Research suggests that LGBTQ adults have higher rates of substance use and mental health issues, compared to their heterosexual cisgender peers. These health disparities are often related to minority stress—the chronic psychological and emotional strain that people experience due to being part of a marginalized social group. For example, some LGBTQ folks may use alcohol and drugs to cope with internalized homophobia or to self-medicate for anxiety and depression. On the other hand, some people use drugs in the context of social settings and connecting with others in their community. Since not all substance use is unhealthy, how do providers know how to identify when substance use is problematic? And what can physicians do to best support their patients who may be at greater risk for harm or even overdose? On this episode, Antonio Urbina, MD, Medical Director for CEI's HIV Primary Care and Prevention Center of Excellence, speaks with Eric Kutscher, MD, MSc. Dr. Eric Kutscher is an Assistant Professor of Medicine at the Icahn School of Medicine, where he practices as a primary care and addiction medicine physician at Mount Sinai's Internal Medicine Associates and REACH Program. He currently serves as the Medical Director of the Mount Sinai Hospital Opioid Overdose Prevention Program and Primary Care Lead for the Mount Sinai Center for Transgender Medicine and Surgery. His clinical work and research focus on harm reduction techniques for patients with substance use disorders, particularly within the LGBTQ+ community. Drs. In this solocast I talk about my recent experience and observations during Hurricane Helene's devastation that hit Western North Carolina and the Southeast. For those of you not local, Hurricane Helene swept through the Appalachian mountains, bringing two days of relentless rain (32 inches at my house) and strong winds. The storm saturated the earth, causing historic flooding and leaving unimaginable damage and devastation in its wake. Since September 29th, the destruction of our community has left many devasted with unimaginable loss. In the wake of this tragedy, there was and is beauty. The beauty of communities far and wide coming to aid and help one another. It has been so heartwarming to see humans helping other humans so selflessly. It solidified my deep belief in creating and fostering good, close, and caring relationships within our communities and neighborhoods. We need to have eachothers back! In this episode, Dr. Marguerite Urban and Dr. Daniela DiMarco discuss Doxy-PEP, a new tool for the prevention of bacterial STIs. Drs. Urban and DiMarco are infectious disease faculty members at the University of Rochester, specializing in sexual health care. They were part of the team who authored the recently released New York State (NYS) Department of Health (DOH) AIDS Institute's guidelines regarding Doxy-PEP. This episode is an introduction to the use of doxycycline as post exposure prophylaxis for bacterial STIs. The hosts will briefly review results from recent clinical trials as well as discuss concerns about risks and side effects. They will also present how guidelines for implementation (who, when and how) of Doxy-PEP have varied thus far and review the current recommendations of the NYS DOH guidelines. But what about the things that happened before marriage? The situations that may have taken place years ago, but the repercussions of which are still creeping into your union? What would change in your life if you genuinely believed that God is always with you? How would this impact how you respond to the fear that swirls around? The world around us wants us to walk in an element of fear. The challenge with fear is that it will keep us from the fullness of our relationship with God. Fear will cause us to become powerless and abort the wonders of the Kingdom of Heaven. In Psalm 34, we are reminded that we seek God; he answers and delivers. In the book of Matthew, we have Jesus calming a storm and the disciples trying to figure out who he is. In 2 Kings, we meet a servant who needed his vision refocused to see from the supernatural rather than the earthly perspective. Have you walked with Jesus for years and still struggling with seeds of doubt? Or, have you witnessed signs, wonders, and miracles, been used mightily of God, and still struggled with doubt? Even after He joins them in the boat and the storm calms down, they still have questions, and their hearts are still hard. There is an invitation to see as Jesus does and recognize God in our midst. However, we also must deal with our fear, unbelief, and hardened hearts to have eyes that see. It is exciting to realize that Jesus was not passing by the disciples but showing them His glory. He invited them to grow in relationship with Him and understand who He is. The same is true for you today. We are called to have a desire or hunger for the things of God. We are called to have a passion or hunger for a relationship with God. We are called to have a desire or appetite for the Word of God. What is your definition of meek? I think of timid and quiet, however, that is not Jesus definition. To be meek is to: - not blow up easily - be humble and submissive to God Do you struggle with being meek? We have to answer this question and make appropriate changes in our lives if we are going to walk in the fullness of who God has called us to be. Jesus tells us that the meek will inherit the earth. When you open your Bible to the book of Genesis it begins with “In the beginning God created the heavens and the earth (Genesis 1:1). However, a 1991 Gallup poll found that some 90% of American's believed that God created the world in some manner. However, by 2019 that number fell to 40% and continues to fall today. As we read through the book of Genesis we see intent, order, and purpose within each day of creation. God planned each step, there was nothing random or haphazard about creation. The truth is, if we don't believe that God created absolutely everything, but himself, with no exception, then we cannot believe that God is sovereign. If God is not sovereign, then we cannot trust Him with our lives.
