Frazer Rice (00:04)I went through a bunch of your articles covering the power industry and energy generation and a lot of things that are happening federally, state level, and it's going to be a lot to get our arms around, but you were the person to do it. So just generally speaking, we're at a point in time with energy and transition ⁓ that policy is moving. Maybe take us through a little bit about the goals and the problem we face. Anna Kramer (00:31)So there are sort of two, I would say, competing problems right now. ⁓ The first one is load growth, which means basically more demand on the electricity grid. And that is something that we haven't seen in this country in decades. for really around 2000 up until maybe a couple of years ago, energy demand on the grid has been fairly constant or even declining slightly. And the reason for that is that everything has become more efficient. Like every appliance you use, every light bulb, your car, everything that could possibly have a demand on the grid is more efficient than it used to be, which is awesome. There's a lot of wonderful benefits that we get from that, including the fact that for a long time utilities and transmission planners and states and the federal government have not really ever had to think about the grid or about like where you get your power aside from these sort of technical conversations that the average person doesn't really pay any attention to. Pannell is a native of Petersburg, Virginia, who began his career of service through his achievement of the Boy Scouts of America, Eagle Scout Rank during his matriculation through high school. Altmann earned a B.A. in African American Studies with a focus in Public Policy from the University of Maryland College Park in 2009 and a Master of Public Administration with a focus in Public Policy from Bowie State University in 2016. Upon graduation, Altmann began his career in the arena of politics and government relations serving under three elected officials in the State of Maryland for over 10 years. In March 2016, Altmann began working in the Non-Profit Sector of the District of Columbia for Food & Friends Inc., as the Manager of Intergovernmental Relations and Public Funding. After a successful service career with Food & Friends, Altmann continued to strive for higher aspirations and worked for the International Association of Campus Law Enforcement Administrators (IACLEA) in 2019 as the Director of Government and External Affairs. During his tenure at IACLEA, Altmann directly interacted with and advocated to, members of the U.S. Congress, the Executive Administration, and federal government agencies in the public safety arena. In pursuit of enhancing best practices and striving to find solutions to problems in the policing community, Altmann worked tirelessly to bring different perspectives to difficult conversations addressing community policing in America. During his time at IACLEA, he was also tasked to lead the COVID-19 Rapid Response Task Force for the nation's Colleges and Universities at the onset and throughout the first year of the COVID-19 pandemic. In the early Spring of 2021, Altmann was presented with an excellent opportunity to join Coca-Cola Consolidated, Inc., as the Director of Government Relations over the Mid-Atlantic Region. Coca-Cola Consolidated is the largest independent bottler of Coca-Cola in the United States. Since that time, Altmann has continued to work to raise awareness surrounding the positive economic, environmental, social, and sustainable impact that his company and The Coca-Cola brand have on the region and country as a whole. Davion Percy is a Government Relations Executive with nearly 2 decades of experience in Government and Politics. He began his career by working in Prince George's County Government. While with the county, he worked for 5 years. The executive branch and another 5 years in the legislative branch before taking so government relations job at Alexander & Cleaver, P.A. (A&C). He quickly rose through the ranks of A&C, becoming Vice President of Government Relations before leaving to launch his firm, Percy Public Affairs. EP410: The Imperative and a 201-Level Financial How-To for Payers and Provider Organizations to Collaborate to Help CKD Patients and Others With Chronic Conditions, With Dan Serrano

In this healthcare podcast, I am talking with Dan Serrano; and we're talking about payer/provider collaboration—blocking and tackling, I'm gonna say—from primarily a financial and revenue point of view. I'd classify this as, say, a 201-level discussion (ie, not entry level, but it's also not super deep in the weeds). We mainly cover the ins and outs of why a provider organization should probably be looking to get paid to better take care of patients with chronic disease and drive better patient outcomes at lower downstream costs and, to some degree, also why payers should be helping provider organizations in their local communities to do so by providing some help and shelter on the journey from here to a capitated payment. The focus today is really, I'd have to say, on the messy middle, where a provider organization does not have capitated contracts nor access to any premium dollars, which, by all accounts, is the holy grail here. The premium is where it's at, and provider organizations might want to be aiming to get a piece of that action. The why for this “get the premium dollar” prime directive is pretty self-evident when you look at the big bucks rolling around in the coffers of those who are collecting said premium dollars. So, this “get the premium” endgame is, for sure, a big piece of the why—why, if I am a provider organization, I might want to take the time and energy and spend the money to embark on a path that might lead me to be able to get compensated for the stuff that patients really want and need to do better, which includes all of the things that I spoke about with Eric Gallagher in episode 405. Also, Vivek Garg, MD, MBA, in episode 407 and Amy Scanlan, MD, in episode 402. Spoiler alert: It's not easy. Now, I asked Dan Serrano, as aforementioned my guest today, to offer up his advice here in the context of CKD (chronic kidney disease) patients. Why did I ask Dan to use the CKD case study, as a touchstone? Well, first of all, talking about this topic in totally theoretical terms is not ideal. We need an actual example for a lot of this to kind of make sense, combined with the first step for most outcomes improvement programs, which is to study your data and pick a patient population to focus on where the data suggests that you can have a big impact. And speaking of impact, did you know that an underlying reason why heart failure patients get hospitalized and rehospitalized is because of underlying CKD? So, impact in the short term and longer term, which I'll get to in a sec. Another reason is—and I'm quoting John Rodis, MD, MBA, here, who is the independent medical director of QC-Health®—Dr. Rodis said the other day, “I sure as heck hope I don't get CKD, because if I do, chances are I'm not going to be diagnosed. And even if I am diagnosed, I won't be treated properly.” So, there's that. And I can see why he's saying that. Two out of five patients with ESRD (end-stage renal disease) don't even know they have kidney disease at all. And the number of patients with progressing CKD on any kind of evidence-based treatment plan is stunningly low. But also, here's another reason I asked Dan Serrano to talk about CKD patient populations specifically as his example: I and Dr. Rodis and the team at QC-Health are not the only ones who have figured out that CKD patients are notoriously expensive and way underdiagnosed. You know who else has figured this out? Payers. Also, private equity. In fact, I was in a meeting with a payer recently, and they stated they had to get CKD patients into point solutions. This payer—and I've heard of others, too—none of these entities are waiting around. And I guess, fair enough, if you look at some of the population health data, that I'm sure these payers and others are looking at. But if you work for a payer and you're listening right now, what I would say, “Okay, with the point solutions, one that you have carefully vetted, of course, because we have patients suffering right now and dollars being frittered away right now.” But I also would submit that those point solutions will perform a whole lot better if we are all gunning for synergies. PCPs (primary care physicians) and traditional FFS (fee-for-service) models in this country need your help. The payment models and admin burden are decimating. Payers certainly are a group with some culpability here. (Sorry to be saying the quiet part out loud.) Instead of forgoing them, please help PCPs. Am I saying be altruistic? Actually, no. Listen to episode 409 with Larry Bauer or episode 391 with Scott Conard, MD, or an upcoming show with Jodilyn Owen and what you will hear is the amazing ability for clinicians rooted in the community to actually drive change in their local markets. In fact, I'd hypothesize that these community-rooted organizations probably have a better track record for actually moving the needle on patient outcomes than any snazzy tech that I have seen, although I am sure that there are one or two very effective snazzy techs out there—the exception proves the rule and all that. Bottom line: As I do so often, I am advocating for payers and provider organizations within communities to collaborate, regardless of whether there's a third party also in the mix. I am reporting all of this in the spirit of being helpful but also with some degree of urgency for any care delivery organization because, I mean, really, forget about the holy grail of trying to capture a percentage of the premium if the money is already going elsewhere to too many point solutions who are already capturing a portion of the premium. IRL, this is what's already going on out there. But where there's a challenge, there is also opportunity. As I have said pretty repeatedly for the past four minutes, because the bar is so low and because CKD patient outcomes are bad news, in general, from a lot of angles, CKD is actually a great place for providers to work hard to improve care and quality. From a financial standpoint, I think there's also a great business case for payers to help provider organizations do so. Doing better than the local standard of care is not hard, sadly. And what that means is that there's so much money that's possible to save due to the expense of this condition. And if you're a payer, even a payer with a third-party CKD solution, if you can help local PCPs and others level up their care, then either you don't have to pay for the third-party point solution for patients who can be managed successfully locally and/or there's a more frictionless path for those patients to be identified and get into the point solutions that are available to them. Let's all keep in mind that patients at rising risk are falling through a lot of cracks. You can have the best point solution in the world, but if patients aren't making it there, then, yeah, no outcomes will improve. No costs will be reduced. Everything I just went through are also all of the reasons why we picked CKD as our focus for a national Groundswell Movement™ that the benefit corp I am co-president of is kicking off to improve CKD patient outcomes. Did you know there is a group of native plants which can be highly adaptable, can serve multiple functions in our gardens, and perhaps even be a backbone component to our home landscapes, but which has historically been overlooked for those purposes? This group of native plants is the sedges, or more specifically, members of the genus Carex. In this episode of the Backyard Ecology podcast, we talk with Sam Hoadley about sedges and their use in the home garden and landscape. Sam is the Manager of Horticultural Research at Mt. Cuba Center in Delaware. Recently, Mt. Cuba Center released their Carex trial report. This report evaluated 70 species and cultivars of Carex over 4 years for garden and landscape use in the Mid-Atlantic Region. During our conversation, Sam and I talk about how Mt. Cuba Center's trials are conducted, the importance of sedges / Carex species, how they can be used in the garden or home landscape, and many other topics. Sedges may not produce the colorful, flashy flowers that we often think about and are drawn to when we are picking native plants to use in our home landscapes or pollinator and wildlife gardens, but they are definitely worth considering. It's our second Mid-Atlantic preview pod, as we dig into the teams in Region 4. In order: CUNYAC (at 2:00) This isn't a relevant conference as long as they don't have an AQ but they are still playing baseball and that's cool MAC Freedom (at 5:35) Arcadia had one of the more surprising dominant seasons in recent D-III history and look poised to be a regional contender once again Misericordia finally got knocked off their MACF throne but look ready to re-claim it Stevens might be able to ride their always-excellent pitching staff back to the postseason but have a lot of offense to replace NJAC (at 18:00) William Paterson finally won the NJAC with one of the nation's best offenses despite extremely poor run prevention; they should be tremendously entertaining again in 2023 Rowan is as solid as it gets but may need to rely on some transfers on the mound to stay at the top of this loaded conference Montclair State can bang with the best of 'em but will need some pitching depth to step up to make noise in a regional Kean had a down year but still boasts some of the best pitching in a suddenly offense-friendly conference TCNJ leads the rest of the pack of several teams that could reasonably make noise in this annual gauntlet UEC (at 43:30) Penn State-Harrisburg has a loaded lineup coming back but a ridiculous amount of innings to replace that could make a title defense challenging Salute to Ryan McCarty but it's somewhat unclear what to expect from Abington moving forward UAA (at 53:55) NYU continues to baffle us but we can usually expect them to be solid even if the SOS is blah We conclude (at 57:10) with our player and pitcher of the year picks, and our teams to beat. Franklin & Marshall has the most talent back of last year's contenders and will obviously be motivated  Johns Hopkins will have a lot of new faces but we know they can be really good Swarthmore is still very talented despite losing half their team to Villanova (slight exaggeration) We could see a lot of moving and shaking up and down the rest of this conference with some coaching changes and intriguing talent on every other team CSAC (at 27:45) Well, well, well, look at Wilson winning 31 games, we love to see it Keystone is always something of a mystery but it's cool that now they have real competition in this conference Landmark (at 34:20) What does the encore look like for Catholic after the best year in program history? Virginia Cruse enlisted in the Army Reserve in 1997 where she trained in interrogation and linguistics. As an Arabic speaker, she served a panoply of tactical units in Ramadi, Iraq from 2003-2004 and subsequently served as a defense contractor and Department of Defense Civilian overseas. Virginia was selected as one of two Direct Commission Officers for the Mid-Atlantic Region in 2008 and served as a HUMINT officer supporting Naval Special Warfare and the Naval War College. Virginia transitioned to the mental health field in 2013 as a Licensed Professional Counselor specializing in Military Issues and Combat-Related Trauma. It's our second Mid-Atlantic pod, as we dig into the teams in Region 4. In order: CUNYAC (at 1:45) We really aren't sure what to make of this conference at this point but it seems like they will still be playing baseball in 2022, so that's good MACF (at 5:30) Misericordia will look to continue their epic streak atop this conference with their super clean style of baseball and HBP superpower DeSales and Stevens will look to finally dethrone the Cougars NJAC (at 16:35) Rowan is losing a good amount of offense from last year's World Series team but might have the best pitching staff in the region William Paterson really jumped up into the upper echelon of this conference and are in good shape to stay there Kean was definitely disappointing in 2021 but have been too good recently to stay down for long Montclair State is a 3x national champion but hasn't been this good in a while TCNJ and Ramapo should all be much better than last year UEC (at 35:10) Penn State-Harrisburg lost an absolute legend in Bret Williams but still is loaded and ready to defend their conference title Ryan McCarty is chasing some D-III records for Penn State-Abington UAA (at 46:14) NYU...has nothing on their website so we don't know much other than they will be playing baseball and are in the UAA which means we will be paying close attention to them We conclude (at 49:05) with our player and pitcher of the year picks, and our teams to beat. In order: AEC (at 3:57) Marymount won the first ever AEC title and look primed to defend their title Centennial (at 11:05) Johns Hopkins is loaded again but will be tested even more with a full schedule and traditionally ridiculous SOS  Swarthmore is back and we're excited to see if they can challenge Hopkins immediately after a full year off F&M, Haverford, and Washington College can all be expected to be in the mix CSAC (at 32:17) Keystone runs this conference and probably always will... ...but could Wilson actually make a run at the Giants? We'd love to see it! Ria Freydberg – The Truman Charities Podcast: A Community of Caring with Jamie Truman Episode 28 Ria Freydberg Ria Freydberg serves as the Executive Director for the Mid-Atlantic Region of the Leukemia and Lymphoma Society. After more than a decade in restaurant management, Ria was nominated to run for LLS' Man & Woman of the Year campaign in memory of her mother. Ria lost her mother to acute myeloid leukemia and after participating in the campaign, new she had found a new love and passion for non-profits, specifically LLS. Shortly after, her mentors, Allison Gurnitz (then LLS Board Chair) and then Executive Director, Beth Gorman, recruited her to work for the National Capital Area Chapter. Fast forward ten years and Ria now oversees the Washington Metro Area and state of Maryland (aka the Mid-Atlantic) for LLS. She works tirelessly toward a world without cancer and, as the top producing region in the country, has an amazing team of volunteers and staff alongside her.
