This is my journey. You are like, Yeah, I need somebody who's been there. You know, I always talk about I have an MBA, and I've been in business for a long time, before I became a coach, I, I was an insurance and I loved it. And I did some other things that add some great, cool stuff. And I always think about, who's the guy that I would come to, who's the person that I would go to, if I needed help running my business. I would not even go to a Yale or a Harvard School Business School professor, I would go to a guy who's actually lived the process. Some of you guys know Gary Vee, cool guy, I listen to his stuff, I don't listen to a guy who works for Harvard. I don't, because I want to know what it looks like to actually be in the trenches. And that's why I use ACC. And the coaching that I do is based in both my real life experience and acceptance and commitment, coaching. And then Acceptance and Commitment coaching, there's a principle called the choice point. It is one of the key principles of ACC that I use in the membership, which has been closed now to members to new members for a little bit, a little while. Ben Funkhouser, Executive Director of The Refuge in Winterset, Iowa, and he is a Commissioned Addictions Biblical Counselor with The Addiction Connection. Ben and Mark finish the series discussing the myths of addiction as noted in Dr. Lance Dodes book The Heart of Addiction: A New Approach to Understanding and Managing Alcoholism and Other Addictive Behaviors (Harper Collins, 2002) How we agree with Dr. Dodes' myths: Myth #9 Are people with addictions impulsive? Romans 14:11-12 "for it is written, “As I live, says the Lord, every knee shall bow to me, and every tongue shall confess to God.” 12 So then each of us will give an account of himself to God." How do habits really help us? Responsibility before God is the truth that transforms someone from feeling helpless, as if there's nothing I can do to change this, so I just have to deal with it. That's hopeless. What seems compulsive is really God's creative power in how He created us to develop habits. 6:45 - The world's lack of hope in their description of the problem of addiction Jay Adams (Godliness Through Discipline) taught the ways habits affect and help our lives as a gift from God. Replacement is a key concept. And he also described the four characteristics of habits as: automatic, unconscious, skillful, and comfortable. 11:35 - Ephesians 4:20-24 13:00 - Episodes of addictive choices have a lot of planning and effort involved even if the actions seem to happen quickly and easily. It's a planned lifestyle and the path becomes so "well-worn" that it's easy to go there. 14:50 - Matthew 5 calls us to be radical -- to cut off the paths of sin so we can replace these addictive habits. 16:12 - A biblical Proverb says, "The way of a transgressor is hard." It does lead to destruction in our lives here, and then eternal destruction without Christ. He also counsels online via video conferencing software. Myth #6 You should be treated by someone who also has an addiction. 2 Corinthians 1:3-7 says "...so that we may be able to comfort those..." "Blessed be the God and Father of our Lord Jesus Christ, the Father of mercies and God of all comfort, 4 who comforts us in all our affliction, so that we may be able to comfort those who are in any affliction, with the comfort with which we ourselves are comforted by God. 5 For as we share abundantly in Christ's sufferings, so through Christ we share abundantly in comfort too. 6 If we are afflicted, it is for your comfort and salvation; and if we are comforted, it is for your comfort, which you experience when you patiently endure the same sufferings that we suffer. 7 Our hope for you is unshaken, for we know that as you share in our sufferings, you will also share in our comfort." (ESV) Explaining the difference between being a sinner, being affected by sin, and sin having dominion over me. All Scripture is useful... so that the man of God can be thoroughly equipped for every good work. (2 Tim 3:16-17) It really is the Spirit of God and the Word of God. Anyone can help an addict. Testimonies of what has happened to us are important but not what transforms the heart. God transforms hearts. God's great and precious promises are given to us. It's His power, His word, His enabling us to escape the corruption of our own heart. Sex addiction, hatred, anger, it doesn't matter what it is, the core of the heart needs to be justified and sanctified. 1 Corinthians 6:9-11 "Or do you not know that the unrighteous will not inherit the kingdom of God? Do not be deceived: neither the sexually immoral, nor idolaters, nor adulterers, nor men who practice homosexuality, 10 nor thieves, nor the greedy, nor drunkards, nor revilers, nor swindlers will inherit the kingdom of God. 11 And such were some of you. But you were washed, you were sanctified, you were justified in the name of the Lord Jesus Christ and by the Spirit of our God." The Bible gives us hope that we CAN be set free. It happens from the Lord who is the Spirit, not my testimony as a counselor, that changes hearts. We have the ability to point them to the One who can set them free. Myth #7 "Addiction is basically a problem of brain chemistry." Dr. Dodes calls out this myth and we agree. Mark went to a training about 25 years ago where he began to witness the transitioning of the terminology and yet maintained the same exact treatment philosophies. The way we were brought up does have an affect on us but it is not binding on us. We still have a choice. Sins have been taken out of the sin category and put them all in the sickness category. Lou mentions David Tyler and Kurt Grady's book, Deceptive Diagnosis: When Sin is Called Sickness (2006, Focus) There is hope. If you are set free by Jesus Christ, you are not an alcoholic, you are a child of God. "As you minimize sin, then being saved is not that big of a deal. The 12 steps are probably not working for you.I started attending 12 step meetings in 2007. I also went to meetings sanctioned by the Church of Jesus Christ of Latter-day Saints, of which I am a member. Those Church sanctioned meetings were full of guys that were pretty similar to me. No one had committed a crime (I don't think) in pursuit of their sexual desires, but none of them seemed able to overcome their pornography use either. So every week for a number of years I would sit in the meetings, I would say, “My name is Zach, I'm a porn addict, it has been x number of days since my last relapse.” If you have ever been to these meetings, the people are earnest, the topic is serious, and the goal is the same for everyone. 12 step programs are the most recognized and ubiquitous type of sobriety focused recovery systems in the country. Judges assign people to attend them. Families swear by them. I found a list https://www.therecoveryvillage.com/treatment-program/aftercare/related/types-12-step-programs/#gref (here) of 32 programs in addition to Alcohol Anonymous. Five of these had the word sex in them. So why, according to Lance Dodes, MD and the Sober Truth, do these meetings only have a 5% success rate? Now, granted, the Sober Truth is targeted specifically to Alcoholics Anonymous, but the “12 Step Program” has been taken and morphed to work with narcotics, pornography and food addictions. I don't have data for those programs, but I think it is safe to say that the data is likely to be similar for similarly structured programs. For me, I worked the 12 steps as best I could, in concert with my bishop and stake president, and had regular meetings with a counselor. All of the world was pulling for me and I was pulling in the direction I was told I should go. Once I had been going to meetings for a few years, I thought, I should be able to go longer than I am. I should have more sobriety. I shouldn't be relapsing like this. I felt completely alone. The truth was, that even though I, like hundreds of thousands, even millions of people before me, had gone through the steps, worked each of them to the best of my ability, apologized, asked for forgiveness, shared the program, done it all, I was still doing what addicts call white knuckling it. I was still living in a place where I was not succeeding to my definition of success. Maybe I wasn't using as much as I once had, but each time the urge came, I was still bearing through it with all the pain that comes from having a kidney stone. I was always just on the verge of going back. Back to pornography, back to lying to my wife, back to hiding from my church leaders, back to buffering my life away with my drug of choice so I didn't have to deal with my feelings. Some of you might be saying in your minds, “oh, then you did it wrong” or “then you really weren't sober” or some other version of blaming me for not getting it right because I wasn't doing it right so I have no right to complain. That's not an atypical response from those dealing with addiction and advocates of the 12 step program. In fact, Dr Dodes talks about this in the Sober Truth. He quotes AA's Big Book saying, “Rarely have we seen a person fail who has thoroughly followed our path. Those who do not recover are people who cannot or will not completely give themselves to this simple program…” Dr. Dodes concluded, as anyone might, that “the program doesn't fail; you fail.” Emphasis his. So how could anyone who has gone through a 12 step program ever step forward and say, “um, sorry guys, this just isn't helping me”? The...

Dr. Dodes is also the former Director of the substance abuse treatment unit of Harvard’s McLean Hospital. Dr. Dodes has developed truly revolutionary insights into the nature of addiction -- concepts he eludcidated in his books, "The Heart of Addiction" and "Breaking Addiction: A 7-Step Handbook for Ending Any Addiction." Dr. Edward Hallowell, a preeminent authority on Attention Deficit Disorder (ADD), called Breaking Addition a "groundbreaking work [that] should become the 'go to' handbook for anyone suffering with addiction." Here are some highlights from the episode: The way most people think about addiction is wrong: they think it's a physical problem; a spiritual or moral weakness; or a neurological problem None of these things = true Dr. Dodes has been talking to people with addictions for decades, and he's learned from them and tested his hypotheses He's come up with a new way of understanding addiction A case history to illustrate this new paradigm Man stuck waiting for his wife became frustrated - spotted a bar and went in When did you start to feel better? "When I was standing on that corner and I decided to get a drink" Illustrative of what he's heard from many people over the years - wasn't the drink itself when he felt better. Something happened when he made the decision. His problem was that he was helpless, trapped. When people feel overwhelmingly helpless, it precipitates addictive behvaior. Once he decided to drink, he wasn't helpless any more. Addictive acts are ways of undoing or reversing overwhelming helplessness. Addiction is not a "thing in itself" -- it's a symptom. It's an "unlucky solution" to the problem of helplessness. Triggers of helplessness are very personal and not conscious "F*ck it: I'm going to have a drink." What does the "f*ck it" mean? It's a fury at being helpless. Analogy to a cave-in. 300 tons of rock trap you in a cave, you're going to freak out. That's a normal reaction. The people who get depressed and inert when helpless don't do well -- rage at helplessness is innate and healthy. It's that power that makes addiction so powerful. This rage has certain properties which give addiction its properties. At the moment of the addictive feeling, nothing else matters. If you break your wrist trying to get out of a cave-in, you're not being self-destructive -- you're just not paying attention to the consequences. Instead of taking a direct action to deal with helplessness, he took an indirect action. All addictive acts are displacements. Helps to explain curious clinical features of addiction - e.g. that you can change focus of an addiction. Drinking alcohol is most common displacement, but people can switch to other drugs or even to gambling, shopping or eating. There is no difference between addictions and compulsions -- this should change the way we think about treatment We know how to treat compulsions! Figure out why they occur, when they occur, etc. Addictions can be treated by a psychologically sophisticated therapy. Conversely, 12 step models don't work well. Giant modern myth about addiction - that it's a chronic brain disease. Comes out of National Institute of Drug Abuse.
