Two Words That Will Change Your Life: Slow Down While recovering from Total Hip Replacement, I engaged with a client on a Zoom call. He shared one of his favorite sayings, “Slow is fast, fast is slow,” which made me angry. Why? First Responders practice “Slow is fast, fast is slow” all the time, so you never see them running to the scene. Pacing themselves is simply safer, more effective, and faster. A more real-life example of why it's a good idea to experience a risk-taking, lane-shifting driver passing you five times only to end up sitting next to you at the stop light—or worse. The reason I was angry was because I had not yet listened to the advice I give high-performance and driven individuals every day of my life — “SLOW DOWN! Step off the hamster wheel of life, wait for the wheel to stop spinning, and only then, begin your journey to whatever you want.” What I realized in my recovery from THR was my pre-surgery hamster wheel was still spinning on pure adrenaline, even though my medications and mobility had forced me to a stop a few days earlier. Once I recognized what was happening, I breathed and began my slow journey back to myself. Total ankle replacement (TAR) is becoming a more common alternative to ankle arthrodesis for the improvement of pain and function in end-stage arthritis of the ankle. The effects of end-stage arthritis of the ankle are similar to those of end-stage hip arthritis. There is a paucity of literature on patient-reported outcome measures (PROMs) following TARs in comparison with total hip replacement (THR) or total knee replacement (TKR). We aimed to study the 1-, 3-, and 5-year outcomes of TAR in comparison with TKR and THR. TAR patients had similar outcomes to THR and TKR patients for disease-specific and mental health domains, and lower patient satisfaction rates in terms of pain relief, ADL, and recreation. Further research is warranted including clinical outcomes along with PROMs with a long-term follow-up.   To view the article click here.

All of these happenings and healings took place over a 5 month time period.

On this episode of the Healthy Wealthy and Smart Podcast, Dr. Fritz Boettner joins me to discuss hip replacements. Dr. Fritz Boettner is a specialist in joint-preserving arthroscopic surgery and minimally invasive total joint replacements of the knee and hip. His practice concentrates on the nonsurgical and surgical treatment of early arthritis. As part of a patient-centered individual approach, he performs arthroscopic cartilage surgery and joint preserving osteotomies, as well as minimally invasive total joint replacements and resurfacings. In this episode, we discuss: -The evolution of hip replacement surgeries -Pre-surgical considerations for anterior and posterior surgical approaches -Post-surgical restrictions on movement -Rehabilitation and pain management considerations following hip replacement -And so much more!   Having the optionality of either surgical approach allows each patient to be evaluated to ensure the best outcomes. Dr. Boettner states, “Having different types of approaches at our availability allows you to select which would be best for the patient.”   Pain and functional management following hip replacement surgery is the same for either approach with the goal being that, “Everything is geared towards early discharge.”   Dr. Boettner stresses that physical therapy providers be mindful of their interventions and avoid adverse outcomes with unnecessary exertion. He reminds, “If you don’t do anything with the hip, patients will be fine after hip replacement.”   For more information on Dr. Boettner: Dr. Fritz Boettner is a specialist in joint-preserving arthroscopic surgery and minimally invasive total joint replacements of the knee and hip.   His practice concentrates on the nonsurgical and surgical treatment of early arthritis. Background: To facilitate the discussion on the increasing number of total hip replacements (THR) and their effectiveness, we apply a joint evaluation of hospital case costs and health outcomes at the patient level to enable comparative effectiveness research (CER) based on the preoperative health state. Methods: In 2012, 292 patients from a German orthopedic hospital participated in health state evaluation before and 6 months after THR, where health-related quality of life (HRQoL) and disease specific pain and dysfunction were analyzed using EQ-5D and WOMAC scores. Costs were measured with a patient-based DRG costing scheme in a prospective observation of a cohort. Costs per quality-adjusted life year (QALY) were calculated based on the preoperative WOMAC score, as preoperative health states were found to be the best predictors of QALY gains in multivariate linear regressions. Results: Mean inpatient costs of THR were 6,310 Euros for primary replacement and 7,730 Euros for inpatient lifetime costs including revisions. QALYs gained using the U.K. population preference-weighted index were 5.95. Lifetime costs per QALY were 1,300 Euros. Conclusions: The WOMAC score and the EQ-5D score before operation were the most important predictors of QALY gains. The poorer the WOMAC score or the EQ-5D score before operation, the higher the patient benefit. Costs per QALY were far below common thresholds in all preoperative utility score groups and with all underlying calculation methodologies.

Background: The aim of the study was to analyze the effect of preoperative patient characteristics on health outcomes 6 months after total hip replacement (THR), to support patient's decision making in daily practice with predicted health states and satisfaction thresholds. By giving incremental effects for different patient subgroups, we support comparative effectiveness research (CER) on osteoarthritis interventions. Methods: In 2012, 321 patients participated in health state evaluation before and 6 months after THR. Health-related quality of life (HRQoL) was measured with the EQ-5D questionnaire. Hip-specific pain, function, and mobility were measured with the WOMAC in a prospective observation of a cohort. The predictive capability of preoperative patient characteristics - classified according to socio-demographic factors, medical factors, and health state variables - for changes in health outcomes is tested by correlation analysis and multivariate linear regressions. Related satisfaction thresholds were calculated with the patient acceptable symptom state (PASS) concept. Results: The mean WOMAC and EQ-5D scores before operation were 52 and 60 respectively (0 worst, 100 best). At the 6-month follow-up, scores improved by 35 and 19 units. On average, patients reported satisfaction with the operation if postoperative (change) WOMAC scores were higher than 85 (32) and postoperative (change) EQ-5D scores were higher than 79 (14). Conclusions: Changes in WOMAC and EQ-5D scores can mainly be explained by preoperative scores. The lower the preoperative WOMAC or EQ-5D scores, the higher the change in the scores. Very good or very poor preoperative scores lower the probability of patient satisfaction with THR. Shared decision making using a personalized risk assessment approach provides predicted health states and satisfaction thresholds.

Evaluierung von Metamizol und Carprofen als postoperative Analgetika nach Hüftgelenksersatz bei Hunden. Das Ziel dieser Studie war es, die analgetische Wirkung der von den jeweiligen Arzneimittelfirmen für den Hund empfohlenen Dosierungen von Metamizol im Vergleich zu Carprofen nach Hüftgelenkersatz zu bewerten. Es ist bekannt, dass Metamizol ein potentes Analgetikum beim Menschen ist. Bis heute gibt es keine Studien zur postoperativen Wirksamkeit von Metamizol beim Hund. Subjektive (Melbourne Schmerzskala (mMPS) und visuelle Analogskala (VAS)) und objektive (Ganganalyse, in welcher die vertikale Spitzenkraft (PVF) und der vertikale Impuls gemessen wurden) Bewertungsverfahren wurden in dieser Studie für die Evaluierung der Schmerzen herangezogen. 39 klinisch gesunde Hunde mit einem Körpergewicht zwischen 5,5 und 60,5 kg (keine Rassespezifität) wurden in diese Studie eingeschlossen. Die Hunde wurden nach Randomisierung in zwei Gruppen verteilt: Tiere der Gruppe M (n = 19) erhielten 50 mg•kg-1 IV Metamizol TID. Tiere der Gruppe C (n = 20) erhielten 4 mg•kg-1 Carprofen IV SID. Die Patienten wurden 3, 6, 9, 12, 20, 24, 28, 32, 36, 44, 48 and 56 Stunden nach Operationsende subjektiv beurteilt. Wurden bei der Evaluierung mittels mMPS bzw. VAS Punktwerte von 12 bzw. 50 Punkten überschritten, so wurde dies als Anzeichen von Schmerzen betrachtet, welche mit einer intravenösen Gabe von Buprenorphin, 10 μg•kg-1, behandelt wurden (rescue analgesia). Eine Ganganalyse wurde einmal präoperativ (preOP) und dann am ersten (OP1) und zweiten (OP2) postoperativen Tag durchgeführt. Drei Patienten in der Gruppe C benötigten in den ersten 3 bis 6 postoperativen Stunden rescue analgesia. Keines der Tiere in Gruppe M benötigte die Gabe zusätzlicher Schmerzmittel. Sowohl bei der mMPS als auch bei der VAS zeigten Tiere der Gruppe M im Vergleich zu Gruppe C über den gesamten Zeitraum niedrigere Schmerz-Werte. Je nach verwendeter Schmerzskala waren diese Unterschiede nach 6 h (mMPS) bzw. nach 24 h (VAS) als signifikant zu betrachten (p < 0.05). Die postoperativen Ganganalysen zeigten bei Hunden der Gruppe M eine bessere Belastung der operierten Gliedmaße (p < 0.05). Diese Ergebnisse zeigen, dass die alleinige Verwendung von Metamizol als Analgetikum eine potente und zufriedenstellende Analgesie nach orthopädischen Eingriffen bei Hunden gewährleistet. Des Weiteren wurde gezeigt, dass die alleinige Verwendung von Carprofen in der vom Hersteller empfohlenen Dosierung nach Hüftgelenksersatz bei Hunden nicht immer eine zufriedenstellende Analgesie hervorruft.

The Grey Bruce Health Network began developing regional clinical pathways in 2002 (Meleskie and Wilson 2003). The Total Hip Replacement (THR) pathway was one of the first developed. A three-stage pathway, the THR pathway has an acute stage for the first five hospital days following surgery, a post-acute stage for convalescent care in hospital and a community-care stage for community physiotherapy after hospital discharge. Pilot results (119 patients) have shown an increase in information transfer, streamlining care and increasing efficiencies (capacity savings of over $92,000), increased usage of best practices, more consistent care across the continuum of care and decreased length of stay (by 1.55 days) at the regional hospital centre.
