Welcome to Episode 84 of the Sustainable Clinical Medicine Podcast! In this episode, we sit down with Dr. Susan Wilson, a former emergency medicine physician who retired in 2017 due to progressive vision loss. Dr. Wilson has since transitioned to coaching, supporting healthcare professionals with issues such as burnout, job satisfaction, and work-life balance. Throughout the conversation, Dr. Wilson emphasizes the importance of peer support, shares her experiences with transitioning out of emergency medicine, and discusses the concept of mental fitness. She delves into the challenges of charting in emergency medicine and the impact of additional responsibilities on her well-being. This episode is packed with valuable insights and practical strategies for navigating the complexities of a medical career, making it a must-listen for healthcare professionals seeking sustainable and fulfilling clinical practice. Here are 3 key takeaways from this episode: Importance of Peer Support: Emphasizing the crucial role of peer support from fellow physicians in coping with the challenges and traumatic events at work. Mental Fitness in Medicine: Recognizing and managing negative emotions without becoming toxically positive is essential for healthcare professionals, and coaching plays a significant role in promoting mental fitness. Transitioning Careers: The journey from emergency medicine to coaching showcases the opportunities for professionals to pivot into intellectually stimulating and beneficial career changes within the medical field. Dr. Susan Wilson Bio: Dr. Wilson has been practicing Emergency Medicine since 1994, after attending Loyola-Stritch School of Medicine in Chicago, and completing her residency at The Medical College of Wisconsin in Milwaukee. She retired from clinical practice in June 2019. Wanting to maintain involvement in Medicine, Dr. Wilson became a certified physician peer-coach, now providing guidance and support to physicians. Her 25 years as a hospital-based, board-certified physician has afforded great insight into the challenges of medicine. Having seen the evolution of medical practice and witnessing firsthand how these changes have impacted her colleagues, Dr. Wilson sees coaching as an important tool to address issues of job satisfaction, work/life balance, and burnout among physicians. Dr. Wilson is using “mental fitness” as the framework for her coaching and helps her clients to shift mindset from negative to positive. She also has a special interest in Clinician Distress Syndrome (AKA Second Victim Syndrome), and offers peer support for those suffering acute, traumatic events. -------------- Would you like to view a transcript of this episode? Susan and Judy Wilson I am so excited to bring you this LIVE taping from the Wild Goose Festival.  Susan and Judy Wilson lit up the Goose Cast and my heart as they told us a love story for the ages.  They shared such courage, such love, and such hope.  In these politically charged moments of our lives it was a breath of fresh air to remember that LOVE does not die and as a community we CAN rise up and refuse to go back into the closet! Judy Wilson was born in the aftermath of WWII. Her dad had been a pilot and was in law school. He read case law to her, which she thought forever scarred my life choices. She was a military brat, the eldest of 5, and raised in the Presbyterian Church. Thus, Judy was a rule follower who could successfully negotiate division of an uneven number of cookies among her siblings. When Judy went to college she met Anne, and learned that there was such a thing as sex between women, and beer is pretty good stuff.  Judy decided she really wanted to be "normal" so she married a man, actually twice, and had 2 children. Judy met Susan in law school, and decided the heck with that being normal stuff. We were in classes and several organizations together, and fell deeply in love. We became a law school scandal, which was fun. (sarcasm implied.) Thirty years later, we have had an incredible life together. We became very involved in the nonprofit world, starting a Family Visitation Center, where children could safely be exchanged by parents in conflict. We have been on several boards, mostly involving children's issues. I worked as a Guardian ad Litem attorney representing children in abuse and neglect cases. That was an amazing experience that allowed me to argue cases in front of both the NC Court of Appeals and the NC Supreme Court. My last job was with Legal Aid's Senior Law Project advising old folks like myself. Judy is now retired, spending an inordinate amount of time crocheting and buying yarn. We have 9 grandchildren and are actively part of their lives. Although my early church years left me with conflict about my sexual orientation, I have found a home in the Episcopal Church, specifically The Cathedral of All Souls, where we have attended since 1991. I am an usher, volunteer at the church office, and participate in Bible study.   Susan Wilson  grew up mainly in Atlanta, Georgia and briefly in Germany and Buffalo, NY (which is why she knows- and I quote- “What Hell is like when it freezes over.”  Susan has a Masters Degree and a Doctor of Jurisprudence, from the University of Georgia and a Masters in Journalism. Susan and Judy, her now wife  were at UGA at the same time getting their Masters Degrees, and yet never met.  It wasn't until several years later that they met in law school where they became the local legal community scandal their third year when they fell in love. Susan reports, “We are less a scandal these days than little old ladies in comfortable shoes who are very much still in love and extremely comfortable with each other.”  Because of my lovely wife, we have two children and 9 grandchildren.  They have lived in the Asheville, NC area since graduating in 1991.  Susan has worked for the County trying Child Support, Juvenile abuse, and neglect cases for about a decade before going into a domestic law private practice where she was also a Guardian ad Litem for both children and adults. Susan ceased her domestic law practice in 2013 and switched to Elder Law - mostly Wills, and Powers of Attorney.  Much of her Elder Law practice was pro bono through a couple of local legal aid offices. Susan stopped practicing law completely in 2022. For the past 10 years, Susan has worked as an Affordable Care Act (Obamacare) Navigator through the Council on Aging of Buncombe County.  They provide unbiased free help to people getting insurance through the Marketplace.  Since we don't get paid by any insurance company, we can talk to people about all of the insurance options that are available and can help them complete the application correctly to minimize problems. Along the way  Susan has been one of the founders of two juvenile restorative justice programs and the Family Visitation Center in Asheville.  Susan was also on several boards and served on the Asheville VA institutional review board (IRB) overseeing medical research projects. Susan has always had a deep spiritual connection to the creator.  When she was quite young,  she told my parents that she played with Jesus up in heaven before she was born, reporting “I established a relationship with the Trinity before I was born.” Susan grew up Methodist and switched to Episcopalian when she fell in love with her now wife, Judy.  They journeyed with All Souls (also know as All Sorts) as it explored Human Sexuality and came to accept same sex relationships. Their current Priest is a lesbian so clearly, “we did something right!” Connect with Judy! Judy Wilson on FaceBook Click the link below to: Learn More about The Wild Goose Festival!

Join your host Andrew Tisser with his guest Dr. Susan Wilson as they talk about maximizing our mental fitness and maintaining a sustainable shift in mindset. In her 25 years as a board-certified Emergency Medicine physician, Susan has seen the evolution of medical practice and the impact of these changes on her colleagues. Through her knowledge as a certified physician peer coach, she provides the tools necessary to navigate these challenging times in modern medicine. And with the rapidly increasing cases of burnout and job dissatisfaction, Dr. Wilson believes that professional coaching is needed now more than ever.  In this episode, you will learn:●      What “Second Victim Syndrome” is and why peer support is essential among medical professionals●      The difference between resilience and mental fitness, and ways to improve the latter●      How to use the power of empathy in learning how to show self-compassion●      The value of “mindfulness” when going into another encounter after a difficult one●      How mental fitness and mindfulness can be applied even outside of professional life●      And much more! ~ About Dr. Susan Wilson: Dr. Susan Wilson has been practicing Emergency Medicine since 1994, after completing her residency at The Medical College of Wisconsin in Milwaukee. She also attended the Loyola-Stritch School of Medicine in Chicago. However, Dr. Wilson retired from clinical practice in June 2019.  Wanting to maintain involvement in Medicine, Susan became a certified physician peer coach, providing guidance and support to other physicians. Her 25 years as a hospital-based, board-certified physician has afforded great insight into the challenges of medicine. Having seen the evolution of medical practice and witnessing firsthand how these changes have impacted her colleagues, Dr. Wilson sees coaching as an essential tool to address job satisfaction, work/life balance, and burnout among physicians.  So now, Susan uses “mental fitness” as the framework for her coaching, helping her clients shift their mindset from negative to positive. She also has a particular interest in “Clinician Distress Syndrome” (also known as “Second Victim Syndrome”), offering peer support for those suffering acute, traumatic events.  You can find Dr. Susan Wilson on... Website: https://www.sjwprofessionalcoaching.com/ LinkedIn: https://www.linkedin.com/in/susan-wilson-md-cpc-2492681a4/  Connect with Talk2Medoc on: Website: https://www.andrewtisserdo.com/ Linkedin: https://www.linkedin.com/in/andrewtisserdo/ Facebook: https://www.facebook.com/andrew.tisser Instagram: https://www.instagram.com/talk2medoc_llc/ Twitter: https://twitter.com/Talk2MeDoc YouTube: https://www.youtube.com/channel/UC0O_Sf3aYLavYaJ_hg7bM8g

"Taking control of distress is done by embracing the concept of 'mental fitness.' Being mentally fit enables one to overcome the negative emotions that have traditionally sabotaged our lives, both personally and professionally. Similar to physical fitness optimizing our bodily strength, mental fitness promotes achievement in our performance, relationships, and sense of well-being. Much research has been done in neuroscience, cognitive and positive psychology, and performance science, showing the true benefit of mental fitness. In particular, the concept of neuroplasticity suggests that with the continued strengthening of certain neural pathways in the brain, modifications in brain architecture can be achieved. These changes have been shown with functional MRI, demonstrating increased density of grey matter in areas that correspond to certain thought processes." Susan Wilson is an emergency physician and physician coach. She shares her story and discusses her KevinMD article, "It's time to reframe second victim syndrome." Dr. Susan Wilson has focused on peer support throughout her career, but when she had to quit medicine due to her own medical issues, she realized just how important it is. Listen as she shares her experience and why she’s so passionate about peer support and coaching today.   When you’re spiraling through grief and depression due to some really terrible medical news, what do you do? If you’re a doctor, having the support of your medical peers is extremely important.    This is also the case if you’re working in high trauma departments or fields. When it comes down to it, we are on the front lines of some of the worst physically and mentally traumatic experiences that the human body and mind can and do experience.    What does that do to us? After having to leave practice as an emergency medical physician, Dr. Wilson wanted to continue her work in peer support. She is now a coach and provides acute support for colleagues who are experiencing traumatic events.    She shares her views on the difference between peer support and coaching and why both are an absolute must in the healthcare field. She’s also talking about her coaching career and how it has changed since Covid.    If you’ve ever experienced a traumatic moment in your career then you really won’t want to miss this discussion.   “The idea of peer support is more of an acute support or assistance for someone that's really needing help right now. They need something right away to buoy them through the event.” Dr. Susan Wilson   In this episode: [03:44] Welcome to the show, Dr. Susan Wilson! Conversations About Second Victim Syndrome with Dr. Susan Wilson Welcome to Difficult Conversations with Dr. Anthony Orsini. My special guest today is Dr. Susan Wilson. After 15 years of practicing Emergency Medicine, she retired from clinical practice, completed her coursework in coaching and formed SJW Professional Coaching Services, LLC with the goal of providing guidance and support to healthcare professionals. Over the past 25 years, Dr. Wilson has seen the evolution of medical practice, witnessing first-hand how the changes in healthcare have impacted her colleagues. She sees professional coaching as an important tool to addressing issues of burnout among healthcare providers. Dr. Wilson is particularly passionate about Second Victim Syndrome, an extremely important topic that most healthcare providers are not aware of but should know about. Dr. Wilson talks about physician burnout and suicide rates, and discusses a JAMA article about the relationship between resiliency and burnout in physicians. Dr. Orsini and Dr. Wilson discuss Second Victim Syndrome (SVS), why it’s such a problem, and how it affects both providers and patients. We learn about the stages of grief that occur with SVS and the final stage which is moving on, in which people will either thrive, survive, or drop out. If someone needs help, Susan lets us know the best resources on how to get it. She talks about the staggering statistics of physician burnout and Second Victim Syndrome and how many healthcare professionals are actually seeking help. We find out how Susan coaches people through EAP on their journey of feeling better. Since Dr. Orsini and Susan both do a lot of coaching, they share why it is so important to them to help others. "The COVID-19 pandemic has impacted everyone, especially those of us in health care.  Our way of practicing medicine has been changed; some would say forever. We find ourselves affected not only clinically but also emotionally.  As a result, clinicians are experiencing more stress and anxiety than ever before. These feelings are not new but have been heightened in the face of the pandemic. Physicians are perceived as self-reliant, emotionally stoic, and pillars of the medical community. Society expects perfection from our health care system and turns to us in times of medical crisis. As clinicians, we need to recognize our humanity;  doctors are people too, with the same emotional needs as any other individual.  In fact, one could argue that medicine demands require an even greater level of emotional support than other professions." PODCAST (22:51) -- We can all make a difference. Elementary-school teacher Diane Trull’s life-defining moment happened when her fourth-grade reading class saw a photo of a cardboard box overflowing with homeless puppies. Trull was no stranger to rescuing abandoned animals. She and her husband Mark had made it their mission to find permanent homes for stray dogs and cats. Now her young students were determined to save these lost pups and others like them. And at that moment, the Dalhart Animal Wellness Group and Sanctuary—known as DAWGS—was born. amzn_assoc_tracking_id = "ksco08-20"; amzn_assoc_ad_mode = "manual"; amzn_assoc_ad_type = "smart"; amzn_assoc_marketplace = "amazon"; amzn_assoc_region = "US"; amzn_assoc_design = "enhanced_links"; amzn_assoc_asins = "0806540346"; amzn_assoc_placement = "adunit"; amzn_assoc_linkid = "eae52365b99ba81208e0d78f41261d52"; This podcast interview with Trull was taken from our 3/15/20 program. How Trull and her fourth-graders started their own animal shelter is a story of dedication, commitment, and perseverance. In this eye-opening, deeply personal book, Trull describes the challenges they faced, from rescuing and caring for the animals to teaching children about compassion and responsibility, to facing local authorities determined to shut them down. She shares inspiring stories about animals and animal lovers of all ages in this moving story of hope and compassion. DAWGS is a testament to how love and a strong measure of determination can offer second chances—one animal, one child, and one day at a time. Reviews: "Inspiring and passionate, this heart-stopping story tells how a teacher and her fourth-grade students became a force for good—one dog at a time. Every town needs a hero like Diane Trull and a sanctuary like DAWGS. This book is a reminder that, regardless of age, we all have the power to enhance the lives of animals and make the world a better place. Diane Trull and her class of nine-year-olds put the humane back in humanity."— Helen Brown, New York Times bestselling author of CLEO, BONO, and JONAH "To read DAWGS is to have witnessed all that is best in humanity and much of what is pretty awful. From its inception, this rescue organization pitted children and their desire to save the city’s homeless dogs against the questionable ethics of a cynical and hidebound adult society that viewed these animals as so much trash. The children prevailed. Thousands of dogs were rescued. It is a testament to the courage of their teacher, Diane Trull, and her extraordinary family that, despite unbelievable challenges and outright hostility, she and her students persevered, and continue to do so. Read this story with tissues near at hand. Read this story and know that there is yet hope in hopeless places."— Susan Wilson, New York Times bestselling author of One Good Dog "I loved DAWGS! Shining from every page is the kindness of Diane Trull and the dedication of her fourth-grade students. If only the whole world could be like those remarkable people. They will inspire you and warm your hearts."— Kristen von Kreisler, bestselling author of A Healing Justice "Get ready to feel a lot. When you realize DAWGS is the only animal sanctuary in America that has been started and run by elementary school children, because they understood that the animals needed their help, it's pretty astonishing. The generous love Diane Trull, her husband Mark, and the children and staff at DAWGS have for these animals is so moving—and they do it out of pure love. Dogs needed to be saved and these special people took it upon themselves to do it. This book is so inspiring all along the way."— Bernadette Peters, actress and animal lover
